Effect of Collateral Flow on Catheter-Based Assessment of Cardiac Microvascular Obstruction.

Cardiac microvascular obstruction (MVO) associated with acute myocardial infarction (heart attack) is characterized by partial or complete elimination of perfusion in the myocardial microcirculation. A new catheter-based method (CoFI, Controlled Flow Infusion) has recently been developed to diagnose MVO in the catheterization laboratory during acute therapy of the heart attack. A porcine MVO model demonstrates that CoFI can accurately identify the increased hydraulic resistance of the affected microvascular bed. A benchtop microcirculation model was developed and tuned to reproduce in vivo MVO

A modified sutureless repair for left pulmonary vein obstruction after catheter ablation.

A 52-year-old man presented with temporal haemoptysis and chest pain 6 months after radiofrequency catheter ablation for atrial fibrillation. Computed tomography revealed severe stenosis in the left upper pulmonary vein (PV) and complete occlusion of the left lower PV. A modified sutureless repair of the left PV obstruction was successfully performed. This modified procedure provides a feasible, safe and effective means of treating PV obstruction, even in cases with distal extension of stenosis.

Distal catheter obstruction from non-infectious cause in ventriculo-peritoneal shunted children.

In hydrocephalic children, ventriculo-peritoneal shunting is the preferred treatment with few complications. However, an obviously non-infectious peritoneal reaction to the cerebrospinal fluid (CSF) may occasionally lead to shunt malfunction. In eight hydrocephalic children, shunt malfunction with distal catheter complication was found with abdominal pseudocyst formation in seven cases and accumulation of the CSF in one. All children had a normal CSF cell count and glucose concentration, and white cell count, and C-reactive protein in peripheral blood were normal. No CSF infection could be det

Balloon catheter dilation for treatment of older children with nasolacrimal duct obstruction.

OBJECTIVES: To describe the outcomes of balloon catheter dilation of the lacrimal duct as the first surgical treatment for older children (>or=18 months) with nasolacrimal duct (NLD) obstruction and to examine the effect of the type of obstruction on outcome. DESIGN: Retrospective noncomparative case series. PARTICIPANTS: Seventy-six children 18 months or older with NLD obstruction who had not undergone previous surgery. INTERVENTION: The patients underwent NLD probing, infracturing of the inferior turbinate, and balloon catheter dilation of the distal NLD. The type of obstruction was noted

Retrograde treatment of ureteropelvic junction obstruction using the ureteral cutting balloon catheter.

PURPOSE: We assessed the efficacy and safety of a new cutting balloon catheter to treat symptomatic ureteropelvic junction obstruction in adults. MATERIALS AND METHODS: A total of 32 adults (mean age 40 years, range 18 to 79) underwent retrograde balloon incision for symptomatic ureteropelvic junction obstruction (27 primary and 5 secondary cases). Treatment outcome was based on improvement of symptoms and resolution of obstruction by excretory urography or diuretic renal scintigraphy. RESULTS: A total of 36 retrograde endopyelotomies was performed on 32 patients. Of 4 patients who underwent

Intracardiac thrombus and tricuspid valve obstruction: a complication of Hickman catheter use.

The risk of catheter-related sepsis is well recognized in immunocompromised patients with an indwelling central venous line, but the hazard of thrombus involving the right atrium is less well known. We report the case of a child who died after intraartrial thrombus obstructed the tricuspid valve. The predisposing factors and available treatment strategies are discussed.

Ureteric orifice obstruction by catheter balloon Post-TURP: A rare cause of obstructive uropathy.

A 50-year-old male underwent small volume TURP for median lobe prostatic hypertrophy. Post-procedure, a 3-way urethral catheter was placed. He subsequently developed flank pain, anuria and creatinine rise. CT demonstrated bilateral obstructive uropathy. In the absence of obstructing lesions, it was suspected that the catheter balloon may have caused obstruction of bilateral ureteric orifices. Balloon deflation (from 30 to 10 mL) and catheter repositioning resulted in rapid resolution of pain and resumption of urine output. Urologists should consider the catheter balloon as a cause of obstructi

Suprapubic catheter insertion complicated by small bowel obstruction requiring laparotomy: a case report.

Suprapubic catheterization (SPC) is a common minimally invasive procedure with various urological indications. However, rare complications like bowel obstruction can occur, causing serious consequences. We report a case of a 70-year-old female who developed bowel obstruction 10 days after SPC insertion. Imaging confirmed the diagnosis and exploratory laparotomy revealed bowel obstruction caused by an adhesive band covering the terminal ileum, which was successfully released. This case highlights the importance of precise catheter insertion techniques, early recognition of complications, and pr

Lymphatic Obstruction and Edema in Neonate due to Left Subclavian Central Venous Catheter.

There are several causes of generalized edema in sick neonates. We describe two newborns that developed progressive and treatment-resistant generalized edema. We suggest this is due to impaired lymphatic flow from the thoracic duct as a result of a central venous catheter in the left subclavian vein.

