The neuroanatomy of Broca's aphasia.

INTRODUCTION: Broca's aphasia, a condition characterized by nonfluent speech and difficulty with language production, results from focal brain damage and is most often caused by stroke. Although traditionally linked to lesions in Broca's area (Brodmann areas 44 and 45 in the left inferior frontal gyrus), recent evidence suggests that the neuroanatomy of Broca's aphasia is far more complex, implicating a broader network of cortical and subcortical regions. This study aimed to delineate the specific cortical and white matter features that, when damaged, lead to persistent Broca's aphasia. METHO

Intraoperative motor speech mapping under general anesthesia using long-latency response from laryngeal muscles.

OBJECTIVE: The aim of our study was to determine any association between preservation of long latency response evoked by electrophysiological mapping of the caudal part of the pars opercularis (inferior frontal gyrus Broca area) and postoperative speech function after tumour removal in patients under general anesthesia. PATIENTS AND METHODS: Twelve native Turkish-speaking patients with tumors in the dominant left frontal lobe, near the Broca area, were included in a single-center prospective cohort study. Hooked-wire electrodes were placed in both cricothyroid muscles (CTHY) before anesthesia

Beyond Broca: neural architecture and evolution of a dual motor speech coordination system.

Classical neural architecture models of speech production propose a single system centred on Broca's area coordinating all the vocal articulators from lips to larynx. Modern evidence has challenged both the idea that Broca's area is involved in motor speech coordination and that there is only one coordination network. Drawing on a wide range of evidence, here we propose a dual speech coordination model in which laryngeal control of pitch-related aspects of prosody and song are coordinated by a hierarchically organized dorsolateral system while supralaryngeal articulation at the phonetic/syllab

Development of information sharing in language neocortex in childhood-onset drug-resistant epilepsy.

OBJECTIVE: We studied age-related dynamics of information sharing among cortical language regions with electrocorticographic high-gamma modulation during picture-naming and story-listening tasks. METHODS: Seventeen epilepsy patients aged 4-19 years, undergoing extraoperative monitoring with left-hemispheric subdural electrodes, were included. Mutual information (MI), a nondirectional measure of shared information, between 16 pairs of cortical regions of interest, was computed from trial-averaged 70-150 Hz power modulations during language tasks. Impact of age on pairwise MI between language r

Fundamental or forgotten? Is Pierre Paul Broca still relevant in modern neuroscience?

The ability to speak is a unique human capacity, but where is it located in our brains? This question is closely connected to the pioneering work of Pierre Paul Broca in the 1860s. Based on post-mortem observations of aphasic patients' brains, Broca located language production in the 3rd convolution of the left frontal lobe and thus reinitiated the localizationist view of brain functions. However, contemporary neuroscience has partially rejected this view in favor of a network-based perspective. This leads to the question, whether Broca's findings are still relevant today. In this mini-review,

Chorea and Broca aphasia induced by diabetic ketoacidosis in a type 1 diabetic patient diagnosed as Moyamoya disease.

We here report one case of hemichorea and Broca aphasia occurred with diabetic ketoacidosis. A 20-year-old woman with type 1 diabetes mellitus had experienced diabetic ketoacidosis fourth time after the onset of diabetes. At the third ketoacidotic episode, the patient was admitted to our hospital for the first time to show hemichorea of the left extremities. Brain computed tomography (CT) demonstrated a high-density area in the right caudate head and low-density area in the right putamen. Magnetic resonance angiography (MRA) demonstrated a stenosis at the root of the bilateral middle and anter

Application of Concomitant Transcranial Direct Current Stimulation (tDCS) and Cognitive Behavioral-Oriented Training (CBT) for Pragmatic Skills Improvement in Young Adults with Autism Spectrum Disorder (ASD): Preliminary Data from a Pilot Study.

