Vestibular symptoms commonly occur in migraine, and episodic vertigo is most frequently seen. Auditory symptoms also occur, but are less common. When Bickerstaff described basilar artery migraine in 1961, he postulated that the many different symptoms were caused by basilar artery ischemia. He documented that neuro-otologic and other symptoms could occur before or during a migraine headache; others later established that these symptoms could also occur during the headache-free period. Case histories of eleven patients with basilar artery migraine are presented in detail. All met the diagnostic

We have investigated 6,972 patients with directional continuous-wave Doppler sonography within the last three and a half years, and have derived criteria for the sonographic diagnosis of basilar artery occlusion or tight stenosis in conjunction with 1,071 retrograde brachial angiograms. By sonographic patterns, we have suspected obstruction of the basilar artery or of both distal vertebral arteries in nine cases. Either bilateral sonographic silence or the absence of a diastolic flow component of the vertebral arteries served as criteria in the sonographic evaluation. Angiography of the verteb

We have investigated 6,380 patients with directional c-w Doppler sonography within the last 3 1/2 years, and have suspected obstruction of the basilar artery or of both distal vertebral arteries in 7 cases. Either bilateral sonographic silence or an absent diastolic flow component of the vertebral arteries were employed as criteria in the sonographic evaluation. Angiography of the vertebro-basilar system, performed in 6 cases, confirmed the diagnoses: basilar artery occlusion was found in 4 patients, 1 patient revealed tight stenosis of the basilar artery in its entire length, and 1 patient ex

Basilar artery occlusion was diagnosed by high-resolution computed tomography (CT) in 11 cases. The normal basilar artery imaged on plain CT scans has an attenuation value not higher than that of whole blood. If the CT attenuation value of the vessel is higher than that of blood, basilar artery occlusion is probably present. The tentative diagnosis can be confirmed by comparison of the attenuation values on the pre- and postcontrast scans, preferably using multiple slices obtained by multiplane dynamic CT.

This report describes for the first time the successful use of transluminal balloon-catheter dilation (angioplasty) for high-grade basilar artery atherosclerotic stenotic lesions in two patients with frequent, severe, progressing symptoms that were unresponsive to anticoagulant therapy. Excellent angiographic and short-term clinical results have been obtained. These results and the relative frequency of these lesions justify further evaluation of this approach.

In a case in which the patient became totally paralysed except for blinking and vertical eye movements, microscopic serial sections of the pons showed bilateral infarcts which were due to occlusion of two small basilar branch arteries, one on each side. One basilar branch was occluded by an atheroma lying at its junction with the basilar artery and the other by an intramural dissection within the wall of the basilar artery. This case provides the clinico-pathological correlation for two further basilar branch infarcts. An unusual finding was that one of the branch arteries supplied the basis p

Variants of the posterior intracranial circulation are important for surgeon, interventionalists and radiologists. Herein, a unique configuration of the basilar artery is reported. A 54-year-old man with a history of COPD, hypothyroidism, smoking, and hyperlipidemia presented to an outside institution with nausea, confusion, altered mental status, and ataxia. The patient was evaluated for stroke. Imaging revealed rotation of the basilar apex of 180 degrees, fetal configuration of the posterior communicating artery, right posterior cerebral artery filling from the left vertebral artery, and dup

Background and Purpose- Factors influencing recanalization success in basilar artery occlusions are largely unknown. Preliminary evidence has suggested that flow arrest in the vertebral artery contralateral to the catheter bearing vertebral artery may facilitate recanalization. The aim of this analysis was to assess the impact of anatomic variations and flow conditions on recanalization success in basilar artery occlusion treated with mechanical thrombectomy. Methods- Consecutive basilar artery occlusions treated with second-generation thrombectomy devices at a single-center were retrospective

BACKGROUND: Recently, the Basilar Artery on Computed Tomography Angiography (BATMAN) score predicts clinical outcome of acute basilar artery occlusion (BAO), yet there is no extensive external validation. The purpose of this study was to validate the prognostic value of BATMAN scoring system for the prediction of clinical outcome in patients with acute BAO treated with endovascular mechanical thrombectomy by using cerebral digital subtraction angiography (DSA).
METHODS: We analyzed the clinical and angiographic data of consecutive patients with acute BAO from March 2012 to November 2016. The

