Chiari osteotomy in cerebral palsy.

Twelve cerebrospastic patients underwent 13 Chiari osteotomies for the treatment of subluxed or dislocated hips. Twelve of these 13 hips were reexamined 24-80 months later. Ten patients had painless located hips. The two failures had pelvic obliquity that resulted in redislocation.

Spontaneous remission of cerebral palsy.

Among 2100 children with a diagnosis of cerebral palsy (CP) twenty carried the diagnosis: Previous CP, now normalized. Seventeen patients could be traced and were reevaluated. Cerebral palsy was diagnosed in these seventeen children (ten boys, seven girls) between the ages of three months and three years (average eleven months). They were found to be normal when reexamined between the ages of one year and five years (average two years two months). Two patients had tetraplegia, three diplegia, nine paraplegia ("paraplegia" were cases of diplegia with minimal affection of the upper limbs - now c

Cerebral palsy: a podiatric overview.

Cerebral palsy is a nonprogressive neurologic lesion occurring during gestation or at birth with resultant motor deficit. It can be classified anatomically, according to the cerebral lesion, or functionally. In order to reach a diagnosis, and subsequently determine the correct treatment, a thorough evaluation of the patient must be performed including muscle testing, persistence of pathologic reflexes, and presence of infantile automatisms. The most frequently seen deformities of the lower extremity are equinus, equinovalgus, and equinovarus. The underlying goal of treatment is to stabilize th

Walking through life with cerebral palsy: reflections on daily walking by adults with cerebral palsy.

Purpose: Walking is a major target in childhood physiotherapy for children with cerebral palsy (CP). Little information exists on the importance or value of walking when these children grow up. The aim of this study was to explore personal reflections on daily walking by adults with CP.Method: Semi-structured individual interviews were conducted and analysed with systematic text condensation, a four-step thematic cross-case analysis. Results: Eight ambulatory adults (26-60 years, four women and four men) with CP were interviewed. Almost all had experienced deteriorated walking ability in adulth

MRI findings in patients with spastic cerebral palsy. II: Correlation with type of cerebral palsy.

The authors studied MR images of the brain in 152 patients, aged 1 to 19 years (mean 3.3), who had spastic cerebral palsy (CP) and were attending two hospitals in Japan in 1993 and 1994. Eighty-one patients had diplegia, 45 had quadriplegia, and 26 had hemiplegia. Of patients with diplegia, 72 had periventricular leukomalacia (PVL) and very few had other types of lesions. In patients with quadriplegia, three main types of brain lesions were observed: PVL in 12 patients, term-type brain injury in 22, and brain anomaly in 10. In the 26 patients with hemiplegia, 17 had a unilateral lesion (rare i

Variability in Cerebral Palsy Diagnosis.

BACKGROUND: Cerebral palsy (CP) is the most common childhood motor disability. The emergence of genetic CP etiologies, variable inclusion of hypotonic CP in international registries, and involvement of different medical disciplines in CP diagnosis can promote diagnostic variability. This variability could adversely affect patients' understanding of their symptoms and access to care. Therefore, we sought to determine the presence and extent of practice variability in CP diagnosis. METHODS: We surveyed physicians in the United States and Canada interested in CP on the basis of membership in the

Exercise interventions for cerebral palsy.

BACKGROUND: Cerebral palsy (CP) is a neurodevelopmental disorder resulting from an injury to the developing brain. It is the most common form of childhood disability with prevalence rates of between 1.5 and 3.8 per 1000 births reported worldwide. The primary impairments associated with CP include reduced muscle strength and reduced cardiorespiratory fitness, resulting in difficulties performing activities such as dressing, walking and negotiating stairs.Exercise is defined as a planned, structured and repetitive activity that aims to improve fitness, and it is a commonly used intervention for

Clinical correlations in cerebral palsy.

