The US government considers cyanide to be among the most likely agents of chemical terrorism. Cyanide differs from many other biological or chemical agents for which little or no defense is available because its individual and public health effects are largely remediable through appropriate preparedness and response. Because the toxicity of the cyanide antidote currently available in the United States renders it ill-suited for use in terrorist incidents and other situations requiring rapid out-of-hospital treatment, hydroxocobalamin--an effective and safe cyanide antidote being used in other c

Effective management of cyanide poisoning from chemical terrorism, inhalation of fire smoke, and other causes constitutes a critical challenge for the prehospital care provider. The ability to meet the challenge of managing cyanide poisoning in the prehospital setting may be enhanced by the availability of the cyanide antidote hydroxocobalamin, currently under development for potential introduction in the United States. This paper discusses the causes, recognition, and management of acute cyanide poisoning in the prehospital setting with emphasis on the emerging profile of hydroxocobalamin, an

In recent years there has been an increased concern regarding the potential use of chemical and biological weapons for mass urban terror. In particular, there are concerns that ricin could be employed as such an agent. This has been reinforced by recent high profile cases involving ricin, and its use during the cold war to assassinate a high profile communist dissident. Nevertheless, despite these events, does it deserve such a reputation? Ricin is clearly toxic, though its level of risk depends on the route of entry. By ingestion, the pathology of ricin is largely restricted to the gastrointe

V-agents are exceedingly toxic oily substances, among which phosphonothiolates VX and VR have been extensively studied. Nevertheless, V-agents encompass a large family of nerve agents with diverse structures, including the phosphonate esters of alkyl acetoacetates or 2-alkoxycarbonyl-1-methylvinyl cycloalkyl methylphosphonates. These agents exist in two geometric isomers, and their properties remain largely unknown. Due to continuous concerns about chemical terrorism and safety, it is necessary to study their properties in order to develop effective countermeasures. Here, we applied computatio

Improvised explosive devices pose a threat to the public by way of terrorism and criminal activities. In the United States a commonly used low explosive in improvised explosive devices is smokeless powder (SP), due to its ease of access. Traditionally, forensic examinations are often sufficient in determining the physical and chemical characteristics of SPs. However, these exams are limited in differentiating or associating SPs when comparing two materials which are physically and/or chemically consistent. Stable isotope analysis of carbon and nitrogen has been used for explosives to further f

Emergency personnel are tasked with the daunting job of being the first to evaluate and manage victims of a terrorist attack. Numerous potential chemical agents could be used by terrorists. The challenge for first responders and local hospital emergency personnel is to prepare for a terrorist event that might use one or more of these agents. As part of that preparation, emergency physicians should have a basic understanding of potential chemical terrorist agents. It is beyond the scope of this article to review all potential terrorist agents. Rather, four potential agents have been chosen for

Weapons of mass destruction can be used to harm and terrorize populations. Such weapons include those with chemical, nuclear or biological properties. Obviously computer viruses can add additional barriers to a quick response. The most effective, least costly and greatest threats are biologicals. Biological terror is not new, and biological weapons have been used for centuries. However, as a result of modern technology, the risks are greater now and the outcomes more terrible. Today they include live pathogens, various toxins, and theoretically "bioregulators"--biochemicals affecting cell sign

In epilepsy research, studies have been made to identify brain areas critical for triggering and/or controlling propagated seizure activity. The purpose of the present study was to focus on a similar approach in nerve agent research by reviewing relevant literature to map potential trigger sites and propagation pathways for seizures. The piriform cortex and medial septal area emerge as prime target areas for soman-induced seizures. The cholinergic hyperactivation in the latter structures seems to induce increased glutamatergic activity in the piriform, entorhinal, and perirhinal cortices along

Concern regarding the use of biological agents--bacteria, viruses, or toxins--as tools of warfare or terrorism has led to measures to deter their use or, failing that, to deal with the consequences. Unlike chemical agents, which typically lead to violent disease syndromes within minutes at the site of exposure, diseases resulting from biological agents have incubation periods of days. Therefore, rather than a paramedic, it will likely be a physician who is first faced with evidence of the results of a biological attack. We provide here a primer on 10 classic biological warfare agents to increa

