Intra-osseous cerebrospinal fluid shunts-Overview of past and present clinical and experimental evidence.

In the history of hydrocephalus treatment, a variety of diversion sites have been explored to ensure an adequate alternative when the peritoneum was not a feasible option. An interesting choice was the elimination of excessive cerebrospinal fluid (CSF) in the skeletal system. The purpose of this review was to evaluate all shunting systems that have been implemented in bone structures and to determine their therapeutic potential. All articles pertaining to bone derivations were selected from PubMed, Medline, EBSCO and Scopus, using relevant search terms. The search revealed 6 types of osseous s

Cerebrospinal fluid shunts in pregnancy. Report of two cases and review of the literature.

With the introduction of silicone indwelling cerebrospinal fluid (CSF) shunts in the 1960s, the prognosis for children with hydrocephalus changed dramatically. These individuals are leading normal and productive lives today. Many women with CSF shunts are not entering the childbearing years. Previously, only 13 cases of pregnancy occurring in patients with CSF shunts have been reported. We present two cases here and review the complications and outcome of the other reported cases. Shunt malfunction appears to be common in pregnancy due to increased intraperitoneal pressure and the enlarging ut

A case of recurrent flight-induced cerebrospinal fluid shunt overdrainage.

Shunted patients often complain of headaches after flights. The effect of air travel on shunt systems is unknown. We describe the case of a patient with longstanding hydrocephalus, who suffered flight-induced clinical deterioration and shunt overdrainage in two independent occasions. The patient, clinically stable for 1.5 and 5 years before each episode, reported severe headaches starting during the descent stages of the air travel. On both occasions, brain MRI imaging demonstrated pronounced ventricular size reduction. This case suggests that flight-induced shunt overdrainage can occur and sh

Cerebrospinal fluid biomarkers of neuroinflammation in children with hydrocephalus and shunt malfunction.

BACKGROUND: Approximately 30% of cerebrospinal fluid (CSF) shunt systems for hydrocephalus fail within the first year and 98% of all patients will have shunt failure in their lifetime. Obstruction remains the most common reason for shunt failure. Previous evidence suggests elevated pro-inflammatory cytokines in CSF are associated with worsening clinical outcomes in neuroinflammatory diseases. The aim of this study was to determine whether cytokines and matrix metalloproteinases (MMPs) contribute towards shunt failure in hydrocephalus. METHODS: Using multiplex ELISA, this study examined shunt

Route of antibiotic prophylaxis for prevention of cerebrospinal fluid-shunt infection.

BACKGROUND: The main complication of cerebrospinal fluid (CSF) shunt surgery is shunt infection. Prevention of these shunt infections consists of the perioperative use of antibiotics that can be administered in five different ways: orally; intravenously; intrathecally; topically; and via the implantation of antibiotic-impregnated shunt catheters. OBJECTIVES: To determine the effect of different routes of antibiotic prophylaxis (i.e. oral, intravenous, intrathecal, topical and via antibiotic-impregnated shunt catheters) on CSF-shunt infections in persons treated for hydrocephalus using interna

Hepatic cerebrospinal fluid pseudocyst: A rare complication of ventriculoperitoneal shunt.

BACKGROUND: Ventriculoperitoneal (VP) shunts are among the most frequently performed operations in the management of hydrocephalus. Hepatic cerebrospinal fluid (CSF) pseudocyst is a rare but important complication in patients with a VP shunt insertion. In addition to presenting our own case, we performed a PubMed search to comprehensively illustrate the predisposing factors, clinical picture, diagnostic methods, and surgical treatment. This article represents an update for this condition. CASE DESCRIPTION: A 40-year-old male was admitted to a hospital complaining of fever, abdominal distentio

Fibrothorax in adulthood caused by a cerebrospinal fluid shunt in the treatment of hydrocephalus.

Ventriculopleural shunts have been used as an alternative to ventriculoperitoneal shunts in the treatment of hydrocephalus. Although recurrent pleural effusion is a known complication of this procedure, severe fibrothorax requiring decortication has been described only once, in a case involving a child. The authors report a case of a severe fibrothorax with an entrapped lung presenting in an adult patient.

