Bone microarchitecture in Rett syndrome and treatment with teriparatide: a case report.

We present the case of a 28-year-old female Rett syndrome patient with low bone mass and a recent fracture who was successfully treated with teriparatide. Bone mineral density and microarchitecture substantially improved after treatment. Rett syndrome (RTT), an X-linked progressive neuro-developmental disorder caused by mutations in the methyl-CpG-binding 2 (MECP2) gene, has been consistently associated with low bone mass. Consequently, patients with RTT are at increased risk of skeletal fractures. Teriparatide is a bone-forming agent for the treatment of osteoporosis that has demonstrated its

What have we learned from clinical studies? Fractures and the interactions of bone mass and remodeling.

The knowledge that we have gained about risk assessment and fracture prediction from the tools we have available at present has provided considerable information allowing the clinician some improved capability to determine those most at risk for fracture. But the tools are imperfect, and consequently, the physician's capability to estimate fracture risk for individual patients is not precise. To put it into statistical terms, while a point estimate of absolute risk can be developed for individual patients the confidence intervals around that estimate are wide. To a degree, the same is true abo

Systematic review of medication related osteonecrosis of the jaw (MRONJ) in patients undergoing only antiangiogenic drug therapy: surgery or conservative therapy?

Osteonecrosis of the jaw is a severe adverse condition affecting patients exposed to specific types of medications. Previous studies have highlighted that osteonecrosis of the jaw is triggered by invasive dental procedures and can be very challenging to manage, especially in patients with cancer. The primary aim of this review was to analyse all available evidence on the management (surgical and/or conservative) of medication related osteonecrosis of the jaws (MRONJ) in patients with a history of antiangiogenic drugs therapy and who had not been previously exposed to any antiresorptive drug tr

Managing Perimenopausal Bone Loss: The Role of Bisphosphonate Treatment.

Preventing the rapid loss of bone density and structure that occurs during the perimenopause is a unique opportunity to prevent the development of postmenopausal osteoporosis and high fracture risk. For women who are not candidates for estrogen therapy, low-dose bisphosphonate therapy for 3 to 5 years can prevent perimenopausal bone loss, after which intermittent bisphosphonate therapy can maintain skeletal health indefinitely. Identifying women at high risk for developing osteoporosis with bone density testing as they approach or enter perimenopause and considering prevention therapy should b

Current outlook on the use of monoclonal antibody therapies for osteoporosis.

INTRODUCTION: The availability of monoclonal antibody therapies impacting on the basic pathways of bone formation; (romosozumab) and bone resorption (denosumab) has contributed greatly to our ability to manage patients with osteoporosis and fracture risk. Monoclonal antibodies offer the potential to optimize therapeutic efficacy with minimal adverse effects. AREAS COVERED: This review discusses aspects of registration clinical trials as well as experience gained from the clinical use of monoclonal antibodies for osteoporosis. The registration clinical trials prove anti-fracture efficacy with

[Clinical efficacy of osteoporosis treatments].

Osteoporosis therapy includes antiresorptive drugs of varying efficacy (estrogens, raloxifene, bisphosphonates, denosumab) and osteoanabolic agents capable of stimulating bone formation through various mechanisms (teriparatide, abaloparatide, romosozumab). This results in notable differences in bone mineral density gain and antifracture efficacy. Several comparative studies and meta-analyses have demonstrated the superiority of osteoanabolic agents over antiresorptive agents, particularly in high-risk women. For all treatments, the risk of severe adverse events remains very low. However, the r

Predictive factors for achievement of treatment goals in patients with postmenopausal osteoporosis treated with denosumab.

OBJECTIVES: To investigate efficacy of long-term treatment with denosumab and predictive factors for achievement of treatment goals in patients with postmenopausal osteoporosis (PMO). METHODS: We enrolled 111 PMO patients who had T-scores ≤-2.5 either at the lumbar spine (L-) or femoral neck (FN-), who had never been treated for osteoporosis, and who could be followed for at least 3 years. We first evaluated changes in bone mineral density (BMD) for up to 7 years. We next defined the treatment goal as the achievement of a T-score >-2.5 at month 36 and performed multivariate analysis to identi

The Effect of Bisphosphonates on Managing Osteoporosis After Spinal Cord Injury: A Meta-Analysis.

BACKGROUND: The increased bone loss after spinal cord injury (SCI) is associated with an increase in the morbidity and mortality of fragility fractures, which can constitute a substantial cost to health care systems. Bisphosphonates (BPs) are now the principal class of medications used for osteoporosis. OBJECTIVE: To demonstrate the effect of BPs on treating osteoporosis after SCI. METHODS: A comprehensive search in PubMed, EMBASE, Web of Science and Cochrane Central databases was undertaken for randomized controlled trials (RCTs), exploring the effect of BPs on osteoporosis after SCI. Bone Health and Osteoporosis Management of the Patient With Duchenne Muscular Dystrophy.

