The Impact of Lipoprotein apheresis on Plasma Protein Levels: A Meta-Analysis.

Lipoprotein apheresis (LA) is used for severe dyslipidemia but can remove non-lipid plasma constituents. In this systematic review and meta-analysis, we synthesized evidence on LA-associated changes in albumin, total protein (TP), and α2-macroglobulin (α2M). PubMed, Scopus, and Web of Science were searched from inception to August 2025 for human observational studies reporting pre-/post-apheresis values for albumin, TP, or α2M. Pooled effects were summarized as weighted mean differences (WMD) with 95% CIs. Prespecified subgroup analyses included apheresis duration (≥ 1 year vs. < 1 year), dise

[European regulation on blood and blood components].

The European regulation on blood and blood components is declined in four directives: the Directive 2002/98/EC known as "mother Directive" and three directives called "daughter Directives" 2004/33/EC, 2005/61/EC and 2005/62/EC. It constitutes a common basis of provisions of quality and safety of blood in the European Union (EU), thus guaranteeing this safety and this quality with the whole of the citizens circulating in Member States of the Union. It cannot prevent a Member State for maintaining or introducing more stringent protective measures. It encourages the anonymous, voluntary and unpai

Effective peripheral blood stem cell collection in a 4.6-kg child.

BACKGROUND: Due to unique technical challenges, effective peripheral blood stem cell collections (PBSCs) have not been consistently reported in patients weighing less than 5 kg. We describe three PBSCs performed in a 4.6-kg child undergoing myeloablative chemotherapy for high-grade glioma. STUDY DESIGN AND METHODS: A multidisciplinary group representing the clinical and apheresis teams adapted a PBSC protocol to accommodate the patient's size and collection targets. Special considerations included timing of the collection relative to chemotherapy, vascular access, strategies for monitoring ad

Logistics, risks, and benefits of automated red blood cell exchange for patients with sickle cell disease.

Red blood cell (RBC) transfusions treat and prevent severe complications of sickle cell disease (SCD) and can be delivered as a simple or exchange transfusion. During an exchange, some of the patient's abnormal hemoglobin (Hb) S (HbS) RBCs are removed. An apheresis device can accomplish an automated RBC exchange, simultaneously removing patient's RBCs while returning other blood components along with normal RBCs. Automated RBC exchange is therefore an isovolemic transfusion that can efficiently decrease HbS RBCs while limiting iron loading and hyperviscosity. However, specialized equipment, tr

When Strong Recommendations Rest on Weak Evidence: Lessons From Therapeutic Apheresis Guidelines.

The American Society for Apheresis (ASFA) guidelines serve as a global standard for therapeutic apheresis practice. However, our analysis of the 2023 guidelines reveals discordance between the strength of recommendation and the quality of evidence. Among 166 indications, one-third carry strong recommendations, yet only 8% are supported by high-quality evidence. Over half (55%) are informed by low- or very-low quality evidence. This mismatch is most pronounced for Category I indications, where apheresis is considered first-line therapy: nearly one-third are based on low-quality data, yet 89% re

The Impact of Lipoprotein Apheresis on Changes of Fibrinogen and aPTT: A Systematic Review and Meta-Analysis.

Lipoprotein apheresis (LA) may influence hemostasis, but reported effects are inconsistent. We quantitatively assessed changes in fibrinogen, plasminogen, activated partial thromboplastin time (aPTT) and thrombin time (TT). Following PRISMA, PubMed, Scopus, and Web of Science were searched to August 2025 for observational human studies reporting pre-/post-apheresis values. Weighted mean differences (WMD) with 95% confidence intervals (CIs) were pooled using random- or fixed-effects models according to heterogeneity. Peripheral arterial disease associated with elevated lipoprotein(a): a review of the evidence and treatment approaches.

PURPOSE OF REVIEW: Peripheral arterial disease (PAD) is an atherosclerotic and thrombotic disease associated with substantial morbidity and mortality. Although risk factors for PAD are mostly modifiable, prognosis remains poor, and patients are at a high risk of cardiovascular events. This review aims to summarize current evidence surrounding the role of lipoprotein(a) (Lp[a]) in PAD and examines the available data on lipoprotein apheresis as an effective management approach for patients with PAD with elevated Lp(a). RECENT FINDINGS: Evidence strongly indicates that elevated Lp(a) is a causal

The Impact of Lipid Apheresis on Changes of Plasma Ions: A Systematic Review and Meta-Analysis.

