Posterior-inferior capsular shift for the treatment of recurrent, voluntary posterior subluxation of the shoulder.

BACKGROUND: The treatment of recurrent posterior instability of the shoulder, especially when it is associated with voluntary subluxation, remains controversial, and operative correction generally is not advised. METHODS: The results of operative correction of recurrent posterior subluxation in a consecutive series of twenty-six shoulders in twenty-four patients were reviewed. Eighteen shoulders were on the dominant side. The average age of the patients was twenty-four years (range, fifteen to thirty-three years). All of the patients had involuntary as well as voluntary posterior instability,

Femoral head allograft for glenoid bone loss in primary reverse shoulder arthroplasty: functional and radiologic outcomes.

BACKGROUND: Several techniques have been adopted during primary reverse shoulder arthroplasty (RSA) to manage glenoid bone defect. Among bone grafts, humeral head autograft is currently the mainstream option. However, autologous humeral heads may be unavailable or inadequate, and allografts may be a viable alternative. The aim of the present study was to evaluate the functional and radiologic outcomes of femoral head allografts for glenoid bone defects in primary RSA. METHODS: We conducted a retrospective study with prospective data collection enrolling 20 consecutive patients who underwent R

Study on long bones: variation in angular traits with sex, age, and laterality.

The biological problems considered in this study are sexual dimorphism in angular osteometric traits, changes in these traits with aging and their bilateral asymmetry in the same individual. The sample comprises the right and left long bones of 200 skeletons (100 males and 100 females) of Sardinian adults (Frasetto collection) for whom the sex and age at death are known. As concerns the sexual dimorphism, some angular traits of upper limb (joint axis angle of the ulna in particular) and the lower limb (condylo-diaphyseal angle of the femur and retroversion angle of the tibia) are significantly

B2 Glenoid in the Active 50-Year-Old With Severe Glenohumeral Osteoarthritis: It's So Confusing! What Should I Do?

In the young, active patient with osteoarthritis, the optimal treatment of B2 glenoid morphology remains a subject of continued debate. Current treatment options have specific advantages and disadvantages. These treatments include hemiarthroplasty with or without glenoid reaming, total shoulder arthroplasty (TSA) with or without eccentric reaming, TSA with bone graft or posteriorly augmented glenoid implant, and reverse TSA. Eccentric glenoid reaming can be effective but has the risk of excessive reaming of subchondral bone for greater retroversion deformities, compromising implant purchase wi

Medial femoral epicondyle upsliding osteotomy with posterior stabilized arthroplasty provided good clinical outcomes such as constrained arthroplasty in primary total knee arthroplasty with severe valgus deformity.

PURPOSE: A modified technique referred to as a medial femoral epicondyle upsliding osteotomy was proposed to address severe valgus deformity with unconstrained posterior stabilized (PS) arthroplasty. The study compared the effectiveness of the technique and PS arthroplasty with constrained arthroplasty during primary total knee arthroplasty (TKA). METHODS: Fifty-three patients presenting with valgus knees with a mean valgus angle (VA) greater than 30° were prospectively randomized and divided into two groups, and both groups received primary TKA. Upsliding osteotomy with PS arthroplasty was p

[Juvenile epiphyseolysis of the femoral head--coxa vara development and treatment].

Children's coxa vara is based on epiphyseolysis of the proximal femoral epiphysis due to relative overload of a primarily healthy metaphysis, or to a "normal" load on diseased structures. These give passively way in the direction of varus and retroversion of the head in the presence of fatigue fractures. Etiologically the idiopathic C. vara infantum can be differentiated from the symptomatic forms with known causes. Broadening of the epiphyseal line, spur formation and cystic bone resorption of the medial-caudad portions of the metaphysis are early x-ray signs. When the process involves the wh

Incidence and risk factors for failure of conservative treatment for valgus impacted femoral neck fractures in elderly patients with high-risk comorbidities-A bi-center retrospective observational study.

