Indexed maximal left atrial volume predicts response to cardiac resynchronization therapy.

AIMS: Cardiac resynchronization therapy (CRT) has shown morbidity and mortality benefits in patients with advanced congestive heart failure (HF). Since about one-third of the patients did not appear to respond to CRT, it would seem reasonable to try to identify patients more accurately before implantation. Left atrial (LA) dimension has been proposed as a powerful outcome predictor in patients with heart disease. Accordingly, the aim of this study is to prospectively assess the predictive value of LA for selecting CRT responders. METHODS: Fifty two consecutive patients with refractory HF, sin

Regional contributions to left ventricular stroke volume determined by cardiac magnetic resonance imaging in cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) restores ventricular synchrony and induces left ventricular (LV) reverse remodeling in patients with heart failure (HF) and dyssynchrony. However, 30% of treated patients are non-responders despite all efforts. Cardiac magnetic resonance imaging (CMR) can be used to quantify regional contributions to stroke volume (SV) as potential CRT predictors. The aim of this study was to determine if LV longitudinal (SVlong%), lateral (SVlat%), and septal (SVsept%) contributions to SV differ from healthy controls and investigate if these parameters can p

Cardiac Magnetic Resonance-Derived Indexed Volumes and Volume Ratios of the Cardiac Chambers Discriminating Group 2 Pulmonary Hypertension From Other World Health Organization Groups.

OBJECTIVE: The aims of the study were to assess the performance of cardiac magnetic resonance (CMR)-derived cardiac chamber volumes and volume ratios to identify group 2 pulmonary hypertension (PH) patients and to determine their cutoff values with the highest sensitivity and specificity. METHODS: One hundred six patients underwent CMR, 2 months after the diagnosis of PH by right heart catheterization. We classified patients with pulmonary capillary wedge pressure of greater than 15 mm Hg as group 2 PH. Cardiac chamber volumes indexed to the body surface area and volume ratios were correlated

Cardiac output and stroke volume dissociate during fluid loading in conscious sheep.

Background. A fluid challenge is judged by a rise in stroke volume or cardiac output, on the twin assumptions that the two report the same physiological effect and that either reflects circulatory benefit. We tested these assumptions across a fourfold crystalloid load in conscious sheep with intact autonomic reflexes. Methods. We performed a secondary analysis of 26 intravenous infusions of 0.9% saline (25, 50 or 100 mL·kg-1 over 20 min) in 13 conscious splenectomized sheep, a within-animal crossover. Cardiac output, arterial and central venous pressures and blood hemoglobin were recorded and

Umbilical Venous Flow Volume and Fetal Combined Cardiac Output in Twin Pregnancies.

This study aimed to establish normal reference ranges of combined cardiac output (CCO) and umbilical venous flow volume (UVFV) in twin fetuses at 20 to 28 weeks of gestation and to evaluate the differences between monochorionic and dichorionic twins. CCO and UVFV were prospectively measured by ultrasound at two centers. The following exclusion criteria were applied: age <18 years or >45 years, first hospital visit at >16 weeks of gestation, monochorionic monoamniotic twin pregnancy, fetal structural or chromosomal abnormality, fetal growth restriction, twin-twin transfusion syndrome, twin anem

Volume Rendering-Based Phrenic Nerve/Pericardiophrenic Bundle Identification Using Cardiac Computed Tomography.

ANATOMY: Phrenic nerves descend anterior to the pulmonary hila, sandwiched between the pericardium and parietal pleura, thus running extrinsically to the heart and lungs. PATHOLOGY: Phrenic nerve injury during catheter ablation or diaphragmatic stimulation during cardiac resynchronization therapy. IMAGING CORRELATION: Conventional approach of preprocedural identification of the phrenic nerve includes slice-by-slice detection of the pericardiophrenic bundle using axial images. However, it is time-consuming with substantial uncertainty and difficulty in precise structural tracing in each axial

Establishing minimally invasive cardiac surgery in a low-volume mitral surgery centre.

INTRODUCTION: Despite early enthusiasm, minimally invasive cardiac surgery has had a low uptake compared with novel techniques in interventional cardiology. Steep learning curves from high-volume centres have deterred smaller units from engaging, even though low-volume centres undertake a large proportion of surgical interventions worldwide. We sought to identify the safety and experience of learning minimally invasive cardiac surgery after undertaking a structured fellowship at Blackpool Victoria Hospital, a low-volume centre. MATERIALS AND METHODS: A retrospective analysis of outcomes for a

Fetal cardiac disease and fetal lung volume: an in utero MRI investigation.

OBJECTIVES: Magnetic resonance imaging (MRI) is a powerful, noninvasive tool to study fetal lung volumes after 18 weeks of gestation in vivo. In neonates with congenital heart disease (CHD), proper lung function is essential for postnatal survival. Antenatal detection of abnormal pulmonary development may help to optimize prenatal and perinatal management of at-risk fetuses. We aimed to investigate lung volumes in fetuses with prenatally diagnosed heart disease. METHODS: A cross-sectional, retrospective study of 105 consecutive singleton pregnancies with CHD and a control, non-CHD group (n =

Airway trauma in a high patient volume academic cardiac electrophysiology laboratory center.