The use of biological and chemical weapons as agents of warfare and terrorism has occurred sporadically, but recent events demonstrate the increasing risk and possibility that terrorist groups with grievances against the government or groups may employ them. Historically, most evaluations of the potential risk for biological weaponry have focused on the military, but the recent release of anthrax in the United States demonstrates that civilian populations are also at risk. More likely than not, most bioterrorism events will be of a small scale; however, agents such as Bacillus anthracis and Ye

Organic phosphate pesticides were used worldwide after World War II and experiences on poisoning and treatment have been accumulated. An organic phosphate "nerve agent" Sarin was used in two terrorist attacks in Japan in the 1990s. Sarin effects on humans were well documented in these two incidents. Sarin gas inhalation caused instantaneous death by respiratory arrest in several victims in Matsumoto. Severely injured victims presenting with coma and generalized convulsion were resuscitated and recovered rapidly without sequelae. Miosis and blurred-dark vision, ocular pain, copious secretions f

The organic phosphorous compounds (OPC) include both the military grade nerve agents and the organic phosphorous pesticides. The major mechanism of OPC toxicity is through inhibition of acetylcholinesterase in neuronal synapses leading to excess acetylcholine and overstimulation of target organs. Signs and symptoms depend on the affinity of the OPC for muscarinic versus nicotinic receptors, and are likely to include both. Muscarinic symptoms may include diarrhea, urination, bronchospasm, bronchorrhea, emesis, and salivation. Nicotinic symptoms such as paralysis and fasciculations may also occu

OBJECTIVE: Sulfur mustard (SM) is a bifunctional alkylating substance being used as chemical warfare agent (vesicant). It is still regarded as a significant threat in chemical warfare and terrorism. Exposure to SM produces cutaneous blisters, respiratory and gastrointestinal tract injury, eye lesions, and bone marrow depression. Victims of World War I as well as those of the Iran-Iraq war have suffered from devastating chronic health impairment. Even decades after exposure, severe long-term effects like chronic obstructive lung disease, lung fibrosis, recurrent corneal ulcer disease, chronic c

Sulfur mustard (HD) is a vesicating agent that has been used as a chemical warfare agent in a number of conflicts, posing a major threat in both military conflict and chemical terrorism situations. Currently, we lack effective therapies to rescue skin injuries by HD, in part, due to the lack of appropriate animal models, which are required for conducting laboratory studies to evaluate the therapeutic efficacy of promising agents that could potentially be translated in to real HD-caused skin injury. To address this challenge, the present study was designed to assess whether microvesication coul

The United States Department of Health and Human Services (DHHS) plays a key role in biodefense, which accounts for almost 80% of the U.S. civilian biodefense budget, which itself has amount to over 500 billion yen a year since the 2001 anthrax mailings and has involved programs based on the HSPD-10 "Biodefense for the 21st Century". To promote R&D on medical countermeasures, the DHHS has invested in basic research and ensuring market, and closely communicated with stakeholders to overcome R&D bottlenecks, fulfill pipelines and increase the number of players in the market. The destruction by terrorists of the twin towers of the World Trade Center and major damage wrought to the Pentagon on September 11, 2001, followed closely by the bioterrorist anthrax attacks via the mails raised the question of whether risk analysis might have a place in defending the United States against terrorist attacks. After first reviewing the multifaceted nature of terrorism and the reasons it is likely to become endemic in world society in the long term, just as other areas of crime are endemic, this article surveys several fields of risk analysis, finding possible short- and long-te

U.S. military and public health experts are increasingly concerned that the general public is at risk for terrorist attacks. Traditional weapons of mass destruction such as explosive and chemical devices remain the most likely forms of terrorism, however the threat of bioterrorism is also present and may be increasing. An intentional biologic event may be covert and if so, will not become apparent for days or even weeks when many ill people present with an unidentified illness. Health care providers will be the first responders during a biologic attack and will be called upon to diagnose disea

Before the sarin incidents in Tokyo and Matsumoto, the Aum Shinrikyo (now Aleph) had tried to conduct bioterrorism with botulinum toxin and Bacillus anthracis. Followers of the Aum could not overcome technical difficulties inherent in developing biological weapons, and the perpetrators had not been prosecuted for their failed attempts of bioterrorism. But the Aum's biological attack revealed several shortcomings in the Japanese law that regulated biological weapons. Since the missile experiment of North Korea conducted in 1998, the Japanese government has come to consider the threat posed by b