Objectives: Individuals with Autism Spectrum Disorder (ASD) exhibit difficulties in the social use of language, regardless of age, cognitive abilities, and symptom severity. The left Broca's area and adjacent cortex are crucial for socio-pragmatic language, particularly in retrieving and integrating context-dependent words. Neuroimaging studies in ASD have shown hypoactivation of the Broca's area and an aberrant pattern of functional connectivity between language-related regions, suggesting their potential involvement in socio-communicative deficits. Given the potential of tDCS to modulate bra

Morphological and hemispheric and sex differences of the anterior ascending ramus and the horizontal ascending ramus of the lateral sulcus.

Broca's area is composed of the pars opercularis (PO) and the pars triangularis (PTR) of the inferior frontal gyrus; the anterior ascending ramus of the lateral sulcus (aals) separates the PO from the PTR, and the horizontal ascending ramus of the lateral sulcus (hals) separates the PTR from the pars orbitalis. The morphometry of these two sulci maybe has potential effects on the various functions of Broca's area. Exploring the morphological variations, hemispheric differences and sex differences of these two sulci contributed to a better localization of Broca's area. BrainVISA was used to rec

Broca's biases in modern aphasiology.

Broca's description of aphemia signals the emergence of aphasiology as a discipline. It also marks the beginning of a long tradition of studies of right-handed patients ("Nous parlons avec l'hémisphère gauche") with lesions in the inferior frontal gyrus (Broca's area) displaying production deficits (Broca's aphasia) in their single, mainly Romance or, as years went by, Germanic language. In modern-day aphasiology, language crosses hemispheric boundaries, production is no longer confined to the left inferior frontal gyrus, and lesions in Broca's area do not necessarily entail Broca's aphasia, w

Neural correlates of syntactic comprehension: A longitudinal study.

Broca's area is frequently implicated in sentence comprehension but its specific role is debated. Most lesion studies have investigated deficits at the chronic stage. We aimed (1) to use acute imaging to predict which left hemisphere stroke patients will recover sentence comprehension; and (2) to better understand the role of Broca's area in sentence comprehension by investigating acute deficits prior to functional reorganization. We assessed comprehension of canonical and noncanonical sentences in 15 patients with left hemisphere stroke at acute and chronic stages. LASSO regression was used t

Functional neuroanatomy of language without speech: An ALE meta-analysis of sign language.

Sign language (SL) conveys linguistic information using gestures instead of sounds. Here, we apply a meta-analytic estimation approach to neuroimaging studies (N = 23; subjects = 316) and ask whether SL comprehension in deaf signers relies on the same primarily left-hemispheric cortical network implicated in spoken and written language (SWL) comprehension in hearing speakers. We show that: (a) SL recruits bilateral fronto-temporo-occipital regions with strong left-lateralization in the posterior inferior frontal gyrus known as Broca's area, mirroring functional asymmetries observed for SWL. Left posterior superior temporal gyrus and its structural connectivity in schizophrenia.

The left posterior superior temporal gyrus (pSTG) is thought to be involved in the pathophysiology and core symptoms of schizophrenia, although its structural connectivity has not yet been systematically investigated. Here, we aimed to evaluate its white matter (WM) connectivity with Broca's area, the thalamus, and the right pSTG. Eighty-three patients with schizophrenia and 141 healthy controls underwent diffusion-weighted imaging and T1-weighted three-dimensional magnetic resonance imaging. Probabilistic tractography was performed from the left pSTG to the Broca area, the left thalamus, and

The Neurobiology of Agrammatic Sentence Comprehension: A Lesion Study.

Broca's area has long been implicated in sentence comprehension. Damage to this region is thought to be the central source of "agrammatic comprehension" in which performance is substantially worse (and near chance) on sentences with noncanonical word orders compared with canonical word order sentences (in English). This claim is supported by functional neuroimaging studies demonstrating greater activation in Broca's area for noncanonical versus canonical sentences. However, functional neuroimaging studies also have frequently implicated the anterior temporal lobe (ATL) in sentence processing m

A Parisian spring: the debate on language localization at the Imperial Academy of Medicine, Paris, April 4-June 13, 1865.

