On July 4, 1982, the Space Shuttle Columbia landed at Edwards Air Force Base, CA, thus successfully completing the fourth and last in a series of Orbital Flight Tests (OFT) of the Space Transportation System (STS). The primary goal of medical operations support for the OFT was to assure the health and well-being of flight personnel during all phases of the mission. To this end, crew health status was evaluated preflight, inflight, and postflight. Biomedical flight test requirements were completed in the following areas: physiological adaptation to microgravity, cabin acoustical noise, cabin at

This study examined the effectiveness of a short-duration but high-intensity exercise countermeasure in combination with a novel oral volume load in preventing bed rest deconditioning and orthostatic intolerance. Bed rest reduces work capacity and orthostatic tolerance due in part to cardiac atrophy and decreased stroke volume. Twenty seven healthy subjects completed 5 wk of -6 degree head down bed rest. Eighteen were randomized to daily rowing ergometry and biweekly strength training while nine remained sedentary. Measurements included cardiac mass, invasive pressure-volume relations, maximal

Objective. To investigate whether LBNP during 21 d -6 degrees head-down tilt (HDT) would modify orthostatic tolerance. Method. 12 healthy males, age 23.7 +/- 5.0, were exposed to -6 degrees HDT for 21 d. 6 of them received -4.0 kPa LBNP sessions for 1 h/d from 15th day to 21st day of HDT. The other 6 served as control. HUT + 75 degrees, 20 min orthostatic tolerance test were done before, on day 10 and on day 21 of HDT. Result. During HUT + 75 degrees, 20 min orthostatic tolerance test on day 10 of HDT, 5 subjects of the control group and 4 of the LBNP group presented presyncopal or syncopal si

The harsh environmental conditions in space, particularly weightlessness and radiation exposure, can negatively affect cardiovascular function and structure. In the future, preventive cardiology will be crucial in enabling safe space travel. Indeed, future space missions destined to the Moon and from there to Mars will create new challenges to cardiovascular health while limiting medical management. Moreover, commercial spaceflight evolves rapidly such that older persons with cardiovascular risk factors will be exposed to space conditions. This review provides an overview on studies conducted

Space travel has necessitated intensive study of certain physical conditions, notably cardiovascular deconditioning, motion sickness, musculoskeletal atrophy and psychological isolation. Benefits of this research are beginning to be available to terrestrial medicine, for example in research on osteoporosis and motion sickness. Other space technology of benefit to medicine includes diagnostic and therapeutic devices, transmission of medical data and satellite communications. A permanently inhabited space station is planned for the 1990s and further research into isolation, occupational hazards

Physical deconditioning often occurs during the acute phase after cardiovascular surgery, and unassisted walking is required to achieve independence, to manage cardiac diseases, and to prevent recurrences. This study aims to investigate the characteristics of independent walking after cardiovascular surgery. We conducted a retrospective cohort study in patients who underwent cardiovascular surgeries (total of 567 patients): 153 in the coronary artery bypass grafting (CABG) group, 312 in the heart valve surgery group, and 102 in the aortic surgery group. We evaluated the effect of each surgery

Postural orthostatic tachycardia syndrome (POTS) is a variant of cardiovascular autonomic disorder characterized by an excessive heart rate increase on standing and orthostatic intolerance. POTS affects younger individuals 15-45 years old with a distinct female predominance (≈80%). The prevalence ranges between 0.2% and 1.0% in developed countries. The onset of POTS is typically precipitated by immunological stressors such as viral infection, vaccination, trauma, pregnancy, surgery or psychosocial stress. The most common complaints are dizziness, weakness, rapid heartbeat and palpitation on st

Bidirectional associations between mood disorders and cardiovascular diseases are extensively documented. However, the precise physiological and biochemical mechanisms that underlie such relationships are not well understood. This review focuses on the neurobiological processes and mediators that are common to both mood and cardiovascular disorders. The discussion places an emphasis on the role of exogenous stressors in addition to: (a) neuroendocrine and neurohumoral changes involving dysfunction of the hypothalamic-pituitary-adrenal axis and the activation of the renin-angiotensin-aldosteron

