[The collector system of the kidney. Applied anatomy based on the analysis of 3-dimensional casts].

The kidney collector system were studied in 120 three-dimensional casts obtained by the injection-corrosion method. To the procurement of the first hundred casts, we made use of a polyester resin; however on the preparation of the last twenty we made use of a mixture containing latex and radiographic contrast as we intended to obtain, besides the casts, the roentgenograms. Two anatomic details, which have a great importance for endourology, were observed: a. the calices' cross draining the meso-renal region and the consequent inter-pelvis-calice (space (IPC) formation which is seen in the roen

WITHDRAWN: Interventions for treating wrist fractures in children.

BACKGROUND: Approximately a third of all fractures in children occur at the wrist, usually from falling onto an outstretched hand. OBJECTIVES: We aimed to evaluate removable splintage versus plaster casts (requiring removal by a specialist) for undisplaced compression (buckle) fractures; cast length and position; and the role of surgical fixation for displaced wrist fractures in children. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (October 2007), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2007, Issue 4), MED

[Surgery for chyloptysis].

Chyloptysis is a rare disease characterized by formation of bronchial casts containing chyle and repeating bronchial tree branching. The authors report a 56-year-old woman with chyloptysis accompanied by cough and expectoration of milky bronchial casts, as well several episodes of asphyxia. Stages of diagnosis and successful treatment including thoracic duct ligation and skeletonization of the root of the left lung are described. The authors also analyze literature data on etiology, pathogenesis and feasibility of conservative and surgical treatment of these patients.

Fibrin airway cast obstruction: Experience, classification, and treatment guideline from Denver.

BACKGROUND AND OBJECTIVES: Plastic bronchitis (PB) is a condition characterized by the formation of thick airway casts leading to acute and often life-threatening airway obstruction. PB occurs mainly in pediatric patients with congenital heart disease (CHO) who have undergone staged surgical palliation (Glenn, Fontan), but can also occur after chemical inhalation, H1N1, severe COVID-19, sickle cell disease, severe asthma, and other diseases. Mortality risk from PB can be up to 40%-60%, and no treatment guideline exist. The objectives herein are to develop a standardized evaluation, classificat

Improving Quality in the Treatment of Pediatric Forearm Fractures: Minimizing the Need for Repeat Intervention With Long-arm Extension Casting.

PURPOSE: Repeat intervention for pediatric forearm fractures after closed manipulation is as high as 46% with flexed elbow (FE) long-arm casts. Casting with the elbow extended (EE) has been advocated as an alternative technique. We documented outcomes of patients treated with closed manipulation and casting with EE long-arm casts for displaced, diaphyseal both-bone forearm fractures. METHODS: We retrospectively reviewed charts for patients who had sedated manipulations and casting for closed, diaphyseal forearm fractures. Open fractures, immediate surgical intervention, metabolic bone disease

Cardiovascular cavities cast in silicone rubber as an adjunct to post-mortem examination of the heart.

We used silicone rubber as a material for making post-mortem casts of cavities in 21 human hearts, 16 of them congenitally malformed, 13 after surgical reconstruction (Fontan and Norwood procedures) and 4 after reconstruction had been performed posts mortem for study purposes. The organs were either fresh, or had been perfusion fixed with formalin prior to casting. When suitable silicone rubber and catalyst were used, we found that casts allowed a very clear view of chamber morphology, and, where surgery had been performed, of any vascular constriction or distortion. We made casts of the aorti

Intraoperative measurement of maxillary repositioning in a series of 30 patients with maxillomandibular vertical asymmetries.

Vertical asymmetry of the maxilla can determine the inclination of the occlusal plane and result in a lack of internal reference points usually employed to measure surgical movements. Maxillary repositioning is therefore based upon surgical planning, which is commonly elaborated on dental casts and model surgery. The absolute precision of surgical planning is not guaranteed by dental casts and model surgery, and the intraoperative assessment of maxillary repositioning is considered to be of paramount importance in such cases. This article describes a simple, noninvasive intraoperative techniqu

Full-body impression technique for conjoined twins: A clinical report.

