Radionuclide computed tomography of the body using routine radiopharmaceuticals. II. Clinical applications.

A whole-body computed tomography system for single-photon emitters was used to investigate the clinical utility of this imaging modality. We have explored its application in brain, lung, liver, kidney, cardiac, bone, and gallium imaging in over 200 patients. Brain images demonstrated better lesion contrast than that in standard scintiphotos. Images of the lung and liver showed radionuclide distribution that was not readily apparent in standard scintiphotos. Moderate or strongly positive pyrophosphate myocardial images demonstrate the potential for infarct quantitation. ECG-gated cardiac blood-

Ventricular voltage-gated ion channels: Detection, characteristics, mechanisms, and drug safety evaluation.

Cardiac voltage-gated ion channels (VGICs) play critical roles in mediating cardiac electrophysiological signals, such as action potentials, to maintain normal heart excitability and contraction. Inherited or acquired alterations in the structure, expression, or function of VGICs, as well as VGIC-related side effects of pharmaceutical drug delivery can result in abnormal cellular electrophysiological processes that induce life-threatening cardiac arrhythmias or even sudden cardiac death. Hence, to reduce possible heart-related risks, VGICs must be acknowledged as important targets in drug disc

Pulsatile Deformations of a Conformable Descending Thoracic Aortic Endograft in Aneurysm, Dissection, and Blunt Traumatic Aortic Injury Patients.

PURPOSE: This study presents analytic techniques to quantify cardiac pulsatility-induced deformations of thoracic aortic endografts in patients with thoracic aortic aneurysm (TAA), dissection (TAD), and blunt thoracic aortic injury (BTAI) after thoracic endovascular aortic repair (TEVAR). TECHNIQUE: We analyzed 19 image data sets from 14 patients treated for TAA, TAD, and BTAI with cardiac-gated post-TEVAR CTs. Systolic and diastolic geometric models were constructed and diametric, axial, and bending deformations were quantified. For patients with cardiac-gated pre-op scans, the damping of pu

[Applications of multi-slice computed tomography imaging in children with congenital heart diseases].

Recently-devised 3-dimensional (3D) diagnostic modalities such as magnetic resonance imaging and multi-slice computed tomography (MSCT) allow visualization of the heart and great vessels from any angle of view and perspective. Among them, the MSCT examination provides highly contrasted 3D images of complicated congenital heart diseases (CHD) and is now widely used instead of cardiac angiography. Here we introduce several applications of the MSCT examination in children with CHD. The MSCT examinations were performed without electorocardiogram (ECG)-gating in order to minimize radiation exposure

Pulmonary atresia with double ductus arteriosus.

We present the case of a neonate with pulmonary atresia and persistent bilateral patent ductus arteriosus imaged by gated multidetector CT. Traditionally, these patients have been assessed preoperatively with invasive angiocardiography or with cardiovascular magnetic resonance under sedation. Our case illustrates that contemporary cardiovascular CT techniques can now be used for preoperative evaluation with minimal radiation penalty, obviating the risks of sedation or cardiac catheterization.

Multiphasic MR imaging: a new method for direct imaging of pulsatile CSF flow.

A new technique is described that allows for the creation of pure pulsatile flow magnetic resonance (MR) images in a single acquisition. Five to 16 electrocardiographically gated images spanning the entire cardiac cycle are obtained with use of a gradient-echo pulse sequence. The section can be varied from 4 mm thick to full thickness projection. Taken singly, each image provides direct assessment of flow direction and velocity. Subtraction of image pairs eliminates signal detected from stationary protons, producing images of pulsatile flow. In this study the technique was used to image the fl

Computed tomography of the heart and great vessels: present and future.

