[Clinical effect of arthroscopy-assisted minimally invasive management of bunion].

OBJECTIVE: To evaluate the effect of arthroscopy-assisted minimally invasive management of bunion and hallux valgus deformities. METHODS: Total 50 patients (53 feet) with bunion and hallux valgus deformities were treated under arthroscopy from July 2008 to July 2011, with an average age of 42.3 years old (ranging from 30 to 65 years old) involving 19 left feet, 28 right feet and 3 both feet. The American Orthopedic Foot and Ankle Society (AOFAS) hallux metatarsophalangeal-interphalangeal(MP-IP) Scale Score was used to evaluate the therapeutic effect. RESULTS: There were no complications such

Radiographic evaluation for AVN following distal metatarsal Stoffella bunion osteotomy.

BACKGROUND: Avascular necrosis of the metatarsal head, delayed bone healing and nonunion are complications that may occur after distal first metatarsal osteotomies. Intraoperative damage to the extraosseous blood supply, the location of the osteotomy and postoperative vasospasm have been cited as possible causes of such changes. We evaluated Stoffella's subcapital osteotomies which were performed at our department for the correction of moderate to severe hallux valgus deformities. MATERIALS AND METHODS: Standardized radiographs of 300 feet, taken 6weeks, 3 months, and 6 months postoperatively

Single absorbable polydioxanone pin fixation for distal chevron bunion osteotomies.

The distal chevron osteotomy is a well-established technique for correction of symptomatic mild to moderate metatarsus primus varus with hallux valgus deformity. Fixation of the osteotomy ranges from none to bone pegs, Kirschner wires, screws, or absorbable pins. We evaluated one surgeon's (J.K.D.) results of distal chevron osteotomy fixation with a single, nonpredrilled, 1.3-mm poly-p-dioxanone pin and analyzed any differences in patients with unilateral or bilateral symptomatic metatarsus primus varus with hallux valgus deformities. All osteotomies healed without evidence of infection, osteo

Hallux valgus inheritance: pedigree research in 350 patients with bunion deformity.

Our objective was to construct 3-generation pedigree charts from 350 patients with hallux valgus. During a 1-year period, all consecutive patients (n = 1174) with a painful bunion deformity evaluated roentgenographically were asked to complete a detailed 3-generation family history questionnaire. We studied 350 probands (22 men, 328 women; male/female ratio, 1:14.9; mean age, 47.8 years). Juvenile hallux valgus was diagnosed in 15 patients. Three or more affected members were observed in pedigrees from 244 probands, 2 affected members in 71, and 1 affected member in 35 (proband) (affected subj

A Multi-Dimensional Systematic Review of Minimally Invasive Bunion Surgery (MIBS).

Background/Objectives: Hallux valgus, or a bunion, is a prevalent foot deformity associated with pain, limited mobility, and reduced quality of life. Surgical treatments include minimally invasive and traditional open techniques, but the optimal approach remains debated. This systematic review evaluates long-term outcomes, patient satisfaction, cost-effectiveness, the influence of patient-specific factors, rehabilitation protocols, and complication rates for these methods. Methods: A comprehensive search of PubMed, Medline, EMBASE, and Cochrane databases identified 22 studies published within

Bunion surgery using locking-plate fixation of proximal metatarsal chevron osteotomies.

BACKGROUND: Symptomatic large hallux valgus deformities commonly require surgical intervention with a proximal metatarsal osteotomy. A number of fixation methods have been described for proximal chevron osteotomies; one of the most recent is locking plates. METHODS: We retrospectively reviewed the records of 16 consecutive patients (20 feet) with severe bunion deformities who had locking-plate fixation of proximal chevron osteotomies. Clinical evaluation focused on osteotomy healing, transfer lesions, and hardware-related complications. Preoperative and postoperative radiographic evaluation i

The zero-plus first metatarsal and its relationship to bunion deformity.

The relationship between first metatarsal length and hallux valgus (HV) deformity was examined. Retrospectively, 210 randomly selected radiographic files were reviewed between 1988 and 1993. The morbid population consisted of 110 feet with HV deformities. The control population consisted of 100 healthy feet (no deformity). Seventy-seven percent of the patients with HV deformity had a first metatarsal length that was equal to or longer than the second metatarsal. This was defined as a zero-plus first metatarsal. Only 28% of the control population had this same proportion in length. Thus, preval

From Etiology to Intervention: A Holistic Review of Bunion Pathophysiology and Care.

