Nephrology, dialysis, transplantation : official publication of the European Dialysis and Transplant Association - European Renal Association2005eng
Over the past 15 years, the shortage of organs for transplantation has worsened. This has forced clinicians to review alternative approaches to organ procurement. These new approaches, however, may have serious implications both for patients and for society as a whole. Ever since the first cadaver organs were used for transplantation, organ procurement has relied on the altruism and goodwill of donors. It now appears that this 40-year-old policy is proving unsuccessful. In order to increase the availability of suitable organs and allow cadaver organ transplantation to continue, it is time to c

BACKGROUND: Artificial hydration (AH) in terminally ill cancer patients remains ethically and clinically controversial. Although evidence suggests limited benefit near the end of life, AH is frequently administered, particularly in East Asian settings where it may carry symbolic meaning. This study conceptualized physician decision-making as a clinical spectrum and examined demographic, clinical, and ethical factors associated with AH decision orientations.

Substance Use Disorder (SUD) is widely recognized as a chronic, relapsing medical condition by major authorities, including the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC), supported by clinical and neuroscientific evidence. Despite this recognition, health systems often respond through episodic, crisis-driven, or punitive models of care. Like other chronic diseases, SUD unfolds through stability, relapse, and recovery, requiring sustained, coordinated, person-centred care. Formalizing SUD as a chronic disease may reduce stigma, attract resources, n

Lower respiratory tract infections are a leading cause of paediatric morbidity and mortality worldwide. Children in low-income countries are disproportionately affected. This is in large part due to limitations in healthcare resources and medical technologies. Mechanical ventilation can be a life-saving therapy for many children with acute respiratory failure. The scarcity of functioning ventilators in low-income countries results in countless preventable deaths. Some hospitals have attempted to adapt to this scarcity by using hand-bag ventilation, as either a bridge to a mechanical ventilator

J. Marion Sims' efforts in surgically curing vesicovaginal fistula are retold. The controversy surrounding his surgical innovation on black slaves is discussed in the context of the ethical principles of autonomy, beneficence, and justice. The relevance of his story to modern surgical innovation in incontinence surgery is emphasized in a discussion of the Pereyra, Gittes, and paravaginal procedures. A call is made to establish a gynecologic ethic to ensure that patients' rights are respected in all future surgical innovation.

This article presents a case in which a patient with mild Alzheimer disease requests a prescription for a newly developed Alzheimer disease drug, after which the patient's daughter inquires about its potential usefulness as a prevention strategy for herself. The article discusses the physician's responsibility to balance the ethical principles of beneficence, nonmaleficence, and respect for patient autonomy when evaluating requests for medications from patients with neurocognitive disease as well as from asymptomatic but at-risk patients.

Although futility is commonly referred to when discussing end-of-life issues, theories of futile care are applied to issues of chronic care, particularly chronic wound care, herein. A case study will be used within the context of defining futility, understanding beneficence in healthcare, and discussing the obligation of the clinician to deliver therapeutic intervention that balances burden with benefit. Open and honest communication will be highlighted as one method to facilitate resolution when dilemmas of futility occur.

A psychiatric advance directive (PAD) is a legal resource that allows a person to document his or her wishes about receiving mental health services in the event that he or she is unable to provide consent at a future time. Advantages for completing a PAD include promoting the person's autonomy and authentic self, minimizing harms, and promoting beneficence. However, more research is needed to describe the positive and negative consequences associated with completing and implementing a PAD in the clinical setting.

Many changes occurred in hormonal contraceptives in the past few years to improve benefice/risks balance and get a better tolerance and observance. These changes concerned the decrease of steroids doses, changes in estroprogestogen sequence, use of less androgenic progestogen and more recently the use of new ways of administration with transdermal system, vaginal ring or subdermal contraceptive implant to avoid daily use. This article makes a review of hormonal contraceptives available in France and benefits of recent innovations.

