Mucolytics for bronchiectasis.

BACKGROUND: Bronchiectasis is predominantly an acquired disease process representing the end stage of a variety of unrelated pulmonary insults. It is defined as a persistent irreversible dilatation and distortion of medium-sized bronchi. Patients diagnosed with bronchiectasis frequently have difficulty exporating the infected sputum. Mucolytic agents target hyper-secretion or changed physiochemical properties of sputum to make it easier to clear. One drug, recombinant human DNase, breaks down the DNA that is released at the site of infection by neutrophils. OBJECTIVES: The objective of this r

The Renaissance of Bronchiectasis.

Bronchiectasis now has an expanding landscape with a research growth rate exceeding that of asthma and COPD (9). We have summarized the top ten major shifts in domains of this landscape. In addition to the gaps mentioned above, further growth also needs to focus on prevention and ensure that consumers and underserved populations are included to strengthen equity (including in medication and RCT access), enhance relevance, and translate advances into meaningful improvements in care and outcomes for both children and adults with bronchiectasis.

Bronchiectasis in immunodeficiency.

PURPOSE OF REVIEW: As the field of bronchiectasis continues to advance, so does the recognition of undiagnosed immunodeficiency as a potential contributor. This review aims to highlight what is currently known about bronchiectasis in immunodeficiency as well as explore future therapeutics and needs. RECENT FINDINGS: Guidelines emphasize the need for evaluation for underlying immunodeficiency in patients with known bronchiectasis. Despite this known association, evaluation continues to be variable in clinical practice resulting in potentially missed therapeutic options, such as immunoglobulin

Eosinophils in bronchiectasis.

PURPOSE OF REVIEW: Bronchiectasis has traditionally been viewed as a predominantly neutrophil-driven airway disease. Recognition of eosinophilic inflammation in a substantial subset of patients has challenged this paradigm. This review summarizes emerging evidence regarding its pathogenesis, clinical significance, and therapeutic implications. RECENT FINDINGS: Eosinophilic bronchiectasis is identified using blood or sputum eosinophilia, although optimal biomarkers and thresholds are unclear. Eosinophilic inflammation occurs in bronchiectasis and overlapping disease states characterized by typ

Biomarkers in bronchiectasis.

Bronchiectasis is a heterogeneous disease with multiple aetiologies and diverse clinical features. There is a general consensus that optimal treatment requires precision medicine approaches focused on specific treatable disease characteristics, known as treatable traits. Identifying subtypes of conditions with distinct underlying biology (endotypes) depends on the identification of biomarkers that are associated with disease features, prognosis or treatment response and which can be applied in clinical practice. Bronchiectasis is a disease characterised by inflammation, infection, structural l

[Radiology of bronchiectasis].

Bronchiectasis is an irreversible bronchial dilatation. It is chronically progressive through a vicious circle of secretion retention, infection, inflammation and structural damage. The underlying causes are diverse and the severity of the disease is variable, which makes both the diagnostics and treatment challenging. Computed tomography (CT) is the gold standard in the diagnosis of bronchiectasis and can be helpful in clarifying the etiology. The type of bronchiectasis, the distribution of the bronchiectasis within the lungs and associated findings are particularly relevant. Imaging is also

Bronchiectasis in Childhood.

Bronchiectasis is one of the diagnostic entities in the spectrum of pediatric chronic wet cough, causing significant morbidity and mortality. The pathobiology involves a vicious cycle of repeated infections, airway inflammation, dysregulated immunity, and tissue remodeling, resulting in impaired airway clearance, destruction of structural elements within the bronchial wall causing them to become dilated, and small-airway obstruction. Pediatric pathobiological studies are lacking, although there are recent data on the role of antibiotics in treating and preventing exacerbations. The focus has m

Cough and bronchiectasis.