In this review paper, we present the common etiology, presentation, diagnosis, and management of the following three common bunion formations: dorsal bunion, tailor's bunion, and hallux valgus (HV). Bunions are common pathologies that present to a variety of clinics, so it is important for providers to have a base understanding of these in order to provide the best care to patients. Many of these bunion formations have a variety of causes which allow providers to manage them before surgical intervention is required. The aim of this review paper is to bring attention and expanded insight on the

Arthrodesis of the first metatarsophalangeal joint for severe bunions and hallux rigidus.

Compression arthrodesis of the first metararsophalangeal joint using a modification of the technique of McKeever is described. This operation consistently produces the best results for hallux valgus and hallux rigidus of the great toe. The operation has been found to be applicable where there have been previous attempts at surgical correction. It also produces good results in patients having rheumatoid arthritis when there is minimal involvement of the interphalangeal joint.

Beyond the bunion: Trends in minimally invasive techniques in foot and ankle surgery.

Minimally invasive surgery (MIS) has revolutionized foot and ankle care by emphasizing delicate tissue handling, reduced postoperative pain, and faster recovery through small incisions and precise instrumentation. The evolution of MIS spans 4 distinct historical "seasons," from its early development in the mid-20th century to its global resurgence in the 2010s, driven by improved techniques, education, and international collaboration. Today, MIS extends beyond hallux valgus correction to include complex procedures such as first metatarsophalangeal joint, subtalar joint, tibiotalar joint, and t

Complications of metatarsal suture techniques for bunion correction: a systematic review of the literature.

To better understand the safety of suture techniques to reduce the intermetatarsal angle in patients with hallux valgus deformity, we undertook a systematic review of the complications associated with the use of this technique. The suture procedures of 197 patients were analyzed for complications. The number of complications in the total group (n = 197) at a pooled mean follow-up period of 23.2 months was 39 (19.8%) and included 21 fractures (10.7%), 11 cases of hardware failure (5.6%), and 7 cases of hallux varus (3.6%). The cohort of patients was further categorized by the specific procedure

Common foot problems. Relieving the pain of bunions, keratoses, corns, and calluses.

Many of the painful foot problems seen in adults are easily treated. Bunions and bunionettes often respond to a change to wide, soft-toed shoes or sandals and the temporary use of a mild anti-inflammatory agent. Initial treatment of plantar keratoses is trimming and use of cushioning shoe inserts. Diffuse keratoses may require reconstructive surgery if metatarsal deformity is the cause. Seed corns and soft corns respond to trimming, and alleviation of pressure with padding and wearing of open-toed shoes is beneficial in these lesions as well as in hard corns. Surgical resection of any underlyi

Application of distal metaphyseal osteotomy for treatment of high intermetatarsal angle bunion deformities.

This is a retrospective study of 10 patients (13 feet) with moderate to severe hallux valgus who underwent a chevron or modified chevron osteotomy with multiple adjunctive soft-tissue releases for surgical treatment. Radiographic and subjective results were evaluated with an average follow-up of 24 months (range, 10-41). Preoperative criteria included an intermetatarsal (IM) angle of greater than 16 degrees (average of 18.4 degrees) and painful hallux valgus deformity. Average preop hallux abductus was 35.4 degrees (range, 25 degrees-48 degrees). The average reduction in the actual IM angle wa

Scarf osteotomy for reduction of tailors bunion deformities: Systematic review and meta-analysis.

BACKGROUND: Tailors bunions are common forefoot deformities affecting the fifth MPJ, with challenging symptomatology often resistant to conservative care. Currently no gold standard exists for the surgical management of tailor's bunions, although the scarf osteotomy has been described as a versatile option for reducing these deformities. METHODS: Relevant electronic databases were searched to collate all studies pertaining to tailors bunion correction using the scarf osteotomy between 2000 and 2021. Both surgeon and patient reported outcomes were required to be included in the systematic revi
