[Meta analysis of the effect of immunoglobulin infusion on neonatal isoimmune hemolytic disease caused by blood group incompatibility].

OBJECTIVE: Neonatal isoimmune hemolytic disease is still one of the major causes of neonatal hyperbilirubinemia. The infants with severe hemolysis even need phototherapy and exchange transfusion. Early intravenous immunoglobulin infusion may block hemolysis to some extent. This study aimed to investigate the efficacy and safety of immunoglobulin infusion on neonatal isoimmune hemolytic disease by meta analysis. METHOD: All randomized controlled trials on the effect of immunoglobulin infusion on neonatal Rh and ABO incompatible hemolytic disease obtained by searching MEDLINE, Cochrane Library,

Combination of ABO blood group incompatibility and glucose-6-phosphate dehydrogenase deficiency: effect on hemolysis and neonatal hyperbilirubinemia.

The incidence (%) of hyperbilirubinemia (serum bilirubin > or = 257 micromol/l) was similar in neonates with a combination of ABO incompatibility and glucose-6-phosphate dehydrogenase (G-6-PD) deficiency (45%), with ABO incompatibility (54%) or G-6-PD deficiency (37%), alone (ns). Carboxyhemoglobin values, corrected for inspired CO, were similarly elevated in all three groups (0.87 +/- 0.32%, 0.82 +/- 0.29%, 0.76 +/- 0.18%, respectively, ns), but correlated with bilirubin only in those with ABO incompatibility alone. ABO-incompatible/G-6-PD-deficient neonates, compared with those with either c

Evidence of immune tolerance to blood group antigens in a case of ABO-incompatible pediatric liver transplantation.

In a 12-year-old patient with blood group O, who had received a partial liver graft 10 years ago from his father with blood group A, the levels of anti-A-specific antibodies (Abs) were persistently undetectable after the transplantation, while the levels of anti-B-specific Abs gradually increased and attained a plateau. Peripheral blood mononuclear cells (PBMCs) from this patient were engrafted into NOD/SCID mouse in order to investigate the immune response to donor-type blood group antigens. Even after sensitization with blood group-A erythrocytes, no anti-A Abs were detected in the serum sam

Forward and reverse typing discrepancy and crossmatch incompatibility of ABO blood groups: cause analysis and treatment.

OBJECTIVE: Analyze the reasons for the mismatch between forward and reverse typing of ABO blood types and the mismatch between cross matching. METHODS: When the forward and reverse typing do not match, use physiological saline method, polyamine method, anti human globulin method, and anti screening positive samples are used for antibody identification. RESULTS: The factors contributing to discrepancies in blood typing between forward and reverse typing include weakened serum typing, condensation, monoclonal immunoglobulin influence, bone marrow transplantation, and blood type subtypes. ABO blood group incompatible renal transplantation in New Zealand: a report of the first 20 cases.

AIM: To report on the first 20 ABO blood group incompatible (ABOi) renal transplants performed in New Zealand. METHOD: Prospective short-term data and retrospective longer-term follow-up data was collected on the first 20 such transplants between March 2008 and April 2015. RESULTS: Patient and graft survival was excellent at 95% at 12 months and final follow-up (11-85 months; median 31 months). Three serious infections were noted in this time period, one of which resulted in patient death (pneumococcal pneumonia). There were no episodes of anti-ABO blood group antibody mediated rejection. Detection of allogeneic blood group A and B enzyme activities in patients with ABO incompatible kidney transplantation.

The phenomenon of accommodation in recipients of blood group ABO incompatible kidney transplantation (iKTx), in which grafts survive despite the presence of blood group A or B antigen in the graft and the presence of corresponding antibodies in the recipient's blood, is not uncommon. alpha1,3-N-Acetylgalactosaminyltransferase and alpha1,3galactosyltransferase associated with the synthesis of blood group A and B antigen (A and B enzymes), respectively, were measured by a highly specific enzyme-linked immunosorbent assay in the sera and transplanted tissues of patients who underwent an ABO iKTx.

Relationship between donor-recipient blood group incompatibility and serum bilirubin after allogeneic bone marrow transplantation from HLA-identical siblings.

Data on 477 patients with hematologic malignancies undergoing allogeneic bone marrow transplantation from HLA-identical siblings were analyzed for correlation between donor-recipient ABO blood group incompatibility and the development of elevated bilirubin levels (over 17 mmol/l) after transplantation. The median bilirubin on day 15 after transplant and the maximum bilirubin in the first 100 days were significantly higher in 155 patients with ABO-mismatched donors compared with 322 patients with ABO-matched donors. In univariate analysis, age > 16 years (P = 0.000006), ABO incompatibility (P =

Blood group O recipients associated with early graft deterioration in living ABO-incompatible kidney transplantation.

BACKGROUND: Blood group O individuals are known to have larger amounts of anti-ABO blood group (anti-A/B) IgG antibodies than A or B individuals. Therefore, in ABO-incompatible (ABOI) kidney transplantation (KTX), it is expected that blood group O recipients are more likely to suffer graft damage, because anti-A/B IgG antibodies are believed to be responsible for worse graft outcomes. METHODS: This study assessed the graft outcomes between blood group O and non-O recipients in ABOI-KTX. A total of 164 consecutive recipients who underwent ABOI-KTX between 1990 and 2007 under three different im

Immunoadsorption for removal of anti-A and anti-B blood group antibodies in ABO-incompatible bone marrow transplantation.

