Suicidal ideation and sobriety: Should acute alcohol intoxication be taken into account for psychiatric evaluation?

BACKGROUND: When evaluating an emergency department (ED) patient who presents with suicidal ideation, it is a common practice to wait until the patient's ethanol level is known or calculated to be less than 80 mg/dl to evaluate patient safety. We know of no study that establishes an association between the degree of alcohol intoxication based on a blood alcohol level (BAL) and reported suicidal ideation (SI) upon recovery. METHODS: We conducted a retrospective review of patients evaluated in a Midwestern ED for the calendar year 2017. Cases were selected if they had a psychiatric social work

Perceived Immune Fitness, Individual Strength and Hangover Severity.

Various factors may contribute to alcohol hangover severity. The purpose of the current investigation was to evaluate the possible impact of alcohol consumption patterns, perceived immune status, and baseline fatigue on hangover severity. A survey was completed by a convenience sample of N = 199 Dutch students who reported on their latest past month's heavy drinking occasion, including subjective intoxication (perceived drunkenness) and next-day hangover severity, which were rated on single-item scales ranging from 0 (absent) to 10 (extreme). In addition, perceived (momentary) immune fitness w

A new alcohol-related traffic law, a further reduction in traffic fatalities? Analyzing the case of Turkey.

BACKGROUND: In June 2013, an alcohol-related traffic law took effect in Turkey. The law 6487 introduced administrative fines for not respecting blood alcohol concentration limits, health warning messages on alcohol containers (bottles, cans), and prohibited the sale of alcohol beverages in retail facilities between 10 p.m. and 6 a.m.. This article examines how this law is associated with traffic fatality variation. METHODS: Data from the Turkish Statistical Institute for the 2008-2019 period were analyzed. Outcomes were traffic fatality rates per 100,000 population and 10,000 motor vehicles.

The 'Grand Rapids dip': could it be real?

The "Grand Rapids dip" refers to a finding in the Grand Rapids study of drinking-and-driving, indicating that drivers with a low blood alcohol concentration had a lower risk of accident involvement than drivers with a zero-blood alcohol concentration. In the Grand Rapids study, it was found that this was a statistical artefact only and did not indicate a real decline in accident risk. This paper asks if a decline in accident risk at a low blood alcohol concentration could be real. It is concluded that it could but is likely to be uncommon. It may occur if: (1) There is a comparatively large a

Investigating the use of PEth, CDT and MCV to evaluate alcohol consumption in a cohort of homeless individuals- A comparison of different alcohol biomarkers.

In a cohort including individuals with suspected high alcohol consumption, the concentrations of the indirect alcohol biomarkers carbohydrate-deficient transferrin (CDT) and mean corpuscular volume (MCV) and the direct alcohol biomarker phosphatidylethanol (PEth) were investigated. Blood alcohol concentration (BAC) was analysed as a marker for acute alcohol ingestion. In addition to questions about subjective alcohol consumption behaviour, 147 homeless persons underwent a physical examination with blood sampling. BAC, PEth, CDT and MCV were determined in the blood samples. Special focus was on

Effect of acute alcohol consumption on blunt bowel mesenteric injury: a retrospective analysis.

BACKGROUND: The effect of alcohol consumption on trauma remains controversial. The effects of alcohol on hemorrhage and peritonitis after blunt abdominal trauma have rarely been discussed. This study aimed to explore the effects of acute alcohol intoxication on the clinical characteristics, injury patterns, and outcomes in a surgical blunt bowel mesenteric injury (BBMI) cohort. METHODS: A retrospective data analysis was performed using trauma cases of patients who had been tested for alcohol and had surgically proven BBMI from a Trauma Registry System from 2009 to 2021. Patients were grouped

Association of cerebellar and pre-motor cortex gray matter density with subjective intoxication and subjective response following acute alcohol intake.

Acute alcohol intake produces subjective intoxication (SI) and response (SR; e.g., valanced stimulation and sedation), which has important implications for alcohol-related risk. Individuals who experience less SI may be more likely to engage in risky behaviors while drinking. Gray matter morphometry in brain regions underlying cognitive and affective processes may help to inform individual differences in subjective intoxication and response. The subjective effects of alcohol vary between limbs of the blood alcohol concentration (BAC) curve (i.e., whether BAC is rising or falling; acute toleran

The impact of alcohol consumption on commercial eye blink drowsiness detection technology.

INTRODUCTION: Driver drowsiness detection technology that assesses eye blinks is increasingly being used as a safety intervention in the transport industry. It is unclear how alcohol consumption to common legal driving limits impacts upon this technology. The aim of the study was to assess the impact of a blood alcohol content (BAC) of 0.05% and of 0.08% on drowsiness detection technology during simulated driving. METHODS: Participants completed a 60-min driving simulation and sleepiness questionnaire under three conditions: 1-0.00% BAC, 2-0.05% BAC and 3-0.08% BAC. During the driving simulat

Use of mindfulness training to improve BAC self-estimation during a drinking episode.

OBJECTIVE: Individuals are often inaccurate at estimating levels of intoxication following doses of alcohol. Previous research has shown that when required to estimate (BAC) at different time points, participants often underestimate their BACs and amounts of alcohol consumed. The present study aimed to increase drinkers' BAC estimation accuracy after drinking alcohol using mindfulness-based feedback to increase their awareness of the interoceptive cues associated with alcohol intoxication. METHOD: Thirty-three adults were given 0.65 g/kg of alcohol and received one of three training condition

Statistical comparisons of 0.08 g/100 ml fortified blood alcohol samples from 6-ml and 10-ml gray-top tubes.

