Curative remnant total pancreatectomy for recurrent pancreatic acinar cell carcinoma: A case report.

INTRODUCTION AND IMPORTANCE: Acinar cell carcinoma (ACC) of the pancreas is a rare pancreatic cancer subtype (incidence, 0.5-2%) with unclear epidemiology and prognosis. Sometimes, repeat resection including total pancreatectomy is required for recurrence. We report a case of ACC recurring in the remnant pancreatic head after distal pancreatectomy that was successfully cured by remnant pancreatic resection following combination therapy with nab-paclitaxel (nab-PTX) and gemcitabine (GEM). CASE PRESENTATION: A 64-year-old woman was referred for pancreatic tumour treatment. CT revealed a 46-mm t

A case of acinar cell carcinoma originating from the accessory papilla of the duodenum.

CASE PRESENTATION: A 61-year-old female was referred to our hospital with a neoplastic lesion in the duodenum. Computed tomography with contrast enhancement revealed a 10-mm tumor in the duodenum. Upper gastrointestinal endoscopy revealed a submucosal tumor-like lesion in the descending part of the duodenum. Endoscopic ultrasound revealed a well-defined hypoechoic tumor. Biopsy and immunohistochemical findings including negative Synaptophysin and Chromogranin A staining and positive Trypsin and BCL10 staining suggested a carcinoma with acinar cell differentiation. Pancreatoduodenectomy was per

Spontaneous rupture of a pancreatic acinar cell carcinoma presenting as an acute abdomen.

INTRODUCTION: Pancreatic acinar cell carcinoma is a rare malignant pancreatic neoplasm. To the best of our knowledge, there has been no report on spontaneous rupture of acinar cell carcinoma. PRESENTATION OF CASE: A 39-year-old Azari male presented with a history of sudden onset, acute epigastric pain of 12-h duration. Eight hours later the patient's general condition rapidly deteriorated, blood pressure was decreased to 90/70mm/Hg and heart rate was increased to 120beat/min. Emergent abdominal computed tomography scan showed a well-defined hypo-dense, necrotic mass, measured 12cm×12cm that w

Acinar cell carcinoma of the pancreas metastatic to the ovary: a report of 4 cases.

We report 4 cases of acinar cell carcinoma of the pancreas, 3 presenting as metastases in the ovary, the first report of this circumstance, which may pose a broad differential diagnosis and caused significant diagnostic difficulty in all the cases. The average patient age was 57 (range: 28 to 81) years. In 3 cases, the ovarian tumors were detected before the pancreatic tumor; in 1 case, a large abdominal mass and ovarian tumors were discovered synchronously. The ovarian tumors were large, solid, white-tan on gross examination, and bilateral in 3 cases; the single case involving only 1 ovary ha

[Acinar cell carcinoma of the pancreas with predominant intraductal growth: report of a case].

Acinar cell carcinoma (ACC) of the pancreas accounts for approximately 1% of all exocrine pancreatic tumours. We report a rare form of ACC in a 66-year-old man. This tumour was revealed by epigastric pain and weight loss. Abdominal computed tomography showed a hypodense, well-demarcated, heterogeneous lesion, in the head of the pancreas, measuring 4.2 cm in diameter. There was a marked dilatation of the main pancreatic duct upstream, with tumour spreading within this duct. The diagnosis of ACC was made on the fine needle aspiration cytology performed during endoscopic ultrasound examination. O

Acinar cell carcinoma of the pancreas: can CT and MR features predict survival?

OBJECTIVE: To evaluate the CT and MRI features of pancreatic acinar cell carcinoma (pACC) and their association with patient outcome and survival. METHODS: This retrospective single-center study included 49 patients with pathology-proven pancreatic acinar cell carcinoma who underwent diagnostic imaging between August 1998 - September 2019. Two radiologists reviewed CT and MRI features independently. Survival was estimated using the Kaplan-Meier method, and Cox proportional-hazards regression model was used to identify factors associated with survival. RESULTS: pACC tended to present as a sol

Prevention of pancreatic acinar cell carcinoma by Roux-en-Y Gastric Bypass Surgery.

Roux-en-Y Gastric Bypass Surgery (RYGB) prevents the occurrence of pancreatic cell acinar carcinoma (ACC) in male and female Ngn3-Tsc1-/- mice. Ngn3 directed Cre deletion of Tsc1 gene induced the development of pancreatic ACC. The transgenic mice with sham surgery demonstrated a cancer incidence of 96.7 ± 3.35% and survival rate of 67.0 ± 1.4% at the age of 300 days. Metastasis to liver and kidney was observed in 69.7 ± 9.7% and 44.3 ± 8.01% of these animals, respectively. All animals with RYGB performed at the age of 16 weeks survived free of pancreatic ACC up to the age of 300 days. RYGB sig

Clinics in diagnostic imaging (157). Acinar cell carcinoma (ACC) of the pancreatic tail.

A 50-year-old Chinese man presented to the clinic with left hypochondrial pain, more than 10 kg of weight loss over a 3-month period, and a firm, large, ill-defined mass in the left upper quadrant. Contrast-enhanced computed tomography of the abdomen and pelvis revealed a well-circumscribed exophytic pancreatic mass with features suggestive of acinar cell carcinoma (ACC). The patient underwent chemotherapy and radiotherapy, with no evidence of local recurrence detected at one-year follow-up. He remains under close surveillance by his oncologist. Treatment for ACC includes surgical resection wi

Pancreatic acinar cell carcinoma extending into the common bile and main pancreatic ducts.

