[Clinical analysis of secondary hemophagocytic lymphohistiocytosis complicated with capillary leak syndrome].

Objective: To investigate the clinical characteristics of secondary hemophagocytic lymphohistiocytosis (sHLH) complicated with capillary leak syndrome (CLS) . Methods: The clinical and laboratory data of 87 sHLH patients, who were treated in our hospital between January 2015 and December 2017, were retrospectively analyzed. Depending on whether they were complicated with CLS, 21 sHLH patients were classified as the CLS-sHLH group, while 66 were classified as the non-CLS-sHLH group. The differences of clinical manifestations, laboratory tests, treatment and prognosis between the two groups were

[Idiopathic systemic capillary leak syndrome: a case report with cardiac involvement].

Systemic capillary leak syndrome (SCLS) is characterized by recurrent hypovolemic shock associated with edema, hemoconcentration and paradoxal hypoprotidemia due to leakage into the interstitial space. We report a new case with cardiac involvement. A 62-year-old man had two recurrent hypovolemic shocks with SCLS's criteria, associated twice with cardiac systolic dysfunction and atrial fibrillation. This report should enhance physician awareness of SCLS, which is a rare and severe disease, and its cardiac involvement.

[A case of brucellosis complicated with capillary leak syndrome and multiple organ dysfunction syndrome].

Brucellosis can lead to pathological changes of multiple systems. Capillary leak syndrome (CLS) is a clinical syndrome caused by different reasons, mainly characterized by hypotension, hypoproteinemia and systemic edema. The condition is critical and the clinical manifestations are complex, and multiple organ dysfunction syndrome (MODS) may occur in severe cases. CLS caused by brucellosis is extremely rare. The diagnosis and treatment of a patient with brucellosis complicated with CLS and MODS was analyzed in this paper, in order to improve the knowledge of clinicians about brucellosis and its

Acute renal failure, compartment syndrome, and systemic capillary leak syndrome complicating carbon monoxide poisoning.

Five Indian labourers sharing one room were found lying on the floor unconscious. A coal fire had been lit to provide heat during the cold night and was still burning inside the room. Two of the men were pronounced dead at the scene. The other three were transferred to the hospital in critical condition. This paper contains the case reports of the three patients who were transferred to the hospital. One of the patients died 60 h after admission. The other two patients were successfully treated and released. The three patients developed severe rhabdomyolysis complicated by compartment syndrome.

Disseminated Herpes Simplex Virus Infection and Capillary Leak Syndrome in Preterm Twins.

Disseminated herpes simplex virus (HSV) can be life-threatening in neonates without treatment. We present a case involving twins diagnosed with disseminated HSV who experienced different clinical courses. Twin A was more severely affected, developing severe capillary leak syndrome (CLS) that led to cardiovascular compromise, acute kidney injury, and hepatic failure. However, with innovative immunologic treatment and expert hemodynamic management, the patient recovered despite high mortality in infants with this presentation. Twin B had a less severe initial presentation but experienced long-te

Capillary leak syndrome in sepsis: the role of intrinsic coagulation pathway activation.

BACKGROUND: The coagulation system is closely linked to the inflammatory response and kinin system activation in sepsis, but its contribution to sepsis-associated capillary leak syndrome (SACLS) remains elusive. This study explored the underlying role of the coagulation system in SACLS. METHODS: Patients with sepsis were recruited to evaluate the early predictive potential of coagulation function-related indicators for capillary leakage. Patients with hemorrhagic fever with renal syndrome (HFRS) and scrub typhus, conditions featuring SACLS as a typical manifestation, were selected to validate

Reducing Capillary Leak Syndrome Mortality in Extremely Premature Infants: A Pilot Investigation.

Neonatal capillary leak syndrome (CLS) is a rare, but life-threatening condition following neonatal sepsis or inflammatory injury. The objective of this study was to describe a standardized treatment approach for CLS that improves mortality and neonatal outcomes. A retrospective cohort study of 10 infants born at 22 to 26 weeks of gestation who developed CLS following a significant inflammatory insult was performed. Time to diagnosis and treatment approaches over 2 epochs were recorded and described. In epoch 2, with increased clinical awareness of CLS and implementation of a standardized trea

Renal Replacement Therapy in Idiopathic Systemic Capillary Leak Syndrome: A Case Report.

