Heart failure-induced cachexia.

Heart failure patients often develop cachexia, which is an independent factor for survival reduction. Cachexia can be diagnosed when there is loss of more than 6% of the body weight, in the absence of other diseases. Even though its pathophysiology has not yet been completely clarified, various factors seem to be involved, such as reduction in food consumption, gastrointestinal tract abnormalities, immunologic and neuro-hormonal activation and changes in the relationship between anabolic and catabolic processes. Since there is not specific therapy for heart failure-induced cachexia, management

Cancer anorexia - cachexia syndrome.

Cancer anorexia-cachexia syndrome (CACS) is a devastating and debilitating aspect at any stage of malignancy. It presents primarily as anorexia, weight loss and muscle wasting secondary to inadequate oral intake and metabolic changes. This syndrome is highly prevalent among cancer patients, has a large impact on morbidity and mortality, and impinges on patient quality of life. The pathogenic mechanisms of CACS are multifactorial. It is suggested to be the result of tumor-host interactions and cytokines have a siginificant role. Diagnosis of cancer cachexia is complex and multifaceted and requi

Cachexia index as a biomarker for cancer cachexia and quality of life in patients with gastric cancer.

BACKGROUND: Cachexia is associated with adverse clinical outcomes in patients with gastric cancer (GC); therefore, a convenient and reliable method for monitoring cachexia is essential. This study aimed to evaluate the utility of the cachexia index (CXI) as a biomarker for estimating cancer cachexia and health-related quality of life (HRQoL) in GC patients. METHODS: The CXI was calculated as the skeletal muscle index (SMI) × serum albumin / neutrophil-lymphocyte ratio (NLR). Diagnosis of cachexia was based on Asian Working Group for Cachexia (AWGC) criteria and Fearon's criteria. Univariate a

Nutrition rehabilitation programs and cachexia clinics for anorexia-cachexia syndrome in patients with cancer.

The anorexia-cachexia syndrome (ACS) is characterized by loss of appetite and unintentional weight loss. Important clinical outcomes are associated with ACS including increased risk of chemotherapy side effects, decreased survival, and quality of life. Because ACS is driven by complex metabolic mechanisms and a chronic pro-inflammatory response, the weight loss and muscle wasting cannot be reversed by conventional nutritional supplementation alone. However, insufficient intake of calories and protein exacerbate weight loss experienced by patients with ACS, while physical inactivity accelerate

Development and application of the Cancer Cachexia Staging Index for the diagnosis and staging of cancer cachexia.

OBJECTIVE: The current tools for evaluating cancer cachexia are either too simple to reflect the far-reaching effects of cachexia or too complicated to be used in daily practice. This study aimed to develop a cancer cachexia staging index (CCSI) that is both practical and comprehensive. METHODS: Patients with gastrointestinal cancers were prospectively included in the study. Clinical data including weight change, body composition, systematic inflammation, nutrition, and function status were entered into regression models to determine the best variable combination as well as their respective c

Salidroside alleviates cachexia symptoms in mouse models of cancer cachexia via activating mTOR signalling.

BACKGROUND: Cachexia has a devastating impact on survival and quality of life for many cancer patients and contributes to nearly one-third of all cancer deaths; also, it is associated with poor responses to chemotherapy and survival. A better understanding of the underlying mechanisms of cancer-associated cachexia (CAC), coupled with effective therapeutic approaches, will improve management of progressive functional impairment in cancer patients. Salidroside, a phenylpropanoid glycoside in Rhodiola rosea L, has been reported to possess potential anti-fatigue, anti-ageing, and anti-Alzheimer's

The impact of cachexia index combined with BMI trajectory on survival outcomes in patients with cancer cachexia.

BACKGROUND: The cachexia index (CXI) has emerged as a recognized prognostic biomarker of cancer cachexia. However, the dynamic progression of cachexia may not be fully captured by a single assessment. This study examined the impact of integrating the CXI with BMI trajectories on the survival prognosis of patients with cancer cachexia to enable early identification of high-risk populations. METHODS: This is a retrospective review of clinical and pathological data from 147 patients diagnosed with cancer cachexia at Xuzhou Central Hospital between January 2019 and May 2024. Based on computed tom

Novel Antibodies for Managing Cachexia.

