Aviation Noise Impacts: State of the Science.

Noise is defined as "unwanted sound." Aircraft noise is one, if not the most detrimental environmental effect of aviation. It can cause community annoyance, disrupt sleep, adversely affect academic performance of children, and could increase the risk for cardiovascular disease of people living in the vicinity of airports. In some airports, noise constrains air traffic growth. This consensus paper was prepared by the Impacts of Science Group of the Committee for Aviation Environmental Protection of the International Civil Aviation Organization and summarizes the state of the science of noise ef

Simulator sickness: a problem for Army aviation.

"Simulator Sickness" describes a symptom complex frequently reported by pilots during or after flight simulator training. There were 112 helicopter pilots at a U.S. Army AH-1 Cobra Flight Weapons Simulator (FWS) who completed a symptom-oriented subjective questionnaire. Of these, 40% reported symptoms of dysequilibrium; pilots developing simulator sickness had significantly more total and AH-1 flight time. Adaptation to the syndrome occurred with increasing FWS experience. The history and aeromedical significance of simulator sickness are briefly reviewed, and a case report presented. A mandat

The controller, aviation medicine and air safety.

Aviation medicine has researched many important facts on pilots, but little on direct relationships between controllers, aviation medicine and air safety. The unsuspecting flying public accepts a 'blind faith' in aircraft and pilots, unaware that aircraft are controlled within 'suspect' ATC systems. The deceptive simplicity of controlling air traffic in apparently limitless skies belies the complexity of man-machine ATC systems operated in ever-crowded airspace, sometimes with antiquated equipment and indifferent communications. The indivisible operational controller/pilot team strives to meet

Navigating Fabry Disease in a Military Aviator.

BACKGROUND: Fabry disease (FD) is a rare, X-linked lysosomal storage disorder caused by deficient alpha-galactosidase A (alpha-Gal A) activity, leading to the accumulation of glycosphingolipids and resulting in a wide spectrum of systemic symptoms, including neurological, renal, cardiovascular, and cerebrovascular manifestations. While the disease affects approximately 1 in 100,000 individuals, its incidence may be underreported, and no cases in aviators have previously been documented. CASE REPORT: A 30-yr-old U.S. Air Force C-5 pilot with a family history of FD was diagnosed with a pathogen

Squamous Cell Papilloma in a Student Aviator.

BACKGROUND: Although oropharyngeal squamous cell papilloma (OSCP) is not uncommon in the general population, reports of OSCP in aviators are rare. This case report serves as a call for flight surgeons to consider all oropharyngeal pathology as a potential source of airway compromise during flight, and highlights regional risk factors for developing malignancies of the oropharynx.CASE REPORT: A 24-yr-old male foreign national student aviator from a central Asian country was at Fort Rucker for flight training. He presented to the clinic with a chief complaint of sore throat and cough for 2 d. He

Urinary tract calculi in military aviators.

BACKGROUND: Urinary tract calculi are a common affliction in the United States and estimates of the adult lifetime risk of developing this condition range from 10 to 15%. Although highly variable, the clinical presentation of someone afflicted with urinary tract calculi typically involves varying degrees of pain, nausea, hematuria, and dysuria. Current disease, as well as a history of urinary tract calculi, is potentially disqualifying for aviation duties in the United States military as well as for commercial aviators. METHODS: Utilizing population based data from the Defense Medical Epidemi

Refractory pemphigus vulgaris in a military aviator.

Pemphigus vulgaris is a rare, chronic autoimmune blistering disease of the skin and mucous membranes that if untreated can be fatal. Current treatment regimens use high-dose systemic steroids to reduce the production of autoantibodies and adjuvant immunosuppressives to compliment and reduce the need for steroids. The disease itself and the pharmacotherapy that makes up the mainstay of treatment present the potential to cause significant disability in aviation personnel. This case report describes the 8-yr treatment course of a military aviator from initial diagnosis to treatment and the achiev

Evaluation of systemic allergy in a jet aviator.

Cholinergic urticaria and exercise-induced anaphylaxis (EIA) are related conditions. Cholinergic urticaria is caused by a rise in body core temperature and usually results in pruritus, skin lesions and, rarely, in serious respiratory and cardiovascular compromise. EIA can result in a cardiovascular compromise and syncope. Ingestion of certain foods may be associated with EIA. A 41-year-old jet pilot complained of 3-month onset of pruritus and urticaria during treadmill exercise. On one occasion, after a routine exercise bout, albeit with pruritus and urticaria, he experienced two short episode

To Fly Or Not To Fly? Aviation and Respiratory Diseases.