Idiopathic systemic capillary leak syndrome (ISCLS) is a rare disease characterized by hypotensive shock, anasarca, hemoconcentration, and hypoalbuminemia. Despite the life-threatening course of the disease, no treatment strategy has been established. A 68-year-old man presented with hypotensive shock following a prodrome. Based on the characteristic blood test findings, ISCLS was suspected. The patient was resuscitated by administering massive amounts of fluids and inotropic and vasopressor agents. After his blood pressure had stabilized, renal replacement therapy (RRT) was promptly initiated

Colchicine: An Impressive Effect on Posttransplant Capillary Leak Syndrome and Renal Failure.

Capillary leak syndrome is a critical condition occasionally occurring posttransplant and is characterized by acute endothelial hyperpermeability leading to systemic protein-rich fluid extravasation and consequent hypovolemia, hypoperfusion, and acute kidney injury. Treatment is merely supportive and is based on osmotic drugs, diuretics, continuous renal replacement therapy, and surgical drainage. However, removal of the underlying inflammatory cause is mandatory to achieve stable resolution. Herein, we report the first successful treatment with colchicine in 2 life-threatening pediatric cases

A Case of Refractory Systemic Capillary Leak Syndrome (Clarkson's Disease) during Pregnancy.

A 32-year-old woman, pregnant with twins, presented with a chief complaint of general fatigue. Her general condition had rapidly deteriorated since her last visit to the primary obstetrician; the patient was then referred to our hospital because of suspected fetal death. She underwent emergency cesarean section because fetal death had indeed occurred, and she was then admitted to the intensive care unit (ICU). On ICU admission, she was found to be in shock. Laboratory analysis revealed extreme hemoconcentration and a low albumin level, and initially, septic shock with obstetric complications w

Two cases of systemic capillary leak syndrome that were treated with pentastarch.

Systemic capillary leak syndrome (SCLS) is a condition that's caused by the shift of fluid and protein from the intravascular space to the interstitial space as a result of repetitive episodes of capillary hyperpermeability. The pathogenesis of SCLS is still unclear, but there's recently been a report showing this syndrome in association with monoclonal gammopathy. This syndrome can be a fatal disease because cardiovascular collapse can occur in the initial capillary leak phase. Although theophylline, diuretics, terbutaline, steroids, calcium antagonist, Ginkgo biloba extracts and plasmapheres

Systemic capillary leak syndrome associated with multiple myeloma of IgG kappa type.

A case of systemic capillary leak syndrome in a 67-year-old woman is reported. The patient suddenly presented with severe edema and hypotension associated with an increased hematocrit and a decreased level of serum albumin, suggesting capillary hyperpermeability. The patient had IgG kappa monoclonal gammopathy in association with not only a slight increase in plasma cells in the bone marrow but also several punched out lesions in the skull, indicating multiple myeloma. Although most of the cases of systemic capillary leak syndrome reported previously were accompanied by monoclonal gammopathy,

Rare Progressive Edema From a Secondary Capillary Leak Syndrome in Autoimmune Disease.

Secondary capillary leak syndrome (SCLS) is a rare and frequently underrecognized cause of progressive edema, hypoalbuminemia, and noncardiogenic pulmonary edema. We present a case of a 55-year-old man with seronegative rheumatoid arthritis and antisynthetase syndrome on chronic immunosuppression who developed progressive bilateral lower-extremity edema and pulmonary edema in the absence of cardiac, renal, or hepatic disease. Extensive evaluation excluded alternative etiologies. The patient met diagnostic criteria for SCLS and demonstrated clinical improvement with albumin-assisted diuresis. T

Idiopathic systemic capillary leak syndrome (Clarkson's disease) presenting with recurrent hypovolemic shock.

A 49-year old male with a past medical history of myocardial infarction and compartment syndromes requiring fasciotomies presented on five occasions with hypovolemic shock. We describe his admissions and presumptive diagnoses which required large volumes of intravenous fluids, admission to intensive care for vasopressors and renal replacement therapy. The presentations were always precipitated by a prodrome of fatigue and pre-syncopal episodes. On his last admission, a diagnosis of Idiopathic systemic capillary leak syndrome (ISCLS), also known as Clarkson's Disease, was reached. He is current

Distributive Shock in the Emergency Department: Sepsis, Anaphylaxis, or Capillary Leak Syndrome?

BACKGROUND: Distributive shock is a hyperdynamic process resulting from excessive vasodilatation. Impaired blood flow causes inadequate tissue perfusion, which can lead to end-organ damage. Although the most common etiology is septic shock, anaphylactic and other etiologies should be considered. CASE REPORT: We report the case of a 30-year-old female who presented to the emergency department with nonspecific symptoms and hypotension after a viral upper respiratory infection. Her physical examination revealed mild edema and rebound tenderness in the right upper and bilateral lower quadrants. S
