Endoscopic cerebral aqueductoplasty: a trans-fourth ventricle approach.

OBJECT: Advances in endoscopic technology have afforded the neurosurgeon new avenues in the treatment of hydrocephalus, rendering many patients independent of shunts, thus averting shunt complications and failure. Cerebral aqueductoplasty has gained popularity as an effective treatment for membranous and short-segment stenoses of the sylvian aqueduct. Traditionally, this procedure has been performed via a coronal approach, passing through the lateral ventricle, foramen of Monro, and third ventricle into the aqueduct. The authors report on the success of a novel technique for this operation, in

The extreme anterior interhemispheric transcallosal approach for pure aqueduct tumors: surgical technique and case series.

Purely aqueductal tumors represent a rare but distinct entity of neoplasms with characteristic morphology and clinical presentation. This study aims to describe the extreme anterior interhemispheric transcallosal approach as a surgical option for purely aqueductal tumors in the upper part of the cerebral aqueduct and present the surgical results. Prospectively collected data of 4 patients undergoing the extreme anterior interhemispheric transcallosal approach for purely aqueductal tumors in the upper cerebral aqueduct was analyzed. The technique is a variation of the anterior interhemispheric

Keyhole Aqueduct Syndrome.

Keyhole aqueduct syndrome is a rare progressive neurodegenerative disorder describing a unique set of neuro-ophthalmologic, neuroimaging, and histopathological findings on autopsy. A midline mesencephalic cleft communicating with the cerebral aqueduct resembling syrinx is seen on imaging and histopathology. There are 9 cases published in the literature. We encountered a patient with vertical nystagmus, internuclear ophthalmoplegia, and progressive ataxia who has a midline cleft connecting the cerebral aqueduct with the interpeduncular cistern highlighting a distinguishing feature of this syndr

Glucoreceptors controlling feeding and blood glucose: location in the hindbrain.

Microinfusion of 5-thioglucose into either the lateral or fourth cerebral ventricle caused increased feeding and hyperglycemia in rats when the cerebral aqueduct was unobstructed. If the aqueduct was obstructed and 5-thioglucose was infused into the fourth ventricle, increased feeding and hyperglycemia persisted, whereas feeding and hyperglycemia in response to lateral ventricle infusion were abolished. Drinking in response to infusion of angiotensin II into the lateral ventricle was not diminished by aqueduct obstruction. These results indicate that glucoreceptors that mediate feeding and hyp

Cerebrospinal fluid flow in normal beagle dogs analyzed using magnetic resonance imaging.

BACKGROUND: Diseases related to cerebrospinal fluid flow, such as hydrocephalus, syringomyelia, and Chiari malformation, are often found in small dogs. Although studies in human medicine have revealed a correlation with cerebrospinal fluid flow in these diseases by magnetic resonance imaging, there is little information and no standard data for normal dogs. OBJECTIVES: The purpose of this study was to obtain cerebrospinal fluid flow velocity data from the cerebral aqueduct and subarachnoid space at the foramen magnum in healthy beagle dogs. METHODS: Six healthy beagle dogs were used in this

Cerebrospinal fluid volumetric net flow rate and direction in idiopathic normal pressure hydrocephalus.

The aim of the present study was to examine cerebrospinal fluid (CSF) volumetric net flow rate and direction at the cranio-cervical junction (CCJ) and cerebral aqueduct in individuals with idiopathic normal pressure hydrocephalus (iNPH) using cardiac-gated phase-contrast magnetic resonance imaging (PC-MRI). An in-depth, pixel-by-pixel analysis of regions of interest from the CCJ and cerebral aqueduct, respectively, was done in 26 iNPH individuals, and in 4 healthy subjects for validation purposes. Results from patients were compared with over-night measurements of static and pulsatile intracra

Mesencephalic developmental venous anomaly causing obstructive hydrocephalus: illustrative case.

