Basal joint arthroplasty and carpal tunnel release through a single incision: an in vitro study.

PURPOSE: Carpal tunnel syndrome coexists with basal joint arthritis in a large percentage of patients. These 2 conditions are often treated surgically through separate incisions. The purpose of this cadaver study is to show the effect of trapeziectomy and transverse carpal ligament release from the scaphoid tubercle on carpal canal pressures. This technique may permit both problems to be addressed through the same Wagner incision. METHODS: In 4 fresh frozen cadaver limbs pressures in the carpal canal were elevated to 30 mm Hg through a percutaneously placed balloon. Pressures were measured us

Trapeziometacarpal joint osteoarthritis and carpal tunnel syndrome: a new surgical approach for concomitant treatment.

A new approach for the concomitant surgical treatment of trapeziometacarpal joint osteoarthritis and carpal tunnel syndrome through the same incision is described. The technique was used in 25 patients (20 women, five men; mean age, 56 years). At a mean follow-up of 27 months, there was complete disappearance of the symptoms of carpal tunnel syndrome in 20 of the 25 patients, incomplete but substantial relief in four patients, and no improvement in one patient. One patient had scar tenderness and another had a superficial wound infection. The surgical technique is simple, safe and cost-effecti

[Traumatic axial separation of the radial mid carpal joint. Case report and review of the literature].

The authors report a case of traumatic axial disruption of the radial mid carpal joint. This unusual type of scaphotrapezo-trapezoïdal dislocation occurred after a motorcycle accident. Open reduction and pinning was performed. At three years and six months follow up, clinical exam showed less power thumb pinch than the other side. Arthritic changes were noted on X-rays control, but a good range of motion was preserved. Radial mid carpal disruption is uncommon. It includes the separation of the trapezium, trapezoïdal, first and second metacarpal bones from the remaining wrist. Only 13 other sim

Free vascularized second metatarsophalangeal joint transfer for scaphotrapezial joint reconstruction after distal scaphoid excision and carpal collapse: A case report.

Treatment of the destroyed scaphotrapezial (ST) joint in young patients is challenging due to the limited available options as they might not tolerate the stiffness, loss of grip strength and long-term complications of partial fusions, bone resections, and interposition implants or arthroplasties. This report presents the results of a free osteocutaneous joint transfer used to reconstruct the ST joint in a young patient suffering from ST destruction and carpal collapse. This 34-year-old patient was previously treated for a right scaphoid nonunion with distal scaphoid excision and interposition

Stress-related bone injuries of the carpal joints in show jumping horses rarely limit long-term performance: a retrospective study of 15 cases.

OBJECTIVE: To describe the type and distribution of carpal joint lesions causing lameness in show jumping horses and describe their 1-year performance outcomes. ANIMALS: 15 show jumping horses (median age, 9 years; IQR, 8 to 11 years) of 3 different breeds, mostly Selle Français, evaluated at a referral center between 2013 and 2023. The inclusion criterion was forelimb lameness localized to the carpus, confirmed by perineural or IA analgesia and/or increased radiopharmaceutical uptake on scintigraphy. Horses with limb conformation abnormalities, a history of trauma, or extra-articular soft ti

Carpal Joint Malalignment With Distal Radius Malunion and Factors in Correction After Distal Radius Osteotomy.

PURPOSE: There is a paucity of data regarding recommendations on when to correct for distal radius malunions and if the initial severity of the radiographic outcomes is correlated with the ability to correct to baseline. We evaluated the effects of distal radius corrective osteotomy on preoperative carpal joint malalignment resulting from distal radius malunions, correlated injury severity and osteotomy timing to radiographic outcomes, and developed a straightforward classification system for predicting radiocarpal and midcarpal maladaptive patterns. METHODS: A retrospective review included 2

Changes in contact site of the radiocarpal joint and lengths of the carpal ligaments in forearm rotation: an in vivo study.

PURPOSE: To examine the contact site of the radiocarpal joint and lengths of carpal ligaments at different forearm rotations in vivo. Rotation of the forearm could exert noteworthy influence on mechanics of the wrist, and understanding how forearm rotation influences wrist mechanics may help treat carpal disorders because wrist position closely relates to forearm rotation. METHODS: We obtained computed tomography scans of the wrists of 8 volunteers at the following 7 positions of forearm rotation: neutral; 30°, 60°, and 90° of pronation; and 30°, 60°, and 90° of supination. Three-dimensional

Equine carpal articular cartilage fibronectin distribution associated with training, joint location and cartilage deterioration.

