Collateral role of the external carotid artery and its branches in occlusion of the internal carotid artery.

In 40 patients with unilateral occlusion of the internal carotid artery, using a transcranial Doppler device, blood flow velocity in the ipsilateral ophthalmic and middle cerebral artery was registered. During compression of the ipsilateral common carotid artery a decrease of ophthalmic artery flow velocity was noted in 39 patients (97.5%) and a decrease of middle cerebral artery flow velocity in 8 patients (20%). The average decrease of mean velocity in the middle cerebral artery was 4.5% of the initial value. During compression of the ipsilateral superficial temporal and facial arteries opht

Retrostyloid and retromandibular courses of the external carotid artery.

BACKGROUND: The external carotid artery (ECA) branching pattern has been extensively studied in the current literature. However, its topographical variability has been barely reported. The aim of the current study was to assess the ECA in relationship with the styloid process (SP) and the mandible. MATERIALS: 100 computed tomography angiograms were randomly selected and retrospectively investigated on the neck region for the ECA course in relation to the SP and the mandible. RESULTS: A retrostyloid course of the ECA was identified on 18 sides (9%). Six patients had a bilateral retrostyloid E

Pseudoaneurysm of the external carotid artery--review of literature.

BACKGROUND: Pseudoneurysms of the carotid artery are rarely encountered in clinical practice. When encountered they are most likely secondary to acute neck trauma, and involve the internal carotid. External carotid artery involvement is very rare. METHODS: We report a case of an external carotid aneurysm following a neck dissection review the literature and discuss the investigation and management of these unusual lesions. RESULTS: Incidence is 0.07%, but mortality can be as high as 30%. Clinical features include swelling and neurological complications. Diagnosis is with a high index of susp

Therapeutic embolisation of the external carotid arterial tree.

Intra-arterial embolisation is a valuable adjunct to the treatment of many vascular lesions, including neoplasms such as glomus tumours or juvenile angiofibromas, and arteriovenous malformations. Its place in the management of the individual patient should be established before any surgical procedure is carried out, as this may prejudice the eventual result. The indications for the procedure, the technique, and possible complications are discussed in this paper, and it is emphasised that the latter are best avoided by the use of a scrupulous technique and adequate technical facilities.

Congenital absence of the right common carotid artery, internal carotid artery and external carotid artery associated with anterior communicating artery aneurysm: a rare case.

Congenital absence of the common carotid artery (CCA), internal carotid artery (ICA) and external carotid artery (ECA) is very rare. We present such a rare case in a 53-year-old woman, associated with a saccular aneurysm of anterior communicating artery (ACA) presenting with subarachnoid haemorrhage (SAH). An embryological hypothesis for the anomalies of the aortic arch is proposed. The correlation between the abnormal haemodynamic stress of the circle of Willis caused by the absence of the right ICA and the development of intracranial aneurysm is discussed.

Ligation of the external carotid artery for severe epistaxis.

Epistaxis which is not controlled by anterior and posterior nasal packing is usually treated by ligation or embolization of the arterial supply to the nose. Transantral ligation of the maxillary artery, or embolization of its branches, have recently been considered the treatments of choice. Ligation of the external carotid artery has the advantage of being a short procedure which can be readily performed under local anesthesia, and does not require particular surgical expertise or the use of specialized equipment. This procedure, in combination with ligation of the ipsilateral anterior ethmoid

Cerebral Protection with a Temporary Ascending Aorta-External Carotid Artery Bypass during Common Carotid Artery Revascularization.

Atherosclerosis is a systemic disease, and multiarterial involvement is common. Involvement of all the supra-aortic arteries may occur in the same patient making cerebral revascularization challenging. In this report, we present complete supra-aortic revascularization, that is, revascularization of the bilateral common carotid and subclavian arteries in a 51-year-old male patient with occluded brachiocephalic trunk, left subclavian artery, and proximally stenotic left common carotid artery. A temporary ascending aorta to left external carotid artery bypass provided meticulous cerebral protecti

[Internal carotid artery stenting with proximal protection systems in patients with concomitant external carotid artery injury].

OBJECTIVE: to evaluate the possibility, efficiency, and safety of using the proximal protection systems in patients with concomitant injury to the ipsilateral external carotid artery (ECA) during internal carotid artery (ICA) stenting. MATERIAL AND METHODS: The results of ICA stenting were analyzed in 39 patients. Their mean age was 66.4 +/- 7.8 years; 29 (74.4%) patients were male. All the patients were symptomatic. The time of ICA blood flow arrest averaged 5.4 +/- 2.1 min. Intolerance to blood flow arrest was noted in 3 (7.7%) patients. The intervention time was 27.6 +/- 8.3 min. The resul

[Internal carotid artery stenosis and external carotid artery occlusion with unusual extracranial collateral pathway: case report].