BACKGROUND: -Establishing the ideal indications for conservative treatment for elderly patients with valgus impacted femoral neck fractures is difficult because few studies have identified predictive factors for the failure of this treatment method. This study aimed to report the outcomes of conservative treatment for elderly patients with valgus impacted femoral neck fractures and determine risk factors that lead to treatment failure. METHODS: -From January 2009 to December 2018, 206 patients with valgus impacted femoral neck fracture were identified at two institutions. Elderly patients wit

Short-term outcomes of anatomic total shoulder arthroplasty with nonaugmented glenoid component for Walch B2 and B3 glenoid morphology.

BACKGROUND: The optimal management of posterior glenoid bone loss in glenohumeral osteoarthritis with anatomic total shoulder arthroplasty (aTSA) remains unknown, as the degree of bone loss and retroversion can vary across a wide spectrum in glenoids with Walch B2 and B3 morphology. The objectives of the current study were to evaluate the clinical and radiographic outcomes of utilizing standard, all-polyethylene, nonaugmented anchor-peg glenoid (APG) components in aTSA for patients with mild to moderate B2 or B3 morphology. METHODS: Between January 2010 and September 2019, we identified 79 sh

Posterior shoulder dislocation shows a higher, flatter acromion, and increased glenoid retroversion compared with anterior dislocation: A matched computed tomography/magnetic resonance imaging analysis.

INTRODUCTION: Posterior shoulder dislocation (PSD) is less common than anterior dislocation (ASD) but is increasingly recognized. Understanding the osseous characteristics of PSD is essential for guiding management. However, direct comparative radiographic analyses between PSD and ASD remain limited. The purpose of this study was to perform a matched-pair radiographic comparison of acromial, glenoidal, and (reverse) Hill-Sachs lesion morphology between patients with first-time traumatic PSD and ASD, and to identify osseous features that may predispose to posterior shoulder instability. METHOD

In vivo characterization of glenoid with use of computed tomography.

Improvement in the treatment of the shoulder could be achieved by accurately describing the pathologic characteristics of the joint. The goal of this study was to characterize, in vivo, glenoids with 3 different diagnoses by using computed tomography (CT): rotator cuff pathology with a limited rupture and without bony changes (group A, n = 15), primary osteoarthritis (group B, n = 13), and rheumatoid arthritis (group C, n = 4). The bone density distribution was assessed by means of the CT value expressed in Hounsfield units. The version angle was also measured. The examination of the CT value

Posterior Glenoid Bone Block Reconstruction: Indications, Techniques, and Outcomes.

Posterior shoulder instability is of particular therapeutic interest, as it typically affects patients with high functional demands such as young athletes and active adults. Although posterior capsulolabral repair has high return-to-sport rates, it is associated with recurrent instability of up to 11%. Posterior glenoid bone loss and significant glenoid retroversion have been identified as risk factors for recurrent instability and failure after primary arthroscopic soft-tissue repair. Therefore, posterior glenoid bone block reconstruction may be indicated for glenoid bone loss 20% or greater

Contralateral slip after unilateral slipped capital femoral epiphysis is associated with acetabular retroversion but not increased acetabular depth and overcoverage.

Overcoverage of the femoral head by the acetabulum, increased acetabular depth and retroversion have been associated with the etiology of slipped capital femoral epiphysis (SCFE). However, limited evidence exists about the impact of the acetabular morphology on the development of a contralateral slip following an initial presentation of unilateral SCFE. We aimed to investigate whether acetabular overcoverage as assessed by an increased lateral center-edge angle (LCEA) and low Tönnis angle, increased acetabular depth assessed by the acetabular depth-width ratio (ADR) and the presence of coxa pr

Femoral head retroposition as a potential compensatory mechanism in patients with a severe mismatch between pelvic incidence and lumbar lordosis.

