Varied effects of experimental diabetes on the autonomic nervous system of the rat.

Disorders of the autonomic nervous system are a frequent late complication of human diabetes. They have been extensively studied clinically, yet their pathologic aspects are still poorly understood. Also, reports on the autonomic nervous system in animal models for diabetes are scanty. Therefore we have investigated sympathetic preganglionic and postganglionic nerve fibers, vagal fibers, as well as sympathetic and enteric neurons of male Wistar rats 1 year after streptozotocin or vehicle injection. By light and electron microscopic morphology we observed: various degenerative changes in sympat

Microneurographic recording from unmyelinated nerve fibers in neurological disorders: an update.

Microneurography is a unique neurophysiological technique allowing direct recording of unmyelinated postganglionic sympathetic or afferent nociceptive fibers by tungsten needles inserted into a peripheral nerve fascicle. In recent years, microneurography has been used to ascertain autonomic impairments in central neurological disorders such as sleep disorders, Parkinson's disease, amyotrophic lateral sclerosis, or vasovagal syncope. Abnormal resting muscle sympathetic nerve activity (MSNA) and skin sympathetic nerve activity (SSNA) or the abnormal sympathetic response to arousal have been desc

Preganglionic fibers in the rat hypogastric nerve project bilaterally to pelvic ganglia.

Stimulation of the hypogastric nerve (HGN) often evokes bilateral responses in some pelvic organs. Retrograde labeling studies indicate that axons of postganglionic neurons often cross to the opposite side. However, there is little information available as to whether preganglionic fibers in the HGN have a contralateral projection to pelvic ganglia. A retrograde tracer was injected into the left major pelvic ganglion (MPG) in rats receiving various lesions of preganglionic nerves (HGN and pelvic nerve, PN). The lumbar spinal cord was then examined for location and number of dye-filled neurons.

Surgical anatomy of the internal carotid plexus branches to the abducens nerve in the cavernous sinus.

OBJECTIVE: Sympathetic branches to the abducens nerve derived from the internal carotid artery sympathetic plexus, while in the cavernous sinus, have been scantly described in the extant literature. Therefore, the present cadaveric study was performed to better elucidate this anatomy. PATIENTS AND METHODS: Eighteen cadaveric sides underwent dissection. RESULTS: The number of branches derived from the sympathetic plexus traveling with the internal carotid artery in the cavernous sinus was one on 11.1 %, two in 11.1 %, and three in 72.2 %. One side was found to have no branches (5.6 %). The me

Gustatory-salivary reflex: neural activity of sympathetic and parasympathetic fibers innervating the submandibular gland of the hamster.

Electrophysiological experiments were performed to clarify the neural control mechanisms subserving gustatory-salivary reflex in anesthetized and decerebrate hamsters. Efferent neural activities of postganglionic sympathetic and preganglionic parasympathetic fibers, innervating the submandibular gland, were recorded when taste stimuli were infused into the oral cavity. Neural activities of primary gustatory afferents were also recorded from the chorda tympani (innervating the anterior part of the tongue) and the glossopharyngeal nerve (innervating the posterior part of the tongue). The parasym

[Localized cardiac uptake defect with 123I-MIBG SPECT in a case with pure progressive autonomic failure].

A 60-year-old man with 3 years' history of autonomic nervous system disturbances such as orthostatic hypotension, anhidrosis and atonic bladder is presented. He had an episode of paroxysmal atrial fibrillation as well. He had neither extrapyramidal nor cerebellar signs, and was diagnosed as having pure progressive autonomic failure. His cardiac SPECT with 123I-MIBG showed the loss of uptake at the apex and the inferior wall and the diminished uptake at lateral walls. But 99mTc-tetrofosmin SPECT following an exercise stress showed a normal cardiac uptake. It was suggested that non-cardiac disea

Sudomotor Dysfunction.

Disorders of sudomotor function are common and diverse in their presentations. Hyperhidrosis or hypohidrosis in generalized or regional neuroanatomical patterns can provide clues to neurologic localization and inform neurologic diagnosis. Conditions that impair sudomotor function include small fiber peripheral neuropathy, sudomotor neuropathy, myelopathy, α-synucleinopathies, autoimmune autonomic ganglionopathy, antibody-mediated hyperexcitability syndromes, and a host of medications. Particularly relevant to neurologic practice is the detection of postganglionic sudomotor deficits as a diagno

Bladder dysfunction in distal autonomic neuropathy of acute onset.

