Carotid Web as an Embolic Source of Acute Ischemic Stroke.

INTRODUCTION: Carotid webs are shelf-like projections of the carotid bulb lumen associated with ipsilateral ischemic stroke in young patients. Given its rarity, a limited number of studies have evaluated the optimal management of symptomatic carotid webs to prevent further ischemic stroke. CASE REPORT: A 40-year-old Caucasian man presented with a sudden onset of left-sided facial droop, hemiparesis, and dysarthria. Computed tomography angiography revealed occlusion of the distal M1 and proximal M2 segments of the right middle cerebral artery and a small intimal flap at the right internal caro

De novo formation of a carotid web: case report.

A carotid web is a shelf-like intraluminal filling defect typically arising from the posterolateral wall of the proximal internal carotid artery. It is recognized as a possible cause of ischemic stroke in young adults. However, its etiopathogenesis is controversial and remains to be fully elucidated. The authors report de novo formation of a carotid web from an intimal dissection documented on serial imaging studies. The findings in this case suggest that a focal intimal dissection could be the underlying cause of a carotid web. Lower shear stress at the posterolateral wall of the proximal int

Discrepant Imaging Findings in Carotid Web: Challenges in Differentiating from Carotid Stenosis Mimics.

Carotid web (CW) is an intimal fibrous thickening, considered a variant of fibromuscular dysplasia. It appears as a shelf-like filling defect in the proximal internal carotid artery (ICA) on angiography and can lead to thrombus formation and embolic stroke. However, distinguishing CW from other carotid pathologies, such as ICA stenosis (ICAS), can occasionally be challenging. A 66-year-old man presented after a motor vehicle accident and was found to have an acute cerebral infarction. Initial computed tomography angiography (CTA) suggested mild ICAS, and carotid ultrasound indicated a non-mobi

Carotid web as a risk factor for large stroke: Prevalence in consecutive carotid endarterectomy cases.

BACKGROUND: Carotid web (CW) is an intraluminal, ridge-like projection arising from the posterior wall of the carotid bifurcation. It is recognized as a risk factor for ischemic stroke and has been increasingly identified as a potential cause of recurrent embolic strokes of undetermined source (ESUS). However, the relationship between CW and stroke size, as well as how CW compares with internal carotid artery stenosis (ICAS), remains unclear. METHODS: A retrospective analysis was conducted of 98 patients who underwent carotid endarterectomy (CEA) for symptomatic ICAS or CW. Demographics, stro

Dual-layer carotid stenting for symptomatic carotid web: Results from the Caroweb study.

BACKGROUND & PURPOSE: Carotid Web (CaW) is a growingly recognized cause of ischemic stroke, associated with a high recurrence risk. Several therapeutic strategies have been proposed as a tertiary prevention including carotid stenting, endarterectomy and antithrombotic medications. Among these, carotid stenting with dual-layer stent may be promising to adequately cover the focal arterial dysplasia. Our aim was to investigate the safety and efficacy of the Casper stent in the treatment of symptomatic CaW. METHODS: We conducted a retrospective analysis of consecutive patients presenting with a s

[A case of recurrent stroke with carotid-web despite dabigatran treatment successfully treated by carotid endarterectomy].

We present a case of a 41-year-old female presenting with recurrence of ischemic stroke on subtherapeutic doses of dabigatran. She had a history of embolic stroke of undetermined sources at the age of 40, and underwent implantable cardiac monitor implantation and had started dabigatran. One year after the first ischemic stroke, she presented with sudden dysarthria and left hemiparesis and was admitted to our hospital. An MRI of the head revealed acute cerebral infarction in the right corona radiata, and an MR angiography revealed right M2 occlusion. Cervical 3D-CTA revealed a protruding struct

Carotid Web as a Source of Thromboembolism in a Young African American Female.

Background: Carotid webs are nonatherosclerotic fibrous bands that may alter hemodynamic flow and increase the risk of platelet aggregation, leading to thromboembolism in young, otherwise healthy individuals. Although rare, carotid webs are important causes of thromboembolic strokes and are often overlooked in the routine workup for a stroke. Treating physicians and radiologists must recognize and properly manage patients who present with carotid webs to prevent recurrent thromboembolism. Case Report: A healthy 30-year-old female presented with slurred speech and unilateral left upper and lowe

[The diagnostic value of ultrasound in carotid web and the characteristics of misdiagnosed cases].