[Ventriculo-peritoneal shunt, hypovolemic shock and cerebrospinal fluid ascites].

A case of communicating hydrocephalus with excessive amount of choroid plexus confirmed by computerized tomography scan in a five-year-old girl is reported. The child exhibited hypovolemic shock and cerebrospinal fluid (CSF) ascites after ventriculo-peritoneal shunting and relief after ventriculoatrial shunting. Some features suggest overproduction of CSF, with the so-called "third-space" fluid losses, due to sequestration of fluids in the body (peritoneum) that might explain shock. They review 22 cases of CSF ascites reported in the literature.

Ventriculo gallbladder shunt as an alternative cerebrospinal fluid diversionary procedure.

BACKGROUND: In the complex management of hydrocephalus, ventriculoperitoneal (VP) shunt insertion is the mainstay for cerebrospinal fluid (CSF) diversion. However, when traditional routes fail or are contraindicated - due to congenital abnormalities, infections, extensive adhesions, or previous abdominal surgeries - surgeons must consider less conventional alternatives. The second line is the ventriculo-atrial shunt then the ventriculo-pleural. The ventriculo-gallbladder (VG) shunt, although rarely used, presents a fascinating option with unique physiological advantages. Here, we present an in

Shunt-related cerebrospinal fluid overdrainage - a multicentre consensus definition.

BACKGROUND: Overdrainage of the cerebrospinal fluid (CSF) is a known complication of hydrocephalus shunt treatment. The concept of CSF overdrainage has evolved alongside advancements in shunt technology and hydrocephalus management. A systematic literature review identified 22 descriptions of CSF overdrainage, highlighting the lack of a universally accepted definition. To address this ambiguity, we conducted a multicentre consensus study to establish a clinically relevant definition of CSF overdrainage that can guide future research and clinical decision-making. METHODS: A modified Delphi con

Optimal timing of cerebrospinal fluid shunting in patients needing cranioplasty.

BACKGROUND: Cranioplasty is performed to repair the cranium after injury or surgery. Cerebrospinal fluid shunts are commonly required to treat associated hydrocephalus. Single-stage shunt and cranioplasty surgery have been associated with increased risks compared with a staged approach. We aimed to assess whether the timing of cerebrospinal fluid (CSF) shunting (pre- or post-cranioplasty) affects complication rates. METHODS: We retrospectively identified all cranioplasty procedures conducted between 11/2017-12/2021 and 1/2004-3/2022 from the Cambridge and Oulu University Hospitals, respective

Quantitative noninvasive measurement of cerebrospinal fluid flow in shunted hydrocephalus.

OBJECTIVE: Standard MRI protocols lack a quantitative sequence that can be used to evaluate shunt-treated patients with a history of hydrocephalus. The objective of this study was to investigate the use of phase-contrast MRI (PC-MRI), a quantitative MR sequence, to measure CSF flow through the shunt and demonstrate PC-MRI as a useful adjunct in the clinical monitoring of shunt-treated patients. METHODS: The rapid (96 seconds) PC-MRI sequence was calibrated using a flow phantom with known flow rates ranging from 0 to 24 mL/hr. Following phantom calibration, 21 patients were scanned with the PC

Abdominal Cerebrospinal Fluid Pseudocyst as a Complication of Ventriculoperitoneal Shunt Placement.

The abdominal cavity has long been used for absorption of cerebrospinal fluid (CSF) in patients with hydrocephalus. Although the procedure is quite common, there are complications that can potentially arise following ventriculoperitoneal (VP) shunt insertion. Here, we report a case of a 39-year-old female patient in which a large abdominal pseudocyst was developed as a complication of VP shunt placement. Ultrasonographical evaluation of the abdomen showed a well-defined cystic mass lesion later confirmed on CT abdomen. She subsequently underwent surgical excision of the pseudocyst with resolut