About 10-15 percent of all patients undergoing allogeneic bone marrow transplantation have a major ABO-incompatibility with their donors. The risk of acute hemolytic reactions due to the infusion of an incompatible donor marrow into the recipient can basically be prevented by recipient antibody depletion or by donor marrow red cell depletion. Nine patients were treated by immunoadsorption using a cartridge with chemically synthesized human blood group A and B antigen as immunoadsorbent for antibody depletion. Within a four-hour-procedure about 2-4 times the patient plasma volume could be proce

Minor blood group incompatibility due to blood groups other than Rh(D) leading to hemolytic disease of fetus and newborn: a need for routine antibody screening during pregnancy.

Minor blood group incompatibility due to blood groups other than Rh(D), although an uncommon cause of neonatal hyperbilirubinemia, has the potential to cause severe hyperbilirubinemia and its sequelae in infants, if left undiagnosed and untreated. Here, we describe clinical presentation, diagnosis and treatment of three cases of minor blood group incompatibility due to anti-E and anti-c antibody. All three neonates presented with pallor, icterus and splenomegaly within the first three days of life. Investigations showed indirect hyperbilirubinemia and a positive direct coombs test. Indirect co

Anti-Blood Group Antibodies in Intravenous Immunoglobulin May Complicate Interpretation of Antibody Titers in ABO-Incompatible Transplantation.

Patients receiving ABO-incompatible (ABOi) kidney transplants are treated before and after transplant with combination therapy, such as intravenous immunoglobulin (IVIG) and therapeutic plasma exchange, to prevent allograft rejection by reducing anti-A and anti-B titers. Although generally considered safe, it is well known that commercial IVIG products contain detectable anti-A and anti-B, which can be associated with hemolysis. Different preparative manufacturing techniques during the production of IVIG affect ABO antibody levels in IVIG preparations; therefore, some manufacturers now use new

Maternal-child blood group incompatibility and other perinatal events increase the risk for early-onset type 1 (insulin-dependent) diabetes mellitus.

The nationwide Swedish Childhood Diabetes Registry, which ascertains 99% of recent-onset Type 1 (insulin-dependent) diabetic children (0-14 years) in Sweden, was linked with the Swedish Medical Birth Registry. A matched case-control study was carried out analysing about 20 perinatal variables concerning mother and child. A total of 2757 infants who became diabetic during the period 1978-1988 were analysed. For each case infant three control children were randomly selected from among all infants born in the same year and at the same delivery unit as the case infant. The following statistically

Infant lung transplantation: First German experience including two cases of ABO blood group incompatible transplantations.

BACKGROUND: Infant lung transplantation (LuTx) has been rarely performed in Europe and poses unique challenges. METHODS: We reviewed referrals for LuTx to our center for patients below 1 year of age from January 1, 2018 to December 31, 2022. Clinical data and outcomes of transplanted infants and data on patients who were declined for evaluation, not listed after evaluation, or died before LuTx were collected. The number of donor lungs from organ donors younger than 1 year of age offered to our center and their utilization were analyzed for the same time period. RESULTS: A total of 18 referra

Solid tumor rejection using personalized incompatible human leukocyte antigen (HLA) and blood group ABH antigens.

Cancer immunotherapies have become mainstream, targeting tumor elimination via various mechanisms, albeit with varied effectiveness. Here, we review briefly the current landscape of cancer immunotherapies and the central role of human leukocyte antigen (HLA) in them. We then propose a new kind of immunotherapy for solid tumors, where the key is the involvement of antigens and antibodies unrelated to the tumor itself. In this approach, we consider the tumor as akin to a transplanted organ, which can be rejected by two different mechanisms of incompatibility. The first involves the intra-tumor a

Successful ABO-incompatible heart transplantation in a child despite blood-group sensitization after ventricular assist device support.

In the first two yr of life blood-group incompatible (ABO-incompatible) heart transplantation can be performed leading to immune tolerance to donor blood group. Antibody titers should be below 1:4. VAD use is correlated with sensitization toward blood-group antigens. A boy was diagnosed with dilated cardiomyopathy at nine months of age and listed for 0-compatible transplantation. Progressive heart failure required implantation of a left VAD. His listing was extended for ABO-incompatible transplantation despite antibody titers of 1:32 anti-A and 1:8 anti-B. After 26 days on VAD, he was transpla

Impact of different blood group incompatibilities in kidney transplantation: a 15-year outcomes analysis from a large kidney transplant center.

BACKGROUND: While ABO-incompatible kidney transplantation (ABOiKT) has demonstrated favorable short-term outcomes, data on its long-term effects remain limited. This study evaluated the short- and long-term clinical outcomes of ABOiKT across various ABO-incompatible donor-recipient combinations. METHODS: We included patients who underwent ABOiKT at our institution in 2007-2024. The outcomes assessed included 15-year data on graft, patient survival, and early AMR rates. RESULTS: Of 239 ABOiKT cases, AMR occurred in 9.2% and was linked to longer hospitalization and higher graft failure. AMR wa
