Intraductal papillary neoplasm of bile duct (IPNB) is one of the precursor lesions of cholangiocarcinoma. Although hepatolithiasis has been extensively studied in its association with IPNBs, there had been no comprehensive study of IPNBs with Clonorchis sinensis infection. Twelve IPNBs were selected from 20 surgically resected cholangiocarcinomas, positive for C. sinensis tests (60%) and compared with eight IPNBs, selected from 51 resected cholangiocarcinomas, negative for C. sinensis tests (16%), by histologic and immunohistochemical studies of mucin core proteins and cytokeratin panels. Endoscopic retrograde cholangiopancreatography or percutaneous techniques are performed for bile drainage and removing bile duct stones. Nevertheless, surgical stone removal can be performed in cases where cholelithiasis with concomitant choledocholithiasis is checked. While forming an anastomosis for bile drainage, the anatomical structure may change from its normal state. Choledochoduodenostomy is one classical and commonly used method for bile drainage anastomosis. The data on long-term complications, including malignancy, associated with this method are limited. Some reports suggest that c

BACKGROUND: Intraductal papillary mucinous neoplasm (IPMN) and intraductal papillary neoplasm of the bile duct (IPNB) are mucinous cystic tumors with intraductal papillary growth and malignant potential. Their concurrent occurrence is exceptionally rare.
CASE SUMMARY: A 58-year-old Chinese man presented with recurrent upper abdominal pain. Imaging and laboratory tests revealed lesions consistent with IPNB and IPMN. Postoperative pathological examination confirmed IPNB with high-grade dysplasia and main-duct type IPMN with low-grade dysplasia. The patient underwent extrahepatic bile duct resec

Invasive biliary tract carcinomas are usually tubular adenocaricnomas with abundant desmoplastic reactions and frequent ductal and periductal invasion at the time of the diagnosis. Recently, several intraductal neoplasms of the bile duct, particularly at a pre-invasive stage, have been recognized. They include intraductal papillary neoplasm of the bile duct (IPNB), biliary intraepithelial neoplasm (BilIN), and others, such as intraductal tubulopapillary neoplasm (ITPN) of the bile duct. IPNBs are grossly visible predominantly intraductal-growing papillary neoplasms covered by well-differentiat

BACKGROUND: Activating point mutations of GNAS at codon 201 have been detected in approximately two thirds of intraductal papillary mucinous neoplasms (IPMNs) of the pancreas. Intraductal papillary neoplasms of the bile ducts (IPNBs) morphologically resemble pancreatic IPMNs. This study sought to assess the mutational status of GNAS at codon 201 in IPNBs.
METHODS: Thirty-four patients were included. DNA from microdissected IPNBs was subjected to a polymerase chain reaction and ligation method for the detection of GNAS mutations at codon 201 and of KRAS mutations at codon 12. Mutational status

OBJECTIVES: To identify imaging features that assist in discriminating intraductal papillary neoplasms of the bile duct (IPNBs) from papillary cholangiocarcinomas (PCCs).
METHODS: This study was approved by the institutional review board. Using the recently proposed histological diagnostic criteria for biliary papillary neoplasms, IPNBs and PCCs were selected from 537 biliary neoplasms consecutively resected in a 12.5-year period. Clinical and imaging features were compared between the two groups.
RESULTS: The histology review identified 19 IPNBs and 48 PCCs, representing an estimated preval

Intraductal papillary neoplasms of the bile duct (IPNBs) have a high propensity for malignant transformation; therefore, surgical resection remains the standard first-line treatment. In cases where hepatic lobectomy is not feasible, however, controlling biliary drainage can be challenging. We present herein a case of unresectable IPNB complicated by jaundice and cholangitis due to mucin production by the tumor. Stereotactic body radiation therapy (SBRT) has been used as a treatment modality, resulting in favorable outcomes. An 80-year-old male patient presented to the emergency room with sympt

Objective: To explore the magnetic resonance imaging (MRI) features and classification of intraductal papillary neoplasm of the bile duct (IPNB). Methods: Data from 90 patients with intraductal papillary neoplasm of the bile duct confirmed pathologically between June 2010 and January 2023 were retrospectively analyzed. The image analysis included the shape and location of the tumor, whether bile ducts had dilatation and the degree of dilation, whether there was a history of liver disease, whether there was a history of schistosomiasis, whether there was cancerous transformation, whether there

Case 1: A man in his 70s was referred to our hospital for further examination of a liver tumor(S3, 3 cm)detected by ultrasonography. Multimodal image examination showed a cystic lesion with solid papillary components located in the S4 accompanied by dilatation of the surrounding intrahepatic bile duct. Although biliary cytology did not indicate confirmed malignancy, the lesion was thought to be an intraductal papillary neoplasm of bile duct(IPNB)with malignant potential, and a left lobectomy was performed. Histopathological examination revealed a papillary tumor in the intrahepatic bile duct w

Intraductal papillary neoplasm of the bile duct is a rare bile duct tumor that displays a range of cytoarchitectural atypia and is recognized as a precursor of invasive carcinoma. We present a 71-year-old woman with a recent diagnosis of lung adenocarcinoma, who presented with acute cholangitis secondary to an obstructive intraductal papillary neoplasm of the bile duct. The patient underwent endoscopic retrograde cholangiopancreatography, which identified the lesion, and on biliary sweep, the polyp presented externally and she underwent successful polypectomy with resolution of the infection.

