Modified Oxford Bone Scores Are Reduced in Patients With Slipped Capital Femoral Epiphysis Compared With Healthy Controls.

BACKGROUND: The etiology and pathogenesis of slipped capital femoral epiphysis (SCFE) are attributable to abnormalities of the proximal femoral epiphysis. This study aimed to examine if there is a difference in the bone age of patients diagnosed with SCFE compared with patients without hip pathology. METHODS: We identified a consecutive series of patients treated for SCFE between December 2012 and December 2019 from a departmental database. Retrospective chart review was performed to collect demographic information and patient medical history. We then obtained a control group of statistically

The narrow window of bone age in children with slipped capital femoral epiphysis: a reassessment one decade later.

A narrow window of bone age (BA) in slipped capital femoral epiphysis (SCFE) was described a decade ago. Children now mature younger. Does the BA narrow window still exist? It is the purpose of this study to investigate this question. A retrospective review of children with idiopathic SCFE (1998-2003) was performed. The initial study data ("past") were used as a comparison. Anteroposterior pelvis radiographs were scored for BA (Oxford method) in 108 children. Only the first radiograph was used for children with sequential bilateral SCFEs. There were 67 boys and 41 girls. There were 73 children

Technical note: bone marrow aspirate needle to assist in-situ screw fixation of slipped capital femoral epiphysis.

Slipped capital femoral epiphysis (SCFE) is characterized by posterior and inferior displacement of the capital femoral epiphysis in relationship to the metaphysis. Although universally accepted as a treatment modality for SCFE, in-situ pinning may be technically challenging in obese adolescents with large body habitus. Adequate screw position is important to achieve stabilization of the epiphysis and to allow closure of the growth plate. During the surgical procedure of in-situ pinning, it is crucial to obtain not only a perfect starting point but also an appropriate trajectory. However, crea

Normative Values for Capital Femoral Epiphyseal Extension of the Developing Hip Based on Age, Sex, and Oxford Bone Age.

BACKGROUND: Recent evidence suggests that increasing capital femoral epiphyseal extension may be an adaptive response that underlies the development of most cam morphology, whereas slipped capital femoral epiphysis is associated with its deficiency. However, there is an absence of rigorous data on the normal development of epiphyseal extension in the hip joint in modern adolescents. The aim of this study was to establish normative values for anterior and superior epiphyseal extension in a normal adolescent control population. METHODS: A total of 210 pediatric subjects (420 hips) between the a

Proximal Femoral Bone Cyst After Single Screw Fixation for Slipped Capital Femoral Epiphysis: A Case Report.

CASE: An 11-year-old female patient underwent bilateral in situ fixation for slipped capital femoral epiphyses using single, cannulated, stainless steel screws. She presented 12 years later with a large osteolytic lesion of the proximal femur, which only involved 1 side. Histological evaluation showed a foreign-body reaction and synovial lining. Infection was ruled out and dynamic hip screw stabilization, and bone grafting were performed. The bone graft healed, and the hardware was removed to prevent a similar reaction. In retrospect, postoperative radiographs at 7 weeks showed subtle osteolys

Treatment of Chondroblastoma of the Capital Femoral Epiphysis in Children Using a Cartilage Fenestration Approach Combined With Artificial Bone Implantation.

BACKGROUND: Chondroblastoma of the capital femoral epiphysis (CHCFE) in children is a rare benign bone tumor that poses significant therapeutic challenges due to its unique anatomic location adjacent to the physeal plate and femoral head articular cartilage. This study aimed to evaluate the clinical efficacy and safety of a cartilage fenestration approach combined with artificial bone implantation for the treatment of CHCFE. METHODS: A retrospective analysis was conducted on 25 consecutive pediatric patients (14 boys and 11 girls; mean age, 10.5 y; range, 6 to 14 y) diagnosed with CHCFE and t

Revised modified Oxford bone score: a simpler system for prediction of contralateral involvement in slipped capital femoral epiphysis.

BACKGROUND: The decision to prophylactically treat the uninvolved hip in slipped capital femoral epiphysis (SCFE) remains controversial. The modified Oxford bone score (mOBS) is predictive of future contralateral involvement in slipped capital femoral epiphysis. The scoring method for this system is challenging to remember because out-of-context irrational number sequences and total score range (16 to 26) are used. This study was performed to evaluate intraobserver and interobserver reliability of the mOBS and to determine whether revising the scoring sequence to 0 to 2 for all 5 categories (t

Displaced femoral neck fractures at the bone-screw interface after in situ fixation of slipped capital femoral epiphysis.

Two patients had displaced femoral neck fractures at the bone-screw interface after in situ fixation of slipped capital femoral epiphysis. Both fractures required closed reduction and internal fixation. Inadvertent derotation of the femur occurred in both patients and allowed fixation of the fracture through the intramedullary canal of the femoral neck without anterior penetration of the pins into the joint; a nonunion occurred in this patient. The other patient developed chondrolysis and avascular necrosis. Because the femoral neck is vulnerable to stress fractures in young adults, there is s

Bone Scintigraphy Prediction of Postoperative Femoral Head Avascular Necrosis in Children With Hip Trauma and Slipped Capital Femoral Epiphysis.

