The impact of improved safety on maintaining a sufficient blood supply is becoming an increasingly real issue facing the National Blood Service in England and North Wales. This paper shows the extent of the impact that safety measures can have on reducing collection levels, without making any value judgement on the safety critera themselves. It demonstrates that underlying trends in collection are making it increasingly difficult to meet demand. Further potential restrictions, perhaps associated with variant Creutzfeldt-Jacob disease (vCJD), will require a reassessment of the safety-sufficienc

Present safeguards for the blood supply consist of three tiers of protection: donor deferral based on a donor's history of risk factors, confidential exclusion of blood units from donors with self-admitted risk factors, and testing of the blood itself. Before the discovery of the AIDS virus in 1983 and 1984, there was no specific test relevant to AIDS that could be used to help improve the safety of the blood supply. The first step was intensified efforts, based on what was then known of the epidemiology of the disease, to take donor histories to identify risk factors. The first specific tests

Emerging infectious diseases (EIDs) have always been one of the major threats to public health. Although the implementation of mandatory testing for 4 classical transfusion-transmitted infectious-human immunodeficiency virus, hepatitis B virus, hepatitis C virus, and syphilis-has reduced the transfusion risk of these pathogens, the potential threat of various EID agents and their constantly evolving variants to blood safety in China is not fully understood. This review presents 9 representative EID agents that are autochthonous and epidemic nationally or regionally in China. The epidemiologic

BACKGROUND: Health history questions identify blood donors believed to pose a higher risk of transmission of infectious diseases. This study assesses the current impact of some of these questions on blood safety as reflected by infectious disease markers.
STUDY DESIGN AND METHODS: Donors who were deferred from donating blood due to health history question(s) were recruited at four different regions of the American Red Cross Blood Services. A blood sample was tested for serologic markers of blood-borne infections as performed for accepted blood donors.
RESULTS: Of 497 deferred donors enrolled

BACKGROUND: Ensuring safe blood transfusion is a public health priority in India, where reliance on serological screening alone may miss early infections. Centralized nucleic acid testing (NAT) offers an opportunity to strengthen blood safety through advanced molecular diagnostics.
AIM: This study aimed to highlight the successful implementation of the Madhya Pradesh centralized NAT hub and spoke model for enhancing blood transfusion safety.
METHODS: The current project was implemented through a public-private partnership (PPP) using a hub-and-spoke model. To assess its merit, a retrospectiv

Antiretroviral therapy (ART) to treat and pre-exposure prophylaxis (PrEP) to prevent HIV infection are effective tools to help end the HIV epidemic. However, their use could affect HIV transfusion-transmission risk. Three different ART/PrEP prevalence analyses in blood donors were conducted. First, blood samples from HIV-positive and a comparison group of infection-nonreactive donors were tested under blind using liquid chromatography-tandem mass spectrometry for ART. Second, blood donor samples from infection-nonreactive, 18- to 45-year-old, male, first-time blood donors in 6 US locations wer

BACKGROUND AND OBJECTIVES: Finding compatible blood donors for patients from minority racial and ethnic populations can be challenging due to differences in blood group antigen frequencies with the existing donor pool. Individuals with sickle cell trait (SCTrait) are allowed to donate in some countries, but in the Netherlands, carriers are currently not permitted to donate. This study estimates the incidence of adverse events among SCTrait donors compared with non-SCTrait donors, to inform potential policy decisions.
MATERIALS AND METHODS: A retrospective study was performed at Sanquin, inclu

Ensuring the safety of blood and blood products is a vital aspect of healthcare. The potential for transmission of pathogens through blood and blood products makes transfusion safety a significant concern. Despite advancements in testing methodologies, donated blood products still pose a risk for infection transmission. Human parvovirus B19 (B19V) is a small, single-stranded, non-enveloped DNA virus transmissible parenterally by blood transfusion. B19V causes a wide range of clinical manifestations, which is generally harmless in healthy individuals. B19V infection may cause severe complicatio

BACKGROUND: Within the coming decades, a steadily growing demand for blood products will face a shrinking blood donor population in many countries. After increasing the donor age of repeat donors for whole blood donation (WB) from 68 to 70 years in 2009 in our Blood Service, we investigated whether this is sufficient as a safe and effective strategy to sustain future blood supply.
MATERIALS AND METHODS: Between 1 March 2009 and 28 February 2011, WB donations from donors aged between 69 and 70 and their proportion of total donations in 2010 were determined. We analysed adverse reaction rates i

Despite measures to mitigate risk of transfusion-transmitted infections, emerging agents contribute to morbidity and mortality. We outline the epidemiology, risk mitigation strategies, and impact on patients for Zika virus, bacteria, Babesia, and cytomegalovirus. Nucleic acid testing of blood has reduced risk of Zika infection and reduced transfusion-transmitted risk of Babesia. Other collection and testing measures have reduced but not eliminated the risk of sepsis from bacterially contaminated blood components. Cytomegalovirus has almost been eliminated by high-efficiency leukoreduction, but

BACKGROUND AND OBJECTIVES: There is a growing concern for the transmission of arboviral infections by blood transfusion in Europe. However, no assessment of the risk of transmission through all European blood supplies has been reported. Risk regulations at a European level should take differences in local transmission risk and the risk of transmission by travelling donors into consideration.
MATERIALS AND METHODS: A risk model and publicly available tool were developed to calculate the risk of transmission by all European blood supplies for arboviral outbreaks within Europe. Data on individua

In the present study, we attempted to assess the safety and the risk of cordocentesis under ultrasound guidance for fetal blood sampling. Forty-five cordocenteses in 43 cases were carried out. These forty-three cases included 23 nonimmunologic hydrops fetalis, 12 congenital fetal malformations, 3 chromosome aberrations, 3 Rh isoimmunization, and 2 idiopathic thrombocytopenic purpura. Gestational age when cordocentesis was performed ranged from 17 to 39 weeks of gestation. In 42 (93.3%) out of 45 procedures, pure fetal blood was obtained. Postpuncture bleeding was observed in 17 cases (37.8%).

Infections with hepatitis B, C, and E virus (HBV, HCV, and HEV) can be transmitted via blood and cause severe acute or chronic liver infections. To ensure the safety of blood donations and protect recipients from virus transmissions, blood donations in Germany are tested for viral genomes using nucleic acid amplification techniques (NATs) as well as for viral antigens and antibodies by serological testing. This article describes the relevant regulations on the safety of blood and blood products in Germany and the various screening methods. The safety of blood products is assessed.Currently use
