Advances in orthotics and bracing.

Understanding of the adult acquired flatfoot deformity (AAFD) continues to grow, as does the sophistication of orthotics and braces used to treat this disorder. This article reviews these advances and some of the devices commonly used to treat patients who have AAFD. Additionally, the recent proliferation and potential implications of mass-manufactured products is discussed.

Prophylactic ankle bracing.

Many choices are available to athletes seeking an ankle support. The time-honored tradition of ankle taping with adhesive tape does offer protection against ankle sprains during activity. Laced stabilizers offer an equal or possibly greater amount of support, are less costly and easier to apply, and can be retightened frequently during activity. The physician should become familiar with one of these two methods and choose one based on availability and feasibility in the community. The air stirrup may be indicated for patients with a history of ankle injury who are undergoing a graduated rehabi

Bracing for the Patellofemoral Joint.

Patellofemoral disorders are common causes of knee pain that result in frequent visitations to musculoskeletal care clinics. Patellar tendinopathy, patellar instability and patellar maltracking, and pain are some of the most common pathologies resulting in patellofemoral dysfunction. For each of these diagnoses, there are unique orthoses and braces available, some of which are uniquely designed to address the pathology involved. While the spectrum of patellofemoral disorders is wide ranging and can often be challenging to treat, bracing frequently plays a large role in the overall treatment al

Bracing for patellar instability.

Knee pain secondary to problems with patellar tracking is common. The initial treatment for these problems is conservative. Patellar orthotics have been used extensively to treat a variety of extensor mechanism problems. Millions of health care dollars are spent annually on the use of these braces. There are many different patellar orthotics on the market today. The use and design of most of these devices are unclear and often lack a solid biomechanical foundation. The practical mechanical function of these braces is to reduce the lateral displacement of abnormal patellofemoral tracking. Ortho

Outcomes for nighttime bracing in adolescent idiopathic scoliosis based on brace wear adherence.

BACKGROUND: This study determined brace wear adherence for patients treated with nighttime braces and evaluated the effect of brace adherence on curve progression. METHODS: One hundred twenty-two patients with AIS ages 10-16 years, Risser stages 0-2, major curves 20°-40° treated with Providence nighttime braces prescribed to be worn at least 8 h per night were prospectively enrolled and followed until skeletal maturity or surgery. Brace adherence was measured using iButton temperature sensors after 3 months of brace initiation and at brace discharge. RESULTS: Curve types were single thoracol

Age and in-brace Cobb angle predict brace failure in adolescent idiopathic scoliosis.

PURPOSE: This study aimed to identify independent risk factors for brace treatment failure, examine the correlation between initial in-brace Cobb angle and curve progression, and assess the relationships among age, curve type, initial in-brace Cobb angle, and treatment success. METHODS: Eighty consecutive patients (76 girls and 4 boys; mean age: 12.0 ± 1.2 years) commenced treatment with an underarm brace and were followed for a minimum of 2 years after brace initiation. Brace treatment failure was defined as a final Cobb angle of ≥ 50°, surgery, or curve progression ≥ 6°. A spinal surgeon ev

Bracing for juvenile idiopathic scoliosis: retrospective review from bracing to skeletal maturity.

BACKGROUND: Juvenile idiopathic scoliosis (JIS) outcomes with brace treatment are limited with poorly described bracing protocols. Between 49 and 100% of children with JIS will progress to surgery, however, young age, long follow-up, and varying treatment methods make studying this population difficult. The purpose of this study is to report the outcomes of bracing in JIS treated with a Boston brace™ and identify risk factors for progression and surgical intervention. METHODS: This is a single-center retrospective review of 175 patients with JIS who initiated brace treatment between the age o

Scapular Bracing is Effective in Some Patients but Symptoms Persist in Many Despite Bracing.

BACKGROUND: A scapular-protecting brace is one option for treating patients with a winging scapula in isolated serratus palsy. However, outcomes after brace treatment have been reported in only a few studies, and to our knowledge, none has results reported at long-term beyond 10 years. QUESTIONS/PURPOSE: We asked: (1) What was the average length of time patients wore the brace? (2) Did scapular winging and ROM improve with brace treatment? (3) Was pain decreased? (4) Did the duration of symptoms before brace treatment influence the outcome? PATIENTS AND METHODS: Between 1980 and 1999, we tre

A 14% Rate of Surgery in Modern Rigo Chêneau Bracing for Adolescent Idiopathic Scoliosis: In-Brace Correction and Brace Wear Contribute to Treatment Success.