A patient with stenosis of the internal carotid artery and occlusion of the external carotid artery associated with an unusual extracranial collateral pathway is presented. A 63-year-old man was hospitalized for sudden onset of black-out after urination. He was alert, and no neurological deficit was found. MRI showed multiple lacunae in the bilateral putamens. Cerebral angiogram demonstrated severe stenosis of the left internal carotid artery at its cervical segment and complete occlusion of the left external carotid artery at its origin. The ascending pharyngeal artery originated from the lef

Anomalous artery directly connecting the external and internal carotid arteries.

An anomalous artery directly connecting the external with the internal carotid artery was encountered on the right side of a 68-year-old Japanese female cadaver. This anomalous artery (5 mm in diameter, 12 mm in length) branched out from the posterior aspect of the external carotid at the level of the origin of the lingual artery, ran obliquely upward posteriorly along the course of the hypoglossal nerve, and was confluent with the anterior aspect of the internal carotid artery. No other variations were found in the morphological aspects of, or in the anatomical relationships between, the caro

Impact of external carotid artery occlusion at declamping of the external and common carotid arteries during carotid endarterectomy on development of new postoperative ischemic cerebral lesions.

OBJECTIVE: The external carotid artery (ECA) is inadvertently occluded during carotid endarterectomy (CEA). The importance of ECA occlusion has been emphasized as a loss of extracranial to intracranial collaterals, a source of chronic embolization, or a site for extended thrombosis during wound closure. This study aimed to determine whether ECA occlusion that inadvertently developed during endarterectomy and that was eventually detected using blood flow measurement of the ECA after declamping of all carotid arteries is a risk factor for development of new postoperative ischemic lesions at decl

External carotid artery revascularization: indications, operative techniques and results.

The external carotid artery (ECA) is an important collateral pathway in patients with ipsilateral internal carotid artery (ICA) occlusion and recurrent symptoms. An ipsilateral ECA revascularization can improve cerebral perfusion or eliminate an embolic source. In the past 11 years 11 patients underwent operation, 6 times for amaurosis fugax and 5 times for transient ischaemic attacks. Eight times ECA endarterectomy was performed for stenosis and/or ulceration. Eight times closure or separation of the ICA stump with thrombus was carried out. There were no perioperative neurological deficits or

Amaurosis fugax from disease of the external carotid artery.

Three patients with proved internal carotid artery occlusion and diseased external carotid artery or common carotid artery had amaurosis fugax. Evidence for retinal embolization through external carotid collaterals is retinal emboli and infarction after carotid artery manipulation, amaurosis during external carotid angiography, and relief of amaurosis by endarterectomy of this vessel. The abundance of external carotid collaterals, their size, and natural incidence are discussed. Reports clinically support embolization to the central retinal artery and ophthalmic artery through these collateral

Successful Reconstruction of Asymptomatic Bilateral External Carotid Artery Aneurysms.

True aneurysms of the external carotid artery (ECA) are extremely rare with an unknown incidence and natural history. We present the successful operative management of an asymptomatic 65-year-old man found to have bilateral internal carotid artery stenosis and bilateral ECA aneurysms. His bilateral carotid arteries were reconstructed with bifurcated interposition grafts in a staged fashion. The patient recovered without sequelae and continues to be asymptomatic 1 year after reconstruction. We present the operative management of this rare case.

Spontaneous intra-parotid pseudoaneurysm of the external carotid artery.

INTRODUCTION: The authors report a case of spontaneous intra-parotid pseudoaneurysm of the external carotid artery, never previously described in the literature. CASE REPORT: A 59-year-old woman presented with a palpable, non-pulsatile, 2 cm retromandibular left parotid mass with no triggering factors. This mass was considered to be a mixed tumour, although it did not present the typical features on MRI. At surgery, the lesion in the lower pole of the parotid gland was found to be pulsatile, in favour of pseudoaneurysm of the external carotid artery. The external carotid artery was ligated to

Increased cerebral blood flow after external carotid artery revascularization.

Ten patients with symptoms of cerebral ischemia were found to have totally occluded internal carotid arteries with significant external carotid artery stenosis or occlusion. Eight patients underwent external carotid endarterectomy, and two patients underwent saphenous vein bypass to the external carotid artery from the subclavian artery. Cerebral blood flow was measured with 133Xe in six patients preoperatively and in five postoperatively. Four of the six patients had diminished mean flow on the affected side, and three had diminished flow in the contralateral side. All patients had abnormalit

[Subclavian artery to external carotid artery bypass with saphenous vein graft in the treatment of common carotid artery stenosis].

A case of TIAs due to proximal common carotid artery stenosis which was successfully treated with autogenous saphenous vein graft between the subclavian artery and the external carotid artery is presented. A 57-year-old, right handed female was admitted to our hospital for the treatment of left common carotid artery stenosis which was pointed out at a local hospital. She had a 7-years' history of repeated transient right hemiparesis and/or left amaurosis fugax. No neurological deficit was revealed on admission. Angiography showed an 80% irregular stenosis of the left common carotid artery at i
