Congenital bicuspid aortic valve, one of the most common congenital heart abnormalities, may become infected, may calcify, and may cause progressive stenosis or progressive insufficiency. While no previous reports suggested cerebral emboli in the absence of valvular infection, we studied four such cases. In all, cerebral angiography was normal and no extracardiac source of emboli was demonstrated. Microthrombus formation and valvular thickening with incompetence could eventuate in embolization.

Bicuspid aortic valve (BAV) is the most common congenital heart anomaly, affecting an estimated 0.5% to 0.77% of the general population. This condition occurs when the aortic valve has only two cusps instead of the usual three, disrupting normal valve function and increasing the risk of various cardiovascular diseases. Often asymptomatic in its early stages, BAV can gradually progress, leading to stenosis, valve insufficiency, and abnormalities of the ascending aorta. One particularly concerning aspect is its potential association with sudden cardiac death (SCD). The aim of this literature rev

Transcatheter aortic valve implantation (TAVI) has become an established treatment option for symptomatic patients suffering from aortic stenosis. A bicuspid aortic valve (BAV) is the most frequent congenital valvular abnormality. With the expansion of the indications for TAVI to patients at lower risk, including younger populations, a BAV is expected to be more frequently encountered. Several challenges are associated with BAVs, from diagnosis and classification to interventional or surgical treatment. Transcatheter prostheses, designed to treat tricuspid aortic valves, have shown promising r

This video tutorial demonstrates the surgical repair of a bicuspid aortic valve in a 45-year-old male patient with severe aortic regurgitation due to annular dilatation and prolapse of the fused cusp. The procedure included external annuloplasty to stabilize and downsize the annulus, and cusp plication to restore effective height and durable coaptation. The repair resulted in a competent native valve with preserved geometry. It highlights reconstructive strategies as a durable alternative to valve replacement.

PURPOSE OF REVIEW: To synthesize and critically assess recent clinical and research advancements in pediatric bicuspid aortic valve (BAV) and its associated aortopathy.
RECENT FINDINGS: In pediatric patients with BAV, progressive aortic dilation (i.e. bicuspid aortopathy) is commonly present and associated with increased risk for aortic aneurysm, dissection, and surgery in adulthood. Ongoing research explores the cause, incidence, and progression of bicuspid aortopathy to promote earlier diagnosis and improve preventive management. Recent findings include: high familial incidence and need for

INTRODUCTION: Bicuspid aortic valve (BAV) is a common congenital valve abnormality. There are no data in the literature regarding the range of aortic valve area (AVA) in normal functioning BAV. We aimed to evaluate the normal range of BAV area and to compare it to subjects with tricuspid aortic valve (TAV).
METHODS: Bicuspid aortic valve subjects were identified from Sheba medical center echocardiographic database and were compared with TAV subjects. Inclusion criteria were normal tissue leaflets appearance and normal functioning valve in the presence of normal echocardiogram. Echocardiograph

Bicuspid aortic valve (BAV) is the most common valvular congenital heart defect in the general population. BAV is commonly associated with the presence of other congenital cardiovascular malformations, which leads to cardiovascular complications requiring surgery in around 27% of cases. Familial clustering of BAV is well-recognized, and international guidelines advocate that first-degree relatives of patients with BAV be screened. Studies of genetic linkage in affected families, syndromic forms of BAV, and sporadic patients led to discoveries of genetic loci harboring genes involved in the dev

BACKGROUND: Bicuspid aortic valves (BAV) are associated with incompletely characterized aortopathy. Our objectives were to identify distinct patterns of aortopathy using machine-learning methods and characterize their association with valve morphology and patient characteristics.
METHODS: We analyzed preoperative 3-dimensional computed tomography reconstructions for 656 patients with BAV undergoing ascending aorta surgery between January 2002 and January 2014. Unsupervised partitioning around medoids was used to cluster aortic dimensions. Group differences were identified using polytomous ran

Bicuspid aortic valve (BAV) is the most common congenital valvular defect and is associated with ascending aortic dilation (AAD) in a quarter of patients. AAD has been ascribed both to the hemodynamic consequences of normally functioning and abnormal BAV morphology, and to the effect of rare and common genetic variation upon function of the ascending aortic media. AAD manifests in two overall and sometimes overlapping phenotypes: that of aortic root aneurysm, similar to the AAD of Marfan syndrome; and that of tubular AAD, similar to the AAD seen with tricuspid aortic valves (TAVs). These aorti

