[Case with probable dementia with Lewy bodies, who shows reduplicative paramnesia and Capgras syndrome].

We report a case of probable dementia with Lewy bodies (DLB), showing reduplicative paramnesia (RP) and Capgras syndrome (CS). The patient, a right-handed 60 year-old male, began to show progressive dementia. At the age of 65, he showed fluctuating cognitive impairment and recurrent visual hallucinations. His SPECT demonstrated hypoperfusion not in the medial temporal cortices, but in the parieto-occipital lobes, where the right hemisphere was dominantly hypoperfused. He was diagnosed with probable DLB. In addition to recurrent visual hallucinations, he showed a sense of self- (or others) tran

Capgras Syndrome Triggered by Marital Separation: A Rare Case of Trauma-Induced Delusional Misidentification.

Capgras syndrome is a rare psychiatric condition classified under delusional misidentification syndromes, characterized by the belief that a familiar person has been replaced by an identical impostor. Although it is most commonly observed in association with schizophrenia, neurodegenerative diseases, or structural brain lesions, it may also arise in the context of acute psychological trauma or severe emotional disturbance. We present the case of a 46-year-old African American woman with no prior psychiatric or neurological history who presented to an outpatient psychiatry clinic accompanied by

OCD Symptoms and Capgras Syndrome in a 13-Year-Old Girl Following Right-Sided Brain Surgery.

This case report documents a rare presentation of obsessive-compulsive-like symptoms co-occurring with Capgras syndrome following surgical resection of a right-sided frontal lobe pleomorphic xanthoastrocytoma in a 13-year-old girl. The patient initially presented with headache and intractable vomiting, leading to the discovery of a right-sided frontal lobe tumor. Postoperatively, the patient exhibited cognitive and behavioral changes manifesting as misidentification beliefs regarding the identities of loved ones and associated distressing thoughts. Comprehensive psychiatric evaluation using va

Capgras syndrome: a review of the neurophysiological correlates and presenting clinical features in cases involving physical violence.

OBJECTIVE: Acts of violence have been frequently reported in cases of Capgras syndrome (CS), a misidentification syndrome characterized by the delusional belief that imposters have replaced people familiar to the individual. CS has been observed in many neuropsychiatric and organic disorders, and neuroimaging studies indicate an association between CS and right hemisphere abnormalities. However, CS has received limited attention from a forensic psychiatric perspective. We propose that elucidating demographic and clinical features noted in cases of violence secondary to CS may highlight importa

Challenging the impostor: a scoping review of the pharmacological management of Capgras syndrome in Parkinson's disease and Lewy bodies dementia.

INTRODUCTION: Capgras syndrome (CS) is a delusional misidentification phenomenon increasingly reported in patients with Parkinson's disease (PD) and Lewy Body Dementia (LBD). Aim of the present scoping review was to provide an overview on current evidence on the pharmacological treatment of CS in PD and LBD, identifying knowledge gaps in the literature. METHODS: The following databases were consulted: PubMed, Google Scholar, the Cochrane Database and Web of Science. RESULTS: The search query covered a time period from 1976 until 2022. Fourteen studies on PD (11 single case reports, 3 case se

Bilateral atrophy of the frontal and temporal lobes in schizophrenic patients with Capgras syndrome: a case-control study using computed tomography.

The computed tomographic scans of 12 patients with Capgras syndrome and 12 matched controls were blindly assessed. A variety of statistical comparisons between the two groups consistently showed that patients with Capgras syndrome had significantly more bilateral frontal and temporal lobe atrophy. Capgras syndrome may be associated with dysfunction of these areas. Interestingly, all of the index patients had paranoid schizophrenia, and these results suggest the hypothesis that in some schizophrenic patients the presence of Capgras syndrome may be a marker for the coexistence of a focal idiopat

Acute onset Capgras syndrome after upper respiratory tract infection: a case report on the possible role of inflammation and drug exposure.

BACKGROUND: Capgras syndrome (CS) is a rare delusional disorder characterized by the delusional belief that a relative has been replaced by an identical person. This syndrome is frequently associated with psychotic disorders and neurological diseases. CASE PRESENTATION: In this case report, we describe a 42-year-old woman who developed Capgras syndrome following a severe upper respiratory tract infection. The patient believed that her husband had been replaced by another person and was found to exhibit delusional misidentification consistent with Capgras syndrome. Paliperidone treatment was i

Case Report: Severe adolescent-onset schizophrenia with Capgras syndrome and remission during clozapine-centered treatment after earthquake-related displacement and peer victimization.

Adolescent first-episode psychosis may combine schizophrenia-spectrum, affective, trauma-related, and developmental features, and Capgras syndrome is uncommon in this age group. We report a confidentially de-identified case of a 16-year-old female high-school student with no previous psychiatric admission or known substance use who developed severe insomnia, reduced self-care, external auditory hallucinations, paranoid and referential ideation, Capgras-type misidentification of her father, and a multithemed bizarre delusional system after cumulative psychosocial stressors including earthquake-

[Treatment and Care for Elderly Patients with Dementia with Lewy Bodies Presenting with Delusional Misidentification Syndrome Including Capgras Syndrome: How to Respond and Intervene from a Neurologist's Perspective].

