The cost of dengue shock and septic shock in Vietnam: a patient-centred economic analysis.

BACKGROUND: Dengue shock (DS) and septic shock (SS) are the most common infectious causes of shock in Vietnam. Little is known about the cost of an episode of DS or SS from the patient perspective. We aimed to describe the direct medical, non-medical and productivity costs associated with DS and SS. METHODS: We recruited adults with DS and SS to a prospective observational study at the Hospital for Tropical Diseases, Ho Chi Minh City, from 2019 to 2021. We collected hospital bills, insurance status and out of pocket payments, and conducted an economic questionnaire at discharge, 1, 3 and 6 mo

Prevalence and Mortality-Related Factors of Multiple Myeloma in Taiwan.

In this retrospective cohort study based in Taiwan, we reported the current epidemiology of patients with multiple myeloma and analyzed the factors affecting mortality. We identified 7285 patients with newly diagnosed multiple myeloma (MM) between 1997 and 2013 in Taiwan. Privileges data from the National Health Institute Research Database was used, as it is made readily available to the public in electronic format for research purposes. From 1997 to 2013, the average incidence of MM per 100,000 people was 1.83. The mortality accounted for an average of 0.44 per 100,000 deaths. In all 7285 inp

Psychological factors influencing healthcare utilization in breast cancer survivors with pain.

INTRODUCTION: Pain is a prevalent side effect seen in breast cancer survivors (BCS). Psychological factors are known role players in pain mechanisms. Both pain and psychological factors contribute to or interact with healthcare use (HCU). However, the association between psychological factors and HCU has never been investigated in BCS with pain, which is the aim of the present study. METHODS: Belgian BCS with pain (n = 122) were assessed via the Medical Consumption Questionnaire, the Injustice Experienced Questionnaire, the Pain Catastrophizing Scale, the Pain Vigilance and Awareness Question

A patient with eosinophilia, hypoalbuminemia and abdominal pain.

Strongyloides stercoralis infections frequently present with eosinophilia and abdominal pain. Since the gastrointestinal symptoms are non-specific, only 15 percent of these patients are correctly considered to have an infectious enteritis or intestinal parasite. In fact, the initial diagnosis is peptic ulcer disease in most patients. The clinical course may be indolent, or patients may develop a sudden catastrophic illness, particularly following the administration of corticosteroids.

Hypertension cost in primary health care facilities in Morocco: a cost-of-illness study.

Hypertension in low- and middle-income countries such as Morocco has an economic importance due to its high prevalence and cost. The aim of this study is to estimate direct health costs (DHC) for hypertension patients followed in primary health care facilities in Morocco. A cost of illness prevalence-based study was carried out on hypertension patients followed in public primary health care facilities in Fez. Data were collected using a questionnaire, including sociodemographic and clinical data and healthcare resources utilization: medical tests, medications, and medical visits. DHC was estim

Transplant Hepatectomy With Portacaval Shunt and MARS Therapy for Perioperative Catastrophe: A Series of Four Liver Transplant Cases.

UNLABELLED: Increased worldwide focus on maximal donor utilization and transplantation of patients once considered too ill to survive liver transplantation may increase the otherwise rare frequency of catastrophic graft failure. Although the deleterious effects of an acutely failing allograft have been established for decades, the optimal strategy in this patient population in the perioperative period remains ill-defined. METHODS: A retrospective review of all liver transplant recipients with perioperative failure leading to transplant hepatectomy between January 1, 2014 and June 30, 2017 was

Imagining the future in health anxiety: the impact of rumination on the specificity of illness-related memory and future thinking.

Individuals with health anxiety experience catastrophic fears relating to future illness. However, little research has explored cognitive processes involved in how health anxious individuals picture the future. Ruminative thinking has been shown to impede the ability to recall specific autobiographical memories, which in turn is related to maladaptive, categoric future thinking processes. This study examined the impact of rumination on memory and future thinking among 60 undergraduate participants with varying health anxiety (35% clinical-level health anxiety). Participants were randomized to

Health anxiety, rumination, and negative affect: a mediational analysis.

OBJECTIVE: A ruminative cognitive style has been associated with a variety of mood and anxiety disorders. This study examined whether a ruminative cognitive style is associated with health anxiety, even when controlling for negative affect. METHOD: College students (N=198) completed measures of health anxiety, rumination, and negative affect and estimated the likelihood that ambiguous symptoms were indicative of catastrophic illnesses. These data were analyzed using structural equation modeling. RESULTS: A ruminative cognitive style was both indirectly associated with health anxiety via its

What we know and what we don't know about catastrophic antiphospholipid syndrome.