The heart rate variability (HRV) features HRV0.75 and HRV0.5, obtained by detrended fluctuation analysis during incremental cycle ergometer tests, have been shown to correlate with indices of cardiorespiratory fitness. Here, we evaluate the ability of these features to track individual changes in cardiorespiratory fitness induced by head-down bed rest (HDBR) among late middle-aged adults. Twenty-two healthy participants (55-65 yr old, 11 women) were exposed to 14-day HDBR. Eleven participants were assigned to a group performing daily aerobic and resistive exercises during HDBR, whereas the oth

The weightless environment of space imposes specific physiologic adaptations on healthy astronauts. On return to Earth, these adaptations manifest as physical impairments that necessitate a period of rehabilitation. Physiologic changes result from unloading in microgravity and highly correlate with those seen in relatively immobile terrestrial patient populations such as spinal cord, geriatric, or deconditioned bed-rest patients. Major postflight impairments requiring rehabilitation intervention include orthostatic intolerance, bone demineralization, muscular atrophy, and neurovestibular sympt

Astronauts exhibit several pathophysiological changes due to a variety of stressors related to the space environment, including microgravity, space radiation, isolation, and confinement. Space motion sickness, bone and muscle mass loss, cardiovascular deconditioning and neuro-ocular syndrome are some of the spaceflight-induced effects on human health. Optimal nutrition is of the utmost importance, and-in combination with other measures, such as physical activity and pharmacological treatment-has a key role in mitigating many of the above conditions, including bone and muscle mass loss. Since t

Several consistent medical problems have been encountered by astronauts during space flights. These include vestibular dysfunction, weight loss, increase in height, upward fluid shift, anemia, cardiovascular deconditioning, muscle atrophy, and bone loss. Almost all of these alterations can be attributed to the absence of gravitational force. Most are adaptive in nature and therefore reversible, but readaptation after returning to earth may cause further problems (e.g., in the case of vestibular dysfunction). The most recalcitrant and disturbing of all these problems is the relentless bone loss

Chronic kidney disease (CKD) is growing worldwide, with increasing numbers of patients facing end-stage renal disease, high cardiovascular risk, disability and mortality. Early recognition of CKD and improvements in lifestyle are crucial for maintaining or recovering both physical function and quality of life. It is well known that reducing sedentariness, increasing physical activity and initiating exercise programs counteract cardiovascular risk and frailty, limit deconditioning and sarcopenia, and improve mobility, without side-effects. However, these interventions, often requested by CKD pa

BACKGROUND: During the past 20 years, significant advances in patient care have resulted in individuals with spinal cord injury (SCI) living longer than before. As lifespan increases, cardiovascular complications are emerging as the leading cause of mortality in this population, and individuals with SCI develop cardiovascular disease at younger ages than their able-bodied counterparts. To address this increasing clinical challenge, several recent studies investigated the central cardiovascular adaptations that occur following SCI. However, a somewhat less recognized component of cardiovascular

Although performing exercise studies in patients with neuromuscular disorders (NMD) is difficult, the number of randomized controlled trials is steadily increasing. There is growing evidence for a positive effect of aerobic exercise in several NMD, on the other hand, the evidence for the effect of strength training is still scarce. Many NMD patients are captured in a vicious circle of physical inactivity, and it is important to let patients adhere to an active lifestyle, in order to prevent further chronic cardiovascular and muscle deconditioning and increased cardiovascular health risks. Exercise is a critical countermeasure to prevent cardiovascular deconditioning during spaceflight; however, exercise does not protect astronauts from postflight orthostatic intolerance. Artificial gravity (AG) by short-arm centrifugation can attenuate reductions in orthostatic tolerance following prolonged head-down bed rest (HDBR), but AG does not protect cardiorespiratory fitness. The European Space Agency hypothesized that exercise and AG countermeasures could be applied simultaneously to protect both cardiorespiratory fitness and orthostatic tolerance following prolonged HDBR. Twenty-four
