Spatial-dependent regularization to solve the inverse problem in electromyometrial imaging.

Recently, electromyometrial imaging (EMMI) was developed to non-invasively image uterine contractions in three dimensions. EMMI collects body surface electromyography (EMG) measurements and uses patient-specific body-uterus geometry generated from magnetic resonance images to reconstruct uterine electrical activity. Currently, EMMI uses the zero-order Tikhonov method with mean composite residual and smoothing operator (CRESO) to stabilize the underlying ill-posed inverse computation. However, this method is empirical and implements a global regularization parameter over all uterine sites, whic

[Regular tachycardia with broad QRS complex: differential diagnosis on 12-lead ECG].

Differential diagnosis of regular tachycardia with broad QRS complex can be challenging in daily practice. There are four different arrhythmias that have to be taken into account when being confronted with a broad QRS complex tachycardia: (1) ventricular tachycardia (VT); (2) supraventricular tachycardia (SVT) with bundle branch block (BBB); (3) SVT with AV conduction over an accessory AV pathway; (4) paced ventricular rhythm. Due to potentially fatal consequences, the correct diagnosis is important in view of both the acute treatment and the long-term therapy. Since SVT with accessory conduct

[Diagnostic approach in complex arrhythmias].

The correct diagnosis of complex arrhythmias requires an exact knowledge of arrhythmia mechanisms and their application in each specific case. In a patient with dilated cardiomyopathy and nonspecific sinus tachycardia, the interpretation of the coronary vein sinus electrogram could quickly raise the possibility of left atrial tachycardia. In a patient with tachycardia, pre-existing left bundle branch block, and syncope, the induction mechanism of the tachycardia in the electrophysiological examination as well as in the detailed analysis of the His bundle electrogram were finally identified as

Extrastimuli-assisted functional mapping improves ventricular tachycardia ablation outcomes: A systematic review, meta-analysis, and meta-regression.

BACKGROUND: In the context of ventricular tachycardia (VT) ablation, functional electroanatomic mapping techniques may help identify arrhythmogenic substrates in scarred and normal voltage areas. In addition, extrastimuli-assisted (dynamic) mapping may be more effective than intrinsic rhythm (static) approaches to uncover key ablation targets, potentially improving procedure outcomes. OBJECTIVE: We aimed to assess the efficacy and safety of functional mapping-assisted VT ablation and to compare procedural outcomes between dynamic and static mapping approaches. METHODS: PubMed/MEDLINE, Scopus

The tetrafascicular nature of the intraventricular conduction system.

The existence of a tetrafascicular intraventricular conduction system remains debatable. A consensus statement ended up with some discrepancies and, despite agreeing on the possible existence of an anatomical left septal fascicle, the electrocardiographic and vectorcardiographic characteristics of left septal fascicular block (LSFB) were not universally accepted. The most important criteria requested to confirm the existence of LSFB is its intermittent nature. So far, our group has published cases of transient ischemia-induced LSFB and phase 4 or bradycardia-dependent LSFB. Finally, anatomical

Value of high-density mapping in the electrophysiology laboratory.

PURPOSE OF REVIEW: This article uses three commonly encountered clinical scenarios to highlight the value of high-density mapping in the electrophysiology laboratory for the identification of the circuits and substrates during catheter ablation of complex arrhythmias. RECENT FINDINGS: High-density mapping gathered with a multielectrode catheter, with its smaller individual electrode size and closer interelectrode spacing, helps during ablation procedures in a number of ways. When high-density mapping is performed during tachycardia, the characterization of macro-reentrant circuits is better.

Clinical Aspects and Ablation of Ventricular Arrhythmias in Tetralogy of Fallot.

Life expectancy of patients with rToF has considerably improved due to refined surgical interventions. Monomorphic fast VTs are frequently encountered in adult patients with rToF. The dominant substrate of VT is anatomical isthmuses bordered by surgical incisions, patch material and valve annuli. Substrate based ablation strategies aim to transect all slow conducting anatomical isthmuses (SCAI) as identified by electroanatomical mapping. Procedural success is defined as non-inducibility of VT and confirmed conduction block over the SCAI resulting in long-term VT free survival in most patients.

Delineation of anatomical relation of important adjacent structures to the left atrium in electroanatomical mapping using fluoroscopy images.

Catheter ablation is being used increasingly for the treatment of atrial fibrillation (AF), and more extensive ablations that include linear lesions on the posterior wall of the left atrium (LA) are performed today. A better understanding of the anatomic relationship between the posterior LA wall and the oesophagus is required to identify potentially high-risk areas for the application of radiofrequency energy. We hereby describe a simple method to delineate the anatomical relationship between important adjacent structures and the LA in electroanatomical mapping using plain fluoroscopy images.