The localization of articulate language (speech) to the posterior third of the third left frontal convolution-Broca's area-did not occur to Broca as he reported the case of his first aphasic patient in 1861. Initially Broca localized articulate language to both frontal lobes, a position that he maintained for 4 years after publishing his first case. In the interval, the Academy of Medicine in Paris had received a copy of a paper authored in 1836 by Marc Dax, in which Dax claimed that the ability to speak resides within the left hemisphere alone. The Academy of Medicine convened in the spring o

[Paul Broca: The Origin of Brain Science of Language].

Pierre Paul Broca studied the brains of patients with articulatory language deficits and reported that articulatory function is localized in the frontal cortex of the left hemisphere. This discovery of functional localization laid the foundation of neuroscience in general and marked the beginning of brain science of language. According to recent reexaminations of the patients' brains, the lesions responsible for apraxia of speech may instead involve the insula or medial fiber connections. Based on the results of functional neuroimaging and lesion studies, we have identified a "grammar center"

A consistent white-matter lesion is associated with network-level structural and functional disconnection in chronic Broca's aphasia.

Broca's aphasia has traditionally been attributed to cortical damage in Broca's area; however, emerging evidence suggests broader network-level dysfunction. This study investigated whether a consistent white-matter lesion underlies Broca's aphasia and how its structural dysconnectivity (SDC) relates to functional network disruption and severity. We analyzed multimodal MRI data from patients with chronic Broca's aphasia (n = 58). Voxel-based lesion overlap mapping (VLOM) revealed a focal white-matter lesion beneath the left central sulcus in 90% of patients with Broca's aphasia. Logistic regres

Language processing in Internet use disorder: Task-based fMRI study.

Internet use disorder (IUD) is generally conceptualized as a fast-growing behavioral addiction. Several structural and functional brain alterations have been revealed in this condition, but previous behavioral studies indicated that language systems may also be impaired. We used a silent word generation task to induce brain activation in Broca's area and other parts of the language system. Blood-oxygen-level-dependent activation analysis and psychophysiological interaction analysis were applied to assess functional brain changes. IUD was measured by the Problematic Internet Use Questionnaire a

Language as a Predictor of Motor Recovery: The Case for a More Global Approach to Stroke Rehabilitation.

Stroke is the third leading cause of death in the developed world and the primary cause of adult disability. The most common site of stroke is the middle cerebral artery (MCA), an artery that supplies a range of areas involved in both language and motor function. As a consequence, many stroke patients experience a combination of language and motor deficits. Indeed, those suffering from Broca's aphasia have an 80% chance of also suffering hemiplegia. Despite the prevalence of multifaceted disability in patients, the current trend in both clinical trials and clinical practice is toward compartme

Microinjection of neurotensin into the ventral tegmental area produces hypothermia: evaluation of dopaminergic mediation.

Neurotensin-producing perikarya and fibers have been identified in the ventral tegmental area of the rat, and recent microinjection studies indicate that neurotensin may function in the ventral tegmental area to regulate body temperature. In this study, the hypothermic response produced by intraventral tegmental injection of neurotensin was shown to be dose-dependent, with a threshold dose between 0.25 and 0.75 micrograms. When fluphenazine, a dopamine receptor antagonist, was microinjected into various forebrain nuclei simultaneous with neurotensin infusion into the ventral tegmental area, it

[Study on brain mechanism of rehabilitation training of articulation disorder in cleft lip and palate patients based on functional magnetic resonance imaging].

The cleft lip and palate (CLP) is one of the most common craniofacial malformations in humans. We collected functional magnetic resonance data of 23 CLP patients before rehabilitation training (Bclp) and 23 CLP patients after rehabilitation training (Aclp), who were performing Chinese character pronunciation tasks, and performed brain activation analysis to explore the changes of brain mechanism in CLP patients after articulation disorder rehabilitation training. The study found that Aclp group had significant activation in the motor cortex, Broca area, Wernicke area and cerebellum. While the