BACKGROUND: Acute basilar artery occlusion (BAO) is considered among the most severe medical emergencies, with very high morbidity and mortality. The aims of this study are to present 5 years experience of 12 centers in Lombardy region on BAO endovascular treatment and to evaluate prognostic factors that may improve clinical outcomes and recanalization rates.
MATERIALS AND METHODS: Registro Endovascolare Lombardo Occlusione Basilar Artery (RELOBA) registry is a retrospective multicentric collection of patients with acute BAO who underwent endovascular treatment between 2010 and 2015. A total

Superficial temporal artery-superior cerebellar artery anastomosis for basilar artery stenosis was performed in a patient who had experienced recurrent nonhemispheric symptoms, and whose angiogram revealed a midbasilar stenosis. Four months after the operation he was readmitted to our hospital complaining of transient motor weakness and numbness of the left extremities. No neurological deficit was found on admission, but 1 hour later the left-sided hemiparesis occurred again and progressed. On the angiograms the upper basilar system was visualized via the superficial temporal artery, and the m

Intracranial aneurysms are focal pathological dilatation of the arteries with a prevalence of 3.2%. Fusiform aneurysms are rare and mainly due to dissection or atherosclerosis. They may manifestas as stroke, cranial nerve palsies, brainstem compression and Sub-arachnoid hemorrhage. A diameter greater than 10 mm is a risk factor for rupture and warrants early intervention.

Basilar artery perforator aneurysms (BAPA's) are a rare entity. Their natural history and treatment are unclear. We describe the largest BAPA reported thus far in literature in a 64-year-old Caucasian woman. This patient did not present with subarachnoid hemorrhage, but with left hemiparesis due to pontine ischemia. The aneurysm was initially misdiagnosed as a tumoral mass in a referring center. Angiography confirmed the presence of a BAPA and a flow diverter was successfully placed. This case shows us that a BAPA can mimic a tumoral mass and can cause ischemia due to mass effect without havin

Strokes in the territory of the posterior cerebral artery (PCA) may rarely cause acute confusion or delirium, especially when bilateral or the dominant PCA are involved. Delirium as the only initial presentation of basilar artery thrombosis (with no brainstem or long tract findings) is an extremely rare occurrence. In this article, the clinical presentation of our case was an acute confusion with septic shock-like features (tachycardia, hypotension, and leukocytosis) for a few days without any focal deficit. These symptoms pointed more toward a non-focal neurological cause, especially meningoe

BACKGROUND AND PURPOSE: Dolichoectasia of the basilar artery is a characteristic finding of Fabry disease. However, its prevalence, severity, and course have been poorly studied. This study quantitatively evaluated, by MRA, a panel of basilar artery parameters in a large cohort of patients with Fabry disease.
MATERIALS AND METHODS: Basilar artery mean diameter, curved length, "origin-to-end" linear distance (linear length), and tortuosity index ([curved length ÷ linear length] - 1) were retrospectively measured on 1.5T MRA studies of 110 patients with Fabry disease (mean age, 39.4 ± 18.6 year

OBJECTIVE: To determine the imaging characteristics of basilar artery hypoplasia (BAH).
METHODS: From October 2009 to August 2013, 10 193 magnetic resonance angiogram (MRA) and 31 716 magnetic resonance imaging (MRI) consecutive cases were retrospectively retrieved from our institutional Picture Archiving and Communication System (PACS) database. BAH was defined as a continuous diameter reduction all over artery and a basilar artery diameter ≤ 2 mm. The same cut-off value was applied for vertebral artery hypoplasia (VAH). We measured the diameters of basilar and vertebral arteries with PACS i

OBJECTIVE: To describe a case of recanalization of a basilar artery occlusion with intravenous (IV) tenecteplase.
CASE: A 74-year-old man with a history of cardiomyopathy presented to an outside hospital with acute vertigo, dysarthria, gaze deviation, and ataxia. Computerized tomography arteriography demonstrated occlusion of the proximal basilar artery. IV tissue plasminogen activator was ordered; however, the patient received a cardiac dose of IV tenecteplase. The patient was transferred to our facility, whereby symptoms resolved, and repeat computerized tomography arteriography displayed r