A patient with cholinergic dysautonomia and a patient with pandysautonomia have each been investigated for disturbances of bladder and urethral function. Both patients suffered from an inability to develop or sustain a detrusor contraction, while retaining normal bladder sensation. Biopsy specimens of bladder muscle stained for acetylcholinesterase revealed a significant reduction in cholinergic nerves compared with controls; however, the prominent cholinergic subepithelial plexus was strikingly preserved. These findings lend support to the view that acetylcholinesterase-containing nerves in t

Auriculotemporal Syndrome (Frey Syndrome).

Frey syndrome is a common sequela of parotidectomy, and although it is not frequently manifested clinically, it can cause significant morbidity for those affected. Frey syndrome results from synkinetic autonomic reinnervation by transected postganglionic parasympathetic nerve fiber within the parotid gland to the overlying sweat glands of the skin. Many surgical techniques have been proposed to prevent the development of Frey syndrome. For those who develop clinical symptoms of Frey syndrome, objective testing can be performed with a Minor starch-iodine test. Some of the current methods to pre

The Pharmacology of Autonomic Failure: From Hypotension to Hypertension.

Primary neurodegenerative autonomic disorders are characterized clinically by loss of autonomic regulation of blood pressure. The clinical picture is dominated by orthostatic hypotension, but supine hypertension is also a significant problem. Autonomic failure can result from impairment of central autonomic pathways (multiple system atrophy) or neurodegeneration of peripheral postganglionic autonomic fibers (pure autonomic failure, Parkinson's disease). Pharmacologic probes such as the ganglionic blocker trimethaphan can help us in the understanding of the underlying pathophysiology and diagno

Interaction between picrotoxin and 5-hydroxytryptamine in the superior cervical ganglion of the cat.

1. Electrophysiological techniques were utilized to study the actions of 5-hydroxytryptamine (5-HT) and picrotoxin on the superior cervical ganglion of the cat.2. The intra-arterial administration of 5-HT to the ganglion elicited both depressant and excitatory actions. In low doses (0.01-0.5 mug) the amine produced a depression of ganglionic transmission. In larger doses (2-50 mug) it produced an excitation of ganglion cells (early discharge) and an initial enhancement of transmission, which was followed by depression. Picrotoxin (25-500 mug, i.a.) blocked the initial excitatory effects of 5-H

Autonomic innervation in multiple system atrophy and pure autonomic failure.

BACKGROUND: Pure autonomic failure (PAF) and multiple system atrophy (MSA) are both characterised by chronic dysautonomia although presenting different disability and prognosis. Skin autonomic function evaluation by indirect tests has revealed conflicting results in these disorders. Here, the authors report the first direct analysis of skin sympathetic fibres including structure and function in PAF and MSA to ascertain different underlying autonomic lesion sites which may help differentiate between the two conditions. METHODS: The authors studied eight patients with probable MSA (mean age 60±

[An electrophysiological study on the artificial somato-autonomic pathway for inducing voiding].

OBJECTIVE: To investigate the possibility of regeneration of somatic motor nerve to replace splanchnic nerve and the electrophysiologic characters of the regenerated nerve. METHODS: An artificial somato-autonomic reflex pathway was established by intradural microanastomosis of L(4) ventral root (VR) to L(6)VR at the left side in 12 male Wistar rats. Then the L(4)VR proximal to the anastomosis was stimulated by silver electrode and the evoked potentials were recorded on the distal end to the anastomosis, pelvic nerve and postganlionic fibers of the major pelvic ganglia (MPG). Chronic idiopathic anhidrosis.

We describe the cases of eight patients with chronic idiopathic anhidrosis. These patients were heat intolerant and became hot, flushed, dizzy, dyspneic, and weak but did not sweat when the ambient temperature was high or when they exercised. Four patients had preganglionic sudomotor lesions and in the remaining 4 the lesion appeared to be postganglionic. The patients did not have orthostatic hypotension, other evidence of generalized autonomic failure, or symptomatic somatic neuropathy. One patient regained thermoregulatory sweat function and no patient's condition progressed to generalized a

Axonal degeneration of the peripheral nerves and postganglionic anhidrosis in a patient with multiple sclerosis.

A 36-year-old woman had, since the age of 24, numerous episodes of visual loss and spinal symptoms and signs at various levels, and was diagnosed as multiple sclerosis(MS). CSF myelin-basic-protein was increased. Neurological and electrophysiological investigations suggested the peripheral nerve involvement. Sural nerve biopsy performed about six years after the onset, revealed severe loss of both myelinated and unmyelinated fibers. Subsequently, histamine skin reaction was defective in the lower limbs. Tests on sudomotor and pupillary functions indicated deficits of both central and postgangl