This clinical report describes a unique impression technique devised for fabrication of a full-body cast of conjoined twins. The authors describe how the cast was used by the medical team to plan the surgical flap design for effective abdominal wall closure during the 27-hour operation sequence that resulted in the twins' separation. The cast, in turn, proved invaluable in planning the complicated surgical procedures necessary to separate them.

The correction of primary knee deformities in children.

One hundred and fourteen patients with primary knee deformity either genu varum, genu valgum or combined varus and valgus deformities were treated with serial plaster cylinder casts and wedging for six to nine months. The children were aged between 2 to 13 years, the peak incidence being in the third year of life. Forty-seven of the patients were found to have active or healed rickets but the remaining patients showed no evidence of rachitic lesions. Results showed that serial corrective plaster casts gave a satisfactory cosmetic appearance in 69 patients but the older the patient the less fav

Applicability of intraoral scans for presurgical orthopedic treatment of cleft lip and palate in infants: a prospective non-randomized comparative study.

OBJECTIVES: This in vivo study evaluated the reliability of digital impressions from intraoral scans compared to digital scans of plaster casts of the edentulous maxilla of infants with cleft lip and palate (CLP). MATERIALS AND METHODS: To compare and evaluate the dimensional accuracy of digital and conventional impressions, 52 infants with CLP were selected. Conventional plaster and digital impressions were taken from each patient before surgical closure of the lips and/or palate. Plaster casts were digitized using the intraoral scanner. Eight linear distances were measured and 3D superimpos

Valve or No Valve: A Prospective Randomized Controlled Trial of Casting Options for Pediatric Forearm Fractures.

The purpose of this study was to determine the rate of cast-related complications when using split or intact casts. A total of 60 patients aged 3 to 13 years with closed shaft or distal third radius and ulna fractures requiring reduction were recruited for this study. Patients underwent closed reduction under sedation and were placed into a long-arm fiberglass cast with 1 of 3 modifications: no valve, univalve, or bivalve. Patients were followed to 6 weeks after reduction or surgical treatment if required. The frequency of neurovascular injury, cast saw injury, unplanned office visits, and cas

Biceps Tendon Lengthening Surgery for Failed Serial Casting Patients With Elbow Flexion Contractures Following Brachial Plexus Birth Injury.

OBJECTIVE: Assessment of surgical outcomes of biceps tendon lengthening (BTL) surgery in obstetric brachial plexus injury (OBPI) patients with elbow flexion contractures, who had unsuccessful serial casting. BACKGROUND: Serial casting and splinting have been shown to be effective in correcting elbow flexion contractures in OBPI. However, the possibilities of radial head dislocations and other complications have been reported in serial casting and splinting. Literature indicates surgical intervention when such nonoperative techniques and range-of-motion exercises fail. Here, we demonstrated a

Improving Lower Extremity Casting Quality by Providing an Experienced Assistant in Pediatric Tibia Fractures Managed by Trainees.

Background: The value of employing an orthopedic technician or advanced practice provider (OT/APP) to assist trainees during on-call hours has not been assessed. As the third most common pediatric long bone fracture, most tibial fractures can be managed with closed reduction and casting. Purpose: We sought to determine whether clinical outcomes could be positively affected for traumatic childhood tibia fractures by using an experienced OT/APP to aid orthopedic surgery residents with closed reduction and casting. Methods: We performed a retrospective chart review of tibial shaft fractures that

Webril-only Soft Spica Cast for Pediatric Femoral Fractures: A Preliminary Study.

We report using a spica cast created with cotton padding and Ace wrap without a rigid component for femur fracture in infants. Outcomes and complications of this soft spica cast were retrospectively compared with other treatments. There were 43 children younger than 6 months (43 diaphyseal fractures) included in the study. Treatment was a Pavlik harness (26 patients), a hard spica cast (8), or a soft spica cast (9) for an average of 3 weeks. All fractures demonstrated healing with similar final angulation and shortening. Hard spica casts caused the most complications. As for material costs, th