Cachexia is a complex, multifactorial syndrome driven by anorexia and systemic inflammation, ultimately resulting in a catabolic state characterized by weight loss, decreased muscle mass, and impaired physical function. Many chronic comorbidities are implicated in the pathophysiology of cachexia, with malignancy being one of the most common. Although approved cachexia-directed therapies are limited to date, the growth differentiation factor 15 (GDF-15)-glial cell-derived neurotrophic factor family receptor alpha-like (GFRAL) signaling axis has been identified as a primary mediator of cachexia

Cardiac Cachexia: A Comprehensive Review.

Cardiac cachexia remains a phenomenon seen more particularly in New York Heart Association Classification class III or IV heart failure patients. This is frequently missed and written off as "old age." It has a mortality rate of 50% within 18 months of diagnosis, so it is imperative to diagnose cardiac cachexia promptly. It remains elusive in the complexity of its underlying pathophysiology, from cytokine release from both the endocardium and the gut, with systemic implications. This review provides an analysis of cardiac cachexia: (1) definition and diagnostic criteria; (2) prevalence and mor

Multimodal interventions for cachexia management.

BACKGROUND: Cachexia (disease-related wasting) is a complex metabolic syndrome which occurs in people with chronic illnesses, including cancer, HIV/AIDS, kidney disease, heart disease, and chronic obstructive pulmonary disease (COPD). People with cachexia experience unintentional weight loss, muscle loss, fatigue, loss of appetite, and reduced quality of life. Multimodal interventions which work synergistically to treat the syndrome could lead to benefits. OBJECTIVES: To assess the benefits and harms of multimodal interventions aimed at alleviating or stabilising cachexia in people with a chr

Cancer cachexia: multilevel metabolic dysfunction.

Cancer cachexia is a complex metabolic disorder marked by unintentional body weight loss or 'wasting' of body mass, driven by multiple aetiological factors operating at various levels. It is associated with many malignancies and significantly contributes to cancer-related morbidity and mortality. With emerging recognition of cancer as a systemic disease, there is increasing awareness that understanding and treatment of cancer cachexia may represent a crucial cornerstone for improved management of cancer. Here, we describe the metabolic changes contributing to body wasting in cachexia and expla

Medical imaging in cancer cachexia.

Cancer cachexia, often referred to as "wasting syndrome," is characterized by fatigue, weakness, and involuntary weight loss. This syndrome is concomitant with progressive skeletal muscle atrophy with or without adipose tissue loss and is frequently accompanied by systemic inflammation. Understanding the complexities of cancer cachexia is crucial for early detection and intervention, and it is also paramount for enhancing patient outcomes. Medical imaging, comprising diverse imaging modalities, plays a pivotal role in this context, facilitating the diagnosis and surveillance assessment of both

Immunoregulation in cancer-associated cachexia.

BACKGROUND: Cancer-associated cachexia is a multi-organ disorder associated with progressive weight loss due to a variable combination of anorexia, systemic inflammation and excessive energy wasting. Considering the importance of immunoregulation in cachexia, it still lacks a complete understanding of the immunological mechanisms in cachectic progression. AIM OF REVIEW: Our aim here is to describe the complex immunoregulatory system in cachexia. We summarize the effects and translational potential of the immune system on the development of cancer-associated cachexia and we attempt to conclude

Cancer cachexia: lessons from Drosophila.

Cachexia, a wasting syndrome that is often associated with cancer, is one of the primary causes of death in cancer patients. Cancer cachexia occurs largely due to systemic metabolic alterations stimulated by tumors. Despite the prevalence of cachexia, our understanding of how tumors interact with host tissues and how they affect metabolism is limited. Among the challenges of studying tumor-host tissue crosstalk are the complexity of cancer itself and our insufficient knowledge of the factors that tumors release into the blood. Drosophila is emerging as a powerful model in which to identify tum

Sex Differences in Cancer Cachexia.

PURPOSE OF REVIEW: Cachexia, a feature of cancer and other chronic diseases, is marked by progressive weight loss and skeletal muscle wasting. This review aims to highlight the sex differences in manifestations of cancer cachexia in patients, rodent models, and our current understanding of the potential mechanisms accounting for these differences. RECENT FINDINGS: Male cancer patients generally have higher prevalence of cachexia, greater weight loss or muscle wasting, and worse outcomes compared with female cancer patients. Knowledge is increasing about sex differences in muscle fiber type an
