Management of the Axilla in the Patient with Breast Cancer.

Evaluation of the axillary lymph nodes is critical in the management of breast cancer because it is a key predictor of survival outcome. Surgeons must not only be able to perform sentinel lymph node dissection with high accuracy but also understand the implications of the results. Management of clinically node-negative and node-positive cases can vary significantly, as described in this review. With emerging data, management of the axilla in breast cancer will continue to evolve.

Contemporary management of the axilla in breast cancer.

The care of patients with breast cancer in the modern era involves a multimodal approach to treating locoregional and distant disease. Recent studies have demonstrated that the extent of surgical intervention in both the breast and axilla can be minimized through a personalized approach based on breast cancer stage, subtype, and planned adjuvant therapies. The older approach focused on complete removal of the axillary contents for appropriate staging and to determine the need for adjuvant systemic therapy and radiation. This approach has been replaced by sentinel lymph node biopsy, which allow

In vivo study of the surgical anatomy of the axilla.

BACKGROUND: Classical anatomical descriptions fail to describe variants often observed in the axilla as they are based on studies that looked at individual structures in isolation or textbooks of cadaveric dissections. The presence of variant anatomy heightens the risk of iatrogenic injury. The aim of this study was to document the nature and frequency of these anatomical variations based on in vivo peroperative surgical observations. METHODS: Detailed anatomical relationships were documented prospectively during consecutive axillary dissections. Relationships between the thoracodorsal pedicl

Castleman's disease of the axilla mimicking metastasis.

Castleman's disease is an unusual inflammatory lymphoproliferative disorder of unknown cause. It most commonly occurs in the mediastinum but rarely in the axillary lymph nodes. We report a case of localized axillary Castleman's disease mimicking metastasis as the patient had a palpable malignant mass in the breast, described by ultrasonography, color Doppler ultrasonography, contrast-enhanced CT, and dynamic enhanced breast MR images.

Myxoid solitary fibrous tumour of the axilla.

Mostly occurring in pleura, solitary fibrous tumour is an unusual soft tissue neoplasm. Solitary Fibrous Tumour with abundant myxoid stroma represents a rare morphologic subtype of the already uncommon tumour. We report a myxoid solitary fibrous tumour of the soft tissue in a 42-year-old man. The patient presented with a large, movable, nontender mass at the right axilla. The MRI showed a well-defined tumour with increased signal after gadolinium injection. During surgery, the mass was found to be closely associated with the axillary vein. The tumour was completely excised. Pathological and im

Lymphadenovarix in the axilla--an unusual presentation of filariasis.

Clinical manifestations of lymphatic filariasis depend on the area of lymphatic involvement and the duration of infection. A 21 year old man, resident in a filariasis endemic region, presented with multiple matted lymph nodes with cystic areas forming a large mass in his left axilla. An ultrasound scan of the axilla using a 7.5 MHz transducer revealed grossly dilated lymphatics but no filarial dance sign. Fine needle (21 G) aspiration cytology (FNAC) from the dilated lymphatics and solid areas in the lymph node mass revealed multiple microfilariae in a background of reactive lymphoid cells. Pe

Mycosis fungoides masquerading as tinea of the axilla.

UNLABELLED: Angel's trumpets (Brugmansia spp.) are important medicinal and ornamental plants belonging to the Solanaceae family, domesticated in the Andean region of South America. Indigenous communities in the Colombian Andes have created unique medicinal cultivars exhibiting a variety of unusual shapes in leaves, and specific uses for ritual or medicinal procedures. These cultivars exhibit leaf symptoms typically associated with viral infections, including mosaic, vein chlorosis, and morphological deformities. Colombian datura virus (CDV) is one of the most studied viruses in Brugmansia, whi

Management of the axilla in primary breast cancer.