BACKGROUND: Femoral head avascular necrosis (AVN) is a devastating complication that can occur in the setting of trauma to the pediatric hip. Bone scintigraphy (BoS) can be used to evaluate femoral head perfusion, but current evidence in support of its use in the pediatric population is lacking. The purpose of this study was to evaluate the sensitivity and specificity of BoS to diagnose femoral head AVN in children. METHODS: We retrospectively reviewed patients who underwent BoS to assess femoral head perfusion following treatment of traumatic femoral neck fracture, hip dislocation, or SCFE a

Low bone mineral density on DXA and slipped capital femoral epiphysis as rare presentation in a child with Rubinstein-Taybi syndrome.

We present a 7-year-old girl with Rubinstein-Taybi syndrome (RTS) and slipped capital femoral epiphysis (SCFE). She underwent bilateral arthroscopy with implant fixation for the SCFE and the symptoms resolved. This was followed by fracture of the femur after minor trauma. Dual energy X-ray absorptiometry (DXA) scan done to evaluate her bone health revealed a low bone mineral density (BMD). Our case highlights the finding of low BMD on DXA and rare association of SCFE in a child with RTS. The conundrum in this case is whether this child can be labelled to have osteoporosis as defined by the cri

Bone remodeling of a femoral head after transtrochanteric rotational osteotomy for osteonecrosis associated with slipped capital femoral epiphysis: a case report.

A rare case of a 12-year-old boy on whom a joint-preserving operation for osteonecrosis after slipped capital femoral epiphysis (SCFE) was performed, is described. Firstly, in situ pinning was performed for acute-on-chronic SCFE. However, osteonecrosis and collapse of the femoral head occurred at 7 months after surgery. Secondly, transtrochanteric rotational osteotomy (TRO) was performed against progression of the collapse of the femoral head. Eight years of X-ray observation revealed bone remodeling at the osteonecrotic region. No documentation has been reported about the potential of bone re

Usefulness of Bone SPECT/CT for Predicting Avascular Necrosis of the Femoral Head in Children with Slipped Capital Femoral Epiphysis or Femoral Neck Fracture.

OBJECTIVE: This study aimed to investigate the usefulness of bone single-positron emission tomography/computed tomography (SPECT/CT) of the hip in predicting the later occurrence of avascular necrosis (AVN) after slipped capital femoral epiphysis (SCFE) or femoral neck fracture in pediatric patients. The quantitative parameters of SPECT/CT useful in predicting AVN were identified. MATERIALS AND METHODS: Twenty-one (male:female, 10:11) consecutive patients aged < 18 years (mean age ± standard deviation [SD], 11.0 ± 2.7 years) who underwent surgery for SCFE or femoral neck fracture and postoper

Early MRI Detection and Closed Bone Graft Epiphysiodesis May Alter the Course of Avascular Necrosis Following Unstable Slipped Capital Femoral Epiphysis.

BACKGROUND: Unstable slipped capital femoral epiphysis (SCFE) has an increased incidence of avascular necrosis (AVN). Early identification and surgical intervention for AVN may help preserve the femoral head. METHODS: We retrospectively reviewed 48 patients (50 hips) with unstable SCFE managed between 2000 and 2014. AVN was diagnosed based on 2 different postoperative protocols. Seventeen patients (17 hips) had a scheduled magnetic resonance imaging (MRI) between 1 and 6 months from initial surgery, and the remaining 31 patients (33 hips) were evaluated by plain radiographs alone. If AVN was

Capitate morphology as a risk factor for healing in scaphoid non-union.

INTRODUCTION: This study investigated the association between capitate morphology and the success rates of non-vascularized bone graft (NVBG) and vascularized bone graft (VBG) operations for scaphoid nonunion. METHODS: We retrospectively analysed 213 patients with established scaphoid non-union who underwent either non-vascularized bone graft (n = 75) or vascularized bone graft (n = 138). We analysed risk factors, including demographics, medical history, implant type, previous surgery, bone graft donor site and capitate morphology for their association with treatment outcome. We also assessed

[Isolated fractures of the capitate: treatment of delayed union].

Delayed and non-union of isolated fractures of the capitate are rare injuries. Initial plain radiographs do not always show the fractures. Especially fractures through the neck of the capitate can lead to delayed union. We report on a patient with persistent localised pain over the capitate 5 months after a fall on the hand. The CT examination showed a delayed union of the capitate, which was treated with an iliac cancellous bone graft and screw osteosynthesis. Bone healing was verified 3 months after operation. Operative therapy is recommended in cases of delayed union or non-union of the cap

Nonunion of the Capitate due to Delayed Diagnosis of an Isolated Fracture: A Case Report.

CASE: A 14-year-old girl was diagnosed with nonunion of an isolated capitate fracture 5 months after she first presented to the emergency department. The fracture of the capitate was treated by cancellous proximal bone graft and screw fixation. This current case provides details of the surgical fixation method and long-term functional outcome after nonunion of an isolated capitate fracture after a follow-up of 18 months following fixation. CONCLUSIONS: Persisting localized tenderness over the capitate should be an indication to perform an additional computed tomography or magnetic resonance i

Management of Isolated Capitate Nonunion: A Case Series and Literature Review.

Background  Isolated capitate nonunion is rare. No consensus on the appropriate treatment for this condition exists. Case Description  We reported two cases of capitate fracture nonunion presenting several months after untreated high-impact wrist trauma. Treatment was delayed as both patients' nonunions were missed on conventional radiographs. Both were ultimately diagnosed with advanced imaging and successfully treated with internal fixation and autogenous bone grafting. The relevant literature pertaining to capitate nonunion was reviewed. Literature Review  Immobilization and internal fixati
