Central venous catheterization: a prospective, randomized, double-blind study.

Central venous catheters (CVCs) are extensively used worldwide. Mechanical, infectious and thrombotic complications are well described with their use and may be associated with prolonged hospitalization, increased medical costs and mortality. CVCs account for an estimated 90% of all catheter-related bloodstream infections (CRBSI) and a host of risk factors for CVC-related infections have been documented. The duration of use of CVCs remains controversial and the length of time such devices can safely be left in place has not been fully and objectively addressed in the critically ill patient. An

Managing inadvertent arterial catheterization during central venous access procedures.

PURPOSE: Approximately 200,000 central venous catheterizations are carried out annually in the National Health Service in the United Kingdom. Inadvertent arterial puncture occurs in up to 3.7%. Significant morbidity and death has been reported. We report on our experience in the endovascular treatment of this iatrogenic complication. METHODS: Retrospective analysis was carried out of 9 cases referred for endovascular treatment of inadvertent arterial puncture during central venous catheterization over a 5 year period. RESULTS: It was not possible to obtain accurate figures on the numbers of

Brachiocephalic veins: an overlooked approach for central venous catheterization.

Central venous access is important in both assessment and treatment of the patient. In modern clinical practice, a percutaneous approach is preferred. The well-established subclavian and internal jugular vein catheterization techniques, however, still carry the risk of major acute complications. In this article we describe a trial study of a percutaneous brachiocephalic vein catheterization technique conducted on a total of 74 cadavers undergoing autopsy. Relying on constant and easily recognizable anatomical landmarks, we performed a total of 128 catheterizations of the right and left brachio

Central venous catheterization via persistent left superior vena cava.

We describe the problems of clinical assessment of subclavian catheter placement whose course was noted to be along the left lateral border of the heart, suggesting malposition. After the catheter's position in a persistent left superior vena cava was confirmed by blood gas analysis, lateral chest X-ray film, and venography, the catheter was used to provide total parenteral nutrition without complications. The evaluation and workup of seeming malposition of central venous catheters and the embryologic development of a left-sided vena cava are discussed.

Surgical, infectious and thromboembolic complications of central venous catheterization.

The authors have analysed the incidence of specific complications in a series of 420 intracaval catheters placed in 388 patients, using six transcutaneous puncture techniques: supraclavicular and infraclavicular subclavian, external and internal jugular, antecubital and brachiocephalic approaches. Strict and moderate criteria were used to evaluate the frequency of complications. Using strict criteria, the lowest rate of surgical complications (5%) was found with the antecubital and external jugular approach, followed by infraclavicular (6.7%) and supraclavicular (9.3%) subclavian techniques; t

External jugular venous catheterization with a Groshong catheter for central venous access.

BACKGROUND: The Groshong catheter (GC) is commonly used as a peripherally inserted central catheter (PICC), and the external jugular vein (EJV) is not a common route for central venous access (CVA). Therefore, external jugular venous catheterization (EJVC) using a GC is rare. However, our experience has shown that this procedure is safe and simple for CVA. PURPOSE: To assess the utility of EJVC using the GC for CVA. METHODS: CVA was done using a GC via the EJV with a single puncture employing the maximal barrier precaution technique. RESULTS: Between March 2005 and June 2007, we attempted t

Percutaneous central venous catheterization in children: first line choice for venous access.

Percutaneous central venous (CV) catheters using the jugular and subclavian veins have been widely used for hemodynamic monitoring and for venous access in difficult clinical situations. However, peripheral venous cutdowns (PVC) still remain the primary mode of short-term venous access in children. To evaluate percutaneous CV line insertion as a routine procedure, a prospective study of 115 patients (75 CV and 40 PVC) was performed. Ages ranged from 1 day to 17 years; age and weight were similar in both groups. Eighty-three CV lines were attempted, with a success rate of 93%, while 82% of 49 P

Accidental Subclavian Artery Catheterization After Central Venous Catheter Placement.

While central venous catheterization (CVC) is used extensively and for a spectrum of medical indications, including dialysis access, the procedure is not without risks. Addressing and acknowledging potential complications is informative, as such insight can improve patient outcomes and establish clinical guidelines. We present the case of a 34-year-old male with dermatomyositis who sustained a subclavian artery injury in the intensive care unit following internal jugular vein catheterization. This case raises awareness of this complication to enable us to promptly identify and manage similar p

Fatal iatrogenic cardiac tamponade due to central venous catheterization.

Fatal adverse events caused by any health professional as consequence of malpractice are uncommon. In this work, the authors report a fatal cardiac tamponade associated with a peripherally inserted central catheter (PICC) by the right jugular vein that perforated the right atrium of the heart. The diagnosis of cardiac tamponade was not detected in hospital during the intrapericardial infusion of total parenteral nutrition and was only registered during the autopsy. The postmortem examination showed a milky liquid inside the pericardial cavity compatible with the total parenteral nutrition admi

Accidental guidewire migration following emergency femoral central venous catheterization.

Central venous catheter placement especially the femoral venous catheter is a common practice in critically ill patients. Awareness of potential complications of the guidewire such as guidewire migration is of utmost importance. Though potentially retrievable by a vascular surgeon or interventional radiologist if it occurs, close supervision by a senior person during passage by a junior or inexperienced person, the use of ultrasound before and after placement of catheter, and use of a checklist may help to identify and prevent its occurrence. We present a very rare complication of central veno

Ultrasound-guided central venous catheterization: what is the evidence?

In recent years, international health quality assurance organizations have been recommending ultrasound guidance for central venous punctures. This article reviews the evidence behind these recommendations. The MEDLINE, PubMed and SCIELO databases were searched for the following MeSH terms: central venous access, ultrasonography, and adults. The search was conducted on September 24, 2010, and selected meta-analyses, randomized clinical trials and reviews, retrieving 291 papers. The 21 most important papers were analyzed in this review. The internal jugular vein is the most studied ultrasound-g

A special anatomic preparation for teaching central venous catheterization.

Catheterization of the subclavian and internal jugular veins is a commonly performed procedure for monitoring the hemodynamic, respiratory, and fluid status of patients as well as for delivery of special potent drugs and nutritive solutions. In inexperienced hands, this procedure can carry significant morbidity. A comprehensive central line placement teaching program has been developed at our institution centered upon an anatomy and catheterization skills workshop. A dedicated anatomic specimen has been prepared to emphasize key anatomical concepts about the subclavian and internal jugular vei

Emergency central venous catheterization during resuscitation of trauma patients.

Central venous catheterization during resuscitation of trauma patients remains controversial. Such catheterizations performed at the UMDNJ-Robert Wood Johnson Medical School at Camden Cooper Hospital/University Medical Center (Camden, NJ) trauma center for the period January 1, 1988 to December 31, 1989 were retrospectively reviewed. Patients with underlying hemothorax, pneumothorax, or resuscitative thoracotomy were excluded. There were 269 catheters inserted using the Seldinger technique. Catheters were placed via the internal jugular or subclavian veins by attending traumatologists or emerg

Thrombosis and infection complicating central venous catheterization in neonates.

To determine the risk of complication associated with Broviac central venous catheterization in neonates, we reviewed the records of 107 infants who were catheterized an average of 5 weeks after birth and cared for in our neonatal intensive care unit. Forty-five of the 107 neonates (42%) had one or more catheter-related complications. Infants with complications had significantly lower birth weights and gestational age, longer duration of catheterization, and more repeat catheterizations than infants without complications. The mortality rate in infants with complications was not different than