BACKGROUND: Lipoprotein apheresis reduces low-density lipoprotein (LDL) cholesterol in high-risk patients, but its effects on plasma ion levels remain unclear. The objective of this study was to systematically evaluate the impact of apheresis on plasma calcium, sodium, potassium, magnesium, and chloride concentrations. METHODS: Following PRISMA guidelines, we searched MEDLINE/PubMed, Scopus, and Web of Science to June 2025. Studies reporting pre- and post-apheresis ion levels with at least two sessions were included. Weighted mean differences (WMD) with 95% confidence intervals (CI) were calc

[Apheresis Techniques for the Treatment of Hyperbilirubinemia in the Nephrology Unit].

Therapeutic apheresis is an important hematological and nephrological method for conditions with altered plasma composition. It is also indicated for the removal of protein-bound molecules, such as bilirubin. Several techniques can remove these compounds, such as the extracorporeal circulation molecular adsorption system (MARS), plasma exchange (PEX), and plasma adsorption and perfusion (PAP). Here we report our experience in the comparison between MARS, PEX and PAP, since current guidelines do not specify which method is the most appropriate and under which circumstances it should be used. Apheresis as emerging treatment option in severe early onset preeclampsia.

Based on an early suggestion by Winkler et al. 2003 and a subsequent successful study by Wang et al. 2006 using lipid apheresis (LA) in 9 patients with preeclampsia to prolong pregnancies, the use of apheresis as therapeutic option in severe early onset preeclampsia has received increasing attention. Further studies using different LA systems also prolonged pregnancy and have been published in the last few years. Albeit using different LA systems and relying on different working hypothesis, all studies demonstrated a promising stabilisation against the disease's progression. Overall time from

[Apheresis therapy in Guillain-Barré syndrome].

Based on previous randomized controlled-trials (RCTs), plasma exchange (PE) has been established as an effective treatment for Guillain-Barré syndrome (GBS). Double filtration plasmapheresis (DFPP) and immunoadsorption plasmapheresis (IAPP) may be effective for GBS, however their effectiveness have not been established because any RCTs have not been available so far. Intravenous immunoglobulin (IVIG) has also been established to be effective for GBS according to several RCTs, however complications have been more frequent in PE than IVIG. Therefore IVIg treatment should be considered as the fir

Membrane processes for plasma separation and plasma fractionation: guiding principles for clinical use.

The use of membranes in blood processing range from the use of large-pore structures for filtration of blood for the removal of large particles (>20 microm transfusion filters) to membranes for the dialysis of blood for the removal of low molecular weight solutes in the treatment of renal failure. Within the past 20 years, membranes have been applied in the separation of plasma from whole blood. Compared to centrifugal plasma separation membrane, plasma separation is preferred when used with online plasma fractionation since the plasma is free of cells. In addition, membranes have also been ap

The Role of Therapeutic Apheresis in the Management of Rheumatoid Arthritis A Systematic Review and Meta-Analysis.

Rheumatoid arthritis (RA) cannot be cured in many patients even with improved drug therapies. Therapeutic apheresis has been explored as an alternative intervention for refractory RA, yet relevant clinical evidence is scattered and mostly derived from early studies. We systematically retrieved eligible studies from PubMed, Web of Science and the Cochrane Library up to December 12, 2024, enrolling randomized controlled trials (RCTs) and case series containing at least five patients that assessed all types of apheresis for RA treatment. A random-effects model was adopted to carry out meta-analys

The Italian registry of therapeutic apheresis in SISTRA: Year of activity 2022.

Therapeutic apheresis refers to a group of extracorporeal blood processing procedures used in the treatment of a variety of systemic diseases. These complex procedures are burdened by adverse reactions related to both procedures and underlying medical conditions. Given the importance of centralizing the collection and the analysis of information on therapeutic apheresis, the Italian National Blood Center (NBC), at the request of the Italian Scientific Society of Hemapheresis and Cell Manipulation (SIdEM), implemented the Italian Registry of Therapeutic Apheresis (IRTA) including it in the Info
