The most challenging step in handling an intumescent cataract is the creation of a well-sized, round, centered, continuous, and curvilinear capsulorhexis. Due to high intralenticular pressure, the capsulorhexis tear has high tendency to extend equatorially leading to development of Argentinian flag sign. To overcome this, we describe a novel technique of capsulorhexis for intumescent white cataracts that does not need special instrumentation. Antero-posterior diameter of lens is more in the center due to its biconvex configuration. This leads to more intralenticular fluid accumulation and thus

In 4 patients with congenital cataract, subtle signs of anterior vitreous face (AVF) disturbance were documented during posterior continuous curvilinear capsulorhexis. The signs were vitreous strands in the anterior chamber, vitreous strands attached to the capsule flap, and distortion of the anterior and posterior capsulorhexis margins; the latter is considered a pathognomonic sign of AVF disturbance. As a result of the early recognition, modifications to the cataract surgery technique were made.

We describe a technique for preventing secondary membrane formation after pediatric cataract extraction. This technique involves capture of an intraocular lens (IOL) optic through a posterior curvilinear capsulorhexis opening in an attempt to maintain a clear visual axis in children after cataract surgery. This maneuver ensures centration of the posterior chamber IOL because the haptics remain in the capsular bag and the optic is captured in the posterior capsular opening. Also, the need for an anterior vitrectomy may be eliminated. Apposition of the anterior and posterior capsule leaflets ant

Cataracts are well known to be associated with myotonic dystrophy. Less well known are the phenomena of recurrent posterior capsule opacification and capsulorhexis contracture post cataract surgery. Two cases are described herein of postoperative capsular complications requiring multiple capsulotomies in patients with myotonic dystrophy. It is proposed that a common aetiology may underlie both posterior capsule opacification and capsulorhexis contracture in myotonic dystrophy cases.

Posterior capsule opacification (PCO) is a common delayed complication of cataract surgery, particularly in younger patients and those receiving multifocal intraocular lenses (IOLs). Intraoperative posterior capsulorhexis has been proposed as a preventive technique, but its use in adult cataracts is less documented. A 27-year-old patient presented with bilateral congenital posterior subcapsular cataracts, high myopia, and astigmatism. Comprehensive preoperative examination, including visual function and retinal imaging, was performed. The patient underwent phacoemulsification with toric multif

BACKGROUND: Although manual capsulorhexis is an ideal anterior capsulectomy technique for adults, it is more difficult to perform in very young eyes because the capsular bag is more elastic. Our mechanized anterior capsulectomy technique has compared favorably with manual capsulorhexis in the laboratory using autopsy eyes. We now report the results of circular mechanized anterior capsulectomy in consecutive pediatric patients receiving a posterior chamber intraocular lens (IOL) after cataract extraction.
METHODS: Twenty eyes of 17 consecutive patients between 6 months and 13 years of age unde
