[Mycobacterium persicum pulmonary disease misdiagnosed as tuberculosis: a case report].

Mycobacterium persicum (M. persicum) is a nontuberculous mycobacterium that predominantly affects the lungs, causing pulmonary lesions, and is readily misdiagnosed as tuberculosis. Here we report a 48-year-old man with AIDS and M. persicum pulmonary disease who was initially misdiagnosed with pulmonary tuberculosis and received standard anti-TB therapy, yet showed no radiological or clinical improvement. Sputum specimens were subsequently subjected to targeted next-generation sequencing (tNGS), which identified M. persicum and established the definitive diagnosis. After switching to an anti-M.

Escaped surveillance: tuberculosis masked by severe hyponatremia in an immunocompromised patient: a case report.

BACKGROUND: Tuberculosis in immunosuppressed individuals frequently presents atypically and may produce false-negative screening results, delaying diagnosis. Hyponatremia and features of inappropriate antidiuretic hormone secretion can mimic paraneoplastic processes, further complicating the clinical picture. This case describes the challenges of diagnosing tuberculosis in a patient receiving prolonged corticosteroid therapy. CASE PRESENTATION: A 74-year-old East Asian woman with long-standing myasthenia gravis treated with prednisone presented with 1 month of progressive weakness, weight los

Unsuspected tuberculosis in COPD and use of levofloxacin: diagnostic challenges.

A 92-year-old female ex-smoker with chronic obstructive pulmonary disease (COPD) GOLD III, was admitted because of communitarian pneumonia in November 2013. She had been using inhaled corticosteroids and bronchodilators and presented five exacerbations of COPD due to pneumonia in the same year, with hospitalizations in March and September. The patient underwent the routine protocol for exacerbated COPD, and bacilloscopy for tuberculosis (TB) was negative. On admission, she had intense dyspnea and a productive cough that improved by administration of levofloxacin. Tests with Ziehll-Neelsen stai

[Scrotal tuberculosis in an immunocompetent patient: an atypical location of Koch's bacillus].

INTRODUCTION: Scrotal tuberculosis is a rare infectious condition that is more common in immunosuppressed patients. CLINICAL CASE: A 19-year-old patient with no known drug allergies presented with testicular pain and swelling for 2 weeks, along with significant weight loss in the last month, with no other systemic symptoms. He reported previous risky sexual relations. Acute right-sided epididymo-orchitis was initially suspected, but empirical treatments were suspended after testing negative for several STDs. However, the patient worsened, showing complicated epididymo-orchitis with fistulizat

A 1-year-old boy with near-complete tracheobronchial obstruction from endobronchial tuberculosis.

A one-year-old boy was referred to our respiratory department for further evaluation of obstructing endobronchial lesions. The lesions were detected on chest computed tomography (CT) performed at another hospital after the patient presented with cough and worsening wheezing. Physical examination revealed tachypnea and diminished breath sounds bilaterally without rales. The patient was receiving supplemental oxygen. Notably, his medical history was significant for an admission at 21 days of age for persistent cough, right upper lung atelectasis, and sputum analysis that revealed Bordetella pert

Recurrent Paradoxical Reactions in Cervical Tuberculous Lymphadenitis During and after Antitubercular Therapy in an Immunocompetent Patient.

Paradoxical reaction (PR) in lymph node tuberculosis (LNTB) is characterized by the worsening of existing lesions or the appearance of new lesions after an initial favourable response to antitubercular therapy (ATT). Although frequently reported in HIV-infected individuals, it can also occur in immunocompetent patients and may be mistaken for treatment failure or disease relapse. We report a 25-year-old immunocompetent woman with microbiologically confirmed cervical LNTB who developed recurrent lymph node enlargement during ATT and again after completing treatment. The diagnosis was establishe

Sensitivity of immunohistochemistry in the laboratory diagnosis of leprosy: a systematic review protocol.

INTRODUCTION: Leprosy is a chronic disease caused by the bacillus Mycobacterium leprae and remains a public health concern in endemic countries. Early diagnosis is fundamental to prevent transmission and irreversible disabilities. Histopathological identification of acid-fast bacilli in tissue specimens is traditionally considered the laboratory reference standard; however, its sensitivity is limited, particularly in paucibacillary forms. Immunohistochemistry (IHC) has been proposed as an adjunctive diagnostic tool for detecting M. leprae antigens in tissue samples, but its diagnostic accuracy

A myco-management problem: improving utilization of fungal and mycobacterial smear and culture.