Use of electroanatomic mapping in the assessment of atrial tachycardia aetiology.

The present case report is of a 36-year-old man who had been symptomatic for frequent palpitations for 4 years after a prolonged febrile episode. The electrocardiogram on admission revealed an iterative supraventricular tachycardia at rate of 110 bpm, with ventriculoatrial interval of 180 ms and superior P-wave axis. Electroanatomic mapping showed substantially normal values in the right atrium other than an area along the posteroseptal region of the tricuspid annulus, with a low-voltage region that correlated with the area of earliest activation.

[Recent advances in radiofrequency catheter ablation].

Radiofrequency catheter ablation has been established as a first-line treatment of various paroxysmal tachycardias, and its developments are still ongoing. As recent advances of radiofrequency catheter ablation, we can point out the following issues: 1) transaortic approach for idiopathic ventricular tachycardia(VT) of LBBB-form with inferior axis, 2) new approach guided by mid-diastolic potential for verapamil-sensitive VT, 3) pulmonary vein(PV) isolation technique guided by PV ostial circular electrogram mapping for paroxysmal atrial fibrillation, 4) new ablation strategies for macro-reentry

Catheter ablation of ventricular tachycardia in patients with structural heart disease.

Radiofrequency catheter ablation has revolutionized therapy of most forms of supraventricular tachycardia and ventricular tachycardia in the absence of structural heart disease by providing arrhythmia cure in almost 90% of patients. However, this treatment has not been nearly as successful in patients with ventricular tachycardia in the setting of structural heart disease, because of a number of factors. Some of these limitations are technical (imprecise mapping tools, multiple regions requiring ablation) although others are patient-related (hemodynamic instability during arrhythmia, progressi

[Noninvasive recording of pre-P wave related to sinus node electrical activity using P wave-triggered signal-averaging technique].

To determine whether the sinus node electrogram (SNE) could be detected noninvasively, a specially modified P wave-triggered signal-averaged electrocardiogram was recorded through the standard V1, V2 and V3 precordial leads in 39 patients. The data between 500 ms before and 300 ms after the trigger point were processed using a digital bandpass filter of 0.5 Hz (12 dB/oct)-20 Hz (18 dB/oct). Pre-P signals were observed just before the P wave resembling SNE recorded directly. Pre-P potential duration closely correlated with direct SACT (r = 0.856, P < 0.0005). Thus, it is believed that signals o

Value of the 12-lead ECG to localize the origin of ventricular tachycardia.

The 12-lead electrocardiogram (ECG) recorded during ventricular tachycardia (VT) remains a simple, universally available tool for VT localization. In idiopathic VT, QRS morphology and polarity often reflect the site of earliest activation and can facilitate targeted mapping. In patients with structural heart disease, however, the surface ECG predominantly represents the VT exit region rather than the critical diastolic isthmus; thus, its main clinical value lies in regionalization and in planning ablation strategy and access. A pragmatic stepwise interpretation integrating V1 morphology, preco

Wave mapping: detection of co-existing multiple wavefronts in high-resolution electrical mapping.

High-resolution mapping makes it possible to reconstruct and display the conduction pattern of the action potential as it propagates through cardiac or smooth muscles. During slow and regular activity, time mapping of the spread of activation muscles. During slow and regular activity, time mapping of the spread of activation is relatively simple and straightforward. However, when frequencies are high or conduction is slow, such as seen during atrial fibrillation or found in the pregnant uterus, the tracking of individual waves may become more difficult and uncertain. In order to reconstruct th

Substrate Mapping in Ventricular Arrhythmias.

Ventricular tachycardia is typically hemodynamically unstable. Strategies to target the arrhythmogenic substrate during sinus rhythm are essential for therapeutic ablation. Electroanatomic mapping is the cornerstone of substrate-based strategies; ablation can be directed within a delineated scar region defined by low voltage. Bipolar voltage mapping has inherent limitations. Specific electrogram characteristics may improve the specificity of localizing the most arrhythmogenic regions within the substrate. Deceleration zones during sinus rhythm are niduses for reentry and can be identified by i

Atrial tachycardias encountered in the context of catheter ablation for atrial fibrillation part ii: mapping and ablation.

Over the past decade, there has been an exponential increase in the number of catheter ablation procedures performed for atrial fibrillation (AF). While for paroxysmal AF, proximal pulmonary vein isolation is sufficient in the majority of cases, ablation of persistent and longstanding AF requires an extensive surgical-like procedure. This approach is correlated with a high rate of AF termination; however, this is achieved at the cost of at least one atrial tachycardia (AT) during the index procedure or during the patient's follow-up in the vast majority of cases. As these ATs are often multipl