For most passengers, even those with respiratory disease, air travel is safe and comfortable. Some travelers may experience hypoxia at sea level but may not need supplemental oxygen during air travel in a hypobaric hypoxic environment. For some individuals compensatory pulmonary mechanisms may be inadequate, causing profound hypoxia. In addition, venous thromboembolism/pulmonary emboli may occur, especially during long haul flights. With adequate screening, patients at risk can be identified, therapeutic solutions can be proposed for the flight, and most can travel can continue safely with sup

Carpal Tunnel Syndrome in Military Aviators.

INTRODUCTION: The incidence of carpal tunnel syndrome (CTS) is increased in occupations exposed to repetitive motion, poor wrist posture, and vibration exposure. While all pilots are exposed to these factors, helicopter pilots are especially exposed to vibration. The study is to identify the incidence and risk factors for CTS in military aviators. MATERIALS AND METHODS: Clearance was obtained from Tripler Army Medical Center IRB. The Defense Medical Epidemiological Database was queried for all new cases of CTS from 2006 to 2015. Incidence rates (IRs) were determined for helicopter pilots, fix

Uncertainties in climate assessment for the case of aviation NO.

Nitrogen oxides emitted from aircraft engines alter the chemistry of the atmosphere, perturbing the greenhouse gases methane (CH(4)) and ozone (O(3)). We quantify uncertainties in radiative forcing (RF) due to short-lived increases in O(3), long-lived decreases in CH(4) and O(3), and their net effect, using the ensemble of published models and a factor decomposition of each forcing. The decomposition captures major features of the ensemble, and also shows which processes drive the total uncertainty in several climate metrics. Aviation-specific factors drive most of the uncertainty for the shor

Prescribing spectacles for aviators: USAF experience.

This special report was written for USAF vision specialists to use as a guide when prescribing spectacles for military aviators and to extend to the civilian sector the knowledge gained from the USAF experience. Visual correction in aviators presents some unique problems, especially for presbyopes. The demands of each individual aircraft environment need to be well understood. Ophthalmologists and optometrists must consider all pertinent aeromedical factors before prescribing spectacles for ametropic aviators.

The use of contact lenses by USAF aviators.

Contact lenses are an alternative method to spectacles for correcting refractive errors, but the U.S. Air Force prohibits the wearing of contact lenses by all aircrew members unless medically or optically indicated (AFR 167-3). USAFSAM has a clinical contact lens study consisting of 55 individuals wearing contact lenses for eye defects such as keratoconus, aphakia, anisometropia, and other special indications. Of these 55 individuals, 33 had medical conditions affecting their vision and were unconditionally grounded. Of these individuals, 31 (18 of 19 pilots, 8 of 9 navigators, 5 of 5 other ca

[Thoracic injuries in the field of aviation medicine].

In civil and war traumatology, 33% of deaths are due to chest traumas although this involves only 25% of cases. Examination of the physiopathy clinical medicine and treatment of the various traumatic lesions of the chest and their complications shows that rapid transfer of such patients to suitable places of treatment following adequate first aid is indispensable. The use of fully equipped aircraft is thus vital. Transport criteria are dictated by the environment, first aid resources, meterorological conditions, cruising altitude, available equipment [an automatic respirator and aspirator are

A Cross-Cultural Anthropometric Analysis in Military Aviation.

BACKGROUND: Aircrew-aircraft compatibility is important in military aviation for flight safety. Anthropometric aircrew selection standards in RAFO were embedded from the selection standards of western defense forces as the aircrafts were imported from there. Henceforth efforts were made to fit local native aviators into aircrafts not initially designed for them. In view of this, this study was carried out to obtain the anthropometric data of Oman aircrew recruits and compare these with published western and eastern data with a hope to understand and highlight the aircrew-aircraft mismatch issu

Human factors in aviation crashes involving older pilots.

BACKGROUND: Pilot errors are recognized as a contributing factor in as many as 80% of aviation crashes. Experimental studies using flight simulators indicate that due to decreased working memory capacity, older pilots are outperformed by their younger counterparts in communication tasks and flight summary scores. OBJECTIVE: This study examines age-related differences in crash circumstances and pilot errors in a sample of pilots who flew commuter aircraft or air taxis and who were involved in airplane or helicopter crashes. METHODS: A historical cohort of 3306 pilots who in 1987 flew commuter