Catastrophic antiphospholipid syndrome (CAPS) is a severe condition with high mortality. Since its description in 1992, an important effort has been made to improve and disseminate knowledge on CAPS. Most of our current knowledge comes from the studies performed using the CAPS Registry, a database created in 2000 to gather as many cases as possible in order to better define this disease. It has demonstrated that this condition has multiple faces and is often triggered by a precipitating factor that leads to a thrombotic microangiopathy and cytokine storm involving almost any organ of the body.

Perceived injustice and its relation to chronic pain outcome in complex regional pain syndrome and chronic musculoskeletal pain.

OBJECTIVES: Clinical observations indicate that patients with complex regional pain syndrome (CRPS) tend to ruminate about their illness. Perceived injustice is a negative cognitive-emotional appraisal regarding the severity of loss associated with blame, unfairness, and pain. We investigated injustice beliefs in CRPS compared with chronic musculoskeletal pain (CMP), where previous evidence indicates clinical relevance for pain-related outcome in this patients' group. The role of perceived injustice in relation to pain intensity and disability was tested through a mediation model including cat

Catastrophic pulmonary haemorrhage after endobronchial biopsy of necrotic lung mass.

Flexible bronchoscopy (FB) is instrumental in the diagnosis and treatment of respiratory illness, with low rates of bleeding post-procedure but unpredictable degrees of severity. Although exceedingly rare, massive pulmonary haemorrhage after FB is often catastrophic. We present a case of massive pulmonary haemorrhage after endobronchial biopsy of a 67-year-old patient with a prior diagnosis of right upper lobe (RUL) necrotic lung mass. Imaging revealed possible lymphangitic carcinomatosis and tumour invasion into the lymphatics and vasculature. Significant RUL tumour burden was visualized duri

Rapid identification of group A streptococcus as the cause of necrotizing fasciitis.

Group A beta-hemolytic Streptococcus pyogenes (GAS) causes a spectrum of highly aggressive, invasive infections. We report two cases of necrotizing fasciitis in which GAS was identified as the presumptive causative organism with the use of the standard rapid streptococcal diagnostic kit. We believe the rapid test kits may be a useful adjunct in the diagnosis and treatment of this catastrophic illness and may play a role in limiting the spread of infection.

Psychoses as the presenting manifestation of Creutzfeldt-Jakob disease in an elderly male.

The differential diagnosis of acute psychosis includes delirium, dementia and primary psychiatric disorders. A comprehensive medical evaluation is crucial to make the proper diagnosis. We report the case of a healthy elderly man who presents with acute psychosis. We describe our diagnostic evaluation based on the patient's history and physical examination, which identified Creutzfeldt-Jakob disease. Avoiding diagnostic error in patients who face such a rapidly progressive and catastrophic illness is paramount.

A cognitive-behavioural approach to the management of idiopathic cervical dystonia.

A case is described in which a patient with idiopathic cervical dystonia is treated successfully with cognitive-behavioural therapy. It was hypothesised that although the dystonia was organic in origin, it was exacerbated and perpetuated by catastrophic thoughts and abnormal illness beliefs, and that modification of maintaining cognitive and behavioural responses would lead to improvement. Self-report outcome measures were consistent with this hypothesis and improvements were maintained at 6-month follow-up.

Risk Assessment of Tuberculosis in Patients With Chronic Mental Illness and Related Factors: A Population-Based Cohort Study in Taiwan.

INTRODUCTION: Tuberculosis (TB) is a globally prevalent chronic infectious disease. The World Health Organization estimates that mental illnesses will become the leading cause of global disease burden in 2030. The inability to detect and provide proper treatment for TB in mental illness patients is an epidemic prevention blind spot. The objective of this study was to retrospectively compare the incidence of TB between the general public and mental illness patients. METHODS: This study used data across Taiwan from 2002 to 2013. The impact of psychological factors on condition-specific, generic and individualized patient reported outcomes in low back pain.

BACKGROUND: An individualized patient reported outcome (PRO) has recently been recommended within LBP research, but no study has evaluated this instrument with commonly applied PROs. Moreover, the impact of psychological factors has mostly been assessed for disease-specific instruments. The objective of this study was to assess the predictive value of illness perceptions, pain catastrophizing and psychological distress on 12 month outcomes assessed by specific, generic and individualized PROs recommended in low back pain (LBP). METHODS: Secondary analysis of patients with sub-acute or chronic

Do we have the guts to go? The abdominal compartment, intra-abdominal hypertension, the human microbiome and exploration class space missions.

Humans are destined to explore space, yet critical illness and injury may be catastrophically limiting for extraterrestrial travel. Humans are superorganisms living in symbiosis with their microbiomes, whose genetic diversity dwarfs that of humans. Symbiosis is critical and imbalances are associated with disease, occurring within hours of serious illness and injury. There are many characteristics of space flight that negatively influence the microbiome, especially deep space itself, with its increased radiation and absence of gravity. Prolonged weightlessness causes many physiologic changes th