Medical innovation and ethical dilemmas are intertwined in gastroenterology and hepatology. This narrative review explores direct-acting antiviral (DAA) therapy for hepatitis C virus (HCV) as a touchstone example of how medical innovation breeds ethical dilemmas. A few quandaries-informed consent as well as informed deferral during the first wave of DAA approvals, sobriety restrictions from payors, and high DAA costs for patients-are addressed through the lens of the foundational principles of clinical medical ethics: autonomy, beneficence, non-maleficence, justice, and utility. By placing the

Kyle van Oosterum and Emma Curran have recently argued that the impairment argument against abortion is weak and accomplishes little. They also claim that impairment fails to explain what makes giving a child fetal alcohol syndrome (FAS) immoral, which is an important premise of the argument. Here, I explain that the impairment argument is not as weak as they believe. Further, I argue that impairment offers a superior explanation for what makes giving a child FAS immoral than their proposal based on creative beneficence.

Hispanic and Latino farmworkers are at risk for negative occupational health and safety outcomes due to issues such as their extreme work conditions, their reliance on employer beneficence, and cultural barriers. The purpose of this article is to explain the unique characteristics of heat-related illness in the Hispanic agricultural work force and to provide an overview of the problems of poor hydration and heat exposure in this population. Culturally appropriate preventive strategies are discussed because industrial-type solutions may not work in a crop production environment where language a

Increasing numbers of children and young adults are now receiving home ventilation. Whereas in some neuromuscular conditions the outcome and quality of life gains are clear cut; in others eg Spinal Muscular Atrophy Type I there are a few outcome studies with conflicting results and it is more difficult to balance the ethical concepts of beneficence, non-maleficence, autonomy and distributive justice. As a result there are widespread variations in the clinical management of these children. This article examines decision-making in these areas seen from the perspective of the individual, the fami

OBJECTIVE: Pediatric palliative care aims to provide a professional, scientific, and compassionate response to the needs of terminally ill pediatric patients, with pain management being one of its fundamental pillars. This systematic review analyzes the ethical aspects involved in the management of pain in pediatric patients at the end of life, using the core principles of clinical bioethics: autonomy, beneficence, non-maleficence, and justice as a framework.

OBJECTIVE: To examine the evidence and practice of antipsychotic dose reduction from the lens of biomedical ethics (specifically principlism) to support evidence-based practice and patient choice and self-determination.
METHODS: An overview of the evidence from randomized controlled trials of antipsychotic dose reduction versus maintenance is presented. This is followed by a theoretical examination of the four key biomedical ethical principles of autonomy, nonmaleficence, beneficence, and justice and how they apply in the case of antipsychotic dose reduction.
RESULTS: Existing clinical trial

Very small premature babies are now kept in neonatal intensive care units for many months to give them a chance at near normal life, but at a cost of up to dollars 1 million. Is that a proper allocation of our finite medical resources (personnel, equipment, supplies, and other necessities) for premies? Or is such action a misallocation of these resources? Both sides of the argument have positive and negative merit. A final philosophical statement is that each human created in the image of God has supreme value. It is it the obligation of fellow humans and society in general to preserve and dig

BACKGROUND: While the number of countries that have introduced or are trying to introduce statutory laws regulating the procedure of assisted dying is increasing, research has yet to address why some legislative attempts succeed and others fail. In Germany, which set out to regulate assisted suicide with an autonomy-based approach, two draft bills were unable to secure a parliamentary majority. This paper sets out to explain the underlying value disputes that contributed to the legislative failure.
METHODS: We conducted an interpretative analysis of 108 speeches and 18 declarations to the vot

Electroconvulsive therapy (ECT) remains among the most effective interventions in psychiatry for severe depression, catatonia, and treatment-resistant psychosis. Despite its well-established efficacy and safety profile, particularly in medically complex and older patients, its use continues to evoke controversy due to the required medical clearance. Ethical dilemmas arise when patients lack insight or capacity to consent, creating tension between respect for autonomy and the duty of beneficence. We present the case of a 62-year-old woman with a longstanding history of paranoid schizophrenia, g

This study was done to evaluate the long-term patient's satisfaction after carpal tunnel syndrome (CTS) electrodiagnostic done between 1989 and 1994 (5 to 10 years follow-up). Mail contact was made to 528 consecutive cases with a questionnaire to be filled; 165 patients responded after 19 exclusions. CTS severity was graded from 0 (incipient) to 4 (severe) after a combination of median sensory distal latency, sensory median-radial latency difference and amplitude of the median compound muscle action potential. Current symptoms ("cure", improved, unchanged or worsed) and the therapy utilized, e
