A randomized study of dietary composition during weight-loss maintenance: Rationale, study design, intervention, and assessment.

BACKGROUND: While many people with overweight or obesity can lose weight temporarily, most have difficulty maintaining weight loss over the long term. Studies of dietary composition typically focus on weight loss, rather than weight-loss maintenance, and rely on nutrition education and dietary counseling, rather than controlled feeding protocols. Variation in initial weight loss and insufficient differentiation among treatments confound interpretation of results and compromise conclusions regarding the weight-independent effects of dietary composition. The aim of the present study was to evalu

Postpartum weight trajectories in overweight and lean women.

BACKGROUND: overweight and obesity in women of reproductive age are increasing and are often linked with excessive weight gain in pregnancy and weight retention after birth. Studies on spontaneous maternal weight trajectory after childbirth are scarce. OBJECTIVE: we describe women's spontaneous weight trajectory during the first six weeks of the postpartum period and its relationship between Body Mass Index and socio-demographical, behavioural and psychological variables. DESIGN: data from 212 women who gave birth in three regional hospitals were collected prospectively between December 2015

Do obese patients after weight loss become metabolically normal?

OBJECTIVE: To compare the metabolic cardiovascular risk factors of obese patients after weight loss and 4-year weight loss maintenance afterwards against a matched control group of subjects with a stable body weight and without history of weight loss. METHOD: 50 obese non-diabetic subjects in the study group who previously lost 10% or more of their weight and afterwards entered a 4-year weight loss maintenance programme were matched by age, BMI and sex and compared to 50 weight-stable subjects without history of weight loss. The comparison between both groups was done in term of anthropometri

Age-dependent lidocaine disposition in patients with acute myocardial infarction.

To evaluate age-dependent changes in lidocaine disposition in patients with acute myocardial infarction, we measured plasma concentrations of lidocaine and its metabolites monoethylglycinexylidide and glycinexylidide after discontinuation of a maintenance lidocaine infusion. Plasma lidocaine clearance was calculated by dividing the lidocaine concentration at the end of the infusion into the maintenance infusion rate. Lidocaine clearance in 35 patients was related to body weight and was reduced by heart failure. Heart failure was more common in the elderly, occurring in 15 of 27 (56%) patients

Third-wave cognitive behaviour therapies for weight management: systematic review and network meta-analysis protocol.

INTRODUCTION: Behavioural and cognitive behavioural programmes are commonly used to assist with weight management, but there is considerable scope to improve their effectiveness, particularly in the longer term. Third-wave cognitive behaviour therapies (CBTs) have this potential and are increasingly used. This systematic review will assess the effect of third-wave CBTs for weight management on weight, psychological and physical health outcomes in adults with overweight or obesity. METHODS AND ANALYSIS: The systematic review will be reported according to the Preferred Reporting Items for Syste

Psychological Determinants of the Weight Cycle: A Review of Perceived Stress and Self-Efficacy.

PURPOSE OF REVIEW: Long-term weight maintenance remains a significant challenge in obesity management. Weight cycling, a common phenomenon in this process, is characterised not only by repeated weight loss and regain but also by an increased metabolic and psychological burden. Previous research has shown that perceived stress and self-efficacy may play a role in processes related to weight cycling, such as adherence to dietary patterns, behavioural regulation and long-term weight management. Therefore, this review aims to examine the potential roles of perceived stress and self-efficacy in the

Estimation of initial phenobarbital dosing in term neonates with moderate-to-severe hypoxic ischaemic encephalopathy following perinatal asphyxia.

WHAT IS KNOWN AND OBJECTIVE: Phenobarbital is the first-line treatment of seizures in asphyxiated neonates; however, due to the high pharmacokinetic variability in this population, there is no consensus on the optimal dosage regimen. This study was conducted to identify variables that affect phenobarbital fate during routine clinical care and then to evaluate the dosage schedule that could be applied in term asphyxiated neonates with respect to achieving the target therapeutic range. METHODS: Phenobarbital pharmacokinetics was calculated based on serum concentrations measurements using one-co

Weight changes after smoking cessation and the risk of hip fractures: a nationwide population-based cohort study.

UNLABELLED: Smoking cessation reduces hip fracture risk; however, subsequent weight loss may offset this benefit. In a nationwide cohort, weight loss after smoking cessation was associated with increased hip fracture risk. Maintaining stable body weight after quitting smoking may be important for fracture prevention. PURPOSE: Smoking cessation reduces fracture risk but is often accompanied by changes in body weight that may influence bone health. However, it remains unclear whether weight change after smoking cessation is associated with hip fracture risk. We therefore examined the associatio

Lifestyle Medicine as Co-Therapy During Incretin-Based Anti-Obesity Pharmacotherapy: Integrating Physical Activity, Nutrition, and Behavioral Strategies for Long-Term Success.

BACKGROUND/OBJECTIVES: Obesity is a chronic, progressive, and relapsing disease that requires long-term, multidisciplinary management rather than episodic weight-loss treatment. Although novel incretin-based anti-obesity pharmacotherapies, including GLP-1 receptor agonists and dual GIP/GLP-1 receptor agonists, have markedly improved the clinical management of obesity, weight reduction alone does not fully capture treatment success. Body composition, lean mass preservation, physical function, nutritional adequacy, psychological well-being, adherence, and long-term weight-loss maintenance are in

Continuous glucose monitoring after discontinuation of incretin-based obesity pharmacotherapy: indirect evidence and a research framework for metabolic stability and weight maintenance.