Computed tomography (CT) has emerged as a new imaging method for the diagnosis and evaluation of cardiovascular disease. With CT body scanners and contrast enhancement, evaluation of aortic dissections and aneurysms, coronary bypass graft patency, cardiovascular thrombus, cardiac tumors, and pericardial disease is possible. On occasion, this technique provides clinically useful information that is not available with other imaging methods. Electrocardiographic gating retrospectively or prospectively improves the image resolution of CT scans, but a new ultrafast CT scanner with a scan time of 30

Dark blood cardiovascular magnetic resonance of the heart, great vessels, and lungs using electrocardiographic-gated three-dimensional unbalanced steady-state free precession.

BACKGROUND: Recently, we reported a novel neuroimaging technique, unbalanced T1 Relaxation-Enhanced Steady-State (uT1RESS), which uses a tailored 3D unbalanced steady-state free precession (3D uSSFP) acquisition to suppress the blood pool signal while minimizing bulk motion sensitivity. In the present work, we hypothesized that 3D uSSFP might also be useful for dark blood imaging of the chest. To test the feasibility of this approach, we performed a pilot study in healthy subjects and patients undergoing cardiovascular magnetic resonance (CMR). MAIN BODY: The study was approved by the hospita

A new method for the study of velopharyngeal function using gated magnetic resonance imaging.

The purpose of this project was to assess the feasibility of imaging the velopharynx of adult volunteers during repetitive speech, using gated magnetic resonance imaging (MRI). Although a number of investigators have used conventional MRI in the study of the human vocal tract, the mismatch between the lengthy time necessary to acquire sufficiently detailed images and the rapidity of movement of the vocal tract during speech has forced investigators to acquire images either while the subject is at rest or during sustained utterances. The technique used here acquired a portion of each image duri

MoCo + ROVir: Synergy Between Respiratory Motion Compensation and Cardiac Receive Region Focusing for Cardiac MRI.

PURPOSE: To improve cardiac motion representation and reduce artifacts for cardiac- and respiratory-resolved imaging through a synergistic combination of retrospective cardiac phased array RF focusing and rigid respiratory motion compensation (MoCo). METHODS: We incorporated cardiac receive focusing using region-optimized virtual coils (ROVir) and MoCo into cardiac- and respiratory-resolved low-rank tensor (LRT) reconstruction, hypothesizing that the combination of MoCo + ROVir would prioritize the LRT representation of cardiac motion over respiratory motion. We compared LRT, MoCo-LRT, ROVir-

Imaging diagnosis of aortic stenosis.

Aortic valve stenosis (AS) is the commonest primary valve disorder with increasing prevalence with age. Trans-thoracic echocardiogram is the main imaging technique used to diagnose AS, but discrepancy in diagnosis has been described in almost one third of cases. Other imaging methods, particularly electrocardiogram (ECG)-gated computed tomography, have now emerged to further clarify the diagnosis of AS by both demonstrating the degree of calcification in the valve as well as aortic valve area. Cardiac magnetic resonance imaging allows accurate quantification of ventricular function and evaluat

Visualization of proximal coronary arteries on high-pitch electrocardiogram-triggered computed tomography in pediatric congenital heart disease: effects of heart rate and body surface area.

BACKGROUND: As CT technology has advanced, techniques for pediatric cardiac CT in congenital heart disease have evolved from retrospective electrocardiography (ECG)-gating with relatively high radiation doses to lower-dose prospective ECG-gating and even single-beat gated scans. Despite these advances, coronary artery imaging in children remains challenging because of their small vessel size and high heart rates, often necessitating retrospective gating. OBJECTIVE: Evaluate coronary artery visualization in pediatric patients (<20 years) who underwent low-dose high-pitch ECG-triggered scans an

Image-derived input function from cardiac gated maximum a posteriori reconstructed PET images in mice.

