OBJECTIVE: We investigated the feasibility of open heart surgery with combined central vascular surgery and present the results from 9 years of experience.
PATIENTS AND METHODS: Of a total of nine patients, eight received coronary artery bypass grafting and one patient aortic valve replacement. Concerning vascular surgery a replacement of the brachiocephalic trunk using a Dacron prosthesis as an end-to-end anastomosis or as a bifurcations prosthesis was performed. OBJECTIVE: To evaluate the results of a technique described by Carreira et al. using bilateral antegrade selective cerebral perfusion by isolating the brachiocephalic trunk and the left carotid artery.
METHODS: Fifteen patients were operated between June 2005 and September 2007. Data analysis were performed using Epi Info and statistical significance was set at p<0.05.
RESULTS: Of the 15 patients, 53.3% were male; mean age was 59.86+/-15.4 years; 60% presented with type A acute aortic dissection; 6.7% type B; and 33.3% aneurysm of aorta and arch. Mean CPB and aortic clamping time was 177.6+/

The article deals with a case report of successful secondary surgical treatment of a patient who endured prosthetic repair of the brachiocephalic trunk for a traumatic false aneurysm of the right subclavian artery complicated by thrombosis of the reconstruction zone. BACKGROUND: The right aortic arch with mirror-image of branching arteries without coexisting congenital heart disease is a very rare anomaly.
CASE REPORT: We report a case of the right-sided aortic arch with aplasia of the left brachiocephalic trunk in a 64-year-old women, presenting difference in systolic blood pressure between upper extremities. The history of the patient and angio-CT findings were described and visualized with images.
CONCLUSIONS: The knowledge of vascular variations is important for the clinical and therapeutic aspects.

Our patient had 80% stenosis of the brachiocephalic artery and total occlusion of the left carotid and left subclavian arteries. Ascending aorta to brachiocephalic artery bypass grafting was performed, with a 10 mm Dacron graft. The right axillary artery was cannulated, and during construction of the distal anastomosis cerebral blood flow was from the right axillary artery. We believe this technique may be beneficial in surgery on an artery in which cerebral blood flow depends exclusively.