BACKGROUND: Developmental venous anomalies (DVAs) are congenital anatomical variants of the normal deep parenchymal veins. DVAs are occasionally found incidentally on brain imaging, and most cases are asymptomatic. However, they rarely cause central nervous disorders. Herein, a case of mesencephalic DVA that caused aqueduct stenosis and hydrocephalus and discuss its diagnosis and treatment is reported. OBSERVATIONS: The patient was a 48-year-old female who presented with depression. Computed tomography and magnetic resonance imaging (MRI) of the head revealed obstructive hydrocephalus. Contra

Rhombencephalosynapsis as a cause of aqueductal stenosis: an under-recognized association in hydrocephalic children.

BACKGROUND: Rhombencephalosynapsis is a rare genetic aberration characterized by variable vermian hypoplasia/aplasia in conjunction with united cerebellar hemispheres. Genetic defects in the isthmic organizer at the mesencephalic-metencephalic junction are presumably responsible for the associated aqueductal stenosis. OBJECTIVE: We performed a retrospective review of 20 children with rhombencephalosynapsis to evaluate for and emphasize the association of aqueductal stenosis and hydrocephalus. MATERIALS AND METHODS: We retrospectively reviewed the MR and CT images of 20 children (0-11 years o

Flow Imaging in Normal Pressure Hydrocephalus.

Flow imaging targeting the cerebral aqueduct has been used in normal pressure hydrocephalus (NPH) to evaluate for imaging features supportive of a clinical diagnosis of NPH and to determine the NPH subtype.1-3 2D cardiac-gated phase-contrast MRI has been used to evaluate for elevated pulsatile flow through the cerebral aqueduct as one imaging feature supportive of the clinical diagnosis of NPH.4,5 However, aqueduct flow measurements have a low sensitivity for NPH and are not well correlated with clinical outcomes.6-9 Structural and flow imaging of the aqueduct is also helpful for differentiati

[Contribution of 3D CISS MRI for pre- and post-therapeutic monitoring of obstructive hydrocephalus].

This report describes the use of the recent three-dimensional Fourier transform constructing imaging in the steady state (CISS) MR sequence in the management of obstructive hydrocephalus. It is a gradient-echo imaging technique with high resolution which remains sensitive to flow. It enables locating the obstruction and determining the upstream impact. It provides anatomical information about third ventricle (V3) morphology and relationships useful before ventriculostomy. Twenty patients with obstructive hydrocephalus at the level of cerebral aqueduct or posterior V3 underwent sagittal 3DFT-CI

Flowing cerebrospinal fluid in normal and hydrocephalic states: appearance on MR images.

The signal intensity of the cerebrospinal fluid (CSF) in the cerebral aqueduct and lateral ventricles on magnetic resonance (MR) images was evaluated in 16 healthy individuals and in 32 patients with various forms of hydrocephalus (20 with chronic normal pressure hydrocephalus [NPH], seven with acute communicating hydrocephalus, and five with hydrocephalus ex vacuo [atrophy]). The low signal intensity frequently observed in the cerebral aqueduct is believed to reflect the pulsatile motion of CSF, which is related to the cardiac cycle. While this "aqueductal flow void phenomenon" can be observe

Reduced subcommissural organ glycoprotein immunoreactivity precedes aqueduct closure and ventricular dilatation in H-Tx rat hydrocephalus.

The H-Tx rat has fetal-onset hydrocephalus associated with closure of the cerebral aqueduct and a reduction in the secretory cells of the subcommissural organ (SCO), a circumventricular organ situated in the dorsal wall of the cerebral aqueduct. The objective of this study was to determine the role of the SCO in hydrocephalus pathogenesis. Serial brain sections through aqueduct regions containing the SCO from H-Tx rats, together with non-hydrocephalic Fischer F344 rats, were studied at E16, before hydrocephalus onset, at E17, the beginning of onset, and at P0 when the hydrocephalus was overt.

Comparative anatomy of dissected optic lobes, optic ventricles, midbrain tectum, collicular ventricles, and aqueduct: evolutionary modifications as potential explanation for non-tumoral aqueductal anomalies in humans.

