Mechanisms underlying vulnerabilities after repeat mild traumatic brain injuries.

Traumatic brain injury (TBI) has drawn national attention for its high incidence and mechanistic complexity. The majority of TBI cases are "mild" in nature including concussions and mild TBI (mTBI). Concussions are a distinct form of mTBI where diagnosis is difficult, quantification of the incidence is challenging and there is greater risk for subsequent injuries. While concussions occur in the general population, it has become a hallmark injury consistently observed among adolescent and young adult athletes and the risks for repeat TBI (rTBI) is significant. Clinical and experimental evidence

Headache in traumatic brain injuries from blunt head trauma.

OBJECTIVE: To determine the risk of traumatic brain injuries (TBIs) in children with headaches after minor blunt head trauma, particularly when the headaches occur without other findings suggestive of TBIs (ie, isolated headaches). METHODS: This was a secondary analysis of a prospective observational study of children 2 to 18 years with minor blunt head trauma (ie, Glasgow Coma Scale scores of 14-15). Clinicians assessed the history and characteristics of headaches at the time of initial evaluation, and documented findings onto case report forms. Our outcome measures were (1) clinically impor

Horseback-riding-associated traumatic brain injuries--Oklahoma, 1992-1994.

Each year, traumatic brain injury (TBI) is associated with 52,000 deaths and accounts for one third of all injury deaths in the United States; in addition approximately 80,000 persons who survive TBI incur some loss of function, residual disability, and increased medical-care needs because of these injuries. Major causes of TBI are motor-vehicle crashes, falls, assaults, and sports and recreational activities. During 1992-1993 in Oklahoma, horseback riding was the leading cause of sports-related TBI. To further characterize horseback-riding-associated TBIs, the Oklahoma State Department of Hea

Traumatic Head and Brain Injuries in Helmeted Motorcycle Crashes.

This study presents an analysis of 364 motorcycle helmet impact tests, including standard certified full-face, open-face, and half-helmets, as well as non-certified (novelty) helmet designs. Two advanced motorcycle helmet designs that incorporate technologies intended to mitigate the risk of rotational brain injuries (rTBI) were included in this study. Results were compared to 80 unprotected tests using an instrumented 50th percentile Hybrid III head form and neck at impact speeds ranging from 6 to 18 m/s (13 to 40 mph). Results show that, on average, the Head Injury Criterion (HIC) was reduce

Maxillofacial injuries and traumatic brain injury--a pilot study.

BACKGROUND: Maxillofacial injuries comprising hard tissue as well as soft tissue injuries can be associated with traumatic brain injuries due to the impact of forces transmitted through the head and neck. To date, the role of maxillofacial injury on brain injury has not been properly documented with some saying it has a protective function on the brain while others opposing this idea. AIM: This cross-sectional retrospective study evaluated all patients with maxillofacial injuries. The aim of the study was to analyze the occurrence and relationship of maxillofacial injuries with traumatic brai

Predictors for traumatic brain injuries evaluated through accident reconstructions.

The aim of this study is to evaluate all the 58 available NFL cases and compare various predictors for mild traumatic brain injuries using a detailed and extensively validated finite element model of the human head. Global injury measures such as magnitude in angular and translational acceleration, change in angular velocity, head impact power (HIP) and HIC were also investigated with regard to their ability to predict the intracranial pressure and strains associated with injury. The brain material properties were modeled using a hyperelastic and viscoelastic constitutive law. Also, three diff

Diagnosis, physiology, pathology and rehabilitation of traumatic brain injuries.

Accumulating clinical and experimental studies continue to elucidate and further define the significance of intra- and extra-cranial factors which determine outcome of traumatic brain injury. These factors include severity of injury, age of the patient, presence or absence of premorbid brain insults, and associated pathophysiological events such as anoxia, respiratory arrest, hemorrage, edema, contrecoup and Wallerian degeneration. Following resolution of acute temporary symptoms, delayed complications include seizures, neurotic and psychotic disorders, earlier onset of stroke, earlier senesce

Traumatic brain injury and delayed sequelae: a review--traumatic brain injury and mild traumatic brain injury (concussion) are precursors to later-onset brain disorders, including early-onset dementia.

