Catheter ablation of fascicular ventricular tachycardia.

Fascicular ventricular tachycardia (VT) is an idiopathic VT with right bundle branch block morphology and left-axis deviation occuring predominantly in young males. Fascicular tachycardia has been classified into three subtypes namely, left posterior fascicular VT, left anterior fascicular VT and upper septal fascicular VT. The mechanism of this tachycardia is believed to be localized reentry close to the fascicle of the left bundle branch. The reentrant circuit is composed of a verapamil sensitive zone, activated antegradely during tachycardia and the fast conduction Purkinje fibers activated

Catheter ablation therapy for supraventricular arrhythmias.

The supraventricular arrhythmias include a wide spectrum of disorders including, in descending order of frequency, atrial fibrillation, atrial flutter, atrioventricular nodal reentry, Wolff-Parkinson-White syndrome, and atrial tachycardia. While not life-threatening in most cases, they may cause important symptoms, such as palpitations, chest discomfort, breathlessness, anxiety, and syncope, which significantly impair quality of life. Medical therapy has variable efficacy, and most patients are not rendered free of symptoms. Research over the past several decades has revealed fundamental mecha

[Catheter ablation of paroxysmal atrial fibrillation].

UNLABELLED: Catheter ablation techniques for atrial fibrillation have undergone an extensive evolution, starting with linear lesions in the right, then the left atria before being superseded by ablation of triggers, mainly from the pulmonary veins. We investigate the feasibility and results of combined pulmonary vein and linear ablation utilizing a specific linear lesion connecting the lateral mitral annulus to the left inferior pulmonary vein (left isthmus). METHODS: 115 patients (101 M: 54 +/- 9 years) with paroxysmal atrial fibrillation (7 +/- 5 years) resistant to 4 +/- 1.6 anti-arrhythmi

[Secondary catheter ablation of atrial fibrillation].

BACKGROUND: In patients with drug-refractory atrial fibrillation there are some non-pharmacologic therapeutic options for heart rate control or recurrence prophylaxis that do not primarily aim at the induction or maintenance of atrial fibrillation itself. AV NODE ABLATION AND MODULATION: Using radiofrequency ablation AV nodal conduction can be completely interrupted (AV node ablation) or partly impaired (AV node modulation), which allows subsequent control of the effective ventricular rate (if necessary by pacer maker implantation). Atrial fibrillation, however, does continue undisturbed in t

[Radiofrequency catheter ablation of ventricular tachycardias].

Management of patients with ventricular tachycardia (VT) is often difficult. Drug therapy is often ineffective. Implantable cardioverter defibrillators (ICDs) can terminate VT episodes but do not prevent them. Radiofrequency (RF) catheter ablation can suppress arrhythmias in selected patients. However, the procedure is often challenging and success rates lower than for ablation of supraventricular tachycardias. The mapping and ablation approach depends on the VT mechanism. Monomorphic VT in patients without structural heart disease is referred to as idiopathic and has a focal origin. These VTs

Radiofrequency catheter ablation of atrial tachycardias.

Atrial tachycardias are frequently unresponsive to medical therapy. His bundle ablation has been proposed as a palliative treatment to treat symptoms and prevent development of tachycardia-mediated cardiomyopathy. Experience with catheter ablation directed at the atrial origin of the tachycardia remains limited. We reviewed the initial success rate and long-term follow-up of radiofrequency ablation of atrial tachycardias. Thirty-six patients underwent electrophysiologic study and radiofrequency ablation of atrial tachycardias, excluding atrial flutter. The suspected mechanism of the clinical a

[Catheter ablation in sinus node tachycardia].

Sinus node tachycardia is an uncommon cause of paroxysmal supraventricular tachycardia. Of those patients who were referred to our clinic for electrophysiologic evaluation and catheter ablation, four patients (1 male, 3 female) were found to have sinus node tachycardia. The diagnosis of sinus node tachycardia required all of the following: The P-wave during sinus rhythm had to be similar to the P-wave during the tachycardia, the atrial activation sequence during sinus rhythm and tachycardia had to be similar, the origin of the tachycardia had to be in the high right atrium. In one patient the

Catheter ablation for paroxysmal atrial fibrillation: a randomized comparison between multielectrode catheter and point-by-point ablation.