OBJECTIVE: Severe mismatch between pelvic incidence (PI) and lumbar lordosis (LL) leads to extra anterior displacement of the gravity line. The objective of this study is to investigate whether femoral head retroposition is a separate compensatory mechanism responsible for the extra anterior displacement. SUBJECTS AND METHODS: Based on the values of PI and LL, 94 patients were divided into the PI-LL match group (PI-LL ≤ 0°), the mild PI-LL mismatch group (20°> PI-LL >0°), and the severe PI-LL mismatch group (PI-LL ≥ 20°). A series of parameters including PI, LL, PI-LL, thoracic kyphosis (TK),

Modified-tension band suturing: 'the pulling the reins technique' - a technique for displaced proximal humeral fracture with medial column instability.

Fractures of the proximal humerus with medial column instability are challenging and present an unacceptable rate of complications and reoperations. Despite good results reported with the use of locking plates and augmentation techniques using bone graft or a second plate, varus subsidence and fixation failure have been frequently reported. We describe the case of a patient presenting with a complex, multifragmentary proximal humerus fractures successfully treated with open anatomic reduction and internal fixation using a locking plate augmented with lateral traction using three bone anchors i

Pelvic deformity influences acetabular version and coverage in hip dysplasia.

BACKGROUND: Although a wide variety of acetabular deformities in developmental dysplasia of the hip (DDH) have been reported, the morphologic features of the entire pelvis in DDH are not well characterized and their correlation with acetabular deformity is unknown. QUESTIONS/PURPOSES: We determined whether there was a rotational deformity of the entire innominate bone, and if so, whether it related to acetabular version and coverage. PATIENTS AND METHODS: We examined the morphologic features of the pelvis using CT for 50 patients with DDH (82 hips). Forty normal hips were used as controls. T

Focal concavity of posterior superior acetabulum and its relation with acetabular dysplasia and retroversion in adults without advanced hip osteoarthritis.

BACKGROUND: Although little is known, a limited number of three-dimensional computed tomography (CT) images of the pelvis present focal concavity of posterior superior acetabulum. The purpose of the present study was to investigate this morphologic deformity and its relation with dysplasia and retroversion in adults who were expected to have the original morphology of the acetabulum after growth. METHODS: Consecutive adult patients with hip pain who visited our hospital and had three-dimensional pelvic CT images were retrospectively analyzed after approval of the institutional review board; e

The thrower's shoulder.

Throwers, or athletes who engage in repetitive overhead motions, are a unique subset of athletes that experience distinct shoulder injuries. Athletes engaged in baseball comprise the majority of patients seeking orthopedic care for throwing related injuries. Injuries specific to throwers most commonly involve the labrum and the undersurface of the rotator cuff. In addition, tissue changes in both the anterior and posterior glenohumeral capsule are common with repetitive overhead motions. These capsular changes alter. This article will examine the pathomechanics of injuries to throwers, elabora

High prevalence of acetabular retroversion in both affected and unaffected hips after Legg-Calvé-Perthes disease.

BACKGROUND: Acetabular retroversion is observed in hips after various pediatric hip diseases. This study sought to examine the frequency of acetabular retroversion in both affected and unaffected hips after Legg-Calvé-Perthes disease and its correlation with the prominence of the ischial spine. METHODS: We retrospectively investigated the version and morphological features of the acetabulum using pelvic radiographs after Legg-Calvé-Perthes disease (107 affected hips treated non-operatively and 72 unaffected hips from the contralateral side). The diagnosis of acetabular retroversion was made b

The presence of a negative sacral slope in patients with ankylosing spondylitis with severe thoracolumbar kyphosis.

BACKGROUND: Pelvic retroversion is one of the mechanisms for regulating sagittal balance in patients with a kyphotic deformity. This retroversion is limited by hip extension, which prevents the pelvis from becoming excessively retroverted, achieving a sacral slope of <0°. However, a negative sacral slope can be found in some patients with ankylosing spondylitis with thoracolumbar kyphosis. The purpose of this study was to analyze this finding. METHODS: We performed a retrospective review of 106 consecutive Chinese Han patients with ankylosing spondylitis with thoracolumbar kyphosis treated at
