Risk factors for Enterobacter septicemia in a neonatal unit: case-control study.

Thirty cases of Enterobacter aerogenes or Enterobacter cloacae septicemia diagnosed over a 32-month period in a tertiary care neonatal unit were enrolled in a case-control study. Each case patient was matched with two controls (patients occupying the cots nearest the case patient when the latter developed septicemia). Of the 32 perinatal characteristics evaluated, 11 were identified by univariate analysis to be significantly associated with the infection. These included parents being residents of the Vietnamese refugee camps, respiratory distress syndrome, necrotizing enterocolitis, umbilical

Heparin-free adventitia pulmonary oxygenation in the treatment of trauma complicated with severe pulmonary infection: a case report.

The application of Venovenous (VV) extracorporeal membrane oxygenation (ECMO) in trauma and patients with severe bleeding tendency has been controversial. However, VV ECMO without anticoagulation contributes to reducing the risk of bleeding during ECMO maintenance. VV ECMO serves critical roles in therapy of patients with severe pulmonary infection and failure in conventional therapy. The common peripheral catheterization approach for VV ECMO is femoral vein-internal jugular vein catheterization, and bilateral femoral vein catheterization can also achieve the purpose of respiratory support for

A vascular access team's journey to central venous catheter and arterial catheter insertion.

Central venous catheters (CVCs) and arterial catheters (ACs) are indispensable tools in modern healthcare, enabling life-saving therapies. However, they carry risks of complications such as bloodstream infections, thrombosis, and procedural mishaps, which impact patient safety and healthcare costs.1 Nurse-driven central line insertion (NDCLI) programs offer a proactive approach to address these challenges by empowering trained vascular access nurses to perform CVC and AC insertions.2 This 4-year follow-up builds on the initial publication, A Vascular Access Team's Journey to Central Venous Cat

[Frequency and colonization rate of intravascular catheters].

Clinical signs are not sufficiently reliable for establishing diagnosis of intravascular catheter-related infection. Therefore, microbiological confirmation, based on the culture of the catheter tip after its removal, is necessary in diagnosing the infection. The aim of this study was to determine the frequency and the degree of microbial colonization of intravascular catheters (IVK), and the risk for the onset of sepsis, by using qualitative, semiquantitative (roll plate) and quantitative (vortexing) catheter culture techniques. During the period April 2001-December 2002, 289 intravascular ca

Effect of local warming for arterial catheterization in adult cardiac surgery: a randomized controlled trial.

BACKGROUND: The increase in internal diameter (ID) and cross-sectional area (CSA) may facilitate better arterial catheterization. Since an increase in body temperature can cause peripheral vasodilation, we aimed to determine if local warming of the radial artery (RA) catheterization site could improve the success rate of catheterization. METHODS: This randomized, controlled study enrolled 160 patients aged >18 years who were scheduled for heart surgery. They were randomized into non-warming palpation (NP), non-warming ultrasonography-guided (NU), warming palpation (WP), and warming ultrasonog

Strategies for decreasing vascular complications in diagnostic cardiac catheterization patients.

Vascular complications are the most common type of complication after a cardiac catheterization. These include hematoma, pseudoaneurysm, arteriovenous fistula, peripheral artery occlusion and dissection, and retroperitoneal bleeding. The Pennsylvania Patient Safety Authority reports that nearly half the medical errors that occurred between June 2004 and December 2006 were cardiac catheterization complications, many of which were vascular related. The American College of Cardiology's National Cardiovascular Data Registry benchmark for vascular complications incidence is less than 1% for diagnos

The Evolution of Upper Extremity Vascular System for the Interventional Cardiologist: A 400-Million-Year Tale for Arm-Based Interventions.

From the delicate fish fins to the highly specialized human arm, the vascular architecture of the "upper" extremity has undergone significant remodeling throughout vertebrate evolution. This fascinating journey leaves behind a complex anatomy and sometimes puzzling vascular variants that are difficult to interpret without evolutionary insight. With the rise of transradial interventions and antebrachial venous access for right heart catheterization, understanding this evolutionary context can offer the interventional cardiologist valuable awareness about the anatomical constraints and potential

Clinical review: alternative vascular access techniques for continuous hemofiltration.