UNLABELLED: Fungal and mycobacterial smear and culture are often inappropriately ordered, straining clinical laboratory resources and risking patient harm from false-positive results. To identify opportunities for optimization, we assessed utilization and yield of fungal and mycobacterial smear and culture over five years at a large academic medical center in an area of low tuberculosis prevalence. Positive rates were generally lower for non-respiratory specimens compared to respiratory specimens, prompting us to focus on collection method (swab vs non-swab) and location (operating room [OR] v

Syphilis, leprosy, and human immunodeficiency virus coinfection: a challenging diagnosis.

The association between syphilis, leprosy, and human immunodeficiency virus (HIV) is not well documented, and the emergence of isolated cases raises the interest and indicates that this triple coinfection can occur. We report the case of a 42-year-old man from Rio de Janeiro, Brazil, who presented with erythematous papules on the trunk, back, and upper and lower extremities; an erythematous plaque on the upper abdomen; and an erythematous violaceous plaque on the right thigh with altered sensitivity. Laboratory investigation showed a reagent VDRL test (1:512) and positive test results for Trep

A Case of Uncommon Tuberculosis Infection Involving the Sternoclavicular Joint.

BACKGROUND Tuberculosis involving the sternoclavicular joint is a rare manifestation of extrapulmonary tuberculosis. Involvement of the muscular and skeletal system in tuberculosis accounts for approximately 10% of extrapulmonary tuberculosis cases. Of these, only 1% to 2% involve infection of the sternoclavicular joint. The presentation of sternoclavicular joint tuberculosis is often insidious without systemic symptoms. Its diagnosis is very difficult because it mostly occurs with an atypical presentation, therefore requiring a multimodal diagnostic approach. Patients can require a combinatio

Laryngeal tuberculosis presenting as an isolated vocal cord lesion in an immunocompetent patient: a case report.

BACKGROUND: Laryngeal tuberculosis (TB), once a common complication of pulmonary TB, has become rare in the antibiotic era but remains an important diagnostic consideration in endemic regions, particularly in patients presenting with persistent hoarseness. While historically associated with advanced pulmonary disease, isolated laryngeal TB can occur even in immunocompetent individuals without systemic symptoms or radiographic evidence of lung involvement. Due to its nonspecific presentation—often mimicking malignancy or chronic laryngitis—misdiagnosis is common, delaying appropriate treatment.

The impact on airborne isolation duration utilizing GeneXpert versus acid-fast bacilli smears to guide discontinuation among individuals with suspected pulmonary tuberculosis: a systematic review and meta-analysis.

OBJECTIVE: The GeneXpert® MTB/RIF nucleic acid amplification test has been proposed as an alternative method to smear microscopy in guiding de-isolation for patients with suspected pulmonary tuberculosis, given its convenience and higher diagnostic accuracy. Currently, there is no comprehensive review summarizing the evidence on the isolation duration using GeneXpert. This systematic review and meta-analysis addressed the length of airborne infection isolation in hospitalized patients initially suspected of having pulmonary tuberculosis where de-isolation decisions were guided by GeneXpert nuc

The changing landscape of diagnosing HIV-associated tuberculosis.

Tuberculosis is the most common cause of hospitalisation and death globally among people with HIV (PWH), even in the antiretroviral therapy era. Diagnosis of tuberculosis is challenging in PWH as bacillary load is lower in sputum and a high proportion have extrapulmonary or disseminated tuberculosis. The clinical features of tuberculosis in PWH are nonspecific and there is a wide differential diagnosis of other opportunistic infections. Tuberculosis progresses rapidly in people with advanced HIV disease, necessitating rapid diagnosis. In this narrative review, key changes are described in the

Palatal Tuberculosis: A Rare Manifestation of Extrapulmonary Tuberculosis.

Tuberculosis (TB) remains a relevant public health concern in Portugal, despite the progress achieved in its control and management over recent years. Extrapulmonary manifestations represent a notable proportion of cases, while oral manifestations are uncommon, particularly those affecting the palate, which are rarely reported. The authors describe a case of a 63-year-old Afghan woman with multiple comorbidities, including poorly controlled diabetes mellitus with end-organ damage and heart failure, who presented with a painless ulcerated lesion on the hard palate with erythematous borders and