Discontinuation of incretin-based pharmacotherapy is frequent and is associated with weight regain and metabolic relapse, including deterioration in cardiometabolic risk factors. New strategies are needed to preserve metabolic stability and body weight after treatment withdrawal. Whether continuous glucose monitoring (CGM) can facilitate early detection of metabolic relapse or support long-term weight maintenance after discontinuation remains unknown. This narrative review examines the indirect evidence supporting CGM use after discontinuation of incretin-based obesity pharmacotherapy, focusin

Effects of Weight Loss and Weight Maintenance on Lipoprotein Insulin Resistance Scores in Adults with Overweight and Obesity.

Background: An elevated lipoprotein insulin resistance (LP-IR) score corresponds to insulin resistance in adults with overweight and obesity, yet data are lacking regarding the impact of exercise interventions on LP-IR. The purpose of this secondary analysis was to evaluate the effects of a weight loss and weight maintenance intervention on LP-IR score in adults with overweight and obesity. Methods: Thirty sedentary adults with overweight and obesity completed a 10-week OPTIFAST® weight loss program with supervised aerobic exercise to achieve clinical weight loss (CWL) (≥7% from baseline). Prospective evaluation of vertical banded gastroplasty as the primary operation for morbid obesity.

Our previous experience with vertical (nonbanded) gastroplasty proved disappointing because of unsatisfactory maintenance of weight loss. Vertical banded gastroplasty seemed to be an attractive alternative operation because it provided an externally reinforced (banded) stoma that would not enlarge over time. In this study, our aim was to determine prospectively the results of vertical banded gastroplasty used as a primary, weight-reducing procedure in patients with morbid obesity. Seventy consecutive patients with morbid obesity (mean weight, 139 kg), all of whom had obesity-related complicati

Strategies and potential molecular targets for obesity treatment.

Obesity is an increasingly prevalent and important health problem. Although treatment is available, the long-term maintenance of medically significant weight loss (5 to 10 percent of initial body weight) is rare. Since 1995 there has been an explosion of research focused on the regulation of energy balance and fat mass. Characterization of obesity-associated gene products has revealed new biochemical pathways and molecular targets for pharmacological intervention that will likely lead to new treatments. Ideally, these treatments will be viewed as adjuncts to behavioral and lifestyle changes ai

Double jeopardy: a review of weight gain and weight management strategies for psychotropic medication prescribing during methadone maintenance treatment.

Methadone maintenance treatment (MMT) is an important treatment tool for the opioid epidemic. One challenge is that many persons who present for MMT also have co-occurring psychiatric disorders. Individually, both methadone and psychiatric medications carry risk of weight gain. Therefore, concurrent prescribing of methadone and psychiatric medications places dual diagnosis patients at even greater risk. As a parallel obesity epidemic grows, results from clinical trials assessing weight gain and weight management strategies among MMT and psychiatric patients can both inform and guide clinical p

Regulation of body weight and food intake by AGRP neurons during opioid dependence and abstinence in mice.

Dysregulation of body weight maintenance and opioid dependence are often treated as independent disorders. Here, we assessed the effects of both acute and long-term administration of morphine with and without chemogenetic activation of agouti-related peptide (AGRP)-expressing neurons in the arcuate nucleus (ARCAGRP neurons) to elucidate whether morphine and neuronal activation affect feeding behavior and body weight. First, we characterized interactions of opioids and energy deficit in wild-type mice. We observed that opioid administration attenuated both fasting-induced refeeding and ghrelin-

Bulls fed a high-gain diet decrease blastocyst formation after in vitro fertilization.

IN BRIEF: Paternal high-gain diet reduces blastocyst development following in vitro fertilization and embryo culture but does not affect gene expression or cellular allocation of resultant blastocysts. ABSTRACT: Bulls used in cattle production are often overfed to induce rapid growth, early puberty, and increase sale price. While the negative consequences of undernutrition on bull sperm quality are known, it is unclear how a high-gain diet influences embryo development. We hypothesized that semen collected from bulls fed a high-gain diet would have a reduced capacity to produce blastocysts fo

Successful weight loss initiation and maintenance among adolescents with overweight and obesity: does age matter?

Treatments for adolescents with overweight/obesity demonstrate mixed success, which may be due to a lack of consideration for developmental changes during this period. Potential developmental differences in weight loss motivations, weight maintenance behaviours and the role of parents in these efforts were examined in a sample of successful adolescent weight losers. Participants enrolled in the Adolescent Weight Control Registry (n = 49) self-reported demographic information and weight history, reasons for weight loss and weight control, weight loss approach and weight maintenance strategies,

Effect of a Leucine/Pyridoxine Nutraceutical on Caloric Intake and Body Composition of Obese Dogs Losing Weight.

The aim of this 29-week randomized, positively and negatively controlled study was to investigate whether a nutraceutical containing 1 g leucine and 13 mg pyridoxine can enhance weight loss while maintaining lean muscle mass in obese dogs. Twenty-four healthy, 2-year-old beagles were initially divided into obesification (n = 18) or ideal body weight groups (n = 6). After obesification, the 18 dogs were divided into three weight loss groups and fed one of the following over 12 weeks: nutraceutical with canned adult diet (CAD; ObN), placebo with CAD (ObP), or a canned therapeutic weight loss die