BACKGROUND: Providing anesthesia and managing airways in the electrophysiology suite can be challenging because of its unique setting outside of the conventional operating room. We report our experience of several cases of reported airway trauma including tongue and pharyngeal hematoma and vocal cord paralysis in this setting. METHODS: We analyzed all of the reported airway trauma cases between December 2009 and January 2011 in our cardiac electrophysiology laboratories and compared these cases with those without airway trauma. Data from 87 cases, including 16 cases with reported airway traum

New molecular mechanisms for cardiovascular disease: cardiac hypertrophy and cell-volume regulation.

Cardiac hypertrophy is an increase in the muscle volume of the ventricle due to the enlargement of cardiac cells. Physiological cardiac hypertrophy is the normal response to healthy exercise, and pathological hypertrophy is the response to increased stress such as hypertension. Intracellular and extracellular aniosmotic conditions also change cell volume. Since persistent cell swelling or cell shrinkage during aniosmotic conditions results in cell death, the ability to regulate cell volume is important for the maintenance of cellular homeostasis. Cell swelling activates a regulatory volume dec

[Volume regulation in cardiac insufficiency : possibilities of telephone coaching and peritoneal dialysis].

Volume retention is the hallmark of progressive heart failure, both systolic and diastolic (heart failure with preserved ejection fraction). It represents the cause of the main symptoms (dyspnea, edema, liver synthesis) and also the main target of drug therapy. Antagonizing excessive volume retention is also the most important therapy element. Many patients can be stabilized with sequential nephron blockade (thiazide + loop diuretics) combined with afterload reduction [blockade of the RAAS (renin-angiotensin-aldosterone) system]. Personal patient coaching combined with telemetric components (w

Ultrasound estimation of volume of postoperative pleural effusion in cardiac surgery patients.

The aim of this study was to establish a practical simplified formula to facilitate the management of a frequently occurring postoperative complication, pleural effusion. Chest ultrasonography with better sensitivity and reliability in the diagnosis of pleural effusions than chest X-ray can be repeated serially at the bedside without any radiation risk. One hundred and fifty patients after cardiac surgery with basal pleural opacity on chest X-ray have been included in our prospective observational study during a two-year period. Effusion was confirmed on postoperative day (POD) 5.9+/-3.2 per c

Myocardial extracellular volume quantification in cardiac amyloidosis: a comparative study between cardiac computed tomography and magnetic resonance imaging.

OBJECTIVES: Myocardial extracellular volume (ECV) on computed tomography (CT), an alternative to cardiac magnetic resonance (CMR), has significant practical clinical advantages. However, the consistency between ECVs quantified via CT and CMR in cardiac amyloidosis (CA) has not been investigated sufficiently. Therefore, the current study investigated the application of CT-ECV in CA with CMR-ECV as the reference standard. METHODS: We retrospectively evaluated 31 patients with CA who underwent cardiac CT and CMR. Pearson correlation analysis was performed to investigate correlations between CT-E

Cardiac Mechanoenergetics in Patients with Acute Myocardial Infarction: From Pressure-Volume Loop Diagram Related to Cardiac Oxygen Consumption.

Acute myocardial infarction (AMI) results in significant changes in cardiac structure and functions, leading to left ventricular remodeling and subsequent systolic and diastolic dysfunction. To improve current approaches in diagnoses, treatments, and prevention of cardiovascular diseases, a better understanding of cardiac mechanoenergetics, including systolic performance and energy demand, becomes paramount. In this review, we summarize cardiac mechanics, cardiac energetics, and their relationship in complications related to AMI using 2 important physiologic frameworks, pressure-volume loops a

Baseline atrial volume indices and major adverse cardiac events following thoracic radiotherapy.

INTRODUCTION: Patients receiving thoracic radiotherapy (RT) have an increased risk of major adverse cardiac events (MACE) posttreatment. We utilized machine learning (ML) to discover novel predictors of MACE and validated them on an external cohort. METHODS: This multi-institutional retrospective study included 984 patients [n = 803 non-small cell lung cancer (NSCLC), n = 181 breast cancer] treated with radiotherapy. Extreme gradient boosting was utilized to discover novel clinical, dosimetric, and anatomical features (CT-based cardiac substructure segmentations) associated with MACE in a coh

Time Course of Cardiac Arrhythmia Following High-Volume Exercise in Recreational Cyclists.

BACKGROUND: The aim of this study was to investigate the time course of cardiac arrhythmias following a bout of high-volume exercise and to identify factors associated with abnormal cardiac arrhythmia. METHODS: Thirty-four recreational cyclists with 12 (interquartile range, 7-20) years of riding experience were recruited for this repeated-measure cohort study. Participants cycled for up to 6 hours at ≈80% heart rate reserve. Cardiac arrhythmias were recorded using a 5-lead Holter monitor for 4 days before exercise, the day of exercise, and 4 days after exercise. Arrhythmias were quantified as

Mean Platelet Volume and Major Adverse Cardiac Events following Percutaneous Coronary Intervention.

BACKGROUND: Mean platelet volume (MPV) has been introduced as a simple and accurate method for assessing platelet function, which can be used as a prognostic marker for cardiovascular events. We investigated whether pre-procedural MPV could predict major adverse cardiac events (MACE) in candidates for elective percutaneous coronary intervention (PCI). METHODS: In this large retrospective cohort, we reviewed the clinical and follow-up data of 4199 candidates (mean age = 59.9 ± 10.3 years; female patients = 1440 [34.3%]) for elective PCI due to unstable angina (UA) or non-ST segment elevation m