PURPOSE: An extensive literature has postulated multiple etiologies for aqueductal stenosis. No publications were found, discussing that evolutionary modifications might explain aqueductal anomalies. This study's objectives were to review the evolutionary modifications of vertebrates' tectum structures that might explain human aqueduct anomalies. Undertaking vertebrate comparative study is currently not feasible in view of limitations in obtaining vertebrate material. Thus, vertebrate material collected, injected, dissected, and radiographed in the early 1970s was analyzed, focusing on the aqu

Current challenges in intrauterine management of obstructive fetal hydrocephalus and future directions.

Aqueductal stenosis is a specific type of non- communicating hydrocephalus, which is characterized by narrowing or constriction of the cerebral aqueduct, also known as the aqueduct of Sylvius. Due to the blockage, the flow of cerebrospinal fluid (CSF) through the aqueduct becomes restricted or obstructed, leading to an abnormal accumulation of CSF within the ventricles and increased intracranial pressure. Progressive expansion of the ventricles leads to increasing cerebral mantle compression during fetal brain development, which can lead to irreversible damage with deleterious consequences. Ea

Hydrocephalus following prenatal methylmercury poisoning.

Prenatal methylmercury poisoning of C57BL/6J mice was followed by the development of communicating hydrocephalus in 15% to 25% of surviving offspring. Although examination of the serially sectioned cerebral aqueduct in hydrocephalic animals revealed the presence of stenosis, complete occlusion of the lumen was not observed. The ependymal epithelium of the cerebral aqueduct was preserved, and there was no evidence of periaqueductal inflammation or reactive gliosis. Edema and vacuolar change were, however, observed subependymally. The cerebral white matter, which bore the brunt of the degenerati

Endoscopic third ventriculostomy in hydrocephalus patients with functioning ventriculoperitoneal shunts: challenging the dictum that shunts treat all types of hydrocephalus. Patient series.

BACKGROUND: Although ventriculoperitoneal shunting is standard for hydrocephalus, shunting may not be ideal for aqueductal stenosis. A cohort of patients with aqueductal stenosis displayed symptoms of over- and underdrainage, despite a patent ventriculoperitoneal shunt (VPS) and optimized valve settings. Endoscopic third ventriculostomies (ETVs) were performed in a subset of these patients with successful treatment of their underlying hydrocephalus, despite a functioning shunt. OBSERVATIONS: All patients who had undergone ETV with a history of ventriculoperitoneal shunting were retrospectivel

Relationship of periventricular overgrowth to hydrocephalus in brains of fetal rats exposed to benomyl.

Benomyl, a benzimidazole fungicide, was administered by gavage to pregnant Sprague-Dawley rats in a daily dose of 62.5 mg/kg of maternal body weight beginning at gestational day (GD) 7. Fetuses examined histologically at GD16 or GD20 revealed a high incidence of craniocerebral anomalies--82.6% of those examined at GD16 and 100% of those examined at GD20. Hydrocephalus occurred in 65.2% of fetuses examined at GD16 and in 58.8% at GD20 but was more severe in the GD20 fetuses. A second common anomaly, termed periventricular "overgrowth" (PVO), consisted of subependymal cell masses that in some fe

Communicating hydrocephalus causing aqueductal stenosis.

The cited evidence establishes that the pathogenetic sequence of generalized communicating hydrocephalus followed by various degrees of aqueductal stenosis occurs in many mammalian species. The aqueductal stenosis is caused by compression of the midbrain by the expanding cerebral hemisphere. Therefore the sharp separation of communicating and non-communicating hydrocephalus is no longer tenable.

Convergence nystagmus associated with Arnold-Chiari malformation.

A case of convergence nystagmus associated with an Arnold-Chiari type I malformation is presented. The nystagmus appeared in the absence of fixation, was provoked during Valsalva's maneuver and neck flexion and extension, and attenuated on deep inspiration. Sagittal magnetic resonance images showed that the diameter of the cerebral aqueduct increased with the neck in full flexion and in full extension. Surgical foramen magnum decompression considerably reduced the nystagmus and abolished the postural variation of aqueduct diameter. It is postulated that this nystagmus was due to a combination