Duchenne muscular dystrophy is associated with an increased risk of bone fragility due to the adverse effects of prolonged glucocorticoid therapy and progressive muscle weakness on bone strength. Osteoporosis manifests clinically as low-trauma long-bone and vertebral fractures (VFs), with VFs frequent, particularly in those treated with glucocorticoid therapy. It is increasingly recognized that bone pain, medical complications of osteoporosis (such as fat embolism syndrome), and the potential for permanent, fracture-induced loss of ambulation can be mitigated with timely bone health surveillan

[Secondary osteoporosis. Disease-Related Osteoporosis in Children.].

Similarly to adult osteoporosis, childhood osteoporosis also is usually divided into primary and secondary causes. Primary osteoporosis includes genetic disorders represented by osteogenesis imperfecta. Secondary pediatric osteoporosis is associated with various diseases and glucocorticoid treatment, and malnutrition, impaired mobility, chronic inflammation and endocrine disorders can be risk factors. In growing children, mild osteoporosis can be spontaneously recovered by elimination of risk factors. However, initiation of treatment using drugs such as bisphosphonate should be considered in p

Efficacy of Bisphosphonates on Bone Mineral Density and Fracture Rate in Patients With Osteogenesis Imperfecta: A Systematic Review and Meta-analysis.

Epidemiological evidence suggests that bisphosphonates are the most promising drugs for patients with osteogenesis imperfecta (OI). However, data on this issue are controversial. We conducted a meta-analysis to assess the efficacy of bisphosphonates on bone mineral density (BMD) and fracture rate in patients with OI. Electronic databases were searched to find relevant studies. Two reviewers independently identified relevant randomized controlled trials, which evaluated the efficacy of bisphosphonates in patients with OI. Outcome measures were fracture incidence and BMD changes in different ske

A systematic review and meta-analysis of efficacy and safety of Romosozumab in postmenopausal osteoporosis.

The study was conducted to illustrate the effect of Romosozumab in postmenopausal osteoporosis patients. Romosozumab decreased the incidence of vertebral, nonvertebral, and clinical fractures significantly. In addition, decreased incidence of falls and increased bone mineral density at lumbar spine, total hip, and femoral neck was observed. Romosozumab is a monoclonal antibody that acts against the sclerostin pathway leading to enhanced bone formation and reduced bone resorption in patients with osteoporosis. Electronic search was performed on Medline (via PubMed), The Cochrane Central Registe

Treatment of postmenopausal osteoporosis with bone-forming and antiresorptive treatments: Combined and sequential approaches.

Efficient therapies are available for the treatment of osteoporosis. Bisphosphonates and denosumab are the most commonly used antiresorptive therapies. Despite differences in the increase in bone mineral density seen with these drugs, the reductions in fracture risk are similar; 50-70%, 20%, and 40% for vertebral, non-vertebral and hip fractures, respectively. The bone-forming treatments; teriparatide and abaloparatide increase bone mineral density more than the antiresorptives and the reductions in fracture risk are 85% and 40-50% for vertebral and non-vertebral fractures, respectively, compa

Current and Developing Pharmacologic Agents for Improving Skeletal Health in Adults with Osteogenesis Imperfecta.

Osteogenesis imperfecta (OI) is a genetic disorder characterized by increased bone fragility largely caused by defects in structure, synthesis, or post-translational processing of type I collagen. Drugs currently used to improve skeletal health in OI were initially developed to treat osteoporosis and clinical trials are ongoing to study their effectiveness in OI adults. Additionally, novel bone-protective agents are in preclinical studies and various phases of OI clinical trials. This review summarizes current knowledge on available pharmacologic agents and current drug trials involving OI par

Twelve Months of Denosumab and/or Alendronate Is Associated With Improved Bone Fatigue Life, Microarchitecture, and Density in Ovariectomized Cynomolgus Monkeys.

Prolonged use of antiresorptives such as the bisphosphonate alendronate (ALN) and the RANKL inhibitor denosumab (DMAb) are associated with rare cases of atypical femoral fracture (AFF). The etiology of AFF is unclear, but it has been hypothesized that potent osteoclast inhibitors may reduce bone fatigue resistance. The purpose of this study was to quantify the relationship between antiresorptive treatment and fatigue life (cycles to failure) in bone from ovariectomized cynomolgus monkeys. We analyzed humeral bone from 30 animals across five treatment groups. Animals were treated for 12 months

Rethinking Osteoporosis Drugs: Can We Simultaneously Address Sarcopenia?

Osteoporosis and sarcopenia are two aspects of the geriatric syndrome that frequently occur together and affect one another in a condition referred to as osteosarcopenia. Preventive and treatment options for osteosarcopenia exist but are mainly focused on the treatment of osteoporosis, as there is still no FDA-approved treatment for sarcopenia. Drugs for osteoporosis include antiresorptive and anabolic drugs and hormonal replacement therapies and are prescribed based on age, BMD and other patient characteristics, which, however, do not include the possible co-existence of sarcopenia. As severa
