Effect of interscalene brachial plexus block on heart rate variability.

BACKGROUND: Interscalene brachial plexus block (ISB) may be followed by cardiovascular instability. Until date, there is no clear picture available about the underlying mechanisms of ISB. In this study, we aimed to determine the changes in heart rate variability (HRV) parameters after ISB and the differences between right- and left-sided ISBs. METHODS: We prospectively studied 24 patients operated for shoulder surgery in sitting position and divided them into two respective groups: R (right-sided block = 14 pts) and L (left-sided block = 10 pts). HRV data were taken before and 30 min after th

Nerve injury complicating ultrasound/electrostimulation-guided supraclavicular brachial plexus block.

BACKGROUND AND OBJECTIVES: Neurologic complications after peripheral nerve blocks (PNBs) are relatively uncommon. It has been postulated that real-time, needle-nerve visualization during ultrasound guided PNBs might further reduce the risk of neurologic or vascular complications. CASE REPORT: In this report, we describe the occurrence of a severe brachial plexus injury after combined ultrasound and nerve stimulator-guided supraclavicular brachial plexus block. CONCLUSIONS: Ultrasound guidance should not preclude development of additional monitoring and protocols to decrease the risk of compl

Bilateral vocal cord palsy following interscalene brachial plexus nerve block.

Acute respiratory complications of an interscalene brachial plexus block include ipsilateral phrenic nerve and recurrent laryngeal nerve palsies. A 71-year-old woman who had undergone a total thyroidectomy for papillary carcinoma 35 years ago was administered a right interscalene brachial plexus block for a shoulder hemi-arthroplasty. Subsequently she developed acute respiratory distress associated with marked stridor secondary to an acute right vocal cord palsy, which was superimposed on what was assumed to be a preexisting left-sided vocal cord palsy. On extubation the patient was noted to d

Novel analgesic adjuncts for brachial plexus block: a systematic review.

This article reviews current evidence for the efficacy of adding novel analgesic adjuncts to brachial plexus block, the goal of which is to prolong analgesic effect without the disadvantage of systemic side effects or prolonged motor block. It may also allow for a reduction in the total dose of local anesthetic used. Novel adjuncts studied to date include opioids, clonidine, neostigmine, and tramadol. Twenty-four studies were reviewed and assessed by using specific inclusion criteria, and only those studies satisfying these criteria were included in the final assessment. Satisfactory studies w

Axillary brachial plexus block for perioperative analgesia in 250 children.

A cannula technique for axillary brachial plexus block in combination with general anaesthesia has been in use since 1994 for children undergoing surgical correction of congenital hand anomalies. During a 4-year period data were collected on 250 procedures in 185 patients of median age 3 years detailing the block technique and the intraoperative and postoperative analgesic requirements. Fifteen patients (6%) required supplemental intravenous opioid intraoperatively and this is taken as a marker of failure of the block. Ninety-five patients (38%) required postoperative codeine phosphate with a

Costoclavicular brachial plexus block for shoulder surgery: a narrative review.

The costoclavicular block is a proximal approach for blocking the brachial plexus in the infraclavicular fossa. Whether the costoclavicular block offers advantages over lateral paracoracoid approaches has been debated. However, diaphragm-sparing anesthesia for shoulder surgery has recently reignited interest in the costoclavicular space. In this review, we examine the costoclavicular block as an alternative to the interscalene block for shoulder surgery, focusing on minimizing involvement of the phrenic nerve. We conducted a systematic search of MEDLINE, EMBASE, and Google Scholar databases us

Tracheal deviation with phrenic nerve palsy after brachial plexus block.

We present a case of a woman who received a left single-injection supraclavicular brachial plexus block for analgesia to facilitate upper extremity orthopaedic surgery. Before tracheal extubation she desaturated, was noted to have a low tidal volume and reduced left-sided air entry on auscultation of the chest. A chest x-ray taken 1 h following tracheal extubation revealed elevation of the left hemidiaphragm and a rightward shift of the trachea and mediastinal structures, with no evidence of pneumothorax. Findings were in-keeping with phrenic nerve palsy complicating the brachial plexus block

Phantom Sensations Following Brachial Plexus Nerve Block: A Case Report.