Brain injuries are too common. Most people are unaware of the incidence of and horrendous consequences of traumatic brain injury (TBI) and mild traumatic brain injury (MTBI). Research and the advent of sophisticated imaging have led to progression in the understanding of brain pathophysiology following TBI. Seminal evidence from animal and human experiments demonstrate links between TBI and the subsequent onset of premature, psychiatric syndromes and neurodegenerative diseases, including Alzheimer's disease (AD) and Parkinson's disease (PD). Objectives of this summary are, therefore, to instil

Traumatic brain injury, neuroinflammation, and post-traumatic headaches.

Concussions following head and/or neck injury are common, and although most people with mild injuries recover uneventfully, a subset of individuals develop persistent post-concussive symptoms that often include headaches. Post-traumatic headaches vary in presentation and may progress to become chronic and in some cases debilitating. Little is known about the pathogenesis of post-traumatic headaches, although shared pathophysiology with that of the brain injury is suspected. Following primary injury to brain tissues, inflammation rapidly ensues; while this inflammatory response initially provid

Contribution of Extracranial Injuries to GOSE Scores after Traumatic Brain Injury TBI: A TRACK-Traumatic Brain Injury Study.

The Glasgow Outcome Scale Extended (GOSE) is the most widely used outcome measure for hospital-based studies of traumatic brain injury (TBI). The GOSE may be administered several ways, the choice depending on the purpose of the research. In this investigation, we evaluated the effect of administering the GOSE to collect functional disability attributed to all injuries sustained (GOSE-All) or excluding the impact of extracranial injuries (GOSE-TBI). We examined the differences in reported disability between the two administration methods at 2 weeks, 3 months, 6 months, and 12 months after injur

Neurosurgical Teleconsultation Safety for Mild Traumatic Brain Injuries and Low-Grade Blunt Cerebrovascular Injuries.

BACKGROUND AND OBJECTIVES: Mild traumatic brain injuries, stable spine fractures, and low-grade blunt cerebrovascular injuries are frequent at level I trauma centers and often prompt neurosurgery consultation, regardless of clinical status. We evaluated the safety and utility of virtual consultations (teleconsultations [TECs]) in the management of these injuries. METHODS: We performed a retrospective review of 260 patients admitted to a level I trauma center from 2020 to 2022. Groups were divided by consultation type, with 210 in-person consultations and 48 TECs. Propensity score matching, ac

Brain interstitial glycerol correlates with evolving brain injury in paediatric traumatic brain injury.

PURPOSE: A better understanding of the complex pathophysiology of traumatic brain injury (TBI) is needed to improve our current therapies. Cerebral microdialysis (CMD) is an advanced method to monitor the brain, but little is known about its parameters in children. Brain glycerol, one of the CMD variables, is an essential component of the phospholipid bilayer cell membrane and is considered a useful marker of tissue hypoxia in adults. This study examined the time course of glycerol and its associations in paediatric TBI. METHODS: In this retrospective cohort study, we collected data on childr

Derivation of the Quebec Brain Injury Categories for complicated mild traumatic brain injuries.

OBJECTIVE: Approximately 10% of patients with mild traumatic brain injury (TBI) present with intracranial bleeding, and only 3.5% eventually require neurosurgical intervention, which often necessitates interhospital transfer. Better guidelines and recommendations are needed to manage complicated mild TBI in the emergency department (ED). The main objective of this study was to derive a clinical decision rule, the Quebec Brain Injury Categories (QueBIC), to predict the risk of adverse outcomes for complicated mild TBI in the ED. The secondary objective was to compare the QueBIC's performance wi

Traumatic microbleeds suggest vascular injury and predict disability in traumatic brain injury.

Traumatic microbleeds are small foci of hypointensity seen on T2*-weighted MRI in patients following head trauma that have previously been considered a marker of axonal injury. The linear appearance and location of some traumatic microbleeds suggests a vascular origin. The aims of this study were to: (i) identify and characterize traumatic microbleeds in patients with acute traumatic brain injury; (ii) determine whether appearance of traumatic microbleeds predict clinical outcome; and (iii) describe the pathology underlying traumatic microbleeds in an index patient. Patients presenting to the

Benzoate Drugs for Traumatic Brain Injury.

Traumatic brain injury (TBI) is an acute disorder with prolonged complexities that affirm to be the leading cause of deaths and disabilities in US. The pathophysiology of TBI comprises of multiple complex processes. An impactful insult results in tissue damage, neuroinflammation, impaired cerebral blood flow, BBB disruption, and demyelination, leading to neurodegeneration and functional impairments. There are no specific treatments for TBI. Recent studies highlight the potential efficacy of the following benzoates for attenuating TBI pathologies in rodents: Sodium benzoate, a metabolite of cin
