Alumina ceramic-on-ceramic total hip replacement with a layered acetabular component.

A modular layered acetabular component (metal-polyethylene-ceramic) was developed in Japan for use in alumina ceramic-on-ceramic total hip replacement. Between May 1999 and July 2000, we performed 35 alumina ceramic-on-ceramic total hip replacements in 30 consecutive patients, using this layered component and evaluated the clinical and radiological results over a mean follow-up of 5.8 years (5 to 6.5). A total of six hips underwent revision, one for infection, two for dislocation with loosening of the acetabular component, two for alumina liner fractures and one for component dissociation with

Two- to 9-year clinical results of alumina ceramic-on-ceramic THA.

From June 1997 to June 2003 we performed 194 total hip arthroplasties on 173 patients using alumina ceramic-on-ceramic bearings as part of a prospective United States Food and Drug Administration/Investigational Device Exemption study. The average patient age at surgery was 49.9 years. Minimum followup time was 2 years (mean 4.3 years, range 2-9 years). We evaluated survival rate, implant- and nonimplant-related complications. Clinical outcomes included the Merle d'Aubigné score. We assessed radiographs for signs of osteolysis, component loosening, and implant wear. No patients had osteolysis

Ceramic-on-ceramic versus ceramic-on-polyethylene bearings in total hip arthroplasty: Results of a multicenter prospective randomized study and update of modern ceramic total hip trials in the United States.

One reason why otherwise well-functioning total hip replacements have a finite service life is eventual aseptic loosening of the implants because of osteolysis induced by wear particles from the artificial bearing. Pain and osteolysis from wear debris can manifest even in the absence of aseptic loosening. Total hip replacements with ceramic-on-ceramic articulations have shown less wear both in vitro and in vivo. A randomized prospective clinical trial was conducted to compare the outcomes of ceramic-on-ceramic articulations to ceramic-on-polyethylene articulations. Two-year data are of interes

Cutaneous metallosis following ceramic insert fracture in total hip arthroplasty: a case report and revision with ceramic-on-ceramic bearing couple.

Ceramic fractures in total hip arthroplasty (THA) are rare complications that pose significant challenges for revision surgery. This case report describes a 68-year-old male who experienced a spontaneous alumina (ceramic) insert and head fracture four years after the initial THA. The first revision with cobalt-chrome and polyethylene components led to severe metallosis, including subcutaneous tissue discoloration. A second revision utilized a ceramic-on-ceramic (CoC) bearing couple, resulting in excellent functional outcomes and resolution of symptoms. Cutaneous pigmentation post-THA is rare a

Building Osteogenic Microenvironments With Strontium-Substituted Calcium Phosphate Ceramics.

Bioceramics have experienced great development over the past 50 years. Modern bioceramics are designed to integrate bioactive ions within ceramic granules to trigger living tissue regeneration. Preclinical and clinical studies have shown that strontium is a safe and effective divalent metal ion for preventing osteoporosis, which has led to its incorporation in calcium phosphate-based ceramics. The local release of strontium ions during degradation results in moderate concentrations that trigger osteogenesis with few systemic side effects. Moreover, strontium has been proven to generate a favor

Ceramics with the signature of wood: a mechanical insight.

In an attempt to mimic the outstanding mechanical properties of wood and bone, a 3D heterogeneous chemistry approach has been used in a biomorphic transformation process (in which sintering is avoided) to fabricate ceramics from rattan wood, preserving its hierarchical fibrous microstructure. The resulting material (called biomorphic apatite [BA] henceforth) possesses a highly bioactive composition and is characterised by a multiscale hierarchical pore structure, based on nanotwinned hydroxyapatite lamellae, which is shown to display a lacunar fractal nature. The mechanical properties of BA a

In vitro cytotoxicity of traditional versus contemporary dental ceramics.

STATEMENT OF PROBLEM: The biocompatibility of new dental ceramics has not been assessed with the same scrutiny as has been applied to alloys and composites. Yet, the biocompatibility of ceramics is critical to the long-term success of dental prostheses because ceramics are in close contact with oral tissues for extended periods. MATERIAL AND METHODS: Five dental ceramics (2 traditional feldspathic veneer porcelains [Vita Omega and Duceragold], 2 lithium disilicate pressable materials [Stylepress and Empress-2], and a pressable leucite-based material [Empress-1]) were tested for their ability

Examples of evolution of microstructure in ceramics and composites.

In this article we describe a number of studies involving the direct observation of microstructural evolution. In general these investigations were carried out to establish the mechanistic paths involved. The materials studied range from fibers being evaluated for use in high-temperature ceramic composites to energetic materials used as propellants. In particular we discuss the room temperature imaging of materials difficult to image by conventional means and the use of the chamber atmosphere to influence microstructural evolution. Imaging of hydroxyapatite formed by chemical means is briefly

Should Ceramic-on-Ceramic or Ceramic-on-Polyethylene Bearings Be Preferred in Revision Total Hip Arthroplasty After Ceramic Head or Liner Fracture? A Retrospective Multicenter Case-Control Study of 33 Cases.

INTRODUCTION: Fracture of ceramic components in total hip arthroplasty (THA), although rare, remains a major concern. The optimal bearing choice in revision surgery following such fractures is still debated. Few studies have specifically compared ceramic-on-polyethylene (CoP) to ceramic-on-ceramic (CoC) in this context. Therefore we built up a retrospective comparative study aiming to compare the two bearing surfaces regarding: (1) complication rates, (2) implant survival, and (3) functional outcomes. HYPOTHESIS: The use of CoP with a monoblock (single-mobility) design is associated with fewe

Ultralight hydrophobic foam ceramics derived from secondary aluminum dross.

Secondary aluminum dross (SAD) is a hazardous solid waste that poses significant environmental risks. To promote its high-value utilization, an ultralight hydrophobic foam ceramics was fabricated using SAD as the primary raw material through particle-stabilized foaming followed by polydimethylsiloxane (PDMS) modification. The thermal insulation performance of the material for exterior wall applications was systematically evaluated. Under the optimized sintering conditions of a sintering time of 24 min, a sintering temperature of 1498 °C, and a heating rate of 10 °C/min, the SAD-derived foam ce

Beyond Strength: Toughening Bioactive Ceramics for Functional Bone Repair.

Apoptosis has an essential function in maintaining the integrity of the gastrointestinal mucosa. Its deregulation is associated with the occurrence of lesions such as in atrophic gastritis, peptic ulcers, intestinal metaplasia, and stomach tumorigenesis. Thus, the aim of the present study was to investigate the frequency of apoptotic cells (apoptotic index, AI) by using two different immunohistochemical techniques, TUNEL and anti-activated caspase-3 antibody (CPP32), in gastric dyspepsia [chronic gastritis (CG, N = 34), chronic atrophic gastritis (CAG, N = 11), gastric ulcer (GU, N = 17), and

Closing diastemas and creating artificial gingiva with polymer ceramics.

A 40-year-old male patient elected orthodontic correction of his crowded anterior teeth, leaving multiple root exposures and spacing in the papilla areas. Connective tissue surgery did not satisfy the patient's expectations and led him to seek alternative treatments. An innovative technique was developed to achieve root coverage and fill-in spaces in the papilla areas. This noninvasive technique used a polymer ceramic material that was bonded to the crown and root surface of the individual teeth. The veneer restorations were inserted on the teeth as a solid unit, then separated into individual
