Improvement of positive blood culture detection by agitation.

To evaluate the advantages of agitation in reducing the detection time and increasing the recovery rate of positive blood cultures, 1,000 three-bottle sets of tryptic soy broth on adult inpatients were analyzed. Two bottles were transiently vented, one of which was agitated (250 rpm) for 7-19 hr at 35 degrees C. The other vented bottle and the anaerobic bottle were incubated stationary at 35 degrees C. Smears and subcultures were performed 7-19 hr after collection on both agitated and nonagitated vented bottles. Subcultures were done on all bottles at 72 hr and smears were performed on the ana

Likelihood of Blood Culture Positivity Using SeptiCyte RAPID.

Background: Early diagnosis and identification of causative pathogens using blood culture in patients suspected of blood stream infection (BSI) and sepsis are critical for improving patient outcomes through earlier and more targeted treatment. There is a need for tools that can guide the use of microbiologic diagnostics, especially where resources are limited, such as in lower- and middle-income countries (LMICs), pandemic and mass-casualty scenarios, and prolonged field care settings during military operations. Methods: Post hoc retrospective analysis of individual patient data from three pro

Neonatal blood culture inoculant volume: feasibility and challenges.

BACKGROUND: Clinicians often express concerns about poor sensitivity of blood cultures in neonates resulting from inadequate inoculant volumes. Our objective was to determine the inoculant volume sent for neonatal sepsis evaluations and identify areas of improvement. METHODS: Single-center prospective observational study of infants undergoing sepsis evaluation. Blood volume was determined by clinician documentation over 21 months, and additionally by weighing culture bottles during 12 months. Adequate volume was defined as ≥1 mL total inoculant per evaluation. For first-time evaluations, loca

Optimized blood culture strategy to document febrile neutropenia.

BACKGROUND: Poor and delayed microbiological documentation of episodes of febrile neutropenia (EFN) deserves improvement. We assessed the impact of a new blood culture (BC) sampling protocol to optimize the diagnosis of bloodstream infection during EFN, compared with standard of care protocol. METHODS: This pre/post intervention included patients who presented an EFN in a pediatric hematology-oncology center. Data were compared between 1-year periods P1 (110 EFN, 53 patients) and P2 (124 EFN, 53 patients). Pre-intervention settings were 1-2 mL of blood cultured per BC set and several sampling

Do Neonatal Infections Require a Positive Blood Culture?

Neonatal sepsis is a major cause of worldwide morbidity and mortality. Blood cultures are considered the gold standard for diagnosis, but results are often delayed for 24 to 48 hours, and sensitivity, although improved by modern techniques, such as automated blood cultures, is variable and affected by the bacterial load. For these reasons, empiric antibiotics are frequently administered to avoid potential devastating consequences of untreated sepsis. Unnecessary antibiotic treatment has been associated with increased mortality and other adverse outcomes; therefore, antibiotics should be discon

[Contribution and evolution of blood culture quality indicators].

The monitoring of quality indicators, combined with a detailed risk analysis, validates the process of automated blood culture. Here we report the methodology of 5 years monitoring for 5 indicators at the Biology Department of Foch Hospital: volume sampled, proportion of contaminants, proportion of positive blood cultures in each instrument and drawer, epidemiological indicator and proportion of false-positive instrument signals. The results obtained were outside the expected target for the volume sampled and were acceptable for the other indicators. The analysis of these results leads us to d

Consequences of Implementing a "Better" Blood Culture System.

BACKGROUND: Blood cultures (BCx) are the gold standard for diagnosing blood stream infections. However, contamination remains a challenge and can increase cost, hospital days, and unnecessary antibiotic use. National goals are to keep overall BCx contamination rates to ≤3%. Our healthcare system recently moved to a BCx system with better organism recovery, especially for gram-negative, fastidious, and anaerobic bacteria. The study objectives were to determine the benefits/consequences of implementing a more sensitive blood culture system, specifically on contamination rates. METHODS: The elec

Pediatric Anaerobic Blood Culture Practices in Industrialized Countries.

BACKGROUND: Routine anaerobic blood culture collection in febrile children is controversial, as clinicians try to account for the severe but relative infrequency of anaerobic bacteremia. Furthermore, clinical and laboratory practice variation among institutions may lead to potentially inaccurate epidemiological data. Our goal was to assess blood culture practices in pediatric patients throughout an international network of hospitals in industrialized countries. METHODS: We conducted a survey of current clinical and laboratory practice patterns in a convenience sample of international institut

The assessment of anaerobic blood culture in children.