Transcatheter aortic valve implantation (TAVI) is an established therapeutic option for high-risk patients with tricuspid aortic valve stenosis. Historically, the presence of a bicuspid aortic valve (BAV) has been regarded as a contraindication to TAVI, on the basis of putative concerns about the associated risks of elliptical deployment, accelerated leaflet degeneration, periprosthetic leaks, and aortic complications. Fortunately, with technological refinements and mounting experience, reasonable success has been achieved with TAVI in selected patients with a BAV. The rate of procedural succe

Bicuspid aortic valve (BAV) is the most common congenital heart defect, affecting 1-2% of the population. It is generally diagnosed late in adulthood when deterioration of the abnormal leaflet becomes clinically evident. BAV patients have an increased risk of developing serious complications, including stenosis, regurgitation, endocarditis, dilation of the aorta, aortic dissection, and aneurysm. BAV is a heritable trait, but the genetic basis underlying this cardiac malformation remains poorly understood. In the last decade, thanks to studies in animal models as well as genetic and biochemical

Bicuspid aortic valve (BAV) is the most common congenital cardiac malformation, affecting 1-2% of the general population. BAV is frequently associated with significant aortic pathology (valve regurgitation, endocarditis, severe aortic stenosis, aortic aneurysm and dissection) accounting for considerable morbidity and mortality. Despite its clinical relevance, the pathogenesis of BAV disease and its complications remain largely unknown. The purpose of this paper is to discuss and summarize our current understanding of BAV in terms of epidemiology, genetics and clinics.

Bicuspid aortic valve is a disease with autosomal dominant inheritance with small penetration. The population of patients with bicuspid aortic valve is heterogenous. In the absence of dilatation of ascending aorta, valvular lesion or degenerative changes of valve leaflets patients are considered to belong to the low risk population. On the other hand patients with the above mentioned characteristics compose a high risk group. Dilatation of the ascending aorta in patients with bicuspid aortic valve has a progressive feature and continues even after replacement of the aortic valve. With progress

Bicuspid aortic valve (BAV) is the most common form of congenital heart disease, with frequent and premature occurrence of cardiac events, dominated by significant valvular dysfunction. BAV has a high prevalence of aortic wall abnormalities such as ascending aortic dilatation. Because more rapid aortic dilatation can occur, once the ascending aorta reaches 40 mm, annual imaging with echocardiography or other imaging techniques is indicated. The most feared complication is aortic dissection. However, the actual incidence of this complication is low (4%). Although limited data exist regarding pr

PURPOSE OF REVIEW: The surgical management of the bicuspid-aortic valve along with associated aortic disease remains a challenge. On the basis of recent literature as well as on our own experience, we offer several recommendations for the most appropriate surgical management of the patients with bicuspid-aortic valve disease.
RECENT FINDINGS: In 2006, the American College of Cardiology and the American Heart Association published guidelines in order to define the standard of care for the management of patients with bicuspid-aortic valve; however, these are consensus-based, mainly from observa

We report the case of a 36-year-old man with calcified bicuspid aorta. Aortic disease was diagnosed after retinal embolism, which caused loss of vision affecting the left eye. Doppler examination did not identify carotid stenosis. Transthoracic echocardiography showed thickness and calcification of two leaflets, moderate aortic regurgitation, high normal left ventricular diastolic dimensions, and high normal interventricular septum thickness. Parasternal long-axis view in the systolic frame showed dilated left ventricular outflow tract, dilated ascending aorta, and calcification and thickening

BACKGROUND: Congenital heart disease (CHD) is a significant health concern affecting approximately 1% of live births. Among these anomalies, bicuspid aortic valve (BAV) is the most prevalent, while bicuspid pulmonary valve (BPV) remains exceptionally rare. This case report presents a unique instance of a 10-year-old girl diagnosed with the combination of BAV and BPV alongside a ventricular septal defect (VSD) and infundibular stenosis, referred to as the Gasul phenomenon.
CASE PRESENTATION: The patient, initially identified with a heart murmur during infancy, exhibited dyspnea classified as N

BACKGROUND: Patients with bicuspid aortic valves (BAVs) are at increased risk of infective endocarditis. Coxiella burnetii is a rare cause of culture-negative infective endocarditis.
CASE SUMMARY: A young male with BAV presented with 3 months of fevers and abdominal distension. Initial workup showed pancytopenia and massive splenomegaly. Blood cultures were negative. Echocardiography revealed endocarditis of the bicuspid valve, aortic regurgitation, and aortic pseudoaneurysm. He underwent annulus debridement, valve replacement, and concurrent splenectomy. Serologic and tissue studies revealed