This experiment supplements the study, "Statistical comparisons of blood alcohol samples from 6-ml and 10-ml grey-top tubes". The initial study analyzed fortified samples for blood alcohol concentration (BAC) using two sizes of gray-top tubes: a 10-ml tube containing a nominal 1% sodium fluoride (NaF), a preservative, and a 6-ml tube containing 0.25% NaF, using the variables of time, storage temperature, fill volume, and concentration. From that study, paired t-tests determined no difference in BAC from data sets of the two tube sizes, however, Analysis of Variance, or ANOVA, testing determine

Comparisons of blood alcohol concentrations between initial testing and reanalysis from unopened tubes preserved with 0.25% NaF following refrigerated storage up to 3.93 years.

Few studies support the usage of <1% nominal sodium fluoride (NaF) to preserve ethanol in antemortem blood. Of these studies, several are limited by short study durations of 90 days or less, and there is limited research of authentic samples preserved with <1% NaF. In this retrospective analysis, data from initial and reanalysis testing of blood alcohol concentration (BAC) in antemortem blood collected in 6 mL gray-top tubes (providing 0.25% nominal NaF) has been compiled, to evaluate changes in ethanol concentration that occurred after periods of refrigerated storage. The time between initial

Emerging issues in the approach to submerged bodies: Water-related deaths in Friuli, northeastern Italy (1993-2020). Analysis of post-mortem reports and some unusual cases.

When analysing bodies recovered from a watery environment, all manners of death, a variety of causes of death and the possibility of body disposal have to be considered. Unclear circumstances of death, unidentified victims and body decomposition may hamper a forensic investigation. The lack of specific and sensitive drowning-related autopsy findings and post-mortem blood alcohol levels (blood alcohol concentrations (BACs)) may also lead to erroneous conclusions. We reviewed the autopsy records for all bodies recovered from water in Friuli, northeastern Italy, over a 28-year period, focusing on

Significant postmortem diffusion of ethanol: A case report.

In the realm of forensic pathology, ethanol is one of the most frequently encountered xenobiotics. The determination of ethanol concentration in blood after death is of great interest in forensic settings. It is important to be able to determine the level of intoxication of the deceased at the time of death, which is directly correlated to the ability to act prior to death, especially when a suicide is suspected. This estimation is not always easy to establish owing to various artifacts that are important to know for a proper ethanol blood level interpretation, among them postmortem (PM) diffu

The prevalence of alcohol and other drugs in fatal road crashes in Victoria, Australia.

BACKGROUND: Driving under the influence of drugs, including alcohol, is a globally recognised risk factor for road traffic crashes. While the prevalence of alcohol and other drugs in fatal road crashes has been examined in other countries, recent data investigating drug driving in fatal Australian crashes are limited. This study aimed to examine how the presence of alcohol and other drugs in fatal road trauma in Victoria has changed over time in different road users. METHODS: A population-based review of road trauma deaths was performed over the period of 01 July 2006 to 30 June 2016 in Victo

Ethanol Kinetics in the Human Brain Determined by Magnetic Resonance Spectroscopy.

In many parts of the world, ethanol is a widely consumed substance that displays its effect in the brain, the target organ for desired, but also negative impact. In a previous study, the ethanol concentrations were analyzed in different regions of the brain by magnetic resonance spectroscopy (MRS). In this study, the same method is used to demonstrate the kinetics of the ethanol concentration in the human brain after oral ethanol uptake. A drinking study was performed with 10 healthy participants. After the uptake of ethanol in a calculated amount leading to a plasma ethanol concentration of 0

Advanced eNose-Driven Pedestrian Tracking Pipeline for Intelligent Car Driver Assisting System: Preliminary Results.

From a biological point of view, alcohol human attentional impairment occurs before reaching a Blood Alcohol Content (BAC index) of 0.08% (0.05% under the Italian legislation), thus generating a significant impact on driving safety if the drinker subject is driving a car. Car drivers must keep a safe driving dynamic, having an unaltered physiological status while processing the surrounding information coming from the driving scenario (e.g., traffic signs, other vehicles and pedestrians). Specifically, the identification and tracking of pedestrians in the driving scene is a widely investigated

Tattoo visualization using cross-polarized lighting and infrared photography.

Identification of the deceased is a critical responsibility of the death investigation system. If visual identification is inconclusive, tattoos can provide secondary identification but may be difficult to visualize at various stages of decomposition. We describe the case of a 35-year-old male found submerged underwater by police after swimming at a pier. The decedent was last seen earlier that day. Signs of immersion including sodden hands, feet, and clothing, wet sand on the torso and legs, and heavy edematous legs were observed. The post-mortem blood alcohol concentration was 427 mg/100 mL;

Smartphone Screen Integrated Optical Breathalyzer.

One third of fatal car accidents and so many tragedies are due to alcohol abuse. These sad numbers could be mitigated if everyone had access to a breathalyzer anytime and anywhere. Having a breathalyzer built into a phone or wearable technology could be the way to get around reluctance to carry a separate device. With this goal, we propose an inexpensive breathalyzer that could be integrated in the screens of mobile devices. Our technology is based on the evaporation rate of the fog produced by the breath on the phone screen, which increases with increasing breath alcohol content. The device s