Over the past 20 years, there has been a decline in the percentage of positive blood cultures yielding anaerobic organisms. Due to the limited blood volume drawn from pediatric patients, we have assessed the value of routine anaerobic blood cultures in children. From January 1994 to December 1998, 9886 paired aerobic and anaerobic blood cultures were analyzed in the pediatric microbiology laboratory at the Taipei Veterans General Hospital. Six hundred and eighteen (6.25%) isolates were considered to be clinically important microorganisms. Staphylococci, streptococci, aerobic gram-negative baci

Activity of cefepime against nosocomial blood culture isolates.

Cefepime is a new aminothiazolylacetamido cephalosporin with a wider spectrum and greater potency than many currently available cephalosporins. Since the blood culture isolates from patients of the study centre in Saudi Arabia are significantly more resistant to antimicrobial agents in clinical practice, we evaluated the in-vitro activity of cefepime and six other beta-lactam antibodies against 390 and 273 isolates of Gram-negative and Gram-positive bacteria, respectively. Cefepime had a broad spectrum of activity against the Enterobacteriaceae (MIC50 < 0.12 mg/L), Pseudomonas aeruginosa, Acin

[Pediatric bacteremia strains grow in blood culture media].

Bacteremia strains may influence the clinical therapy, the isolation of bacteremia strains is important for patients. In order to understand the distribution and the growth in the blood culture media of bacteremia strains, from January 1982 through December 1988 we studied the blood culture sent to us from the pediatric ward. These were three hundred and forty one bacterial strains (3.1%) isolated from patients. In the meantime we also studied the bacteremia strains growth in the blood culture media. In these positive cultures, there were 83 gram-positive, and 258 gram-negative strains. The 83

Volume of blood submitted for culture from neonates.

We prospectively examined 298 sets (298 aerobic, 299 anaerobic, and 73 resin cultures) of blood cultures from 161 critically ill newborns. The attending physicians were unaware of the study. The mean blood volume per patient (aerobic and anaerobic) was 1.05 (range, 0.11 to 3.04) ml. The mean blood volume per aerobic bottle was 0.53 (range, 0.01 to 1.90) ml. Among aerobic samples 2.7% were less than or equal to 0.1 ml, 16% were less than or equal to 0.3 ml, 33% were less than or equal to 0.4 ml, and 55% were less than or equal to 0.5 ml. For anaerobic cultures the mean blood volume was 0.52 (ra

Survival of leptospires in commercial blood culture systems.

Laboratory diagnosis of leptospirosis depends on the serological findings and, if possible, the isolation of the organisms. During the period 1981-1982 there were 40 blood cultures submitted from serologically positive patients and a successful isolation rate of only 12% was attained. Correct media, method of collection of suspect blood and laboratory technique all contribute to the successful isolation of the organism. Because of the low isolation rate an investigation was undertaken of various blood culturing systems used in Great Britain. Results obtained showed that certain blood culture m

Counterimmunoelectrophoresis for rapid identification of blood-culture isolates.

A total of 303 blood cultures that were positive by examination of Gram-stained smears were tested immediately by counterimmunoelectrophoresis for detection of bacterial antigens. Antigen was detected in all 82 blood cultures containing Streptococcus pneumoniae and 11 of 22 with Klebsiella pneumoniae, two of two with Haemophilus influenzae, and one of one with Neisseria meningiditis. False-positive cross-reactions in 265 tests occurred only with pneumococcal Omniserum in two cases of nongroupable streptococcal bacteremia and with Klebsiella antiserum in one case of Escherichia coli bacteremia

Impact, barriers, and facilitators of blood culture diversion devices to reduce blood culture contamination and improve patient safety: a scoping review.

Blood culture contamination (BCC) is a frequent and costly challenge in clinical diagnostics. BCC leads to extended hospital stays, unnecessary antimicrobial therapy, diagnostic delays, and increased healthcare costs, sometimes exceeding $100,000 per case depending on the scope of analysis. It also contributes to environmental waste and reputational harm. Blood culture diversion (BCD), particularly via blood culture diversion devices (BCDDs), has emerged as a promising strategy to reduce BCC. BCDDs divert initial blood flow, likely contaminated with skin flora, thereby improving diagnostic acc

Evaluation of Optimal Blood Culture Incubation Time To Maximize Clinically Relevant Results from a Contemporary Blood Culture Instrument and Media System.

Timely diagnosis of microorganisms in blood cultures is necessary to optimize therapy. Although blood culture media and systems have evolved for decades, the standard interval for incubation prior to being discarded as negative has remained 5 days. Here, we evaluated the optimal incubation time for the BacT/Alert Virtuo blood culture detection system (bioMérieux) using FA Plus (aerobic) and FN Plus (anaerobic) resin culture bottles in routine clinical use. Following institutional review board (IRB) approval, a retrospective review evaluated the outcomes of 158,710 bottles collected between Nov