BACKGROUND: Cerebral palsy (CP) is a group of persistent (but not necessarily unchanged), movement, posture, muscle tone and motor skills disorders non-progressive, with early onset, due to non-progressive impairments, occurring on an immature brain or a brain under development (prenatal, perinatal, postnatal during the first 3-4 years of life). It is associated to a variable extent with: cognitive disorders, epilepsy, sensory deficits, behaviour disorders. AIM: The study of the correlations between the clinical forms/subtypes of CP, comorbidities, and severity of functional impairment. MATE

Handedness in diplegic cerebral palsy.

OBJECTIVE: To determine if the frequency of left-handedness is high in children with spastic cerebral diplegia. DESIGN: Case-control study. METHODS: One hundred and eleven children with spastic diplegic cerebral palsy and 444 age- and gender-matched controls were studied. The handedness of each child was assigned on the basis of responses to questions on the hand preference for writing and drawing, feeding and throwing a ball. The data were analysed by conditional logistic regression and computing the odds ratio and 95% confidence intervals for left handedness. RESULTS: Of the 111 children

Epidemiologic associations with cerebral palsy.

OBJECTIVE: To estimate epidemiologic risk factors for cerebral palsy. METHODS: Data were collected by linkage to state-based perinatal repositories and cerebral palsy registers and using a maternal questionnaire. The cohort included 587 individuals with cerebral palsy and 1,154 non-cerebral palsy controls. RESULTS: The following factors were associated with cerebral palsy: recorded maternal infection during pregnancy (41.4% patients compared with 31.3% controls; odds ratio [OR] 1.55, 95% confidence interval 1.26-1.91), small for gestational age ([birth weight less than third customized centi

[Acetabuloplasty in cerebral palsy patients].

PURPOSE OF THE STUDY: Acetabuloplasty as an isolated surgical procedure is one of the options allowing for hip joint realignment in cerebral palsy patients. Also, it is often involved in combined techniques used for hip joint reconstruction. MATERIAL: In the years 2004-2005, 20 cerebral palsy patients, aged from 4 to 13 years, were indicated for acetabuloplasty. The group included children with varying degrees of locomotor disability, ranging from inability even to crawl to ability to move without support (stages 2 to 7 on the Vojta scale). METHODS: Clinical and X-ray findings were evaluated

Hip displacement in cerebral palsy.

BACKGROUND: Hip displacement is considered to be common in children with cerebral palsy but the reported incidence and the proposed risk factors vary widely. Knowledge regarding its overall incidence and associated risk factors can facilitate treatment of these children. METHODS: An inception cohort was generated from the Victorian Cerebral Palsy Register for the birth years 1990 through 1992, inclusive, and multiple data sources pertaining to the cohort were reviewed during 2004. Gross motor function was assessed for each child and was graded according to the Gross Motor Function Classificat

Perinatal trauma and cerebral palsy.

In this article "perinatal trauma" is restricted to injuries that are sustained by the infant during the labor and delivery primarily as a result of mechanical factors, with the understanding that even under optimal circumstances, the process of birth is traumatic. Mechanical insults to the perinatal brain may result in primarily a hypoxic or ischemic injury to the cerebral tissues; those conditions are not discussed in this article. Although there are multiple types of perinatal trauma, this article is restricted mainly to those types that impact upon the subsequent development of cerebral pa

Neuropathologic substrate of cerebral palsy.

Animal models have assisted in understanding the mechanisms of brain injury underlying cerebral palsy. Nevertheless, no such models replicate every aspect of the human disease. This review summarizes the classic and more recent studies of the neuropathology of human perinatal brain injury most commonly associated with cerebral palsy, for use by researchers and clinicians alike who need to analyze published animal models with respect to their fidelity to the human disorder. The neuropathology underlying cerebral palsy includes white-matter injury, known as periventricular leukomalacia, as well

Birth asphyxia and cerebral palsy.

There has been a societal presumption that most, if not all, cases of hypoxic ischemic encephalopathy-induced cerebral palsy occur during the 3 hours that are related to the events of labor and delivery; society has tended to overlook the remaining 7000 hours of the pregnancy. As a result of this societal perspective, often times the obstetrician has been targeted unfairly as the person who is responsible for a given child's neurologic injuries. Rather, the entire pregnancy, labor, delivery, and well beyond birth require examination to understand fully the pathophysiologic mechanisms that are