Following the administration of brachial plexus anesthesia for right thumb carpometacarpal arthroplasty with ligament reconstruction, a 54-year-old woman with all limbs intact developed phantom limb sensations, including the misperception of the placement of her right arm and frozen limb sensations in her fingers. Immobility of her fingers in a stacked position was experienced for ~3.5 days after surgery, and she described her phantom sensations as the hand experiencing "tingling" and feeling "heavy." While the onset of these phantom sensations occurred almost immediately after administration

[A case of prolonged hypercapnea after interscalene brachial plexus block].

An 83-year-old woman was scheduled for surgery of the left upper and lower extremity fracture. She had past history of lung partial resection for lung cancer and rheumatoid arthritis, and recent history of pneumonia. She also had fluid retention in the thoracic cavity. Open resection of the femoral neck fracture was first performed uneventfully under spinal anesthesia with bupivacaine 0.5% 2 ml. Then, interscalene blaxioplexus block was performed with 0.75% ropivacaine 15 ml and 1% lidocaine 10 ml for tension band wiring of the fractured olecranon. Delayed Horner's syndrome during a continuous infraclavicular brachial plexus block.

Horner's syndrome is a potential, albeit rare, feature of continuous infraclavicular brachial plexus local anesthetics infusion, mainly the result of anatomical considerations. Horner's syndrome may be described as an "unpleasant side effect" because it has no clinical consequences in itself. Nevertheless, patient discomfort and anxiety may reduce acceptance of the analgesic technique. Reassurance and close clinical monitoring of the patient are essential to enhance patient's safety and acceptance of the technique.

Vertical infraclavicular brachial plexus block in children: a preliminary study.

BACKGROUND: Brachial plexus blockade is a well-established technique in upper limb surgery. Among the infraclavicular approaches, the vertical infraclavicular brachial plexus (VIP) block is easy to perform and has a large spectrum of nerve blockade. The aim of this preliminary study was to determine the ease, effectiveness, safety, and duration of the VIP block in pediatric trauma surgery. METHODS: Fifty-five patients (ASA physical status I and II, age range 5-17 years old) scheduled for upper limb trauma surgery received a VIP block under light general anesthesia, using 0.5 ml x kg(-1) of ro

[Axillary brachial plexus block with lidocaine HCl and carbonated lidocaine].

An accordance with the clinical examination recommended by Lanz and Theiss, we studied 26 patients undergoing a blockade of the axillary plexus with lidocaine-CO2 or lidocaine-HCl. Furthermore, we determined the blood concentrations of these local anaesthetics. The study showed no significant differences between the two groups of patients, except that the onset of the blockade was more reliable in one group. There was less impairment of motor function than sensory function. Blood lidocaine concentrations amounted to 2.46 mg/l (mean value) within 25 min. Slight cerebral intoxication could be se

Plasma concentrations of local anaesthetics after interscalene brachial plexus block.

Plasma concentrations of local anaesthetic agents have been measured after 40 interscalene brachial plexus blocks in 39 patients, using lignocaine, prilocaine, bupivacaine and etidocaine. Lignocaine produced greater concentrations than prilocaine, and bupivacaine greater concentrations than etidocaine. The addition of adrenaline resulted in much lower concentrations in the case of all four agents.

Effects of Ultrasound-Guided Superficial Cervical Plexus Block With Clavipectoral Fascial Plane Block Versus Superficial Cervical Plexus Block With Interscalene Brachial Plexus Block in Clavicle Surgery: A Prospective Study.

Background Perioperative pain control in clavicle fixation is challenging due to complex innervation and limitations of conventional anesthesia. Regional techniques provide safer, opioid-sparing alternatives. This study compared the efficacy and safety of superficial cervical plexus block with clavipectoral fascial plane block versus interscalene brachial plexus block in clavicle fixation surgery. Methodology In this prospective randomized study, 60 patients scheduled for open reduction and internal fixation of clavicle fractures were allocated into two groups of 30 each. Group 1 received supe