Treatment of the axilla with either radiotherapy or surgery remains an integral part of the management of patients with invasive breast cancer. In general, the standard treatment of the axilla involves a partial ALND (surgical clearance of axillary nodes from levels I and II). There is as yet no evidence that axillary treatment improves survival, but the issue remains controversial. Axillary lymph node dissection is an effective staging procedure and is essential for local control of disease in the axilla, although, with increased emphasis on mammographic screening and early detection, the inc

Usually pathologic changes of the axilla are due to diseases of the lymph nodes. Infrequently there are soft tissue tumours, inflammations, diffuse changes of the axillary soft tissue, vascular diseases and pathologic findings of the shoulder joint. Regional fasciocutaneous flaps for the burned axilla.

The conventional approach for secondary correction of unstable axillary scars or contractures with skin grafting commonly risks failure of adherence and early recurrence of the problem, even in highly motivated patients. Potential early mobilization and the avoidance of postoperative splinting may be advantages of reconstruction with vascularized tissues instead. The local fasciocutaneous flap is advocated as a simple, rapidly elevated, single-staged option that preserves function while correcting these deformities. Although the dorsal thoracic fascial flaps are the most versatile in the axill

Fasciocutaneous flaps for burn contractures of the axilla.

Fasciocutaneous flaps do not compromise the function of joints, are less bulky and are easier to dissect. Their use prevents the uncomfortable prolonged splintage of the axilla with the shoulder in abduction which follows split skin grafting. Fasciocutaneous flaps are reliable and failure is minimal, if at all, unlike split skin grafting. Parascapular, lateral thoracic and cervicohumeral fasciocutaneous flaps have been used in 23 patients (aged 8-36 years) with a total of 30 flaps. The mean hospital stay was 12 days and no recurrence of the contracture was observed in the follow-up period of 3

Neurovascular injuries of the thoracic outlet and axilla.

In this retrospective series of 46 patients with neurovascular injury of the thoracic outlet or axilla, our management strategy emphasized prompt recognition of trauma that may be superficially innocuous. Whenever possible, the precise site of vascular injury was identified arteriographically to help plan an incision that would provide proximal vascular control as well as expedient injury exposure. Vascular reconstruction was accomplished with either end-to-end primary anastomosis (n = 17) or autogenous saphenous vein graft interposition (n = 26). No case of ischemic limb loss occurred, althou

Surgical treatment of hidradenitis suppurativa of the axilla.

We review our surgical experience with hidradenitis suppurativa of the axilla at Cook County Hospital during the years 1963 to 1972. The operation for this entity described by Pollock et al has (1) reduced the length of hospitalization, (2) increased the rate of primary healing and reduced the period of secondary healing, (3) allowed a great amount of tissue to be excised, (4) permitted both axillae to be operated on at once (without incapacitating the patient), (5) avoided the need for skin grafting, and (6) has had less morbidity for those patients in whom primary healing is not achieved. We

Case report: Axilla fibroadenoma - An atypical presentation.

INTRODUCTION: Fibroadenoma is the most common benign breast tumor among women between 15 and 35 years, however, a fibroadenoma arising from accessory breast tissue is a rare occurrence. CASE PRESENTATION: We encountered this uncommon presentation in a 37 years old female with a gradually increasing left axillary lump associated with discomfort. On ultrasound it was a 17.3 mm × 10.6 mm x 17.5 mm well defined solid nodule with internal vascularity, BiRADS IVa lesion. Core biopsy revealed fibroepithelial lesion and the patient electively underwent excisional biopsy. Histopathology confirmed the

Percutaneous trans-axilla transcatheter aortic valve replacement.

The left axillary artery is an attractive alternative access route for transcatheter aortic valve replacement (TAVR) and may provide better outcomes compared to other alternatives. Nevertheless, there remain concerns about vascular complications, lack of compressibility, and thorax-related complications. Between March 2019 and March 2021, 13 patients underwent transaxillary TAVR for severe aortic stenosis at the University Hospital Bonn. The puncture was performed with a puncture at the distal segment of the axillary artery through the axilla, with additional femoral access for applying a safe
