Staphylococcal toxic shock syndrome in a paediatric burn unit.

A case of staphylococcal toxic shock syndrome (TSS) in a 15-year-old patient undergoing reconstructive burn surgery is reported. This syndrome, first described in menstruating women using tampons, can also occur as a postsurgical complication. The epidemiology, microbiology, clinical presentation and management of non-menstrual TSS are discussed and correlated with the patient present. We advise caution in the use of occlusive dressings and recommend constant isolation of patients with open wounds colonized with a TSST-1 producing strain of Staph. aureus.

Burn unit treatment of acute, severe exfoliating disorders.

In this chapter, we review the experience gained in the burn unit treatment of two of the severe exfoliating disorders, toxic epidermal necrolysis (TEN) and the Stevens-Johnson syndrome (SJS). Septic mortality was higher if high doses of corticosteroids were given versus the current protocol, which avoids these drugs. Current controversies include the role of topical therapy, leukopenia, and invasive catheters in septic complications as well as nutritional requirements. Concomitant viral infection and the macrophage may each play a role in the etiology of TEN and SJS.

Determining the Value of Screening and Decolonization in Burn Unit Management.

Methicillin-resistant Staphylococcus aureus (MRSA) is consistently associated with more severe wound complications and worse outcomes in burn patients. Many burn centers who have implemented routine MRSA screening and decolonization protocols have reported reduced MRSA transmission, lower rates of device-associated infections such as CAUTI and CLABSI, and decreased LOS. At the Ohio State University, Wexner Medical Center (OSUWMC), we initiated a multidisciplinary quality improvement two-phase project to implement routine screening. Phase 1: all patients who screened positive for MRSA received

The use of Xbox Kinect™ in a Paediatric Burns Unit.

BACKGROUND: The popularity of video game use in burns rehabilitation has grown because, in addition to facilitating maintenance of range of motion (ROM), the virtual imaging characteristics of these games provide distraction from pain. OBJECTIVES: The effect of using Xbox Kinect™ as an adjunct to physiotherapy in a Paediatric Burns Unit (PBU) has not been established. This study aimed to investigate the effect of using the Xbox Kinect™ on outcomes of children in the PBU at Chris Hani Baragwanath Academic Hospital. METHODS: This non-equivalent, post-test only control group study took place ov

Survival status and predictors of mortality among burn patients admitted to intensive care units at burn centres in Addis Ababa, Ethiopia: a retrospective follow-up study.

INTRODUCTION: A burn injury is a severe global healthcare burden and is associated with high mortality risk. The mortality rate is marked, particularly in low-and-middle-income countries. Despite its clinical importance, recent epidemiological data on burn injury and mortality are limited in the area of critical care units in Ethiopia. Therefore, this study aimed to determine the survival status and predictors of mortality among burn patients admitted to intensive care units. MATERIAL AND METHODS: This multicentre retrospective cohort study was conducted at Addis Ababa governmental hospitals i

Modifying burn care in a low- and middle-income countries (LMIC) to meet global best practice: A burn unit or burn isolation ward?

The overwhelming burden of burns in low-income (LIC) and low-middle-income (LMIC) countries has been well-documented. Though best global practice is for major burns to be treated in burn units, the prohibitive cost makes it difficult. In this article we attempt to present the improvement in outcome recorded over a period of 3 years as we transitioned from nursing our burns patients in the general surgical wards (Group A), then a separate cubicle within the wards (Group B) and then an isolated burn ward (Group C). Other practices such as physiotherapy, traditional wound dressings, and limb spli

Acute Kidney Injury After Burn: A Cohort Study From the Parkland Burn Intensive Care Unit.

Acute kidney injury (AKI) is a common and morbid complication in patients with severe burn. The reported incidence of AKI and mortality in this population varies widely due to inconsistent and changing definitions. They aimed to examine the incidence, severity, and hospital mortality of patients with AKI after burn using consensus criteria. This is a retrospective cohort study of adults with thermal injury admitted to the Parkland burn intensive care unit (ICU) from 2008 to 2015. One thousand forty adult patients with burn were admitted to the burn ICU. AKI was defined by KDIGO serum creatinin

Predictors of mortality in cases of thermal burns admitted to Burn Unit, Ain Shams University Hospitals, Cairo.

In cases of death due to burn injury, it may be of great medico-legal importance to determine if the death is caused by fatal burn injury or due to other factors related to treatment. Therefore, this study aimed to investigate early predictors of mortality in patients with thermal burns admitted to Burn Unit, Ain Shams University Hospitals during a period of one year (2011). The study included 152 cases, mortalities represented 20.4% of the included cases (31 cases). Binary logistic regression analysis showed that, total body surface area (TBSA) of the burns and APACHE III score were significa