Aged patients with dementia with Lewy body (DLB) present with delusional misidentification syndrome and occasionally Capgras' syndrome. It is difficult to treat the DLB patients presenting with these psychiatric symptoms, as well as dementia, parkinsonism, sleep disorders, and autonomic dysfunction. In advanced stage of DLB, it is necessary to pay careful attentions to the appropriate choice of drugs as well as to improve the environment surrounding the DLB patients. At an earlier stage of DLB, before the exacerbation of illness, it is important to explain the clinical features and treatments

Tactile restoration of familiarity in Capgras syndrome following right hemorrhagic stroke: Lesion mapping, Disconnectome analysis, and lesion network characterization of multisensory person recognition.

BACKGROUND: Capgras syndrome is characterized by the delusional belief that familiar individuals have been replaced by identical impostors. Current models attribute the syndrome to disrupted affective familiarity despite preserved perceptual recognition, but the underlying network mechanisms and the role of alternative sensory modalities remain unclear. METHODS: We studied a patient with Capgras syndrome following a right hemispheric hemorrhagic stroke. Neuropsychological assessment, behavioral experiments, and galvanic skin response recordings evaluated multimodal person recognition. Structu

[Treatment and Care for Elderly Patients with Dementia with Lewy Bodies Presenting with Delusional Misidentification Syndrome Including Capgras Syndrome: Severe Psychiatric Symptoms Associated with Dementia with Lewy Bodies].

The treatment and care for severe psychiatric symptoms associated with dementia with Lewy bodies is challenging. This is especially true for elderly patients because the use of antipsychotics is associated with an attendant mortality risk. In this article, dementia patients with Lewy bodies who presented with severe psychiatric symptoms such as Capgras syndrome (delusional misidentification syndrome), are described, and pharmacological and non-pharmacological strategies to address these symptoms are discussed. Measures to be avoided include antipsychotic administration and physical restraint,

Disturbances of person identification in Alzheimer's disease. A retrospective study.

Person identification disturbances in Alzheimer's disease (AD) add to the suffering of both patients and caregivers. We assessed the prevalence of person identification disturbances in the records of 217 outpatients with AD. These disturbances occurred in 25.4% (N = 55) and included transient misidentifications of familiar persons (N = 34), the Capgras syndrome (N = 11), misidentification of themselves in mirrors (N = 5), prosopagnosia (N = 3), misidentification of unfamiliar persons as familiar (N = 1), and misidentification of another person as oneself (N = 1). Transient misidentifications w

Capgras delusion for animals and inanimate objects in Parkinson's Disease: a case report.

BACKGROUND: Capgras delusion is a delusional misidentification syndrome, in which the patient is convinced that someone that is well known to them, usually a close relative, has been replaced by an impostor or double. Although it has been frequently described in psychotic syndromes, including paranoid schizophrenia, over a third of the documented cases of Capgras delusion are observed in patients with organic brain lesions or neurodegenerative disease, including Parkinson's Disease. Variants of Capgras involving animals or inanimate objects have also been described. The etiology of Capgras in

Delusional misidentification, AIDS and the right hemisphere.

A patient with AIDS developed the Capgras' syndrome as part of an acute confusional state resulting from an opportunistic infection. Neuropsychological testing suggested non-dominant hemisphere dysfunction with impaired facial recognition. Serial CT scanning showed a right parietal lesion which resolved after the psychosis improved. These findings provide further evidence for the hypothesis that selective visuospatial deficits underlie the development of Capgras' syndrome.

"Capgras" Delusions Involving Belongings, Not People, and Evolving Visual Hallucinations Associated with Occipital Lobe Seizures.

Capgras syndrome is characterized by the delusional belief that a familiar person has been replaced by a visually similar imposter or replica. Rarely, the delusional focus may be objects rather than people. Numerous etiologies have been described for Capgras to include seizures. Similarly, visual hallucinations, both simple and complex, can occur secondary to seizure activity. We present, to our knowledge, the first reported case of visual hallucinations and Capgras delusions for objects that developed secondary to new onset occipital lobe epilepsy. We then discuss the possible underlying neur

Twin studies have shown that our sense of humor has an underlying genetic component, but less investigation of the origins of stand-up comedy has been undertaken. This article briefly reviews twin research findings on humor, then describes the working partnership and social affiliation of a pair of monozygotic male twins who perform together as stand-up comedians. The abilities, personalities and temperaments of these twins suggest future avenues for research in this interesting area. Next, findings from twin studies and case reports of twins with cleft lip and palate, dental caries, noninvasi