PURPOSE: The purpose of this study was to determine if accurate image-derived input functions (IDIF) can be measured from cardiac gated positron emission tomography (PET) images reconstructed using ordered subset expectation maximization-maximum a posteriori (OSEM-MAP) without further correction. PROCEDURES: IDIFs from the left ventricle were measured from cardiac gated PET images reconstructed using OSEM-MAP with computed tomography (CT)-based attenuation correction for five C57/BL6 mice. The accuracy of the IDIF was tested against blood samples using Bland-Altman analysis. RESULTS: Image-d

Diagnostic quality (feasibility and utility) of cardiac CT imaging in pediatric patients on mechanical circulatory support.

BACKGROUND: Pediatric patients on ventricular assist devices (VAD) and Venoarterial extracorporeal membrane oxygenation (VA-ECMO) increasingly undergo cardiac computed tomography (CT) for device assessment, complication evaluation, and transplant planning. Imaging this population is challenged by metallic device components, contrast timing in altered hemodynamics, need for sedation, and small patient size. The diagnostic quality and utility of cardiac CT in pediatric patients on mechanical support have not been systematically described. METHODS: We retrospectively reviewed electrocardiogram (

Cerebellar Tissue Strain in Chiari Malformation with Headache.

OBJECTIVE: The pathogenesis of Chiari malformation type 1 (CM-1)-associated Valsalva headache is unknown, but it may be caused by abnormal cerebellar tonsil tissue strain. Advances in cardiac-gated magnetic resonance imaging (MRI) techniques such as balanced fast-field echo (bFFE) allow quantification of the motion of anatomic structures and can be used to measure tissue strain. The current study investigated the relationship between Valsalva heachache and tonsillar motion in patients with CM-1. METHODS: A retrospective review of patients with CM-1 who had undergone cardiac-gated bFFE MRI was

Central thrombi in pulmonary arterial hypertension detected by MR imaging.

Differentiation of thrombi from slow flow in the pulmonary arteries, sometimes observed in the presence of pulmonary arterial hypertension, can be equivocal. Magnetic resonance (MR) imaging was performed in a patient with chronic pulmonary thromboembolism and pulmonary arterial hypertension using an electrocardiographically gated technique that allowed visualization of the pulmonary arteries at the end of diastole and multiple times during systole. These images were compared with those of a patient with primary pulmonary hypertension and those of healthy subjects. Thrombi were discrete structu

Fetal cardiac cine magnetic resonance imaging in utero.

Fast magnetic resonance imaging (MRI) led to the emergence of 'cine MRI' techniques, which enable the visualization of the beating heart and the assessment of cardiac morphology and dynamics. However, established cine MRI methods are not suitable for fetal heart imaging in utero, where anatomical structures are considerably smaller and recording an electrocardiogram signal for synchronizing MRI data acquisition is difficult. Here we present a framework to overcome these challenges. We use methods for image acquisition and reconstruction that robustly produce images with sufficient spatial and

Intrinsic gating for small-animal computed tomography: a robust ECG-less paradigm for deriving cardiac phase information and functional imaging.

BACKGROUND: A projection-based method of intrinsic cardiac gating in small-animal computed tomography imaging is presented. METHODS AND RESULTS: In this method, which operates without external ECG monitoring, the gating reference signal is derived from the raw data of the computed tomography projections. After filtering, the derived gating reference signal is used to rearrange the projection images retrospectively into data sets representing different time points in the cardiac cycle during expiration. These time-stamped projection images are then used for tomographic reconstruction of differ

Accelerated free-breathing 3D T1ρ cardiovascular magnetic resonance using multicoil compressed sensing.

BACKGROUND: Endogenous contrast T1ρ cardiovascular magnetic resonance (CMR) can detect scar or infiltrative fibrosis in patients with ischemic or non-ischemic cardiomyopathy. Existing 2D T1ρ techniques have limited spatial coverage or require multiple breath-holds. The purpose of this project was to develop an accelerated, free-breathing 3D T1ρ mapping sequence with whole left ventricle coverage using a multicoil, compressed sensing (CS) reconstruction technique for rapid reconstruction of undersampled k-space data. METHODS: We developed a cardiac- and respiratory-gated, free-breathing 3D T1ρ