Acinar cell carcinoma (ACC) of the pancreas is relatively rare, accounting for only approximately 1% of all exocrine pancreatic tumors. A 69-year-old man was found to have a mass lesion measuring approximately 4 cm in diameter in the pancreatic head on ultrasound, abdominal dynamic CT, and percutaneous transhepatic cholangiography. Magnetic resonance cholangiopancreatography showed defect of the lower common bile duct (CBD) due to obstruction by the tumor cast. Histopathologically, the pancreatic head tumor invaded the main pancreatic duct (MPD) and CBD with extension into the CBD in a form of

Differentiating pancreatic acinar cell carcinoma from solid pseudopapillary neoplasm using contrast-enhanced computed tomography.

BACKGROUND: Solid pseudopapillary neoplasm (SPN) and pancreatic acinar cell carcinoma (PACC) represent two uncommon pancreatic tumors with distinct biological behaviors and management strategies. Both entities can present with overlapping appearances on contrast-enhanced computed tomography (CECT), posing a diagnostic challenge preoperatively. Therefore, reliable imaging differentiation between these tumors is clinically important. This study aimed to evaluate computed tomography (CT) imaging features for differentiating SPN from PACC and to develop a combined model to improve preoperative dia

Case Report: Pseudoprogression of pancreatic acinar cell carcinoma after PD-1 blockade integrated treatment.

Pancreatic Acinar Cell Carcinoma (PACC) is a rare subtype of pancreatic cancer, and its systematic treatment protocol is still pending for further exploration. There have been a few reports that PACC patients benefited from immunotherapy-integrated treatment. Here, we present a metastatic PACC patient who had new hepatic lesions but lowered serum AFP and CA19-9 after 2 cycles of integrated treatment that included chemotherapy, PD-1 blockade and antiangiogenic therapy, which was later confirmed to be pseudoprogression (PsP) by pathological diagnosis. So far as we know, this is the first reporte

Pancreatic acinar cell carcinoma with extension into the main pancreatic duct: a case report.

BACKGROUND: Pancreatic acinar cell carcinoma (PACC) is a rare exocrine malignant tumor. Its widespread intraductal extension into the main pancreatic duct (MPD) is also rare. CASE PRESENTATION: We report the case of a 71-year-old man with PACC with MPD extension. The patient was assessed with laboratory and radiographic investigations that facilitated a preoperative diagnosis. Endoscopic ultrasonography (EUS) and dynamic thin-slice multi-detector row computed tomography (MDCT) were useful for determining the resection line of the pancreas. EUS-guided fine needle aspiration (EUS-FNA) was also

Pancreatic acinar cell carcinoma presenting with panniculitis, successfully treated with FOLFIRINOX: A case report.

Pancreatic acinar cell carcinoma (PACC) is a rare tumor of the exocrine pancreas, representing only 1% of all pancreatic malignancies. A 50-year-old man presented with edema of the thumb joints bilaterally, followed by an appearance of masses in the bilateral lower extremities and fever (38°C). The masses were diagnosed as panniculitis by skin biopsy, and multiple intraperitoneal masses were incidentally detected on pelvic magnetic resonance imaging performed to investigate the leg abnormalities. The patient was referred to the Kyushu University Hospital for further investigation, and fluorode

Functional pancreatic acinar cell carcinoma extending into the main pancreatic duct and splenic vein.

INTRODUCTION: Pancreatic acinar cell carcinoma (ACC) has several unique characteristics, such as its progression pattern, spreading into the pancreatic duct and large blood vessels, and its secretion of pancreatic exocrine enzymes, which induces a paraneoplastic syndrome. CASE REPORT: A 79-year-old Japanese man, with medical history of chronic renal failure, was referred to our institution for the examination of his abdominal pain and hyperglycemia. Plain computed tomography demonstrated a mass lesion, 4 cm in diameter, in the body of pancreas. Abdominal ultrasonogram demonstrated a bulky, hy

[Extension of an acinar cell pancreatic carcinoma with cystic changes invading the Wirsung canal].

We report a case of acinar cell carcinoma of the pancreas with misleading cystic changes. A 32-year-old woman presented with symptoms suggesting acute pancreatitis on chronic pancreatitis. The abdominal computed tomography and the endoscopic retrograde pancreatography demonstrated hypertrophy of the pancreatic head associated with global dilatation of main pancreatic duct and secondary canals and a 5 cm communicating cyst. A intraductal papillary-mucinous tumor was suggested. Microscopic findings showed a poorly differentiated adenocarcinoma. Six months later, a liver metastasis was detected.

SEER-based survival nomogram for salivary acinar cell carcinoma using clinical and surgical factors.

Acinar cell carcinoma (AcCC) has a certain risk of recurrence, metastasis or even death. This study aimed to explore the relationships between clinicopathological characteristics and survival in AcCC patients, and a nomogram model was developed and validated for predicting overall survival (OS). AcCC patients were identified from the Surveillance, Epidemiology, and End Results Program. An external validation was conducted using an independent cohort from our hospital. Independent prognostic factors for OS were determined using univariate/multivariate Cox regression analyses, and a nomogram was

RNA methyltransferase 3 drives pancreatic acinar cell carcinoma growth and is a therapeutic target.

Pancreatic acinar cell carcinoma (ACC) is a rare and aggressive malignancy whose molecular basis remains poorly understood. N6-methyladenosine (m⁶A) RNA modification, mediated particularly through methyltransferase 3 (METTL3), has emerged as a critical regulator in various cancers. Here, we investigated the role of METTL3-mediated RNA methylation in ACC development and progression. We used transgenic mouse models overexpressing Mettl3 and SV40 large T antigen under the pancreatic elastase I promoter. Comprehensive analyses included m⁶A-methylated RNA immunoprecipitation sequencing (MeRIP-seq),