Data of 42 patients with carotid web diagnosed by ultrasound or confirmed by computed tomography angiography (CTA) or surgical pathology in the First Affiliated Hospital of Zhengzhou University were retrospectively collected, including 20 males and 22 females, aged 35-77 (55±11) years. According to the accuracy of ultrasound diagnosis of carotid web, the patients were divided into Group of misdiagnoses by ultrasonoscopy (MD group, 10 cases) and Group of correct diagnosis by ultrasonoscopy (CD group, 32 cases). Compared with CD group, carotid webs in MD group were more likely to be associated w

Multimodality imaging of carotid web: A case report and literature review.

OBJECTIVES: The carotid web is a compelling potential mechanism of cryptogenic stroke. It is easy for it to escape diagnosis or be misdiagnosed, since it rarely causes hemodynamic stenosis. Currently, there is a lack of consensus on the prevalence, etiology, imaging evaluation, and treatment strategy of carotid artery. We aimed to present the multimodal imaging of carotid web and comprehensively review the characteristics of carotid web. METHODS: A 38 year-old man with carotid web, who presented with acute left hemispheric syndrome, was not identified by computed tomography angiography and hi

Carotid web: an under-recognized and misdiagnosed ischemic stroke etiology.

BACKGROUND: Carotid web (CaW) constitutes a possible cause of ischemic stroke, particularly large vessel occlusion syndromes. We aim to evaluate misdiagnosis rates and diagnosis trends for CaW. METHODS: Based on CT angiography (CTA), we prospectively identified a cohort of patients with symptomatic CaW treated at two comprehensive stroke centers (CSC) from 2014 to 2020 to assess misdiagnosis. Official CTA reports from the CSCs and referring hospitals were then reviewed for mention of CaW. For diagnosis trends, we retrospectively analyzed a CSC electronic medical record, identifying patients w

Ultrasound imaging of carotid web with atherosclerosis plaque: a case report.

BACKGROUND: To the best of our knowledge, no previous studies on carotid webs with atherosclerosis plaque have been conducted. Thus, both radiologists and clinicians have insufficient knowledge of this disease, which could lead to misdiagnosis and missed diagnosis. An accurate diagnosis is beneficial to clinical management and prevention of stroke. Here, we present a case of a carotid web with an atherosclerotic plaque, which was confirmed by histopathology and was treated at the Department of Neurosurgery, Beijing Tiantan Hospital. CASE PRESENTATION: We report a rare case of a carotid web wi

Outcomes after carotid revascularization for symptomatic carotid artery web: A multi-institutional cohort study.

ObjectiveCarotid web can serve as a nidus for clot formation and subsequent thromboembolic stroke. Although treatment historically involved antiplatelet therapy, recent evidence demonstrates notably elevated recurrent stroke rates despite best medical therapy. We examined the safety and efficacy of carotid artery stenting (CAS), which involves placing a coil in the artery, and carotid endarterectomy (CEA), a surgery to remove the clot, for treatment of symptomatic carotid web.MethodsA multi-institutional retrospective registry including adult patients with carotid web with history of ipsilater

Carotid web: an underrecognized cause of ischemic stroke in a young patient.

Carotid web is an underdiagnosed cause of ischemic stroke, particularly in young and middle-aged patients without major atherosclerotic risk factors. It is characterized by a shelf-like intraluminal filling defect arising from the posterior wall of the carotid bulb, promoting thrombus formation through local hemodynamic disturbances. We report the case of a 45-year-old woman with a history of well-controlled hypertension who presented with acute right upper limb weakness. Brain imaging confirmed an acute ischemic stroke. CT angiography of the supra-aortic vessels demonstrated a carotid web at

Carotid web: Pathophysiology, diagnostic, and therapeutic options. A narrative review.

A carotid web (CaWeb), otherwise known as a carotid bulb diaphragm, is a spur of intimal fibrous tissue extending into the carotid bulb. It is a rare, underdiagnosed cause of ischemic strokes in young people. The purpose of this narrative review was to provide an update on CaWebs, highlighting recent evolutions in their management. We undertook a comprehensive literature search on main electronic databases - MEDLINE/PubMed, Cochrane Library, Web of Science, and EMBASE - using a dedicated equation to include studies up to February 13, 2024. We also searched for the most recent guidelines about