Processes involved in equine carpal osteochondral injury have not been established. In other species, fibronectin appears important in chondrocyte-matrix interactions, and levels are increased in osteoarthritis. This investigation aimed to (a) describe fibronectin immunoreactivity in the middle carpal joint of 2-year-old Thoroughbreds, (b) determine topographical variations, (c) compare strenuously trained (Group 1) or gently exercised horses (Group 2) and (d) describe sites with early osteoarthritis. Group 1 (n = 6) underwent a 19 week high intensity treadmill training programme. Group 2 (n =

Ultrasound of the wrist in low disease rheumatoid arthritis patients: Intercarpal joint is more sensitive than radio-carpal and distal radio-ulnar joint.

OBJECTIVES: Musculoskeletal ultrasound (MSUS) assessment of disease activity is a valuable tool for guiding therapeutic decisions within a treat-to-target strategy. This study aimed to assess synovitis activity in the wrist joint at different levels using ultrasonography (US) in patients with rheumatoid arthritis (RA). METHODS: Gray scale synovial hypertrophy (SH) and power Doppler (PD) ultrasound were performed on the dorsal radio-scaphoid, inter-carpal, and radio-ulnar joints of the dominant wrist. A one-way ANOVA with multiple comparisons was conducted for each joint. RESULTS: A total of

Changes in Ultrasound Parameters of the Median Nerve at Different Positions of the Radiocarpal Joint in Patients with Carpal Tunnel Syndrome.

Ultrasound imaging (US) is being increasingly used to aid in the diagnosis of entrapment neuropathies. This study aims to evaluate the shear modulus and cross-sectional area (CSA) of the median nerve in patients with carpal tunnel syndrome (CTS). A total of 35 patients with CTS participated in the study. CSA and shear modulus testing were performed in shear wave elastography (SWE) mode in five positions of the right and left radiocarpal joints (intermediate position 0°, 45° of extension, maximum extension, 45° of flexion, and maximum flexion). There were significant side-to-side differences in

High-resolution ultrasound approach to quadrangular joint in carpal boss: Everything musculoskeletal sonographer should know.

At the quadrangular joint (QAJ) of the carpus, a rare bony protuberance called carpal boss (CB) may occur. This bone abnormality may be due to osteophytes development or os styloideum. Symptomatic patients may complain pain, swelling, and restrictions in hand motion. These symptoms result from joint degenerative-inflammatory changes, development of ganglion cyst/bursitis, or tendons pathology. Correct diagnosis and appropriate management can be achieved through high-resolution ultrasonography (HR-US). The purpose of this review is to define the pathology spectrum around and within the QAJ in C

Preliminary Assessment of Pisotriquetral Joint Kinematics Following Transverse Carpal Ligament Release: A Cadaveric Pilot Study.

Introduction Transverse carpal ligament (TCL) release is the standard treatment for carpal tunnel syndrome; however, its potential to alter carpal biomechanics is unclear. Specifically, it has been hypothesized that TCL transection may destabilize the pisotriquetral (PT) joint or unmask latent arthritis. This pilot study investigated the immediate kinematic effects of TCL transection on the PT joint in an embalmed cadaveric model. Methods Six embalmed cadaveric wrists were mounted on a custom stabilization apparatus. Colored markers were affixed to the ulna, pisiform, and fourth metacarpal bon

Radiographic Appearance of the Fore Digit and Carpal Joint in the Mule Foal from Birth to 3 Months of Age.

Few studies have established the normal radiographic anatomical development of the donkey foal and, to date, no data are available for mules. Our aim was thus to evaluate the radiographic development of the fore digit and carpal joint in the mule foal from 0 to 3 months of age. Ten forelimbs of five healthy full-term mule foals were included. Radiographs of the fore digit lateromedial and dorsopalmar and the carpus dorsopalmar were performed weekly for the first month of age, and bi-monthly during the following two months. Fore digit growth plate closure times, morphological, angular, and line

[Histochemical and morphometric studies of the musculature of the forelimb of sheep with reference to its function. 2. Flexor and extensor of carpal and toe joints].

Muscle tissue was obtained from eight forearm muscles of six male sheep (180 days old) and stained for NADH tetrazolium oxidoreductase and myofibrillar ATPase after preincubation at pH 4.3. The fiber diameter and the percentage were determined of three fiber types: slow twitch oxidative (STO), fast twitch oxidative (FTO) and fast twitch glycolytic fibers (FTG). The extensor carpi radialis muscle had the lowest percentage of STO fibers. It is a fast extensor of the carpal joint. The superficial digital flexor muscle (FS) is located in the superficial region of the forearm has shown the highest
