[A case of blast injury of the brain].

Cases of craniocerebral injuries associated with foreign body penetration into the cranial cavity are not frequent in peacetime. The reported case was that of shot wound of the head caused by a steel pin used in construction works. The foreign body penetrated into the cranial cavity through the right eyeball and across the brain to the occipital lobe. The foreign body was not removed, and after 34 days in hospital the patient was discharged home with only a slight neurological deficit, i.e. low-grade paresis of his left foot. No remote complications were observed during 4 years of follow-up.

[An experimental study of blast injury].

This paper presents some aspects of the authors' experimental research on blast injury in the past two years. The main results are as follows: (1) A new designed 39 meter-long shock tube for biological test has been built in the laboratory. Its maximal overpressure values are 215 kPa (in open condition) and 505 kPa (in closed condition). It may meet the need for inflicting blast injuries with various degree of severity. (2) A study of the effect of simulating gun muzzle blast wave on sheep indicated that in the single explosion, the threshold overpressure values inflicting the injury of intern

Blast injury: Impact to the cornea.

Visual disorders are common even after mild traumatic brain injury (mTBI) or blast exposure. The cost of blast-induced vision loss in civilians, military personnel, and veterans is significant. The visual consequences of blasts associated with TBI are elusive. Active military personnel and veterans report various ocular pathologies including corneal disorders post-combat blasts. The wars and conflicts in Afghanistan, Iraq, Syria, and Ukraine have significantly increased the number of corneal and other ocular disorders among military personnel and veterans. Binocular vision, visual fields, and

Primary blast injury: an intact tympanic membrane does not indicate the lack of a pulmonary blast injury.

The tympanic membrane (TM) has long been viewed as an indicator of primary blast injury. A primary blast injury occurs due overpressure occurring as a result of the detonation of high explosives. Cadaver studies indicated pressure required for perforation of the tympanic membrane to be 137 kPa for adults. The accepted range in which other organs (lung, colon, and intestines) are damaged by the pressure wave emanating from an explosion is in the 400-kPa range. The use of the perforation of the tympanic membrane as an indicator of a primary blast injury missed a range of up to 50% of those suffe

Blast From the Past: A Retrospective Analysis of Blast-induced Head Injury.

Because of the sharp increase in the number of military personnel exposed to explosive blasts in combat, research has been dedicated toward understanding the impact of explosions on the brain. It is important to consider that potential injuries that military personnel sustain may be both in the form of physical injury as well as "invisible" neuronal and psychological damage. Since the inception of the study of blast science in the Medieval and Renaissance eras, significant improvements have been made in the historical record keeping and biomedical analysis of blast injuries. Blast injuries: The experience of a level 1 trauma center.

BACKGROUND: There is a global surge in blast injuries, which are associated with high morbidity and mortality. To our knowledge, there are no guidelines for the management of blast injuries in the trauma bay. METHODS: This single-center retrospective cohort study utilized data on all patients admitted to our emergency department (ED) with terror- or combat-related injuries between October 7, 2023 (Gaza Iron Swords War onset) and February 4, 2024. The primary outcome was trauma severity indicated by either an injury severity score (ISS) >15 and/or need for acute care. We also analyzed the mech

Ophthalmic injuries from the beirut blast: Managing long-term consequences.

The 2020 Beirut Port explosion was one of the largest non-nuclear urban explosions in history, and resulted in a plethora of oculofacial injuries. In this retrospective study, we present the two year follow up ophthalmic outcomes of the survivors of the blast. Only 16 out of 39 patients continued follow up at our center, with 13 having delayed complications and 7 requiring further surgery. The most common delayed complications related to the eyelid, lacrimal system, and orbit. Treatment of disfiguring facial and peri-ocular scarring with laser-assisted drug delivery of topical 5-fluorouracil s

Fatal head and neck injuries in military underbody blast casualties.

INTRODUCTION: Death as a consequence of underbody blast (UBB) can most commonly be attributed to central nervous system injury. UBB may be considered a form of tertiary blast injury but is at a higher rate and somewhat more predictable than injury caused by more classical forms of tertiary injury. Recent studies have focused on the transmission of axial load through the cervical spine with clinically relevant injury caused by resultant compression and flexion. This paper seeks to clarify the pattern of head and neck injuries in fatal UBB incidents using a pragmatic anatomical classification.

Blast injuries to the lungs: clinical presentation, management and course.

Five patients with blast injuries to the lungs after bomb explosions are reported. In each patient radiological changes were apparent on the initial chest film taken within 4 hours of the explosions. Arterial hypoxaemia was also present. Four patients were actively treated with continuous positive-pressure ventilation, which was adjudged effective therapy. Two patients died, one owing to bilateral pneumothorax which occurred during anaesthesia, and the other owing to overwhelming infection. Hypoxaemia persisted for 4 months in one of the survivors. Lung function tests which were performed on t

Reconstruction of firearm and blast injuries in Syrian war refugees.

BACKGROUND: War injuries differ from other injuries owing to the large tissue defects they cause and their high risk of contamination. As fragments scattered by high-energy firearms and explosives cause serious composite tissue damage, repair of such injuries is difficult and requires a long treatment period. We discuss the treatment methods used for injured Syrian War refugees admitted to our clinic and present the most effective repair methods for war-related tissue defects for each region of the body. METHODS: A total of 61 patients treated between June 2012 and April 2015 were retrospecti

Burn injuries from the london suicide bombings: a new classification of blast-related thermal injuries.

In July 2005, four suicide bombers detonated explosive improvised high explosive devices (IEDs) in three separate underground carriages and a double decker bus in London, resulting in 56 deaths and 775 injured. This study aims to understand the mechanisms and patterns of burn injuries from high explosives, and the related factors that determine mortality. The types and patterns of burn injuries in survivors and fatalities in the confined underground train carriages and the bus were analysed, evaluating injury severity score and the victims' relative position from the detonation point. The data

[Are the injuries caused by the "water bombs" dropped by the Canadair airplane blast injuries?].

During the forest fire extinguishing in summer using the fire-fighting amphibia (the Canadair) seven soldiers were injured by the "water bomb" dropped from the amphibia and two soldiers died. The way of injury occurrence as well as type and nature of injuries, imposed the question to the author: could it be a case of blast injuries, especially of primary blast injuries? Except for pure scientific reasons, a positive answer could have a practical importance both in regulation of work of persons engaged in fire combat on the ground during extinguishing of fire by the amphibia as well as for phys