Bronchiectasis is a chronic lung disease with permanent airway dilatation, mucus retention and recurrent lower respiratory tract infections. Bronchiectasis is increasing in prevalence and has a significant morbidity and an excess mortality rate over age matched controls. It is increasingly identified during investigations into chronic cough and is evident high resolution CT scanning. There remain significant knowledge gaps in our understanding of the epidemiology, pathophysiology, prognosis and optimal treatments in bronchiectasis. This article reviews current concepts in bronchiectasis and fo

BACKGROUND: Bronchiectasis is predominantly an acquired disease process that represents the end stage of a variety of unrelated pulmonary insults. It is defined as persistent irreversible dilatation and distortion of medium-sized bronchi. It has been suggested that with widespread use of high-resolution computed tomography, more bronchiectasis diagnoses are being made. Patients diagnosed with bronchiectasis frequently have difficulty expectorating sputum. Sputum therefore is retained in the lungs and may become infected, leading to further lung damage. [Surgery for bronchiectasis].

The incidence of bronchiectasis has declined significantly in industrialized countries and its management has also changed because of the progress of antibiotic therapy. However, for some patients, medical treatment is not sufficient to control the disease and the quality of life is affected. Surgical treatment is then a very good alternative, when a gesture of complete resection of the affected areas is feasible in terms of lung function and it allows, with a low morbidity and mortality, for very satisfactory long-term results and slows down the progression of the disease. In cases of diffuse

Imaging of bronchiectasis.

Despite limitations, high-resolution computed tomography (HRCT) continues to be the gold standard for establishing the anatomic extent and severity of bronchiectasis. HRCT allows insight into the physiologic consequences of bronchiectasis. Recent reports show that there is good correlation between measurements of the density of airway walls and airway obstruction in patients with chronic obstructive pulmonary disease. Measuring peak airway wall density may prove of value for assessing the severity of bronchiectasis as well as potentially serving as a biomarker for measuring response to therapy

Pathogenesis of bronchiectasis.

Non-cystic fibrosis bronchiectasis is a heterogeneous condition and its pathogenesis is still not well defined. A combination of a defect in host defense and bacterial infection allows microbial colonization of the airways resulting in chronic inflammation and lung damage. An ongoing cycle of infection and inflammation may be established. Typically, the walls of the small airway are infiltrated by inflammatory cells causing obstruction whilst mediators, such as proteases released predominantly by neutrophils, damage the large airways resulting in bronchial dilatation. Adjacent parenchyma is al

The pathophysiology of bronchiectasis.

Bronchiectasis is defined by permanent and abnormal widening of the bronchi. This process occurs in the context of chronic airway infection and inflammation. It is usually diagnosed using computed tomography scanning to visualize the larger bronchi. Bronchiectasis is also characterized by mild to moderate airflow obstruction. This review will describe the pathophysiology of noncystic fibrosis bronchiectasis. Studies have demonstrated that the small airways in bronchiectasis are obstructed from an inflammatory infiltrate in the wall. As most of the bronchial tree is composed of small airways, t

Bronchiectasis in children.

Bronchiectasis is a chronic disease of the conducting airways that produces persistent productive cough, recurrent respiratory infectious exacerbations, and obstructive lung disease in children and adults. This article focuses on the grading and recommendations for chronic therapies of bronchiectasis caused by cystic fibrosis (CF)- and non-CF-related conditions. The scope of this article is to focus on outpatient treatment and not include as-needed treatment for mild or severe pulmonary exacerbations associated with bronchiectasis.

Management of bronchiectasis.

Around 1 in 1,000 adults in the UK has bronchiectasis, a potential outcome of various underlying lung diseases. It results from a 'vicious circle' of recurrent infection, inflammation, excessive mucus production, reduced mucociliary clearance, and dilatation and destruction of bronchi. Associated damage to blood vessels can lead to haemoptysis, which is usually slight, but may be severe. Bronchiectasis may be limited to one area (localised) or be more widespread (generalised). It can reduce lung function and quality of life, and may lead to respiratory failure and premature death. Here we revi