Acute Urinary Obstruction in a Tetraplegic Patient from Misplacement of Catheter in Urethra.

A male tetraplegic patient attended accident and emergency with a blocked catheter; on removing the catheter, he passed bloody urine. After three unsuccessful attempts were made to insert a catheter by nursing staff, a junior doctor inserted a three-way Foley catheter with a 30-mL balloon but inflated the balloon with 10 mL of water to commence the bladder irrigation. The creatinine level was mostly 19 µmol/L (range: 0-135 µmol/L) but increased to 46 µmol/L on day 7. Computerized tomography urogram revealed that the bilateral hydronephrosis with hydroureter was extended down to urinary bladder

A Case of Pyelonephritis Caused by Ureteral Obstruction due to a Urinary Catheter.

An 88-year-old man with long-term catheterization developed sepsis and obstructive pyelonephritis due to catheter migration into the left ureter. After recovery with treatment, recurrence was revealed following routine exchange. Clinicians should suspect ureteral migration in elderly, bedridden patients with long-term urinary catheters who present with peri-catheter leakage or renal dysfunction.

Relief of bronchial obstruction using a Fogarty catheter in a patient with bronchomalacia.

Tracheobronchomalacia can be latent without showing any clinical manifestations and may be incidentally found during anesthesia. In such cases, hypoxia may occur during anesthesia. We experienced obstruction of the left main bronchus caused by bronchomalacia that was incidentally found during open-heart surgery in a 4-yr-old patient. We could not reopen the airway by routine techniques, such as positive pressure, and had great difficulty in weaning the patient from cardiopulmonary bypass. The use of a Fogarty catheter allowed the relief of airway obstruction and weaning from cardiopulmonary by

[The use of the double-J catheter as urinary obstruction prophylaxis in lithiasis].

Our experience in the use of the double-J catheter in the prevention and treatment of urinary obstruction in stone disease is presented. This retrospective study spans the period January, 1985 to June 1989. Fifty-five double-J catheters were placed in 51 patients (catheter replacement was required in 4). Catheter placement was done antegradely in 17, retrogradely in 34 and intraoperatively in 4 cases. The indications were for treatment post-ESWL in 45 patients, stone remnants following percutaneous nephrolithotomy in 1, post-ureteroscopy in 2, and after open surgery in 2 patients. Complication

[Percutaneous revision of shunt with obstructed ventricular catheter (author's transl)].

Shunt dysfunction due to an obstructed ventricular catheter can be avoided, at least in part, by placing the of the catheter anteriorly to the Foramen of Monro. However, once the catheter is obstructed, surgical removal under general anesthesia is almost inevitable. Irrigation method for an obstructed ventricular catheter enables us to gain scarcely anything and accumulation of instilled fluid in the ventricle often causes the dangerously increased intracranial pressure. 1) Technique for placement of the ventricular catheter. The skin incision is a semicircular. After the skin flap is reflecte

Obstructive Uropathy: a retained urethral catheter within an ileal conduit.

Following an ileal conduit (IC) urinary diversion for intractable urinary incontinence, a woman in her 50s presented with deteriorating renal function during a routine follow-up. This decline prompted further investigation with an ultrasound scan (USS), which demonstrated bilateral hydroureteronephrosis and an atrophic left kidney. Although the mercaptoacetyltriglycine (MAG-3) renogram was inconclusive in revealing urinary obstruction, a subsequent computed tomography (CT) scan uncovered a calcified tube-like structure within the IC and hydroureteronephrosis extending to this level. To address

Contemporary techniques for catheter-based intervention for hypertrophic obstructive cardiomyopathy.

Hypertrophic obstructive cardiomyopathy (HOCM) is the most frequent genetic cardiovascular affection and is associated with high mortality and morbidity. Invasive treatment of symptomatic patients with HOCM refractory to drug therapy was limited to surgical myomectomy for years. In the mid 1990s, alcohol septal ablation (ASA) emerged as a new and less invasive option for septal ablation (SA) and is now considered a good alternative with excellent short- and long-term outcomes. Besides ASA, other techniques have been promoted to treat HOCM. The present review aims to summarise current practice

[Obstructive pyelonephritis caused by ureteral orifice catheterization with a bladder catheter].

This article reports a rare case of acute pyelonephritis secondary to left ureteral obstruction by a bladder catheter. The patient was 93years old man hospitalized in the hospital emergency department with a 39°C fever and pyuria from an indwelling catheter. Blood test found hyperleukocytosis, inflammatory syndrome and acute renal failure. Diagnosis was confirmed by non-contrast abdominal CT scan showing distal part of the catheter inside left ureteral orifice with ureterohydronephrosis. Treatment consisted in replacing the catheter by a three-way catheter for irrigation and parenteral antibio