We present the first case of an intraductal papillary neoplasm of the bile duct (IPNB) accompanying a mixed adenoneuroendocrine carcinoma (MANEC). A 74-year-old woman presented with fever of unknown cause. Laboratory data revealed jaundice and liver injury. Contrast-enhanced computed tomography revealed a 20 mm polypoid tumor in the dilated distal bile duct, which exhibited early enhancement and papillary growth. Upper gastrointestinal endoscopy revealed mucus production from the papilla of Vater, characterized by its protruding and dilated orifice. Endoscopic ultrasonography visualized the po

We study the clinic, surgical pathologic aspects of 81 patients with cancer of the gallbladder and extrahepatic bile ducts, dividing them in 4 groups by localization. The majority of cases were women, the average in age was 57.05 years with the greatest incidence between the 5th and 6th decade of life. We can conclude that the symptoms and signs are not useful as well as the laboratory and radiologic test. Adenocarcinoma was the most common type of cancer and the outcome of the patients was very poor because cancer has grown beyond the limits of resectability before it is clinically apparent.

No changes in prognosis of extrahepatic bile ducts malignant tumors was observed in the last years, in spite of modern tomographic procedures: only occasionally an early diagnosis resulting possible. Jaundice is the presentation sign of the disease, allowing in most cases when the site of tumor is the papilla of Vater or distal common bile duct a radical surgery but only palliative procedures when the tumor arises in the gallbladder or at the hepatic duct junction. Survival data from literature and personal experience indicate limited late results for radical surgery together with high operato

Intraductal papillary mucinous neoplasm of the bile duct (IPMN-B) belongs to a group of biliary disorders that are premalignant lesions and have a relatively low incidence. We describe a case of a patient with IPMN-B who presented unusually with cholangitis and underwent surgical resection as a definitive treatment. The patient was a 65-year-old male who presented with a six-month history of recurrent right upper abdominal pain and loss of appetite, with a recent history of jaundice. Blood investigations showed leukocytosis with altered liver function test, with elevated bilirubin and liver en

Intraductal papillary neoplasm of the bile duct (IPNB) is a heterogeneous disease similar to intraductal papillary mucinous neoplasm of the pancreas. These lesions have been recognized as one of the three major precancerous lesions in the biliary tract since 2010. In 2018, Japanese and Korean pathologists reached a consensus, classifying IPNBs into type l and type 2 IPNBs. IPNBs are more prevalent in male patients in East Asia and are closely related to diseases such as cholelithiasis and schistosomiasis. From a molecular genetic perspective, IPNBs exhibit early genetic variations, and differe

BACKGROUND: Intraductal papillary neoplasm of the bile duct (IPNB) is considered a pre-cancerous biliary lesion and/or an early cancer lesion, although its classification remains unclear. The 2019 revised edition of the World Health Organization Classification of Tumors of the Digestive System proposed type 1 and type 2 as new classification categories, and meta-analyses and/or multi-center cohort studies are beginning to be reported. However, treatment for IPNB recurrence and metastasis remains unclear.

Intraductal papillary neoplasm of the bile duct (IPNB) is a rare entity characterized by papillary growth within the bile duct lumen1, currently regarded as a biliary counterpart of intraductal papillary mucinous neoplasm of the pancreas. The most common clinical findings are abdominal pain, jaundice or cholangitis, although some patients are asyntomatic. The diagnosis requires careful assessment of imaging findings and cito-histological evaluation, with endoscopic ultrasound guided fine needle aspiration (EUS-FNA) playing a very important role. IPNB is a premalignant lesion which can evolve i

PURPOSE: We propose a morphological classification of intraductal papillary neoplasm of the bile duct (IPN-B).
METHODS: A retrospective analysis of 16 patients with IPN-B who had undergone surgical resection was conducted. These 16 cases were classified into three types based on the primary lesion's anatomical location: branch duct type (6 cases), main duct type (5 cases) and mixed type (5 cases). In this paper we have analyzed the characteristics of IPN-B according to our new classification.
RESULTS: All branch duct type IPN-B was located in the left lobe and 5 of them were resected by left

CONTEXT: Intraductal papillary mucinous neoplasm (IPMN) of the pancreas occasionally penetrates to others organs. We present a case of IPMN penetrating to the stomach and the common bile duct.
CASE REPORT: A 75-year-old man was admitted to the hospital because of epigastric pain. Computed tomography (CT) showed a papillary tumor protruding into the markedly dilated main pancreatic duct and splenic vein obstruction. The tumor was diagnosed as IPMN arising in the main duct, but he rejected surgery and he was followed without treatment. One year later, gastroduodenoscopy revealed gastropancreati