Obtaining or maintaining vascular access for continuous hemofiltration can sometimes be problematic, especially in the child or adult in multiple organ failure with edema and/or coagulopathy. Problems commonly encountered include obstruction of the femoral vein by the catheter, insertion difficulties, safety concerns when cannulating the subclavian vein in coagulopathy, and catheter and circuit occlusion due to disseminated intravascular coagulation. For access in infants we describe a technique utilizing two single-lumen thin-walled vascular sheaths. For infants and small children initial acc

Duloxetine contributing to a successful multimodal treatment program for peripheral femoral neuropathy and comorbid 'reactive depression' in an adolescent.

In the United States, duloxetine has been approved for the treatment of major depressive disorder, diabetic peripheral neuropathic pain and fibromyalgia in the adult population. Data regarding the use of duloxetine in the pediatric population, however, are very limited. Femoral nerve injury is a rare complication of cardiac catheterization. In the case described, duloxetine contributed to a successful multimodal treatment program for peripheral neuropathic pain due to femoral neuropathy in an adolescent with 'reactive depression' and conversion symptoms. To the best of the authors' knowledge,

Peripheral intravenous cannulation: reducing pain and local complications.

Caring for patients with peripheral intravenous cannulas/catheters (PIVCs) is an important part of the nurse's role and insertion of PIVCs has traditionally been a post-registration nursing skill. With the introduction of the Nursing and Midwifery Council Standards of Proficiency for Registered Nurses in 2018, insertion of PIVCs is to be incorporated in pre-registration nursing programmes for all four fields of nursing practice. Although IV cannulation is a commonly performed clinical procedure, it is associated with significant risks and complications, which can cause pain for patients. This

[II BAXTER-SENPE workshop: peripheral parenteral nutrition].

During the II BAXTER-SENPE workshop, a multidisciplinary nutrition expert committee discusses on indications, advantages and drawbacks of Peripheral Parenteral Nutrition (PPN), being defined as PN compounded by the three essential elements, vitamins, and minerals. Its composition implies a CH: lipids ratio of 60:40, a limited lipid daily intake (1.3 g/kg of body weight/day) and no more than 30% of total calories), and a nitrogen load not higher than 10 g of nitrogen in a maximum volume of 2500 mL. PPN can be administered by the peripheral route since its osmolarity is lower than 800 mOsm/L and

Preventing complications of subclavian vein catheterization.

The proximity of the subclavian vein to several important structures of the arm, neck and chest make serious complications in subclavian catheterization occur in 0.4% to 9.9% of cases. Complications can be in immediate, chronic and late categories. An understanding of the anatomy, venography and a step-by-step technique of catheterization will help prevent complications and assist the physician in recognizing them when they occur.

Impact of accelerated Seldinger technique long peripheral catheters on catheter-related outcomes: a systematic review.

INTRODUCTION: In recent years, the long peripheral catheter (LPC) has been introduced into clinical practice, filling the gap between short peripheral catheters and midline catheters. Heterogeneous nomenclature has created ambiguity regarding clinical outcomes. METHODS: A systematic search of CINAHL, Cochrane Library, Google Scholar, and PubMed (2000-2023) identified studies evaluating Accelerated Seldinger Technique (AST) placed LPCs in adults with difficult intravenous access (DIVA). Only Prospective and retrospective studies were included. Data extraction and quality assessment was indepen

Upper Extremity Vascular Access.

Vascular arterial access is one of the key steps of percutaneous interventions. Both upper and lower extremity arteries can be used for percutaneous entry point, but choice of the arterial access depends on the interventional procedure type, knowing arterial anatomy, patient's clinical conditions. Proper choice of the vascular access plays crucial role in planning, safety, and effectiveness of interventional procedures. Upper extremity vascular access becomes dominant in interventional cardiology practice, and therefore, interventionalists should be familiar with technical details how to succe