PURPOSE: Rigo Chêneau-style orthoses (RCSOs) have gained popularity for treating adolescent idiopathic scoliosis (AIS) due to their emphasis on 3-dimensional correction. However, previous studies lack objective measures of brace wear compliance and have not described in-brace correction (IBC) thresholds associated with treatment success. This study evaluated the relationship between IBC, brace wear, and treatment outcomes in AIS patients treated with RCSOs. METHODS: A retrospective review at a single academic institution included skeletally immature patients aged 10 to 18 years with AIS and a

Automated Caries Detection Under Dental Restorations and Braces Using Deep Learning.

In the dentistry field, dental caries is a common issue affecting all age groups. The presence of dental braces and dental restoration makes the detection of caries more challenging. Traditionally, dentists rely on visual examinations to diagnose caries under restoration and dental braces, which can be prone to errors and are time-consuming. This study proposes an innovative deep learning and image processing-based approach for automated caries detection under restoration and dental braces, aiming to reduce the clinical burden on dental practitioners. The contributions of this research are sum

Retention procedures for stabilising tooth position after treatment with orthodontic braces.

BACKGROUND: Without a phase of retention after successful orthodontic treatment, teeth tend to 'relapse', that is, to return to their initial position. Retention is achieved by fitting fixed or removable retainers to provide stability to the teeth while avoiding damage to teeth and gums. Removable retainers can be worn full- or part-time. Retainers vary in shape, material, and the way they are made. Adjunctive procedures are sometimes used to try to improve retention, for example, reshaping teeth where they contact ('interproximal reduction'), or cutting fibres around teeth ('percision'). This

Design of personalized scoliosis braces based on differentiable biomechanics-Synthetic study.

This work describes a computational methodology for the design of braces for adolescent idiopathic scoliosis. The proposed methodology relies on a personalized simulation model of the patient's trunk, and automatically searches for the brace geometry that optimizes the trade-off between clinical improvement and patient comfort. To do this, we introduce a formulation of differentiable biomechanics of the patient's trunk, the brace, and their interaction. We design a simulation model that is differentiable with respect to both the deformation state and the brace design parameters, and we show ho

Study on Photocuring Printing Technology of Polyurethane Acrylate-Based Transparent Braces.

A polyurethane acrylate-based photosensitive resin system was developed for 405 nm DLP printing. The formulations were prepared by blending commercial urethane acrylate RT75A with TPO-L, BBOT, and one of four reactive diluents (ACMO, IBOMA, TPGDA, or CTFA). Double-bond conversion was determined by ATR-FTIR, whereas viscosity, UV-Vis transmittance, working-curve parameters (critical exposure and penetration depth), volumetric shrinkage, and mechanical properties were evaluated. The reactive diluent markedly affected both the liquid-state behavior of the resin formulations and the properties of

Unloader braces for medial compartment knee osteoarthritis: implications on mediating progression.

BACKGROUND: For persons with unicompartment knee osteoarthritis (OA), off-unloader braces are a mechanical intervention designed to reduce pain, improve physical function, and possibly slow disease progression. Pain relief is thought to be mediated by distracting the involved compartment via external varus or valgus forces applied to the knee. In so doing, tibiofemoral alignment is improved, and load is shifted off the degenerative compartment, where exposure to potentially damaging and provocative mechanical stresses are reduced. OBJECTIVES: To provide a synopsis of the evidence documented i

Tibial shaft fractures treated with functional braces. Experience with 780 fractures.

We have reviewed our recent results with functional bracing of tibial shaft fractures in adults in order to define its role in management. We also analysed several parameters of these fractures to discover those which influence healing. A total of 780 tibial fractures treated in prefabricated functional braces were followed to union; shortening of less than 10 mm and angulation of less than 5 degrees in any plane were our parameters for successful treatment. The average time before applying a brace was 3.8 weeks for closed fractures and 5.2 weeks for open ones. Closed fractures healed in an av