INTRODUCTION: Catheter ablation for paroxysmal atrial fibrillation is widely used for patients with drug-refractory paroxysms of arrhythmia. Recently, novel technologies have been introduced to the market that aim to simplify and shorten the procedure. AIM: To compare the clinical outcome of pulmonary vein (PV) isolation using a multipolar circular ablation catheter (PVAC group), with point-by-point PV isolation using an irrigated-tip ablation catheter and the CARTO mapping system (CARTO group; CARTO, Biosense Webster, Diamond Bar, CA, USA). METHODS: Patients with documented PAF were randomi

Clinical Usefulness of Virtual Ablation Guided Catheter Ablation of Atrial Fibrillation Targeting Restitution Parameter-Guided Catheter Ablation: CUVIA-REGAB Prospective Randomized Study.

BACKGROUND AND OBJECTIVES: We investigated whether extra-pulmonary vein (PV) ablation targeting a high maximal slope of the action potential duration restitution curve (Smax) improves the rhythm outcome of persistent atrial fibrillation (PeAF) ablation. METHODS: In this open-label, multi-center, randomized, and controlled trial, 178 PeAF patients were randomized with 1:1 ratio to computational modeling-guided virtual Smax ablation (V-Smax) or empirical ablation (E-ABL) groups. Smax maps were generated by computational modeling based on atrial substrate maps acquired during clinical procedures

Typical Flutter Ablation as an Adjunct to Catheter Ablation of Atrial Fibrillation.

During the past decades there has been a consistent evolution of both surgical and catheter-based techniques for the treatment of stand-alone atrial fibrillation, as alternatives or in combination with anti-arrhythmic drugs. Transcatheter ablation has significantly improved outcomes, despite often requiring multiple procedures and with limited success rates especially in presence of persistent atrial fibrillation. Surgical procedures have dramatically evolved from the original cut-and-sew Maze operation, allowing nowadays for closed-chest epicardial ablations on the beating heart. Recently, th

[Ablation of the cavotricuspid isthmus. Randomized prospective study of radiofrequency ablation with irrigated catheters versus standard catheters].

INTRODUCTION AND OBJECTIVES: An important limitation of the ablation with standard catheter is the volume and limited depth of the lesions created. The irrigated catheters, due to a larger and deeper lesion could be useful in patients with typical atrial flutter. The aim of this study was to prospectively compare the ablation procedure with an irrigated-tip catheter versus the standard catheter in this group of patients. METHODS: A total of 37 consecutive patients referred to ablation of the cavotricuspid isthmus for typical atrial flutter were randomized either to be performed by an standard

Management of recurrent atrial fibrillation after catheter ablation: 5-year outcomes of repeat catheter ablation vs. antiarrhythmic therapy.

AIMS: Recurrence of symptomatic atrial fibrillation (AF) after catheter ablation (CA) is not uncommon. However, real-world long-term data on clinical outcomes of rhythm control strategies in patients with recurrent AF after CA remain limited. METHODS AND RESULTS: Using the TriNetX database, we identified adult patients (≥18 years) with recurrent AF after index CA. A 3-month blanking period was applied, during which no rhythm-control therapy was received. Patients were classified into two groups: repeat CA or initiation of antiarrhythmic drug (AAD) therapy. Propensity score matching (PSM) was

The challenge of optimising ablation lesions in catheter ablation of ventricular tachycardia.

Radiofrequency catheter ablation has become an established treatment for ventricular tachycardia. The exponential increase in procedures has provided further insights into mechanisms causing arrhythmias and identification of ablation targets with the development of new mapping strategies. Since the definition of criteria to identify myocardial dense scar, borderzone and normal myocardium, and the description of isolated late potentials, local abnormal ventricular activity and decrementing evoked potential mapping, substrate-guided ablation has progressively become the method of choice to guide

Single-center experience of catheter ablation for atrial fibrillation using multi-electrode mapping and ablation catheters.

PURPOSE: Radiofrequency ablation (RFA) is an established therapy for the treatment of paroxysmal and persistent atrial fibrillation (AF). Many techniques have been reported to achieve RFA. We report a single-center experience of RFA using three multi-electrode catheters. METHODS: We collected data of the patients who had RFA for AF using custom-designed multi-electrode mapping and ablation catheters between May 2007 and November 2009 at this center. RESULTS: A total of 105 patients aged 56 ± 9.6 years underwent RFA using three multi-electrode catheters. Eighty-seven patients were new and 18
