Continuous decompression with intramedullary nailing for the treatment of unicameral bone cysts.

PURPOSE: To evaluate the efficacy of decompression of unicameral bone cysts (UBCs) of the long bones with intramedullary nailing and to compare responses to treatment according to location. MATERIALS AND METHODS: We evaluated 48 consecutive patients treated between January 1988 and June 2000. Mean age was 10.3 years. Mean follow-up was 9.8 years. Evaluation was performed according to the radiographic criteria of Capanna. RESULTS: UBCs were located in the proximal humerus (n = 24), humeral shaft (n = 2), proximal femur (n = 19), distal tibia (n = 2) and fibula (n = 1). A total of 62.5% presen

Extreme complications of Fixion nail in treatment of long bone fractures.

The authors present their experience related to extreme complications in treatment of diaphyseal fractures of the long bones with application of the Fixion expansion intramedullary nail in a total of 48 surgeries. We have encountered six (12.5%) extreme complications in the management of fractures of 3 humeral, 2 tibial, and 1 femoral bones during its application procedure and postoperative follow-up. Of six cases, two with humeral and tibial fractures developed nonunion and rotational instability because of failure of inflation of the Fixion nail. One of the Fixion nail in humerus was broken

[Locked nailing of fractures of the long bones. 1. Fractures of the leg].

The authors present an account of their initial experience with the use of a method of locked nailing of fractures of the leg, applying the technique described by Grosse and Kempf. They use original implants of Howmedica Co. They indicated for osteosynthesis closed fractures of the leg in the middle three fifths of the bone length, without major damage of soft tissues (grade 0 and 1 of Tscherne's classification). In injuries with imminent compartment syndrome they operated after a delay, in all others they operated early after injury. Using the described method, the authors operated 16 patient

[Stable flexible nailing of fractures of both bones of the forearm in children].

Flexible medullary nailing is a method of choice for fractures of the forearm in children. This technique is indicated when orthopaedic treatment has fallen, or for instable fracture. We reported forty four fractures of the forearm concerning children with an age ranged from 2.5 to 15 years. We obtained 38 good results (83.3%) and 6 complications (14.7%) which were related to growth disturbance, leading to limitation of pronation; in one case we obtained a rupture of extensor pollicis longus.

[In osteogenesis imperfecta no intramedullary nailing and especially no bone plates in childhood].

This is a review of 24 patients with osteogenesis imperfecta of all age groups. 7 severe cases have been followed closely during growth for at least 10 years after multiple series of osteotomies with stabilisation by rodding and plating. The conclusion of the disappointing late results consists in that plating has to be abandoned completely during growth. The indication for rodding has to be restricted severely because of the necessity for replacement of the nails about every two year until cessation of growth. Special operative indications are outlined.

Elastic Intramedullary Nailing of Pediatric Both-Bone Forearm Fractures.

BACKGROUND: Elastic intramedullary nailing of both-bone pediatric forearm fractures is used to treat unstable fractures that fail conservative management with closed reduction and casting, open injuries, or injuries with neurovascular compromise. DESCRIPTION: Small incisions are used to enter the medullary canal of the distal end of the radius and proximal part of the ulna, avoiding injury to the adjacent physis. The elastic nail is advanced to the fracture site under fluoroscopic guidance. The fracture is reduced with traction and manipulation, and the nail is passed across the fracture site

Aneurysmal bone cyst healing response with intramedullary lengthening nail.

We report the treatment process of a pediatric patient with deformity and shortening in the arm after a recurrent aggressive aneurysmal bone cyst (ABC) in the proximal humerus. The patient was treated with curettage of the lesion and lengthening on an intramedullary nail following an osteotomy just distal to the ABC. The period of lengthening was approximately 50 days. At the end of the treatment the lengthening goal was achieved without any neurovascular complication. There was a minimal loss in shoulder hyperabduction due to the deformity of the humeral head.

Angiolymphoid hyperplasia with eosinophilia affecting the nail bed and underlying bone.

A 69-year-old Japanese woman had a dark red, 1-cm nodule located in the nail bed of the middle toe of her right foot. An X-ray examination revealed an osteolytic lesion of the distal phalanx of the right middle toe. Histopathology showed a proliferation of blood vessels surrounded by epithelioid cells and an infiltration of many eosinophils and some inflammatory cells. There has been no recurrence after surgical excision of the skin lesion and the distal phalanx of the right middle toe.

The huckstep nail. Stable fixation of mechanically deficient femoral bone.

A Huckstep intramedullary nail was used to treat 23 fractures in which the femur was mechanically deficient due to pathologic fracture, nonunion, or complex fracture in osteoporotic bone. All nailings were performed by open surgery, and in some cases this was augmented by corticocancellous bone grafting. The Huckstep nail allows the use of multiple cross screws at 15-mm intervals, providing immediate and adequate stability with successful early postoperative weightbearing. The results at followup were good in functional and radiologic terms, with no cases of fixation failure or infection. The

Shortening/re-lengthening and nailing versus bone transport for the treatment of segmental femoral bone defects.

Segmental femoral bone defects are a severe challenge for orthopedic trauma surgeons. The objective of this study was to compare the efficacy of the shortening/re-lengthening and nailing (SRN) technique with the traditional bone transport (BT) technique in treating patients with such defects. A retrospective study was conducted involving 46 patients with segmental femoral bone defects, with 21 cases treated using the SRN technique (SRN group) and 25 cases managed with the traditional BT technique (BT group). The mean length of the bone defect was 5.8 ± 1.1 cm in the SRN group and 6.1 ± 1.6 cm

Closed Compression Nailing Using a New-Generation Intramedullary Nail without Autologous Bone Grafting for Humeral Shaft Nonunion.

INTRODUCTION: Although the recommended treatment for humeral shaft nonunion is compression plating with autologous bone grafting, we treated a case of humeral shaft nonunion with an intramedullary nail (IMN) without bone grafting. Presentation of Case. Osteosynthesis with IMN was performed on a 24-year-old man with a humeral shaft fracture at another hospital. However, bony union was not obtained 1 year after the first surgery, and he was referred to our institution. We treated the nonunion with exchange nailing without autologous bone grafting using compression function of the nail, leading t

Delayed primary reconstruction of subtotal nail bed loss using a split-thickness nail bed graft on decorticated bone.

Split-thickness nail bed grafting is the accepted method of treatment for injuries involving loss of nail bed tissue. The nail bed of the great toe may be used without donor-site morbidity, and nail bed grafting may be combined with other procedures for fingertip reconstruction. A case of fingertip avulsion injury with loss of the nail plate, nail bed, and periosteum over the exposed distal phalanx of the thumb was reconstructed by a split-thickness nail bed graft placed directly on granulating decorticated bone. The length, appearance, and function of the injured dominant thumb were preserved
